I am submitting this recall request because my vehicle has experienced repeated incidents of a serious safety issue. While driving—most often on the highway—a warning appears that says “Smart system brake malfunction. ” immediately after, the car begins braking on its own without driver input. This has happened multiple times and, based on online reports, it appears to be affecting many Mazda vehicles across different models and years. Each time, I have been forced to pull over, turn off the car, and restart it to temporarily resolve the issue. This is extremely dangerous, especially at highway speeds, as it creates a high risk of rear-end collisions and sudden loss of vehicle control. When reported to Mazda, the concern was dismissed as a “sensor issue,” but the repeated nature and widespread reports suggest a potential systemic defect that requires urgent investigation and recall.

2 Automatic Emergency Braking problem of the 2018 Mazda CX9

Failure Date: 04/15/2025

1. The navigation has a frosted crackle appearance framing the screen. It is difficult to read the navigation when needed. I know this has happened to other models of this vehicle and those were recalled and covered. 2. The brake senor keeps going off stating "Smart brake system malfunction". This goes off repeatedly while driving. 3. Then a second message pops up and reads "front radar sensor system malfuntion".

Smart city break malfunction warning light appears while driving. Simultaneously the tire pressure warning appears. Also, at the same time, the temperature reads -20 to -40 degrees. Yes, vehicle is available for inspection. Fearful that car could randomly break while driving putting myself and family in danger since there is a malfunction. Mazda service center says the problem can’t be reproduced so there’s nothing they can do. The problem seems to appear when the outside temperature is 60 degrees fahrenheit or higher.

4 Automatic Emergency Braking problem of the 2020 Mazda CX9

Failure Date: 05/29/2024

Vehicle randomly displays "Smart brake system malfunction," "forward Smart city brake malfunction" and " front radar system malfunction. " this has been going on for at least a year. I will be driving on a highway, trying to accelerate and these messages will pop up. I'm not able to accelerate when these messages pop up and I have to turn on my hazard lights and pull over to the side of the road. The only thing that clears these warnings are for me to completely shut down my car and restart it. Luckily I have been able to get over to the side of the road on the highway without an accident, but these warning lights pop up when it's sunny out and there is no reason for the car to act the way it does. I took my car to the dealer the first time it happened and paid several hundred dollars for them to tell me that they don't know why it happened. There are several forums posted online with people having the same issues. Online posts say to make sure the sensors are clean and that the malfunction can activate if it's too hot, rainy, snowing or foggy? in my experience, it has happened only when it has been sunny. When the system decides to malfunction, the car will not accelerate, and it could make it easier for the car to be hit by another car. It's like the car just shuts down.

5 Automatic Emergency Braking problem of the 2019 Mazda CX9

I was driving down the street with my foot in the gas going about 35mph. No cars in front or behind me. All of the sudden my car brakes for no reason. It was a hard brake. My foot was on the gas pedal when this happened. Then a bunch of warnings displayed on my screen: forward Smart city brake malfunction, front camera sensor system malfunction, brake override system malfunction and Smart brake malfunction. My check engine light, traction control light and parking brake light also came on. This was extremely dangerous and could have caused an accident the way the car decided to brake all by itself.

6 Automatic Emergency Braking problem of the 2017 Mazda CX9

Failure Date: 02/24/2023

My vehicle has several issues related to a water pump failure in which the check engine light did not remain on. The vehicle began to over heat and slow down on the highway/ roads despite having my foot on the gas. My vehicle is now at heritage Mazda catonsville and unable to be read by the obd reader which may be related to a weak wiring harness reported by other owners of the Cx9. It also my be why my check engine light is not on despite having an engine issue. Many others have reported a failed water pump which caused cracks in the head which causes the car to over heat and slow down without notice. Despite having thousands of customers with this issue Mazda has yet to issue a recall.

Failure Date: 04/21/2022

On 04/21/22, I took my car to the dealer after noticing antifreeze on our garage floor and a burning smell. No check engine lights had illuminated. They proceeded to tell me that there was a coolant leak from head/block and cavity leak (something about channel pressure). They also stated this was a casting flaw during manufacturing of the engine. They told me I needed a whole new engine for $8,479. 97! Mazda corporate refused to replace the engine at their cost because I had 64,575 miles on it and it was out of powertrain warranty. I called them and asked to speak to a supervisor and never heard from them. I called on 4/26, 4/27, 4/28, 5/4, 5/5 (5 times), 6/9, & 6/20. I finally gave up agreed to pay $7,479. 97 (the dealer gave me $1,000 off). I received the car back on 05/20 and it would not start later that day. They towed it back to Mazda and they replaced the crankshaft sensor that was damaged upon install of the new engine. I received it back again on 05/31 and later in the day, the check engine light came on and the automatic braking system engaged while I was driving (thankfully only 40 mph). I managed to pull over in a turn lane before the engine died. I was so thankful I did not make it onto the interstate I was about to enter! Mazda came and picked me up and later got my car. They could not figure out why it was dying and finally got it to recreate it. They replaced another engine sensor and then drove it a lot to make sure it was truly fixed. I finally received my vehicle back on 07/07. I am afraid to take my car out of town and it makes me nervous it will die and automatically start braking again. They know this is a manufacturer issue and they refuse to take responsibility. I am so disappointed in them. This is a huge safety issue. I am also lucky I didn't blow a head gasket before I took it into the shop the first time.

On several occasions, the system would activate the panic braking while driving at low speeds in a few different situations. One situation is while pulling into a 2 car garage with objects along the garage walls, I. E. Shelves in front of vehicle, lawn equipment on the side, grill on opposite side. While entering the garage the system would engage well before the front of vehicle is close to the shelving. Another occasion happened in a parking lot while driving at low speeds with no other vehicles or objects near the vehicle.

Failure Date: 09/12/2021

On 10/12/2021 we were driving on and urban street when the vehicle infront of us signaled and initiated a right hand turn. At that point were we traveling approximately 20 mph, and we were driving with active cruise disengaged. When we were around 150 feet behind the vehicle taking a right when our cx-9 falsely engaged the emergency braking. Our car with our whole family in it locked up the brakes and stopped in 80 feet short of the car taking a right. The vehicle behind us only managed to stop 10 feet short of hitting us in the rear. It is in the 4 people in our vehicles opinion that this operation was both terrifying and dangerous to use and those following.
