When driving in "autopilot" mode which uses adaptive speed control, the car suddenly slows down without any warnings. This happens even when there is no vehicle in front of the car and therefore slowing down is not expected. This has happened several times while driving on freeways. If it weren't for my immediate reaction, serious multiple-car accidents would have happened. My reaction has been to quickly disengage the "autopilot" system and manually accelerating to avoid rear collisions. The other issue is the car suddenly steer the wheel and drag the car to the side even when there is no reason to do so. In such cases, I have to forcefully steer the wheel to the opposite direction to avoid collision with other cars moving on either side, or parked cars on the streets.

The contact owned a 2022 telsa Model 3. The contact stated that while driving at 18 mph the vehicle veered to the right hitting an electrical pole. The contact stated neither the autopilot or cruise control was set to on, and the vehicle failed to stop. The contact stated it happen so fast he was unable to depress the brake in time. No warning lights illuminated and the air bags in the vehicle failed to deploy. The contact sustained knee injury but did not require immediate medical attention. However, the contact stated he was traumatized from the crash. The vehicle was towed and deemed destroyed. A police report was filed. The contact notified the manufacturer of the failure. The failure mileage was approximately 21,372.

There was a forward collision warning I got with a parked car when going less than 20 mph when the road was curving. The car is defective and I shouldn’t have gotten a forward collision warning which causes distraction from the loud and sudden noise it emits. It also negatively impacts my Tesla insurance safety score which is a scam.

The alarm is so loud that everybody in the car panics including me and it dangerously interrupts my driving. I received a warning message that displayed, “front left safety restraint system fault. ” this is an engineering problem as the cables associated easily detach under the driver’s seat. Tesla has been long aware of this problem. They came out to my home and replaced the wire connection by installing sturdier connectors. I was subsequently was charged $329. 40.

The following recalls have been pending on my vehicle: 23v-838; tc2023-657, 24v-051 Tesla is unable to push over-the-air (ota) updates to my vehicle to remedy these recalls due to an issue with the onboard computer being unable to receive updates. This issue arose when Tesla pushed a software/firmware update to my vehicle which effectively "bricked" the advanced drive system portion of the computer. In other words, I am unable to safely receive an update to resolve the issues (solved by the recall) because Tesla's failed software update damaged the onboard computer.

It was a raining day. When I turned 90 degree to my right into the parking lot from local road, I hit the brake and the car slowed down for turning. Fsd or autopilot were off for sure. Then I felt the car was moving down suddenly and made a big noise due to the a slightly slope down when I turned. I saw a parking slot right in front of me. I hit the accelerator a fraction of second to prevent the car to stop. In a fraction of second, the car accelerated like crazy. I immediately hit the brake very hard (95% of my force on brake). I have 30 years of driving experience. I knew that I hit the brake tightly, but the car still kept accelerate like crazy. I could forever remember the feel and moment of my foot was in the brake. Even after the accident, I went to test the Tesla car. I could feel the difference of brake and accelerator since the accelerator is much easier to push down and I needed to push much deeper in the right of my right foot for accelerator. And the brake is hard, and close to my foot. I thought the car would stop like what I had been driving this car for one year and about 20,000 miles. However, the car was stopping at all, but keeping accelerating for 2 seconds. No warning lights and aeb were on at working. The car hit the short of concrete wall and steel fence and then dropped and hit the tree in right front of me. Finally the car dropped into the short cliff. The airbags were off and warning lights were on. My son and me could use the manual door opener to go out.

As I was approaching my off ramp (which has a very tight turn), my automatic brakes failed which caused me to slam onto the brakes, and skid into one of the off ramp signs. All 3 of my children were in the back, the back driver side window shattered, I lost my driver side mirror, and there was damage done to the back door and wheel well (all on the driver's side). Although the damage looked minimal, no one was willing to work on the vehicle and I was strongly encouraged by my insurance company to claim it as a total loss. Granted, I had this vehicle for approximately 2 months, and had put at least $4,000 into it.

Dear NHTSA, I am writing to report a safety defect with my Tesla Model 3, VIN 5yj3e1ea9jf009000. On July 28th, 2023, at 3:34 pm pst, my wife deepa was driving into my driveway at my home at 4103 filan way, san jose, CA, 95135. The weather was sunny and car was traveling at 5-10 mph. The car was not in autopilot and the driving mode was set to chill mode. When pressed the brake pedal to slow down, the car did not stop and went straight into my garage, causing significant damage to my home and the car. Very experienced driver and I am confident that wife was applying the brake pedal correctly. However, the car simply did not stop. I am very concerned that this is a software glitch in the Tesla car, and I am worried that it could happen to other Tesla drivers. I have tried to contact Tesla to report the issue, but I have not been able to reach anyone. I had the car towed to a body shop, but the car is now totaled and I cannot take it back to Tesla. I am requesting that NHTSA investigate this safety defect and take appropriate action to protect Tesla drivers. I experience frequent "phantom" braking while driving north or south on highway I-75. Many times this happens near state route 70 traveling north, but I have had this phenomenon happening driving south or headed north on 275. Cruise control is on, autopilot may or may not be on (I have had it happen in both modes). It's a frightening and very dangerous problem. Speeds decelerate from 75 or so to about 35 or 40. It's sudden and scary and potentially life threatening. I have immediately hit the gas in the multiple times this has happened.

I was stopped before turning left into a parking space. I was driving slowly. The car stopped but I did not engage the brake as a Tesla will stop once the foot is removed from the accelerator. At that point the car leaped forward at full speed hitting parts of a building and a fence. I did not step on the accelerator. No one was injured. The airbags did not deploy even though there was $17,000 damage to the front of the car. There were no warning signals. The person inside the building was waiting for me and saw the car come to a stop.

During a head-on collision on [xxx] in georgia, the electrical system in my 2022 Tesla Model 3 lost power and the electric door releases stopped functioning. The vehicle interior caught fire and I was trapped inside. I was unaware of the location of the hidden mechanical emergency door release because it is not visibly labeled, not explained upon delivery, and not intuitive in an emergency. I was forced to climb to the back seat and break the rear passenger window with my legs to escape while the interior was burning. I suffered a broken hip, broken arm, and later required a full hip replacement. The mechanical emergency door release was concealed and unlabeled, resulting in entrapment during a fire, which is a violation of fmvss 206 and presents a continuing and life-threatening safety hazard to all drivers and passengers. Tesla has not provided warnings, recall guidance, labeling, or owner education regarding how to exit the vehicle during a power-loss emergency. This defect can result in deaths. Unintended acceleration during public street and highway while approaching a stop to make a right turn on November of 2022 in orange county California onto a highway I let go of the pedal ready to press the brake and while turning it made a unnecessary acceleration to speed up making steering difficult, another incident where I believe in February 2023 as I was driving in the right lane a truck made a sudden stop and the accident avoidance never deployed nor warn me and the acceleration maneuver my steering pedal as I felt it degrease on the brake and increase on the acceleration pedal this was very traumatic and scary knowing I might have bodily injuries or even death also while setting the speed limit I notice while using the regenerative braking the brake pedal would vibrate very aggressively also while using the back up cam and side views it would glitch a lot after a bug update I feel this is very concerning for me and my kids and a lot of reviews and even attorneys or aware of the situation most of all it's a safety issue for California.

I am writing to report several safety issues that I have experienced with my Tesla Model 3. These issues pose a significant risk to my safety and the safety of other road users. I kindly request that you investigate these matters thoroughly and take appropriate action to address them. 1. Steering wheel failure: on three separate occasions, the steering wheel of my Tesla Model 3 suddenly stopped working. Two of these incidents occurred while driving on the highway, which made it extremely difficult for me to maintain control of the vehicle. This sudden loss of steering functionality is a serious safety concern and could potentially lead to accidents and injuries. 2. False forward collision warnings: the car's forward collision warning system has been providing false warnings, which has had a negative impact on my insurance premiums. Despite the fact that my insurance company is aware of the issue and acknowledges that it is a system failure, they have been automatically increasing my insurance rates on the leased vehicle. This has resulted in a significant financial burden, with my insurance premium increasing from $120 to almost $400 per month. Furthermore, I have been informed that next month, my premium will be raised to $600. I would like to emphasize that I have a clean driving record with no previous accidents or traffic violations. 3. Slow charging speed: another issue I have encountered is the extremely slow charging speed of the vehicle. Despite the fact that the Tesla Model 3 is rated at 160 kw/h, it consistently charges at a maximum rate of 50 kw/h, regardless of the charging location or time. This significantly limits the usability. Took the vehicle to the Tesla service center twice. However, they said that they have been unable to replicate the steering wheel failure which would be crazy if they could replicate it every time the car is driven. And they have dismissed the slow charging speed as a normal variation and it's not. Disregarding safety!.

Complete shutdown or blanking screen while driving causing god knows what is working and what not. Definitely driver won't know his/her speed, car systems status like blinker, auto pilot,. . . Etc until the system comes back to become available. 1st time this happened, Tesla sent me its service mobile service person who mentioned that he could not find anything wrong and if it happened again he will schedule the car for an in-house service center appointment for the main computer unit to be replaced. When the incident happened again Tesla scheduled the service appointment but after inspection the report came back that this is a recurring incident in many Teslas and require a firmware update! service report attached (job number 4 and 5).

The undercarriage shield of my car disconnected and started dragging while I was driving in the rain. I did not collide into anything or hit a bump. I inspected my wheel well to discover a set of screws that were bunched up and not installed during manufacturing. I had to pull the shield from under my car so that I can continue to drive the vehicle. The shield is connected to the forward warning and pedestrian warning collision system which became inoperative. The faulty manufacturing that caused the shield to disconnect caused a dangerous condition while operating. After I removed it, a second dangerous condition was created since all of the safety senors were disconnected.

This is a report of suddent unintended acceleration on a 2018 Model 3 performance with 47,800 miles on the odometer - the first known occurrence on this car. At 50% state of charge, I drove the Tesla on a 10 minute trip and pulled into an underground parking garage with 3 lanes of valet parking. At about 5-10 mph, the forward collision warning came on as I was driving towards the cones and stop sign. I brought the vehicle to a full stop with the friction brakes (not regenerative braking, which was set to low). The drive mode was set to hold, which lets the car roll forward but not backwards and doesn't creep when foot is off the brake. About 15-30 seconds after coming to a complete stop, my foot was hovering over the brake with maybe slight pressure up to 5%. The car suddenly lurched forward with wide open throttle type acceleration. It has over 450 electric horsepower from 2 motors. The car moved between 2 feet and 5 feet. My passenger noticed and asked what I did, if anything. I did not ever once touch the accelerator. Since my foot was on the brake pedal but not pushing hard on it, I was able to depress it with my foot and stop the car. There was a concrete wall perhaps 25 feet in front of the car. No people were in the front of the car. I was at the stop sign in the garage where valet takes over. I do not know if the car would have kept accelerating if I wasn't able to engage the brakes hard.

Full self driving was engaged, my hands were on the steering wheel, and vehicle made very unexpected and wrong turn sharply to the right directly into a barrier with front bumper. I have the incident recorded on the dash cam. The time from the sharp turn until the collision was less than 0. 5 seconds, not allowing driver response. I have the video of the dashcam recording of the accident. Tesla also has the cabin recording of my engagement during the accident. I cannot send the video through this web page unfortunately.

Tesla knowingly disabled the hardware sensor suite in a recent update of the software in their push toward "Tesla vision". Unfortunately the disabling of the hardware sensor suite has made the car more dangerous to drive in certain conditions such as dark roads and inclement weather where the camera cannot detect a danger situation in advance the way it could with a hardware sensor suite. The disabling of this sensor is costing me $6000 in damage due to Tesla vision not seeing a large, dead deer in the middle of the highway causing me to drive over it and damage critical components. Tesla's decision to use a plastic shield instead of a metal shield to protect the components lead to the piercing of the battery coolant line and damage to other components.

Ghost forward collision warnings appear from time to time even tho there is no one in front of the vehicle. Sometimes when the vehicle in front of me is turning left/right the collision warning will alarm me of possible impact even tho there was no real threat for anything to happen. Also, it happens when you want to park behind a parked vehicle (it happened to me today) and alerted me of a possible crash, while I was crawling to park. I wouldn't mind these ghost warnings, but since I am using Tesla insurance which gives monthly insurance premiums based on the safety performance of the driver (fcw is one of those and the most important one) this is always impacting me financially at the end of the month. Tesla should be forbidden to use a technology that is not perfect in order to determine the price of the insurance.

Phantom braking issue. Cruise control set at 68mph. Medium freeway traffic at 7pm. Noticed disabled car ahead on the shoulder. No danger. As we approached the car, my car went into a panic stop as if it thought a collision was ahead. Car braked and skidded. I was able to regain control and accelerate and avoid a rear end collision. No waring or repair issues.

During rush hour traffic on highway 101 southbound near redwood city, I experienced an incident while utilizing autopilot and autosteer. Despite being in stop-and-go traffic, the vehicle did not respond to the car in front which suddenly came to a complete stop. Neither the automatic emergency braking system nor warning sounds, which has been known to activate erroneously, were triggered. As a result, I was forced to manually brake at the last moment, but there was not enough time for manual intervention and the vehicle collided with the car in front. It is worth noting that every other instance in which I have utilized autopilot and autosteer, which occurs on nearly every trip I take, the vehicle has been able to respond and brake, albeit with slow reaction times and close distances.

Tesla promised in Nov 2022 that software update enabling parking assist will be available within "next couple of weeks". 3 months passd since then and there is still no parking asist.

Constantly issues with the car braking hard without reason - also the car not braking when needed, and poor estimation of distance to the vehicle directly in front. When the car phantom brake, it causes traffic behind the vehicle to become in danger of a forward collision. When the car incorrectly estimates the distance to the next vehicle, it causes the car to apply maximum braking force to rectify the miscalculation. The "autopilot" and "cruise control" are dangerous and hazardous to use in normal vehicle operation. The vehicle/component and behavior is repeatedly certified as "within spec" and no issues found with the vehicle or components.
