3 Front Seat Belt Anchorage problem of the 1996 Plymouth Grand Voyager

Failure Date: 09/05/2001

Drivers side seatbelt "d" ring has 3 fractures, all in different places. One fracture is very large and the other two are of good size. The vehicle has never been involved in a collision and no abnormal stress has been placed upon this componet to cause such fractures. It is reasonably new and was not there as of about 2 months ago. I just discovered it on or about 09-05-01.

4 Front Seat Belt Anchorage problem of the 2000 Plymouth Grand Voyager

Failure Date: 04/25/2001

I was involved in an accident on the date given. The other party involved pulled out in front of me. I hit her at about 45 mph. The main impact was around the location of my left headlight, driver's side and front. My airbag did not deploy, my seatbelt did not lock and restrain me and my back seat collapsed upon impact. Chrysler has sent an engineer to look at the van and I still have not heard back from them. The estimate is still not complete by stan's auto body and the total is already close to $5000. I had to seek medical treatment due to the safety features not performing. I was diagnosed as having a concussion, a contusion and a minor neck injury. As of today I have had a severve headache and some memory loss since the accident.
