On [xxx], at approximately [xxx], I was driving westbound on the [xxx], just before the [xxx] and [xxx] off-ramp, with my wife and our [xxx] baby in the car. While in traffic, I noticed the regenerative braking system was not slowing the vehicle down adequately, despite having a clear and sufficient distance from the big rig ahead of us. As the gap closed, I slammed down on the brake pedal, but it felt abnormally stiff and did not respond with the urgency or force expected. The vehicle failed to stop in time, resulting in a rear-end collision with the big rig. Thankfully, we were not injured due to the speed of the accident. The breaks have not been inspected due to it being in the collision center there was an alarm but it sounded off too late and the car ended up colliding. Shortly after I purchased this car it began to put itself into park while the car was moving slowly and even when it was in car wash mode going through a brush carwash that requires cars to be in neutral. I reached out to Tesla support to investigate and they explained this is a safety feature that engages if the driver leaves the seat while the car is not in park. I explained to them that in both cases I had been sitting firmly in the seat and there should have been no reason for the car to put itself in park and asked them how I turn this auto feature off. They said there was no way to turn it off. I explained to them that all automation features should be optional, they argued with me. Came to find out some sensor in my driver's seat was malfunctioning. They took the car to replace the seat/sensor, but I reiterated that all these auto features should be optional and a consumer should be able to turn them off. My car was rendered unsafe to drive due to the malfunctioning sensor, I should have had a way to turn off the feature, I asked them to open a ticket with their engineers to add settings to turn off this automation feature. Fast forward to 6/25/25, as I am pulling my car into a parking space at a store the car slams itself in park, so hard that I thought maybe I had hit something hard with one of my wheels. I followed back up with Tesla support and am being told there is still no way for me to turn off this automation feature. No automation feature should ever be hard coded into a motor vehicle. The risk presented by malfunctioning automation features is too great, and if I want to completely control every aspect of my car I should be able to, regardless of how much automation it is capable of. I am filing a complaint now because Tesla is clearly not taking these concerns seriously.

Incident details: date & time: [xxx] driver: my spouse, purpose of trip: personal (heading to a nearby soccer park) location: [xxx] weather & road conditions: clear weather, dry roads, normal traffic conditions driver safety: the driver was wearing a seatbelt at the time of the incident incident description: my spouse was driving to the nearby soccer park with the vehicle in fsd mode. Upon approaching a railway crossing, the signal was green, indicating it was safe to proceed. However, instead of maintaining a controlled speed, the car suddenly accelerated while attempting to cross. Before my spouse could take control, the vehicle struck the boom barrier, which was either lowering or had already lowered at that moment. The incident happened so suddenly that she did not have enough time to react and override the system. Fortunately, no injuries occurred, but the vehicle sustained damage to the front bumper, windshield, and frame. The unexpected behavior of fsd at a railway crossing raises serious safety concerns regarding its decision-making. Safety concerns: I have been a long-time appreciative user of fsd, but this incident has shaken my confidence in the system. I now fear using fsd. Given the serious nature of this incident, I request an urgent review and response regarding my concerns, as well as coverage for the necessary repairs. Request for investigation & cost coverage: access to vehicle logs: I request access to the vehicle's telemetry and camera recordings to determine why this happened. Fsd system review: please investigate whether this was an fsd-related miscalculation and what corrective actions are necessary. Cost of repairs: as this incident was caused by an unexpected fsd action, I request Tesla to cover the repair costs for the vehicle and any associated damages. Tesla has told me the ap4 computer has failed in my car which has resulted in them having to replace it. I had this issue start on 1/12, and I scheduled the earliest appointment available for 1/30. They did not offer me a rental or any assistance during this time, and I need my vehicle to commute to work and conduct other essential activities. It has just over 1,000 miles on it and is ~3 months old now. This issue impacts: backup camera, emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, vehicles built in gps to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously, and I have received no reparations for it.

Park assist error message when put car in reverse. No visualizations on display. Early collision not working. Several times (not in this instance) backup camera has been black and does not show an image. Sentry system error message. Fsd offline. (may not be a safety issue but wanted to provide information as it would show other errors in the system affecting safety) I have a scheduled appointment with the service center in February.

I was driving at a speed of 60 miles per hour in rainy weather when the car’s emergency braking system suddenly activated. At the same time, the steering wheel locked due to the braking, and I completely lost control of the vehicle. The car spun multiple times before crashing into the center divider of the highway and finally coming to a stop. This was an extremely dangerous incident from a safety perspective. This was the first time such a braking malfunction occurred. The incident has not yet been investigated by the insurance company, and no warning or notification was displayed by the vehicle at any point.

After a software update pushed by Tesla, the ap4 computer in my 2024 Tesla Model Y shorted out, causing a complete failure of all safety systems controlled by this computer. The following safety-critical and driver-assist features have become non-functional: 1. Cruise control (adaptive or otherwise) 2. Forward collision warning 3. Lane departure warning 4. Lane keeping assistance 5. Blind spot warning 6. Parking collision warning 7. Full self-driving and auto park features 8. Cameras required for autopilot functionality 9. Auto headlight dim / auto wiper system 10. Back up camera rendered inoperable additionally, because the gps system has failed, the vehicle’s navigation system is inoperable. This impacts Tesla’s unique route planning feature that sets up supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. This system also initiates preconditioning of the battery (heats it up to be able to charge), so super charging is extremely slow without the gps system availability. Safety concerns: the failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic. Rear visibility in the car is very limited without the federally required backup camera. The autopilot computer failed. This has rendered all safety systems and camera on the vehicle inactive, including the rear view camera, all forward and side cameras so no active safety systems are functioning. This includes crash avoidance, lane keeping, blind spot warning, cruise control, and rear camera during reverse, the vehicle is nearly impossible to reverse as the view is terrible with no cameras.

Vehicle cameras stopped working. Navigation stopped working. Screen also stopped working for a time. Was able to get it back on but it went off multiple times since. If I can't see the screen, I can't tell how fast I'm going to do anything else. Cameras control many of not all of the safety systems. So those are all inoperable. Issue is being reported across multiple forums all over the internet as "short in ap4 module. ".

All cameras stopped working. With this, all safety features are not working. I can’t see to back up at night. No collision avoidance, no lane departure warning, high beams don’t stay on, turn signal doesn’t stay on. It has been two weeks and Tesla has no fix. There was no warning, just noticed it one day as I was driving it and was very dangerous in dark snowy conditions. I called Tesla service immediately and they gave me an appointment for following week. Then next day I got message from them that there are a large number of vehicles having the issue and they said they don’t have a fix for it and moved my appt out 7 weeks. I have gotten no help from them and I told them I don’t feel safe driving the car. They have not provided another car.

Tesla has told me the ap4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around thanksgiving, specifically [xxx]. I scheduled the soonest appointment available for 12/13 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 2500 miles on it and is ~2 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, vehicles built in gps to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. Additionally, the vehicle came to a stop on the highway when my wife and I were in the car on the night of [xxx], forcing us to perform an emergency pullover and abandon the car 22 miles north of rochester, mn overnight. My Tesla Model Y experienced a complete computer failure, rendering all active safety systems inoperable. This included backup cameras, speedometer display brightness, and collision avoidance system. After receiving assistance from Tesla service, the issue was diagnosed as a shorted computer. A computer replacement was performed, resolving the problem. I am submitting this case as a reference for potential investigations.

While driving between 50-55 mph the Tesla applied emergency breaking even though there was nothing in front of us and no risk of accident. The car went from 55 to about 5-10mph in the matter of a second. We are lucky no one was directly behind us or it would have led to a high speed rear end accident.

In a 90 mile drive I experience phantom breaking 4 different times. No car was in front of me, in fact nothing at all was in front of me when my car slammed on the brakes 4 separate times. One time there was a car somewhat close behind me. I also twice experienced audible warnings and flashing alarms to take control immediately of the car when my hands were on the wheel and I was already in control of the car, it was alarming and shocking to get these alarms while already in control and driving. I believe this car, as currently configured, can pose a danger to other drivers on the road.

This happen once before earlier this year on a two lane highway when a truck was coming toward me in the opposite lane. I was driving on a street at the speed limit there were no cars in front of me and behind me thankfully the car just braked very hard suddenly for no reason and I lost complete control over the vehicle after the car came to a complete stop I was able to continue driving. Thankfully there were no cars behind me because if there were I don’t even want to imagine the accident I could’ve caused aside from this the breaking was so hard that my whole body hurts and I am afraid to drive the vehicle as I do not know when to expect this breaking since it can happen so suddenly at any time.

I was driving and going about 50mph and there was no other car near me. Suddenly the car slammed on its brakes. Dropped to probably 10mph. My mom and I were both wearing seatbelts but our heads were thrown forward. My neck and shoulder are sore and stiff today now. There were dark skid marks in the road ahead of us and I believe the car saw those and broke because of them. If there had been a car behind us, we would have be rear ended. There were no warnings beforehand. When the car broke, lights flashed and there was also an alarm. The vehicle has not been inspected by police or manufacturer.

The incident occurred on July 14th, 2024 at 9:49 p. M pst in sammamish wa, where my car (2024 Tesla Model Y) incorrectly enabled automatic emergency braking , and unexpectedly applied brakes when I was traveling at 45 mph on an empty road. The sudden braking reduced the speed of my car to less than 15 mph in a span of 2 seconds and eventually brought the car to a halt . I strongly believe this was done in error due to a system malfunction, as there was no potential cause of a collision given there were no incoming vehicles or any vehicles ahead. There were also no pedestrians or any objects such as animals or wildlife on the road. One this incident occurred, an alert with code “app_w050” appeared on the car screen. (photo attached). Due to this abrupt unexpected braking, it caused a significant treat to my safety, as I could have potentially lost control of the car and slid off the road into a ditch or another vehicle traveling behind me could have potentially collided with me in the rear end.

During my trip, traveling at speeds ranging from ~30 to ~75mph my car suddenly and with no advance warning braked to a full stop, four separate times. I was not using cruise control or any driver assisted features. There were no obstacles in the road. I did not get in an accident because there were no other cars near me on the two different roads I traveled on.

This is my fifth Tesla, so I. Am very familiar with the self driving feature which is mostly good. However, recently I got a Model Y and with in 24 hours I was rear-ended on the freeway! I had the car in full self driving mode (fsd)! for some reason it just braked hard in the middle of my driving however, traffic was backing up. I injured my lower back and neck and I'm now having to go in for therapy and schedule a cat scan. Luckily the person behind me was fine. After the incident I downloaded the zip disk which records incidences. The car had documented pre and post accident! but the actual incident there is no record. . . Almost like it got wiped out!?!.

Unknown. Drove car in front of garage. Car came to a full stop. About to put car into reverse from drive in order to back into the garage, when car suddenly accelerated forward. Turned wheel sharply to the left in order avoid hitting neighbors fences. Vehicle traveled in an arch and collided with a tree next to an fence and stopped.
