37 Wheel problem of the 2022 Tesla Model 3

Failure Date: 07/16/2023

I was driving my 2022 Tesla Model 3 long range. This car does not have autopilot (because we do not trust that technology). We were traveling eastbound on I-8 just east of exit 61 from san diego to arizona on Sunday July 16 at about 2:30pm. The weather conditions were not an issue; it was sunny and warm. I was in the left lane doing 77mph with the cruise control on. I had passed a vehicle in the right lane and it was now maybe 500 feet behind us, still in the right lane. Suddenly, the Tesla Model 3 braked hard for no reason. The car braked so hard that the tires were starting to squeal. We were well inside the lane (had not drifted to either side). There were no obstacles in front of us. There was no discoloration on the highway from any repair or car fire. The road was completely visible. The car's speed had dropped from 77mph to about 57mph before I could press on the gas to resume normal driving. Had there been a car behind us, it's possible we could've been hit from behind. The car in the right lane did wind up passing us and I'm pretty sure they thought our driving was dangerous -- borderline insane. There were no warning sounds, no alerts on the monitor. It was completely random, completely sudden and completely dangerous. The car did this 2 more times before I disengaged the cruise control for the rest of the trip. I do not have a dashcam recording of this incident, but I do have one of a similar behavior from the car a few days earlier on July 13. It's hard to tell from the video exactly where it happens.

38 Wheel problem of the 2023 Tesla Model 3

Failure Date: 03/11/2023

On 3/11/23, around noon time, I drove on I-280 n bound, just past edgewood road. It was nice weather, no wind, nothing extra loaded in the car, smooth clean road surface. All of a sudden I saw black pieces flying at my left side of the car through the window, I heard some noise and the car started unbalanced. I pulled the car slowly to the shoulder . . . It was a left rear tire blowout! this car was only for 4 months, and drove ~2700 miles. No warning sign beforehand and no damage on the tire, the rim and the tire cover after carefully examining the Tesla test center at 680 el. Camino real, santa clara, CA. Attached are four photos taken on 3/11/23. New tire was replaced on 3/14/23 at SC site, CA. It was charged $441. 67 (receipt is not included here).

39 Wheel problem of the 2022 Tesla Model 3

Failure Date: 03/08/2023

I ran over an object in the road too small to see. The tire burst and went flat. The burst also created a hole in the wheel itself. The alloy/material from which the wheel is constructed was not strong enough to keep a hole from being created when the tire burst.

42 Wheel problem of the 2021 Tesla Model 3

Failure Date: 11/06/2022

The tpms alerted me that the front driver tire was low at 36 psi. Pulled into gas station within a mile from initial alert and when I unscrewed the valve cap the valve stem broke and shot out of the tire and the tire deflated. The components were found and are available for inspection. The car has not been repaired yet and is currently at the Tesla service center in agoura hills at 28721 bldg a canwood street agoura hills, CA. I drive two children under 5 and am lucky this did not occur at high speeds or while the vehicle was in operation at any speed. There is no reason this should have happened. There was no prior damage to the wheel or tire.

43 Wheel problem of the 2018 Tesla Model 3

Failure Date: 10/18/2022

Car is 4 years old with 22k miles only and never was involved in an accident or high impact situation. When backing out of driveway and then turning wheels going forward at low speed, a loud noise occurred in the front passenger wheel area and car came to a complete stop. After going out to investigate, the car body was resting on the wheel and two bolts were on the road and the lower control arm was detached from the underbody and dragging to the floor. Had this happened at high speed I can’t imagine what would have been the outcome. No early warning signs occurred prior to this incident.

44 Wheel problem of the 2018 Tesla Model 3

Failure Date: 10/15/2022

My vehicle, was parked and pulled out of the parking lot on a doing a complete left steering turn, couple of nuts&bolts carrying the vehicle control arm fell to the ground and the entire control arm disengaged out of the shafts. This could have been very fatal, if my family or me were riding the car. Tesla was trying to tell me that saying that, this is caused due to a high impact etc, not doing wheel alignment etc. And they want to charge me $3561+ to fix this shaft to put back the nuts and bolts that fell off from the car.

45 Wheel problem of the 2022 Tesla Model 3

Failure Date: 09/22/2022

The car was advertised at 312miles on the battery. It is 265. The charging takes 45 to hr and I was lied to about the mileage overage and the ability to purchase the car. This car isn't as advertised. I reached out to Tesla and they do t offer any solution. I was scammed. I want to return the car. The tires do t last as the cars are too heavy. If the tires don't last 1 yr that's a defect.

46 Wheel problem of the 2022 Tesla Model 3

Failure Date: 08/28/2022

I recently got into a car accident with a Tesla. I was injured and my car crashed against a residence’s entrance putting myself and others in danger. The components or systems that failed are the regenerative breaking on the Tesla, the torque or force that is distributed to each wheel, and the rear passenger tire shows more wearing than any of the other tires of the vehicle (not only on my car, but also on one of my friend’s Tesla. ) it was raining. I was driving at a normal, slow speed, on a straight line on the road. I stopped pressing the gas pedal, the regenerative breaking activated, and my car glided and crashed. In the past, I have had some experiences where even on a dry road, sunny day and no rain, my car would slide and loose traction when letting go of the gas pedal therefore activating the regenerative breaking and making the car loose traction and slide. I have videos and pictures, filled a police report, and recently filed a claim with my insurance. The vehicles components have been inspected by the police. They have not been inspected by the insurance as of now. The only lights/car signals that turned on automatically after the incident occurred were the emergency lights. I do not recall any lights/signals showing up on the dashboard or screen while the incident was happening.

47 Wheel problem of the 2018 Tesla Model 3

Failure Date: 08/14/2022

Vehicle developed a loud squeak coming from driver’s side front wheel well that sounded like a something vibrating or rubbing a spring at ~54300 miles. Reported issue to Tesla via the Tesla app. Tesla’s recommended solution is to replace both the right and left upper control arm. Further research found that this is a widespread problem reported by Tesla owners on the web. Indeed when I was at the Tesla service center the women in line in front of me was bring in her vehicle for the same problem. The upper control arms include a ball joint that needs to be lubricated. This vehicle’s upper control arms do not have a built in means to lubricate that joint like a zerk fitting. The vehicle is available for inspection. The safety issue here is a defective and/or poorly designed part which if left un-lubricated would likely experience water intrusion, corrosion and possible catastrophic failure – maybe breaking at speed. Our vehicle has lost lubricant at the ball joint given the noise and is likely susceptible to/experiencing water intrusion. Also found “sb-20-31-006 service bulletin reseal front upper control arm ball joints”, but this curiously does not include our vehicles VIN. My suspicion is our upper control arm has a different part number than the upper control arm in sb-20-31-006 but is likely a very similar part. My concern is part number whack-a-mole is taking place. Tesla did not even have to inspect the car to know what the problem was. No other parties have inspected the part and no warnings, alerts, messages or other symptoms have occurred except for the horrible noise.

48 Wheel problem of the 2022 Tesla Model 3

Failure Date: 06/22/2022

While backing out of my garage the car accelerated uncontrollably and climbed the curb of my driveway. I tried braking without any success. A message saying, to the best of my recollection, "automatic braking disabled" appeared on the dashboard. After climbing the curb the vehicle came to a stop and "hazard lights" came on. There were less than 50 miles on the odometer at this time. After waiting for a few minutes I gently pulled the car off the curb and slowly drove it back into the garage. I discovered broken lug nut and what appeared to be its casing lying in the driveway. Tesla service center sent tow-truck to tow the vehicle away to its service center. I handed over the broken lug nut and and its casing to the Tesla tow-truck person. While towing my car suffered even more damage as the passenger side front wheel became almost horizontal and it considerably damaged the front bumper. The photos of all these are uploaded with this incident report.

49 Wheel problem of the 2022 Tesla Model 3

Failure Date: 02/26/2022

I am reporting this incident on behalf of our daughter who lost her husband in this Tesla 3 single-car accident. Photographs of what I believe is a failed left front wheel are attached. Sample pieces of the wheel are not in my possession. The accident occurred on a clear day in the late afternoon on a dry stretch of narrow, unlined road that was in a degree of disrepair. Video from the front camera of the vehicle does not indicate a high rate of speed prior to the final minute, which is not captured on the vehicle flash drive. (the flashdrive or specific video minutes can be submitted at your request. ) the first reponder and witness at the scene of the accident described the vehicle approaching and accelerating very rapidly toward an impending curve. The sound of the acceleration frightened his children who were playing in the front yard. The witness indicated hearing a loud bang prior to the vehicle lifting up on its left side before exiting the road and hitting a tree resulting in the instantaneous death of the driver who was alone in the car. Although the driver had attended a bourbon festival prior to the incident, witnesses who participated with him and others who spoke with him after the fest and prior to the accident indicated no impairment in speech or movement. Component part photographs and accident report are included with this report. You will notice there is no apparent damage to the tire. A claim was regisered with state farm automobile insurance company, which is attached, as well.

50 Wheel problem of the 2021 Tesla Model 3

Failure Date: 02/01/2022

The tire fell off the car while driving. The lateral link bolts have come unfastened and cause the link to come out of place and consequently damage several suspension components. As the bolts unfastened, the wheel was separated from the car and fell off, causing the car to be undrivable.

52 Wheel problem of the 2021 Tesla Model 3

Failure Date: 10/24/2021

I want to report an issue I have with Tesla concerning the damage that occurred to my vehicle as a result of a failure of Tesla fsd version 10. 3 firmware release 2021. 36. 5. 2 recalled by Tesla on 10/29/21 NHTSA recall number 21v-846. At noon on 10-24-21, I was testing the latest release of fsd as part of the beta program. My Tesla was in self-driving mode on a city street at 35 mph. This was the same route it had already successfully done earlier in the day, when suddenly the car jumped over the curb, causing damage to the bumper, to the wheel and a flat tire. This appeared to have been caused by a discolored patch in the road that gave the fsd a false perception of an obstacle which it tried to avoid. Immediately after the accident, the car was towed to the service center. The repair was finished on 10/27/21 at which time I was asked to pay $2,332. 37 to take possession of the car. I explained that the damage was caused by the recalled version of fsd NHTSA recall number 21v-846 and that Tesla should be liable for the damages. I was advised that the engineering team would conduct an investigation and based on the results the money would be reimbursed. However, I did not agree to pay hoping that after the investigation Tesla would honor the recall. After two months of waiting, there is still no communication on the results of the investigation even though the case is quite simple. The car was going straight at a very low speed in self-driving mode. There is nothing except the recalled version of fsd that could have changed its trajectory. Therefore, it is obvious that nobody looked at this issue and there is no commitment to doing so anytime soon. I communicated using the Tesla app chat that I am not willing to wait indefinitely and gave notice that I intend to file a petition with the court. I gave e-mail notice to Tesla’s resolution team as well to make sure that my notice reaches Tesla’s legal department. I have paid the invoice on 12/22/21 to get my car back.

55 Wheel problem of the 2018 Tesla Model 3

Failure Date: 10/12/2020

We have a 2018 Tesla Model 3, purchased new in late July 2018, which currently has about 32,000 miles. We have now had 8 flat tires in the span of 26 months or on average one every 4,000 miles, all by puncture. We live and drive in the san francisco bay area. Our other vehicles have gone 10+ years without a flat tire so the Tesla flat tires are not due to the area we live in. . On our Tesla we have had to replace 7 tires, 5 with assistance from michelin at a total out of pocket cost of about $2,000. Plus much time, pain and suffering. The flat tire issue is an obvious design flaw as Tesla specs and delivers the car with michelin 235/45r18xl 98w mic primacy tires that are prone to puncture due to the weight of the vehicle, the low profile soft rubber tire and the car does not come with a spare tire. We have talked at length with Tesla service and they admit that the car is a 'flat tire magnet' as it is one of the heaviest vehicles on the road, the low profile performance tires are puncture prone and flat tires are the most common repair. We have been stranded several times due to getting a flat in an area with poor cell phone coverage, which means you are basically hitch hiking. On 2 occasions we were on a freeway in the fast or second lane when the puncture occurred and the vehicle automatically slowed down with alarms screeching forcing drivers behind us to pass on the right precluding us from pulling over for a long period of time, which destroyed the flat tire, requiring replacement. This was an extremely dangerous situation and left my wife and I terrified. We no longer trust the vehicle for trips outside of our immediate area.

56 Wheel problem of the 2018 Tesla Model 3

Failure Date: 10/12/2020

Third party product called an alloy gator (https://alloygator. Com/us/) which is sold by alloy gator north America (us office 9035 dearborn Ave, blaine, wa 90238) tel 1=855=534-2867 partially detached from my wheel while I was on the highway, began flying around, and smacked into my passenger side rear door, rear quarter panel and rear bumper. It then snapped off and shot away from the vehicle like a high speed projectile. The product contains no warnings about the risk of this happening, seems to be defectively designed, and it could very well have hurt a person or property when it snapped off at high speed. The product is unsafe and should not be permitted to be sold in the u. S. Attaching pictures of the damage caused as well as a photo of the remaining part of the alloy gator as it appeared on my wheel after the damage was done.

57 Wheel problem of the 2019 Tesla Model 3

Failure Date: 06/26/2020

I have a 2019 Tesla Model 3 with premium wheel upgrade and the continental pro contact RX tires put on by Tesla are clearly defective. The car has 7k some miles on it and the tire blew out randomly. There was no impact/crash of any kind, we were going approx 30 miles per hour on a paved road and heard a large boom. Then almost instantly our tpms indicated extremely low tire pressure pull over now. We had a blow out with these basically brand new tires, no other damage to car so that's it. Could kill someone in the wrong circumstance!.

58 Wheel problem of the 2020 Tesla Model 3

Failure Date: 06/26/2020

Friday June 26th, 6:00 p. M. Public roads. Sunny, dry. Something snapped while driving 5 mph at the moment I started a slow left turn. The car lurched to a halt. The right front wheel was angled left at about a 35 degrees, while the left wheel pointed straight. Tesla sent a flat bed. The truck technician reported by phone that the wheel was separated from the suspension and a Tesla bolt was available for him to temporarily hold alignment for safe loading. I understand he left a note and the bolt inside the car. I believe the bolt was part of the broken suspension. The car had 3,300 miles. During my ownership, I noticed a light clicking (or clunking) when the car passed over a 3 inch change in road elevation, such as occurs during repaving when striking a man hole cover or new layer of asphalt. I took this to be a peculiarity of the Tesla suspension combined with the 18 inch wheels. The sound was rare, as most roads are quite smooth and I made it a point to drive slowly over sudden bumps. More recently, as our town is repaving, I heard the click more frequently. On father's day, while a friend was test driving the car, he being a car geek, heard the click, called it a "clunk," and told me to get it looked at. I had not yet made the appointment when something snapped on June 26th. Over the weekend I left voice mails and sent to Tesla service an email stating Tesla should consider giving me a replacement vehicle, but I wished first to hear their report on what had occurred. Tesla service in milford is not open on weekends. Tesla called on Monday afternoon seeking my input on what had occurred. Later that day they reported it appeared to them that a critical bolt had not been welded onto the sub-frame. Tesla service in milford ordered a new sub-frame, part number 1044521-00-r, from California. Tesla is researching what occurred.

59 Wheel problem of the 2019 Tesla Model 3

This incident happened because of sudden acceleration issue in Tesla mode 3 in our parking lot. I was parking between 2 cars on my right. So, was really slow. While turning there was a sudden acceleration and the vehicle was at a high speed. Since, I was turning, the car rotated at the high speed and in the process my car hit car on my left, 3 cars on my front. There was severe damage to my car and a flat tire. 61 Wheel problem of the 2018 Tesla Model 3

Failure Date: 12/28/2019

Driving on highway 10 lake city to jacksonville FL, suddenly heavy rain caused car to skid driving in autopilot mode at 70mph , the road changed from asphalt to concrete and car could not handle that change , car went into hydroplan/skid mode with 15 snake lapses before I could bring it to rest on shoulder , we were lucky to have survived as a semi truck was just 20 seconds behind us. The car even after halting accerelated on its own causing another small burst of skidding. . To add to the woes - the auomated wipers on this car are too slow , sudden rain pour on windshield cause lack of visibility and to turn the wipers to full mode , it takes 3-4 swipes on cars computer , which during crucial moments is a very bad design and can cause unitended accidents.

63 Wheel problem of the 2018 Tesla Model 3

Failure Date: 09/24/2019

Hello- I was trying to park my vehicle in a parking spot in santa clara, California when the car automatically accelerated and hit another car. It then immediately had a flat tire. I did not hit the wrong pedal nor was I accelerating it. It was quite dangerous. It came to a halt after hitting another car. Both the cars were damaged. Prior to that we had difficulty trying to connect to the car using the app on the phone but thought it could have been due to an app error. Not sure if it was correlated. This event happened on Sep 24. Tesla advised me to tow the car to the service center. Since then my car has been with Tesla service. They have not given us any updates about the diagnostics and keep telling us to take the car back. They have cleared to drive the car for 20 miles. My car was extensively damaged. I request you to help. This is quite serious for the safety of everyone. While I am willing to accept if Tesla proves beyond a doubt that it was my error, I am quite confident that it did it automatically. I am also concerned that Tesla has not given us any update for the last 3 weeks and have not provided us with any details of what the course of action is beyond asking us to take the car back and clearing it to drive for 20 miles. I have 2 kids and want a thorough investigation for the safety of everyone involved. Please help.

65 Wheel problem of the 2018 Tesla Model 3

Failure Date: 05/07/2019

The contact owns a 2018 Tesla Model 3. While driving 25 mph in the rain, the contact heard an abnormal noise. The contact pulled over and noticed that the rear bumper and rear wheel hubcap on the passenger side had fallen off. The contact called the manufacturer who stated that the failure was due to an act of god and they would charge for towing the vehicle to a certified collision center. The contact called her insurance company and the vehicle had been at the certified collision center for over a week. The vehicle was not repaired. The manufacturer reiterated that an outside influence caused the parts to fall off and they would not pay for the repairs. The contact wanted the manufacturer to cover the costs with the initial warranty since the vehicle was just purchased in July of 2018. The contact suspected that the parts being used were of inferior quality. The dealer was not contacted. The failure mileage was unknown.

67 Wheel problem of the 2018 Tesla Model 3

Failure Date: 02/01/2019

I hit a medium sized pothole on a freeway in 2019 and cracked 2 rims. After replacing the rims and tires, I still had vibration. Now, over about 100-1000 miles, my alignment goes out and my suspension and steering begins to loosen (or feel that way), leading to steering instability, bounciness and vibration through the steering wheel. There are no warnings to identify this, but the car's own "applied offset diagnosis" will show this problem. I've been to Tesla over 10 times to attempt to repair this and they say the car is working as designed after redoing the alignment and torqueing suspension. On about 3-4 of these visits, Tesla has reproduced the problem on a test drive. In 8 of 10 of the visits, the alignment printout afterward shows the front left toe is out. Because they say this is how it is designed, I believe this is a design issue that could put myself and others at risk. I've replaced so many parts, I can only assume through process of elimination that it's the steering rack or something in the subframe being loose that's causing this. Maybe all the bolts in Teslas just get loose too easily. Overall my issue is that Tesla is unwilling to correctly diagnose or repair the problem, not necessarily that this happened in the first place. There could be thousands of other customers driving vehicles and Tesla deems as safe, but really are not.
