On 8/16/2022 Tuesday, I went to work at about 10:24 and parked by the road next to mcdonalds to call my husband. I wanted him to take the car to park elsewhere as that spot had no parking starting at 10:30. My husband was not available so I was about to move the car and had my feet slightly touching the brake. At that point the car suddenly accelerated and jumped forward without any control and hit a car that was coming out of mcdonalds driveway. That car was about 15/20 feet away from my car and I hit on the driver’s side. My car was in such speed that it pushed the other car 6 feet on the other side which in turn hit a pedestrian crossing the road. I am 100% sure that I did not press the accelerator and the car moved by itself in such force that I had no chance of controlling it. I’ve asked Tesla to investigate as well.

The contact owns a 2022 Tesla Model X. The contact stated that after his wife had entered the parked vehicle and the driver door was still open, the vehicle experienced unintended acceleration, causing the vehicle to drive over a curb, into a ravine and then crash into a tree. No warning lights were illuminated. A police report was not filed due to the contact's wife not sustaining injuries. The vehicle was towed to a local dealer; however, the vehicle was not diagnosed or repaired and was then towed to an independent mechanic, where it remained awaiting diagnosis and repairs. The manufacturer was made aware of the failure. The approximate failure mileage was 200.

This issue most likely plagued our Model X during it's original factory warranty based on the class action, nthsa inquiry, and nthsa and Tesla recall for the same issue which affects Model X buil between 2016 and 201. These are the things we have personally experienced 12 times over our short ownership, 6 times these nearly caused accidents - this catastrophic malfunction has occurred (on average) once every 500 miles during our ownership: primary driving instrument and safety info screen (small screen/driver dash screen) and larger infotainment screen both: freeze, “lock up” or “go blank” turn black (an issue known by some drivers as “the black screen of death”) fail to turn on repeatedly reboot display a “touchscreen unresponsive” message stop working completely otherwise malfunction these issues generally occurred at the worst possible times, most likely when the deficiently designed computer was over taxed (night, weather, traffic, cold - though it also occurred twice under basic conditions). This happened with my new Tesla Model X plaid with installed self drive software. The autopilot or self drive mode was not enabled when the incident happened. Turning into a parking spot at a target superstore, (next to a damaged at the rear end car, parked to the left of the spot I was pulling into), the car went out of control with unintended acceleration as if the car's emergency evasion system got activated unintentionally. It went over the curb and made a turn on it's own on the grassed divider and came to a stop on it's own, while I was trying to apply breaks all along. It caused damage to other cars but fortunately no people were harmed. This incident happened end of July of 2022 and I have since repeatedly tried to get answers from Tesla. There have been multiple other incidents of unintended acceleration reported by other drivers of Tesla with two incidents resulting in loss of lives. My car finally got repaired and has been released to me saying they have tested and found it safe to drive although I was told with regards to my incident "the engineering case is still open with no resolution at this point". I had requested a master technician to check my car but that has not been arranged yet. I have repeatedly contacted the local Tesla service center manager who has not been able to get an answer from her team with regards to the cause of the problem but at the same time was told that the current issues do not warrant repurchase of the car like I had requested.
Recurring phantom braking every drive.
Travelling at highway speed with cruise control on, the brakes came on hard as if there was an upcoming collision, slowing the car by 25-30 mph. But there was no car or obstacle ahead of mine. I was able to immediately accelerate back to speed without further incident. And fortunately, the cars behind me were able to slow down without hitting me. This is the third (3rd) time this has happened. The previous was less than two weeks ago on city streets without cruise control and I don't remember the first which happened shortly after getting the car in March 2022. I noticed that others have had a similar experience, which led me to submit this report.
While on cruise control (autopilot) on the highway, the vehicle occasionally violently slows down. The majority of the time, this happens near or just before an exit/entrance ramp and if the car is in the rightmost lane. It happens regardless of traffic conditions and even if there are zero cars around, it will still "hiccup" and hit the brakes. I've noticed that the cruise control system sometimes automatically changes the maximum speed for the cruise control to 35-45 mph which explains the sudden braking. It is semi-replicable by setting the cruise control to a comfortable highway speed (say 65 mph) while the car is in the rightmost lane, then driving past a (normally) heavily trafficked entry/exit merging area. If requested, I can probably find a safe time and area to replicate this and send in my dashcam footage of the incident.

The vehicle brakes are aggressively applied at random times when traveling on the freeway. This only appears to happen when the vehicles tacc (traffic aware cruise control) is enabled. Thankfully I have not been rear ended yet. My family's safety is in danger when tacc is enabled. Not only am I concerned we could be rear ended, but I am concerned someone could think I am "brake checking" them and then proceed to act violently (road rage). There is a particular stretch of highway where this happens 100% of the time that would allow me to reproduce the occurrence on demand for investigative purposes.
While driving at speed approximately 4:20pm est April 9th, 2022 on i95 near the georgia/south carolina border the onboard computer froze and became unresponsive, including the speedometer, the radio, navigation system, etc. . I became aware of the issue because the car started to slowly slow down but the display continued to show 70 mph. I forced the computer out of autopilot but the car continued to show it was in autopilot. Shortly after the display behind the steering wheel went blank then rebooted, followed by the center console. Thankfully I was paying attention and did not panic, had I panicked I could very easily been in an accident as I did not know what was going on with the car. I reached out to Tesla multiple times following the incident and the agents and agents were non-caring and treated it as no big deal, and acted as if I should not be concerned about it. I could not even get assistance from them on getting service for the car. They kept pointing me to the mobile app on the phone which would not function properly and after typing the incident into as I wrote above 3 times I gave up.
Vehicle repeatedly applied the brakes while on cruise control. This sometimes seemed to be a response to other vehicles approaching in the oncoming traffic lane, but sometimes seemed totally unexpected. Once, the vehicle even sounded the collision alarm when no collision danger was present. If someone had been following me closely, the inexplicable braking could have caused an accident. A search of Tesla forums shows this is a common problem with these vehicles.
The car started giving error messages of systems failing one after another on the highway. It slowed down instantly as if someone pressed the brakes and I could not accelerate. Obviously I did not press the brakes. There were a lot of vehicles behind me and this could have easily caused an accident putting a lot of people at risk. I had to get off the highway ASAP and get the car towed. The error messages included charge port disabled, automatic emergency braking is disabled, power reduced, traction control disabled, stability control disabled, cruise control disabled, power reduced. Tesla diagnosed and confirmed this being an issue with the onboard charger being faulty and replaced that with charge port itself for a total of $3727. 60. They refused to cover any part of this which I think should have been taken care of by the company.

1) cruise control randomly resets itself to different speeds. Happens several times daily. I can set it to 55 and 5 seconds later it will reset itself to 80 and accelerate suddenly, or vice-versa. It can change to any speed-either higher or lower than what I've set. Happened yesterday afternoon (3-17). First noted it being a problem 2-28 at about 6 pm mst while driving on 4 lane highway in colorado, with multiple episodes of randomly resetting speeds itself. Given the rapid acceleration/deceleration of a Tesla, when the cruise control resets itself, the acceleration/braking can be abrupt and significant. 2)ghost braking has been and remains a problem with this car. It was serviced by Tesla 2-18 and a "chip" was replaced per Tesla. I reported about a dozen incidents of ghost braking to Tesla shortly after picking it up but don't have exact times/days. There are 3 episodes of ghost braking that were truly dangerous. I was driving on the interstate at 70-75 mph and the car slammed on the brakes for no reason, and it seemed it would have stopped completely in the middle of the interstate if I hadn't accelerated manually. Speed went from 75 to 50 within 1-2 seconds before I was able to react and accelerate. Twice this happened at about 1-2 am on a completely empty interstate in nebraska with no one around me at all, but the 3rd time was on a busy highway (I-55) near chicago. I was nearly rear-ended by another car when this braking occurred. He was able to swerve around me, but was unhappy and made his displeasure known. Even if it didn't directly cause an accident it certainly triggered road rage, which is probably more dangerous than actually having an accident these days. Have told Tesla about it before but they always tell me it's just "software" and will get better with updates I have notified Tesla of these incidents (with written record) but haven't heard back from them. I have a service appointment 3-21-22 to get them to try and address these issues.
The car, while "autopilot is engaged" will slam on the brakes on the highway for no reason. It is extremely scary and happens frequently.
At 1:10 pm on Wednesday, November 10, 2021 we were travelling sb on I-5 near longview, wa. The day was clear and dry. We were in lane 2 of a 3-lane stretch of interstate. There were no vehicles in either lane right next to us; a semi-truck was in lane 1 (outside lane) several car lengths in front of us; a passenger car was 1-2 car lengths behind us in the same lane as us (lane 2). No overpasses, trees, or any other discernable distractions were present. We were travelling at 70 mph on cruise control. With no warning, the car braked suddenly in an effort to bring the car to a full stop. Fortunately, the driver directly behind us was paying attention and was able to swerve into lane 3 to avoid rear-ending us. I was able to bring my car into the Tesla location in lynnwood, wa for service on November 12, 2021 to have the issue checked. While in for service, I did some research on the issue and found blogs dating to 2018 about "phantom braking"; I determined this described our situation. When I collected the car, the service tech said they were unable to replicate the issue and therefore, were not able to establish a problem to correct. I inquired if they knew about a phantom braking issue; they indicated that telsa had heard of it and was working on a "fix" but there was no known timeline for the "fix". I have not used the cruise control feature in my car since 11/10/21 and will not because of the significant safety concern. Tesla need to be made to make this safety issue a priority to correct. If musk's team can build a rocket and send it to space, they certainly can correct a cruise control issue.
On 10-16-21 Saturday around 7:40 pm, I was going to my brother-in-law's house which is on the left side of the road I was driving. It is a single lane road in a busy residential area. I wanted to park outside of the house by the road. I took a left turn towards the gate, then slight reverse and put in drive to pull to park parallel to the road. My Tesla model-x went into 'sudden unintended acceleration' mode, went left to right to left in a violent way, picking up speed like a rocket. I applied the breaks repeatedly, but the car did not stop and continued to race on the road for about 0. 3 mile where the road takes slight left turn. It went and crashed into a 40 ft tall big pine tree at probably more than 150 mph. It knocked off part of the compound wall of the owner, and passenger front tire blew away and landed in their front yard. I had the seat belt on. No other vehicle involved. I was the only occupant in the vehicle. No fire. I called 911 from my cell phone. I opened the driver side door manually and got out. Police took the report from me and took pictures. The report will be in another 3 days. Ambulance took me to hospital. Broke rt ulna, lumbar 2 vertebra, pneumothorax lt lung with collapse of left lung, s/p chest tube. Also cervical muscle strain/ rupture. Discharge after 3 days. Still lot of back pain. Unable to go to work. Car is total loss. I have Tesla insurance. Reported the accident to insurance. Adjuster still working on the claim. Requested to 'preserve the car' for further investigation. 7-14-when I stop at a light, the front end of car goes up by about 4 inches. Took to Tesla service center and I was told it is adaptive suspension and it is normal. 8-10-21. When I apply break to stop at a red light, and when I turn on left turn signal, power shuts off completely, everything blank in the middle of busy traffic, had to reset power. Very scary. Happened twice. Complained. Tesla remotely updated sd card. Happened again on 10-15-21.

The mcu (media control unit) and the driver's dash goes out while driving under normal driving conditions. When the mcu and dash goes dark/dies, the turn signals don't work, the rear view camera goes out, the blind spot warning and lane departure warning is disabled and the speedometer doesn't display. Basically I was driving in the dark. It was a very dangerous situation. Fortunately I was driving and not my wife who is the primary driver. This is no longer a technical safety concern but rather a safety issue for us and for others. I brought it in for service 2 times and the outcome was: we're aware of the bug and working on an over the air fix but no eta. This is not an isolated issue since it occurred many times captured below: Jun 22 - 8:33 pm pst Jun 30 - 8:32 am pst Jul 2 - 12:32 pm pst Jul 8 - 5:58 pm pst Jul 13 - 8:09 am pst Jul 16 - 6:46 pm pst this is similar to the situation that occurred in the collapse of the condo in florida. In that case: 1. They were aware of the issue 2. They knew they had to fix it 3. But they didn't take responsibility nor accountability. They should have evacuated the building instead of killing 100+ innocent lives. In our case: 1. Tesla knows there's an issue 2. Tesla knows it needs to be fix 3. Tesla refuses to take immediate action until something drastic occurs. The car is practically brand new with only 5k miles on it. I have videos to capture this frightening ordeal.
Since delivery in December 2020 the computer mcu has shut down for two minutes while driving over a dozen times. The computer reboots after a couple of minutes but during this time it disables all instrumentation, turn signals and other driver information. I brought it in to the Tesla service center in highland park, illinois with a list of dates and times that it shut down and they told me it was a known firmware issue and that there would be a solution sometime in the future. I don't believe a 2021 Tesla that cost nearly $100 k would force me to stick my arms out the window to make turn signals.

The contact stated that the vehicle was in the parking lot parked for approximately 1 minute, the vehicle independently accelerated forward as the park assist unavailable warning light illuminated causing the vehicle to crash into a concrete wall where it came to a stop. No airbags deployed. A police report was filed. A police report was filed. There were no injuries sustained and no medical attention was sought. The vehicle was towed by the police department to a lot where the contact was awaiting the insurance company estimates for the damages. The dealer was not contacted. The manufacturer was notified of the failure and started an investigation. The contact mentioned NHTSA action number: dp20001 (power train, electrical system, vehicle speed control, electronic stability control) as a possible solution to the failure however the VIN was not included. The failure mileage was approximately 12,548.
Repeated violations of autopilot safety systems has forced Tesla to activate in cabin cameras to be used as an additional safety device for driver monitoring. Not all vehicles have in cabin cameras and should be installed for increased safety concerns.
The contact's wife owns a 2017 Tesla Model X. The contact stated while his wife was driving at slow speeds, the vehicle independently accelerated, without warning. The contact's wife forcefully applied pressure on the brake pedal to stop the vehicle. The vehicle resumed regular driving after the failure and proceeded to drive at slow speeds. As the contact's wife approached the driveway to park the vehicle in the garage, the same failure recurred without warning. The contact slammed on the brake pedal to stop the vehicle. The manufacturer was notified of the failure and stated that a diagnostic would be performed on the vehicle. The vehicle was not yet repaired. The failure mileage was approximately 13,000.
Around 1:50 p. M may 27, 2021, I left my house driving my Tesla 2020 Model X with my son in the front passenger seat to his school. We returned to our home around 2:19 p. M. At our front gate, I slowed down to a close stop as I turned the vehicle into my driveway. Once descending the driveway, my Tesla vehicle suddenly accelerated to an extremely high speed. While I attempted to brake; however, none of the speed controls seemed to work. The vehicle hit my 2009 Toyota camry parked on the left side of my driveway first, then took out the heavy duty wrought iron fence post and gate door located about 100 ft from my front gate. My Tesla vehicle kept moving at a very high speed and knocked down and damaged all trees, plants, and garden arbors along the vehicle’s path and, then, crashed through the rear wrought iron fences about 200 ft from my front gate. Finally, the vehicle stopped after crashing into two large trees and stop there. My son got injured and was taken to an urgent care facility after the crash. Two of my vehicles including 2020 Tesla Model X and 2009 camry le were totaled and a large portion of my backyard, fence and gate were destroyed due to this Tesla crash. This Tesla vehicle suddenly accelerated for no reason, indicating that there must have been something seriously wrong with this vehicle at that time. The resulting skid marks on my driveway discovered after the incident demonstrates that braking was applied even though the vehicle accelerated. I had reported this crash to Tesla on may 27 and followed up on may 29, June 1 and June 3 and have not received a response from Tesla at this time. The investigation shall be performed to find why this Tesla vehicle suddenly accelerated to prevent other Tesla vehicle drivers from similar dangerous incidents caused by the vehicle’s sudden accelerations. I have the videos and photos from the crash. Please let me know should you need this information for the investigation.
All screens in the car shut down while driving and there is no access to your speed, blinkers, control of lights, control of car safety features, wipers, etc. Tesla was notified multiple times and each time they tried to correct the issue, it was not resolved. It has been a year since the issue started and has happened over a dozen times, resulting in unsafe driving conditions. Tesla keeps stating they "remotely diagnose" the problem and told me to keep an eye on it and see if it happens again, although they told me less than 2 weeks prior to call immediately if it happened again. They have refused to fix the problem and it has become a safety issue with driving the vehicle.
Most times when I take my foot off the gas pedal, the car seems to accelerate on its own. However, when merging onto the highway, acceleration is reduced. As of the past couple of weeks, steering around turns has become a problem. The car operates like it's sliding on an ice patch.
I was driving my Model X (June 2020) home last evening on a road I normally drive. (6 lane road) I was approach a traffic light that turned yellow and I determined I didn't have enough time to stop - so I hit the accelerator to get through the light before it turned red and then everything on the car stopped working. No alarms, no sounds, no warnings, nothing - music even stopped playing. The only indication something was wrong was the dashboard flashing yellow light indicator that the car thought it was slipping (complete dry @ 5:50pm -- temperature was 45 degrees). The car would no longer accelerate (I hit the acceleration pedal and nothing happened), regenerative braking stopped working and the car was only coasting and quickly slowing down (it is a heavy car). I managed to get it into a nearby parking lot and called Tesla roadside. The car would not accelerate after rebooting 2+ times and when it finally managed to begin to move the car behaved like it thought it was "slipping". No alarms showed up until I was instructed to reboot by Tesla emergency roadside - at which point they recommended I bring the car to service.
Tesla issued a recall for the mcu after I had the mcu replaced because it was failing. The screens in my car would go blank and I would have no idea how fast I was going while driving. The a/c wound shut off in the middle of the drive. I didn't have turn signals. These were all the reasons why the NHTSA ask Tesla to issue a recall, and finally Tesla agreed to this recall. Tesla is however not reimbursing me for the replacement of the mcu. I have spent months trying to get Tesla to honor their recall. I would like to be reimbursed for this recalled mcu that I had to pay for myself as it was a major safety issue. I would appreciate you help.
On the beginning of October 11, my reverse camera stops functioning. At night, when I park my car in the garage, my front screen (behind steering wheel) was blank. Shortly after, my car suddenly self-accelerated while I was parked. This resulted in the car smashing through my garage wall, causing loads of damage in the room opposite of the wall. Some damage included: a broken wooden bar, shattered glasses & liquor bottles, and damages to the front of my Tesla. This car was newly purchased, with a milage of simply 40 miles.
I've created several bug reports for the autopilot system which is, a very real danger. The vehicle accelerates towards parked or stopped cars, slams on the brakes without explanation on the freeway, begins to change lanes on the freeway then violently swerves back into the original lane without explanation. I've taken the vehicle into 3 different service centers. The explanation is "software" and they recommend "updating the software" when new versions come out. The version of software released are not ready to be released to the general public. As a pilot, I understand how autopilot systems should behavior. The Tesla autopilot behaves so dangerously it doesn't appear the software engineers even bothered to test it before release.
Main computer system died. All screens stopped functioning (including speedometer). Car wouldnt lock/unlock. Tesla said the mcu had to be replaced and they were going to charge me because it was 5 days out of warranty.

The contact was able to depress the brakes pedal and stop the vehicle. The contact stated no warning light was illuminated. The contact stated she had experience the failure again on 12/14/2020, after parking the vehicle and while the vehicle was off when it accelerate independently and drove into a building. The airbags did not deploy. The contact did not sustain any injuries or seek medication attention. A police and fire department report was filed. The contact called local dealer Tesla (6701 amador plaza rd, dublin, CA 94568; (925) 361-1173) and was made aware of the failure. The vehicle was not diagnosed or repaired as of yet. A the manufacturer had been informed of failure. The failure mileage was approximately 200. The consumer stated the vehicle's lights independently turn on and of.
I was driving on freeway 99 near chowcilla, California in my Tesla Model X with under 7,000 miles on autopilot and there was no cars in any direction near me. This freeway was a straight line with no bends or curves. The car was traveling at 75 mph in the middle lane on autopilot and the car suddenly decelerate very aggressively for no reason. The tires screeched as the car's speed suddenly came to a near standstill at the speed of 10-15 mph. The driver ([xxx]) took manual control and drove the car without any potential fatalities. If there was a car or truck behind us, we would have died. This has happened on two previous occasions, but the deceleration was not as sudden as this life threatening occurrence. I was the fourth vehicle in line at a bank drive thru and when the vehicle in front of me moved up in line, I placed the vehicle in drive and suddenly the vehicle accelerated out of control and hit two vehicles and then went over a curb into a parking lot and crashed into two other vehicles, which resulted in a serious accident involving four other vehicles. Fortunately, no one was seriously hurt. My vehicle was a total loss.
My vehicle's center display (mcu) is starting to show signs of failure on account of a flawed design that depends on an emmc memory component that is known to fail catastrophically after several thousand write cycles, causing loss of critical adas and rear view camera functions, hvac controls, and other basic vehicle functions. Now, I am facing the prospect of having to pay Tesla thousands of dollars to replace the entire mcu in my just-out-of-warranty vehicle. Tesla has tacitly acknowledged the safety implications of their original design by modifying it to use a more robust memory component in 2018, and they should be made to recall pre-2018 models to ensure that nobody else has to endure what I and many hundreds of others have experienced.
At approximately 6:18 pm on 3/21/2020, I was driving my brand new 2020 Tesla Model X (7 miles starting odometer) home from taking delivery of it (!), and after 20 miles of mostly highway driving, I was nearing home and stopped at a traffic light (with several cars stopped ahead of me). After about 30 seconds of sitting dead-stopped, the car vigorously accelerated with no action on my part, and I had to turn the car hard to the right to avoid a full-on collision with the stopped car in front of me. Fortunately, to my right there was a curb and then an empty parking lot and so I was able to avoid any serious property damage or injury. Both of the passenger side tires on my vehicle were flattened, and there was some very minor damage to the front drivers side bumper, and to the rear passenger side fender of the vehicle in front of me. (I had left enough distance when stopping at the light in between my car and theirs and I also somehow reacted very quickly given how in shock I was. ) the car was not drive-able because of the flat tires, and so I contacted Tesla roadside assistance. They towed the vehicle back to the Tesla facility from where I had just originated. After the incident, I immediately contacted the sales person because -- you know -- I had literally just purchased the car. Someone else on the sales team answered the phone. My sales person called me back shortly thereafter, and I eventually spoke with the sales manager around 7:45pm. All four of those individuals (3 sales, 1 roadside assistance rep) said something to the effect "we've never heard of a vehicle doing this before. " thankfully I thought to google the issue, and an article in car & driver about your pending investigation popped up. So I feel like they were probably gaslighting me per some script from legal, which is a really great feeling after you spend $103,000 on a car.
An accident occurred in my Tesla Model X 90d that was due to the fault of the car. We have owned the car for 3+ years. What occurred today in the car is something that has never happened before. The car was functioning perfectly fine during my whole 20 minute commute. As I pulled into my office's parking lot, the car seemed fine. However, as I was finishing parking the car in the parking space, the car unexpectedly started accelerating at a frightening speed. More clearly put I was almost done parking and my foot was partially on and off the brake. While my foot was slightly off the brake the car accelerated so fast that it ran over a curb and climbed a grass slope and ran over and tore down a small tree. The car accelerated so quickly that it ran over the tree before I could even react. After this experience I no longer feel that it is safe to drive a Tesla. I'm just happy that there weren't any people or cars in front of me. This is a major safety issue and needs to be investigated.
