"automatic braking system failed to engage" on a offramp lane from freeway 5 southbound to crown valley parkway off ramp while traveling approximately 25mph I rear ended a vehicle that was at dead stop and that vehicle rear ended a second vehicle. California highway patrol was called and report was documented which is attached. This happened on December 22,2020 at about 7pm, dark clear evening in the city of mission viejo. It was on the straight freeway lane leading to the offramp with other lanes on the freeway moving freely. Upon discussion with Nissan investigation (claim no. 42994813) 615 725 7590, they summarized 10 page disclaimer from owners manual stating that braking system is not 100%. Its understandable if the vehicle was old but this is a new vehicle with what is advertised as a automatic braking system and that system did not work. I manually braked to a crash.

The intelligent key system allowed the vehicle to be driven off without the key present (it was in the pocket of a person standing outside the car). The driver and passengers then went on a 15 mile trip, wherein the vehicle was parked (safely). After awhile the vehicle was re-entered, but the driver could not start the car because the key was not present. 2 young children were left waiting in the hot sun, but were not injured. Eventually another person retrieved the keys and brought them to the disabled car and driver using a second vehicle. It was very inconvenient and stressful for all parties involved and luckily no one was injured. Note: later on, after testing the vehicle in a controlled situation, the warning system did not indicate that the key was not present as the owner's manual states it should.

Page 1-21 of the 2012 Nissan Leaf owner manual has an illustration of a forward-facing convertible child restraint incorrectly installed, with the caption "latch webbing-mounted attachment". The illustration shows the forward-facing convertible child restraint installed with the lower anchorage flexible connector strap (latch webbing) routed through the rear-facing child restraint belt path. No webbing is routed through the forward-facing child restraint belt path. This illustration may suggest to users that this is how to route the "latch webbing" in a forward-facing child restraint illustration, which may lead to injury. This illustration is common to many models of 2003 and later Nissan vehicles, not just the Leaf.
