Failure Date: 12/25/2025

The contact owns a 2023 Tesla Model 3. The contact stated that while driving with the autonomous self driving feature activated in rainy weather, the vehicle suddenly turned towards the left onto railroad tracks. The contact regained control of the vehicle and was able to drive off the railroad tracks. The tires and rims were damaged due to the failure. The contact owns a 2025 Tesla Model 3. The contact stated while his daughter was driving 30 mph at night with full self-driving(fsd) supervised mode engaged, the vehicle suddenly made a left turn. The contact stated that the vehicle drove over a curb and onto the grass, and the vehicle drove into a residence's driveway, where the front of the vehicle crashed into the rear passenger's side of three unoccupied vehicles that were parked in the driveway. The front passenger's side and driver's side air bags deployed. The exterior hazard lights were illuminated. The contact's daughter did not sustain any injuries. A police report was filed but the contact did not have the police report information available. The vehicle was towed to a tow lot, where it was waiting to be picked up by a certified Tesla autobody repair shop. The manufacturer was notified of the failure, but no assistance was provided. The contact owns a 2022 Tesla Model 3. The contact stated while operating the vehicle in "self-driving" mode, the error message "camera malfunction" was displayed. During the failure the vehicle drove through red lights and erroneously steered the vehicle off the road and into the wrong lane. The vehicle was taken to the local dealer who diagnosed that the main computer was faulty and needed to be replaced. The computer was replaced but the failure persisted. The contact stated a notification was not sent to the car screen allowing the contact to read the instructions and activate the feature.

The contact owns a 2020 Tesla Model 3. The contact stated that while driving 75 mph at night with self-driving assist mode, the auto high beams lights engaged three times within 20 seconds. The contact stated that the driver who was ahead of him then decelerated and veered to the side of the road because it appeared that the contact was sending him an alert through the activation of the high beams. Additionally, the contact was pulled over by a police officer. The contact stated that the auto high beam feature was distracting to other drivers. The contact stated that he was unable to disengage the feature. The manufacturer was notified of the failure and informed the contact that the driver did not have control over the feature. Failure Date: 02/07/2022

On trip from grand island ne to pueblo CO on colorado state hwy 76 when driving at highway speeds the auto steer function repeatedly (at least 8 occurrences) invoked emergency braking for no apparent reason. This occurred during daylight hours, in brilliant sunshine, on clear and dry road conditions with no other vehicles in the vicinity, in both the right and left lane. In some cases the car would brake upon cresting a slight rise where the horizon is not visible until the vehicle is at the top of the rise. In another case there was highly reflective silver steel guard rails on both sides of the road. Also experienced phantom braking when there was identical signage on both sides of the road at about head height of a person. We also experienced occurrences on flat sections of road with no signage. The braking is unpredictable and severe enough to cause a rear end collision if a vehicle had been behind us during any of the events we experienced.

Failure Date: 11/30/2021

On November 30 at 6pm I was driving north on I-75 in the tampa area with navigate on autopilot activated. I was in the middle lane, it was dark, and there was lots of traffic. My screen asked me to confirm the autopilot request to move to the left lane. I tapped the stalk to confirm. I did not look in the left lane and was not controlling the steering, though my hands were on the wheel. The car immediately turned into the left lane and side swiped a police vehicle. Nobody was injured and the damage to both cars was minor. Previously I had received alerts that my left pillar camera was "blocked", but the alerts went away, so I assumed that there was no problem. When the alerts appeared, auto steer was deactivated, but when the alerts went away auto steer worked again. I do not remember when the first alert appeared, but probably earlier in November 2021. Subsequently, the alerts have appeared and disappeared. I have requested service from Tesla, but the appointment is scheduled for next month. The officer in the police vehicle and two other officers who came to the scene looked at my vehicle and took pictures. I received accident report, crash number [xxx] from hillsborough county sheriff's office (case number[xxx]), but no citation. Failure Date: 11/24/2021

The contact owns a 2020 Tesla Model 3. The contact stated that while driving on the right side of the road at 35-45 mph, on autopilot mode with a passenger in the front passenger seat, the vehicle drove off the road independently and crashed into a tree. The air bags failed to deploy. There were no warning lights illuminated. The contact's face hit the steering wheel. The contact sustained a black eye with laceration, bruised ribs, head, neck and back pain, sprained left ankle, bruises on her thigh, may have sustained nerve damage on the right side of the face that hit the steering wheel and sciatic nerve damage on the right side of her body. The occupant of the front passenger seat sustained neck, back and clavicle injuries. The contact sought medical assistance the next day.
