On July 1, 2025, I was involved in a severe rear-end collision while driving my 2024 Mazda3 manual hatchback. The impact was forceful enough to shatter the rear windshield, leave a deep dent in the tailgate, and bend the trailer hitch mount under the vehicle by several inches. Despite this, none of the airbags deployed, and there were no airbag warning lights before or after the crash. After the incident, I checked my vehicle’s VIN on NHTSA’s recall lookup tool and discovered that recall 7525e had been issued on June 6, 2025, for a critical airbag sensing system defect (sas module), which can disable airbag deployment after battery depletion or improper power conditions. According to Mazda and NHTSA documents, this defect can completely disable airbag function, increasing the risk of injury in a crash. As of July 11, 2025, I have not received any recall notice from Mazda by mail, email, phone, or dealer contact—despite being the registered owner. Mazda’s documentation shows that: dealers were notified as early as June 3, 2025 owner letters are not scheduled until July 28, 2025 this means that I was driving a defective vehicle without knowledge of a serious safety issue at the time of the crash. I believe that: Mazda failed in its legal obligation to notify me in a timely and adequate manner. The non-deployment of airbags in my crash is consistent with the failure described in recall 7525e. Other affected Mazda owners may remain unaware and at significant risk. I respectfully request that NHTSA: investigate whether recall 7525e was the cause of airbag failure in this incident. Determine whether Mazda’s recall notification practices met regulatory requirements. Ensure immediate outreach to affected owners to prevent further injury or risk. In 2023, the reverse camera would either flicker or completely shut off (displaying a blank screen) while the reverse gear is engaged. While reversing the vehicle to park onto the property the rear camera image failed to display in the vehicle, leading to a collision with the curb. The limited visibility out of the rear windshield for the Mazda 3 leads to a typical driver relying on the camera display to show obstructions not visible through the rear and side mirrors. This failure lead to the car impacting the curb and the passenger being shocked that the car was in an accident. If that was child or small animal, they would have been hit or run over. Luckily, this failure only resulted in property damage, a large dent in the bumper, and possible exhaust damage (see attached). A recall notice for this known Mazda camera issue was sent out in the mail by the manufacturer at the end of 2023. Mazda failed to address the issue when inquired over the phone, instead stating they "would be in contact once a solution was found. " Mazda did not reach out or notify me at any point with a solution until the incident above occurred. After contacting Mazda by phone to address the issue, they said there is still no solution found but to take the vehicle down for a diagnostic test that would be the owner's responsibility to pay for if it was found to be a "owner issue". After a brief discussion and multiple phone calls, Mazda stated they would make the repair as it was confirmed to be a Mazda issue that had just been solved. However, the car would need to be kept over the weekend and a loaner vehicle would be provided. Mazda also stated the collision was a driver issue and Mazda will not be responsible for anything that occurs while using the vehicle. At that point, the dealership was unwilling to assist me and I left. This seems to be a Mazda originating issue, if the rear camera functioned as intended, the curb would have been visible and the impact could have been avoided.

The contact owns a 2015 Mazda motor. The contact stated that occasionally while the vehicle is shifted into reverse, the review camera image would not display on the digital screen. In addition, the contact stated that the image looked distorted. The contact received a notification of 23v487000 (back over prevention) however, the part to do the recall repair was not yet available. The local dealer was contacted, but the vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 85,000.

Manufacturer recall no 6023g NHTSA recall no. 23v-487 rear view camera has not been repaired. How long more do I have to wait for the repairs to start? it has been over 6 months since the recall notice went out but I have had the issue for years now. Please step in and make Mazda make good on their products. I have called several dealerships but they all tell me that only Mazda headquarters knows when the repairs kits will become available. So, when is that? do we wait until more accidents happen so the lawsuits increase over this matter? last time I checked they are still putting out thousands of cars on the road. . . Does that mean they are too busy to take care of these issues since, after all, they have already pocketed our monies and now we are pushed to the side as if we don't matter? I await great news.
