This issue involves critical safety systems, including the rearview camera, front camera, and autopilot/driver assistance features. The vehicle was taken to Tesla for a recall related to the mcu system. Tesla marked the recall as completed, but no effective repair was performed. The rearview camera remains unavailable, the front camera system is not functioning properly, and driver assistance features continue to be disabled. Tesla returned the vehicle without resolving the safety issue. This condition poses a serious safety risk, as essential visibility and safety systems are not operational despite a completed recall.

No issues with summons for over 4 years $2200 damage to adjacent vehicle but Tesla no damage left front wheel caused damage on start.

I used summon on [xxx] to bring my car to me because it was parked way at the end of the parking lot at discount tire. There was only 1 car in the whole lot, which was parked next to my Tesla. While using the “come to me” feature in summon – my Tesla pulled forward and then turned and drove right in to the car parked next to mine. It did quite a bit of damage to my Tesla and the mercedes parked next to me it ran into. When I called my local Tesla service center in austin to ask what I am supposed to do, they told me they needed to run diagnostics to see if they can fix the “summon” issue. After running diagnostics they blamed the issue on the wrap I have on my car. They did not test the sensors obviously though, because they work fine through the wrap. Not to mention that Tesla offers wraps from the factory now. They also stated that summon was aborted due to collision, so it did report their was a wreck. Repair notes: the proximity sensors on the vehicle are being obstructed by the vehicle wrap, which results in interference or compromised sensor functionality. This interference can disrupt the accurate detection of surrounding objects, impacting the vehicle’s ability to engage safety features like autopark. Data analysis reveals that the autopark function was aborted during collision, due to the seat belt not being fastened at the time of the incident. The infotainment unit in the car that controls functionality, I. E. Navigation used to monitor battery availability is malfunctioning because of glue seeping from the unit and causing the screen to go black intermittently while driving creating a safety hazard for the driver and others due to a distraction. When this happens the driver loses visibility to navigation and other controls central to the vehicle functionality. Tesla is aware of this manufacturing defect that is a safety concern because it requires the driver to not focus on the road.

Since the installation of software version 2024. 27. 25, which includes fsd version 12. 5. 4, on sept 26, 2024, any driving which occurs on fsd results in emergency braking events for green lights, unprotected/unsignaled crosswalks without pedestrians present, parallel parked cars not encroaching on the lane of travel, vehicles traveling the same direction and similar speed (+/- 5mph), not encroaching on my lane. These frequently reoccurring events were not remedied by any of the subsequent updates received: v2024. 32. 10 (fsd v12. 5. 4. 1) on oct 8, 2024 v2024. 33. 5 (fsd v12. 5. 4. 2) on Nov 2, 2024 2024. 44. 25. 2 (fsd v12. 5. 4. 2) on Dec 10, 2024 while not all events occurred with surrounding traffic, those that did posed a hazard to not just occupants of my vehicle, but those in and around other vehicles, including pedestrians. Most other drivers do not expect and are unprepared for a vehicle to apply panic-level emergency braking and come to full stop at green lights, before unprotected crosswalks with no pedestrians present, or along well-traveled streets without significant errant other drivers. No warning lights, chimes, or other indications ever appeared on any of the displays or instrument clusters within the vehicle during any of the events. The fsd traffic display did not indicate or highlight any features of the surrounding area that were causing it to slow or stop. The problem has been regularly experienced by many other drivers, as evidenced by the large number of similar reports across the various Tesla driver communities online. I and my vehicle are available for inspection and I'm additionally willing to take an inspector on a ride-along to demonstrate the issue.
The horn is in a difficult place to access compared to all other vehicles in the USA making it a safety concern and hazard will driving should an emergency arise and you can’t get to the horn quickly. This has happened to me driving the 2022 Tesla Model S and I could not get to the horn swerving violently to avoid a collision.

Vehicle suddenly stopped on highway luckily there were no drivers behind me. The vehicle all of a sudden started beeping even though there was a clear path and no obstructions. Then stopped. I was panicked and in awe at what was happening. I couldn’t get the vehicle to drive I was just stuck for 10-15 seconds. I was finally able to get the car moving again but the problem hasn’t happened since. Any information as to what may have gone wrong may help other Tesla owners who may or may have not experienced this… yet.

The mcu that controls the climate and advanced driver assistance systems stops working while driving. I sent a service request to Tesla and they replied that my older mcu1 is failing because of the new software updates. It isn't the emmc card hardware failure, but the symptoms and safety concerns are the same. None of the controls work, as the main screen goes blank and resets while driving. Tesla told me the fix for this problem is to upgrade to mcu2, which they fail to provide for free as a safety recall. There was already a recall for the mcu memory chip failing and causing the same safety concern. The car disables the advanced driver assistance systems when this problem occurs. It happens about once or twice a month for the last year.
The vehicle was involved in an accident due to a failure of the autopilot system. There was a recall letter sent out of 23v-838 a month prior, but changes either did not persist or was the primary cause of the accident.
I believe my software is hacked. Car was purchased 6 months prior to when the vehicle shows registered to me. The vehicle shows no location with Tesla and has no direct contact from the vehicle with Tesla. My display screen blacks out while I’m driving on the freeway or streets. Leaves me feeling unsafe in bad areas I am driving to pick up drop off kids from college, school, or film school projects . Some of the safety hazard warnings do not work at times. Does not give me correct supercharger locations which leaves me in scary situations. Traveling on 405 freeway June 19,2023 my car excelled to 91mph without warning and didn’t use any sort of automated system as I do not have that capability. Tried to step on brake but did not work and I changed lanes which suddenly caused the car to slowly bring the car to driving mode again. Very scary. The car speaker had a male voice and all the car occupants pulled out their phones to see if it was coming from their phones. But it definitely came from the vehicle speaker system. My iphone was hacked with nso israel' company pegasus spyware and now my car is being hacked. I never connect my device to my car. Although my kids do periodically. Dear NHTSA, I am writing to file a complaint regarding a sua incident that occurred on October 20, 2023, involving my 2016 Tesla Model S. On the day of the incident, I pulled into my garage and began to park my car. When I pressed the brake pedal, the car suddenly accelerated forward, crashing into the garage wall and totaling the vehicle. The garage wall was severely damaged and will need to be rebuilt. This is not the first time that my Tesla has experienced sua. In 2017, I filed a complaint with NHTSA regarding a similar incident. In my previous complaint (issue # 11000097 dated June 19, 2017), I described a similar incident that occurred in December 2016. At the time, Tesla denied that the incident was caused by sua and no further action was taken. I am convinced that the sua incident on October 25, 2023, was caused by a defect in my Tesla Model S. I am also concerned that other Tesla owners may be at risk of similar incidents. I urge NHTSA to investigate this matter and take appropriate action to protect consumers. I believe that Tesla should be held liable for the damages to my vehicle and my garage wall, and that the company should be required to recall all Tesla Model S vehicles that are at risk of sua. "Tesla vision" is the adas sensor system in the car which relies exclusively on video cameras with no lidar or ultrasonic sensors to measure distance. It is highly inaccurate and prone to failure. It consistently reports false warnings and fails to identify actual threats.
There was a trash can on the edge of the road and my vehicle was in autopilot/fsd mode and for some reason it swerved over too close to the edge of the road. I intervened in time to avoid hitting the trash can head-on but it still struck the passenger side mirror and scraped across the passenger rear fender. The mirror got slammed into the passenger side door causing damage to the door and trim at the bottom of the passenger side window.

Vehicle is typically operated in fsd beta mode. I have had approximately a thousand driver initiated disengagements due to unsafe or potentially unsafe operation in beta mode. At times there have been indirect indications of unintended vehicle operation potentially due to anomalies with system hardware and communications between modules and control systems. The refreshed s and y models feature a vehicle display behind the yoke steering wheel. When at stop signs, stop lights, or when turning, the yoke (in an upright or near upright position) blocks safety related displays indicating intended vehicle path, bollards and critical safety-related driver annotations. Visibility of these safety critical displays is further degraded when stopped making unprotected turns with the yoke erratically moving. The ability of the driver to safely takeover control of the vehicle is further degraded by the chaos associated with the yoke whipping back and forth on unprotected turns, and drivers and passengers being unable to see critical displays. When the vehicle, driver and passengers all have a reduced capability to ensure safe operation the chaos further degrades safe vehicle operation. At a minimum an over the air recall is required to add safety critical displays/annotations to the large rectangular display between the driver and front passenger similar to y and 3 models. As noted above I also have hardware related concerns on my vehicle which at times appear to be masked/accommodated by the firmware but are more evident on secondary systems independent of safe vehicle operations.
Door handle not working and not popping out as expected and driver can’t go inside the car. A few complaints already from the internet and a lawsuit already filed by other owner and this seems to be recall already but nothing was mentioned yet camera get blinded or get block easily with rain or sun. This is an issue if you’re using the autopilot.
I have had multiple phantom forward collision warnings while driving and phantom braking while in autopilot. It happened twice today on a short 10 mile drive.
The vehicle still exhibits random forward collision alerts. I can be multiple car lengths away from the car in front traveling at no more than 40 mph, and it will still trigger. Tesla is using these to determine a safety score, which it then uses to determine monthly auto insurance rates. There is no way to request review of a potentially false forward collision alert. Worse, on the freeway, the autopilot still randomly emergency brakes. This is a well known reported issue for all Tesla vehicles. Over-the-air updates have not fixed this issue. Lastly, the passenger side mirror keeps getting stuck in rear view mode (faces down) when shifted to drive gear. I've brought this up with the service center, and they claim nothing to fix. This has been ongoing since I purchased the vehicle. I have had near misses when attempting to shift to the right and cannot see vehicles on the right lane.
When driving with autopilot enabled, I am experiencing the same issues as many others. Without any warning or reason, the car will sometimes slam on the brakes which puts the occupants in my vehicle and the cars behind me in danger. I have had to punch the throttle quickly to correct the situation but often the car has slowed 20 mph by that point. My estimate is that it happens ~5 times per hundred miles travelled on freeways.
While using wildly falsely marketed $10k "full self driving" add-on across multiple software revisions and after reporting each and every bug to company I have experienced potentially fatal phantom breaking while at highway speeds and could have been killed by rear ending on at least 4 occasions. Have experienced 20+ erroneous forward collision errors with automatic breaking or lane assist corrections for obstacles such as hills, mailboxes, trees, absolutely nothing at all, etc. When driving around mildly banking left turn corners with 45mph speed limits whenever approaching a left turn lane within median the car will violently lurch into the turn lane causing panic for passengers and nearly causing a collision within resuming concrete median. While using autopilot the car failed to stop, correct or warn when a vehicle entered my lane and caused an accident with several thousand in damages. Car does not properly break for turns on highways requesting driver take over for what should be an easily navigable turn. Doesn't take apex or break early and will drift lanes making other surrounding drivers nervous. Moving the horn from muscle memory location to awkwardly placed touch button has prevented timely use of horn on multiple occasions which could easily result in fatal accidents. Overall this car is a literal death trap not at all even remotely ready for production use on public roads and neither I nor any of my passengers feel safe using the feature that was over priced, over promised and under-delivered. Tesla service center employees too worried about retaliation from management to inquirer about problems because of previous reparations from corporate offices. List of problems is shockingly egregious and should not be legally permitted on roads.
Beta autopilot was engaged, driver had hand on steering wheel, vehicle was traveling less than 35mph (speed limit) and coming around a corner when it misjudged the road curve. The road curved to the left and as the car took the turn it took too wide of a turn and veered slightly off the road. Unfort the front right side of car went up and over the begining of a rock incline. The front right tire blew out and only the side airbags deployed (both sides). The car traveled about 500 yards along the road and then turned itself off. I could not get the horn to work, nor put it in neutral. There were no warning signs before incident. In fact the I belong to a beta driving scoring rating and for that day Tesla rated me a 91/100. I called Tesla roadside assistance and they said they would not provide any help as it was viewed as an accident and not a self-driving issue. It was reported to Tesla and I have called several times to try and understand why the car didn't stay in the lane and why only the side airbags deployed. I reported this to Tesla customer support and was now told they would not provide any information based upon their analysis, although initially I was told that it would take 5 weeks for engineering to review and get back to me. Vehicle is at repair facility and was told damage to car will be almost $28-30k. Car previously had issues with forward collision warning system going off, although I was not aware there was a recall regarding this exact problem, back in October 2021.
No early alert before slamming on the brakes, only tapping a rear bumper on a 1999 Toyota tundra. No damage to the Toyota but to my surprise, the insurance company said the Tesla's totaled. The airbags for the driver were deployed only, with no other passengers in the car. Quite a loud noise. I have an edr report, not sure how to interpret it. Did not hear any warning lamps. Fortunately, both the other driver and I suffered no injuries. I would like to find out exactly what happened.
When using auto pilot on the freeway my vehicle sometimes comes to a halting stop for no reason putting myself and passengers in great danger.
My Tesla Model S has the v1 hardware, also known as mcuv1, with the daughterboard emmc replacement from the previous recall running on firmware 2020. 48. 37. 1. The sound for the entire car will cut out. It might come back, but even a reboot of the car does not solve anything. These sounds may include blinker noise, speed limit chimes, lane departure warning, autopilot engage/dis-engage chimes, gear warnings, collision safety chimes, early braking warning, and a lot more safety-related sounds. This happens every time I drive the car, not just the reported date. I have brought it in for repairs numerous times, but this has never been addressed. I am only told to wait for a new firmware upgrade, which I have been for years now since I first reported the issue to Tesla. This only happens with mcuv1 cars and they have refused to provide a free hardware upgrade even after my incessant reports and appointments stating that it is unsafe.
Center console gets black screen occurrences and loss of audio has happened at least 15-20 times. Purchased car from Tesla in mid October. Screen issues since having the car and no audio, Tesla states to patiently await fe update have had the car serviced multiple times for the issue. Several appointments canceled by Tesla stating there is no fix and to perform reset on car which im now doing every 5-10 days. This has happened while vehicle was stationary in park and have had to drive multiple times on the freeway and surface roads with no screen or audio. Has occurred 11-17-20, 11-28-20, 12-8-20, 12-19-20, 1-2-21, 1-8,21,. When black screen occurs no warning chimes or audio present. Tesla refuses to address the he issue despite being aware of it and having been giving exact dates of occupancies pictures of black screen etc.
Repeated unexpected emergency braking incidents, similar to those described in the recent washington post article. The first one happened on the day that I bought the car, and has never improved. One scenario is the same as described in the article - oncoming car/truck on a two-lane road. But it also happens on freeways. If cruise control is on, it's likely that there will be at least one event during a 30 minute or longer session. As others have mentioned, these are often in congested traffic. When a car is following closely, they have to react very quickly. In several cases they have gotten angry because they think I'm slamming on the brakes to discourage them from tailgating. The very first incident, on the day I bought the car, was hair-raising. The car slammed on the brakes in the middle of an interchange where a semi was close behind and there was dense traffic all around both of us. Fortunately the driver was very alert and avoided me without endangering anybody else. But things could have gone the other way. I've mentioned it to local Tesla service people who (at the beginning) said that the car was in the learning period, calibrating the sensors. The user documentation mentions this and suggests that the training interval could be as much as 100 miles. It gets worse and better as Tesla fiddles with the software. It did indeed get quite a bit worse with the release of the full self driving beta -- which I have qualified for but never actually used because. . . Heck, if they can't get cruise control right, I sure don't want them to take on any more responsibilities.
The mcu (master control unit) is the 17" touch screen display that controls most of the functions of the vehicle. Vehicle has been plagued over it's live with freeze up's and reboots. April 2019, screen locked up and would not restart, which caused vehicle to loose propultion, power assisted brakes, power steering, window controls (had to use emergency manual release to exit vehicle, then go back in and squeeze between the seats to get my disabled son out of the back as the back door handles did not operate either and had to try and find the well hidden manual release hidden under the carpet with no markings to get the rear door open and him out of his car seat). Vehicle also would not go in to park, I had to use my shoes (with steel toe) under the wheels to keep the vehicle from rolling down an incline. This also prevented the hood from being opened to potentially pull the power to the vehicle to force a reset as the reset buttons did not work. Also trunk would not open and had to climb through the inside to get to the manual release for the trunk to get diapers for my disabled son while we waited 4 hours for a tow. Once doors were shut, no way to get back in. Head lights also would not turn off. Had it towed to Tesla service of highland park, illinois from milwaukee county, wisconsin. They said the headlights stayed on all night, and that the car "fixed" itself the overnight some how. No additional information would be provided. That was the worst failure, however, it still locks up and reboots at random times as it's in use. Either while driving (when that happens I loose turn signals and other safety features as well as ability to control vehicle settings) or while parked preventing it from being driven.
