I continue to get completely false forward collision warnings. Part of this is because my front camera housing keeps fogging up and over the past 529 weeks I’ve gotten a half dozen false fcws. When these faulty alerts occur, a loud audible beeping is heard and red alerts are displayed on the screen. These faulty alerts are dangerous distractions, causing panic, fear, and uncertainty in the driver, causing them to take their attention off of the road while they assess what just occurred. This is a critical flaw in the functionality of Tesla vehicles that the company has refused to address. This also makes me insurance rate spike way higher and I’m getting charged extra for this.

I initially visited the milford, CT Tesla service department and communicated through the Tesla ap on 12/14/2024 regarding my 2024 Tesla Model Y, which has only 2,357 miles on it. The car's cameras, sensors, & navigation stopped working, making driving unsafe. Now, the bright lights do not go on. The vehicle's camera/sensors are critical for safe operation. Tesla said the car may need a new computer, but they have none in stock. They texted me that they have many people facing the same issues.
Hello , I just bought the new Tesla Model Y 2024 in the October and all of a sudden all the driver assistance programs are not working, including the rear camera(reverse camera) I took it for the service on December 4th and they fixed issue on December 5th. After few days on December 13th the same issue happened again and Tesla service center is saying I need to wait till February 20th 2025 for the repair. All cameras are not working in the car, it does not show any backup camera, no forward collision warnings or no lane change assist Tesla is not considering this as a safety issue I am including a chat summary with the service center via pdf file. I have included previous repair invoice also when the same issue happened on 4th December.
Tesla has told me the ap4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around thanksgiving. I scheduled the soonest appointment available at 12/24 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 4000 miles on it and is ~2 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, vehicles built in gps to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously. . . Tesla has not seemed to publicly acknowledge the issue yet either.

Ap4 computer failure has led to navigation, cameras, blind spot monitoring, lane departure and other systems being inoperable. Tesla has stated they don’t have the parts to fix and have pushed out my service repair time twice already. Issue reported on December 4th, since then no time table for repair has been given. Tesla keeps stating the car is operable, leaving out the facts it’s dangerous to drive due to small rear windows and blind spots based on the cars design to work with operable cameras that give the drive clearer angles to view.
My new 2024 Model Y Tesla is having a computer failure - no navigation / gps (stuck at last address when issue occurred) and no cameras (including rear camera). With limited rear windshield visibility and no backup camera functionality, we've been put in dangerous situations. They keep pushing out the service dates waiting on new parts / computers. Around the end of November, my 2024 Tesla Model Y's cameras stopped functioning. The sensors for blind spots and possible collision prevention stopped working. The gps was also not working, the car believes it's been in the same spot for over a month. The lights of the car also do not work as promised, as now I have to manually turn them on on my dashboard and it goes on and off. I have been trying to get into Tesla to have this fixed but the appointment keeps getting pushed out due to the part that needs to be fixed not being in stock. I brought my car into Tesla for them to confirm that it is the part that is missing and that is the case but they couldn't fix it because they don't have the part. It's been over a month and my date was pushed out again, probably the sixth or seventh time. Tesla claims that there will be a software update that will fix this issue, however, my car is not updating and won't even accept a software update.
Back in late November 2024, Tesla pushed an over-the-air (ota) update to my 2024 Tesla Model Y. Immediately after pushing this ota, all of my car's cameras stopped working (including my rear or "backup" camera). This is in violation of federal law as all new vehicles are required to have a backup camera. Additional safety-critical items were also impacted: the rear-view and both of the side mirrors remain tinted to the darkest level, reducing my visibility. The side mirrors are also tilted in the "downward" position, making lane changes incredibly difficult. Since the front-facing and side-facing cameras do not work the vehicle's forward and side collision detection systems are inoperable. The vehicle suffers from additional failures (gps navigation, maps, autopilot, among many others). I have presented these issues to Tesla and they have continously pushed out my repair date. I have also asked for a loaner vehicle due to the failure of the car's safety systems (some of which are federally mandated!!). Tesla has ignored my pleas for a loaner vehicle and has also remained silent when I have repeatedly asked to return the vehicle under washington state lemon laws. My safety and the safety of others have been put at grave risk due to the backup camera and collision detection cameras not working. Tesla has not inspected the vehicle as they continuously push my service date further out. No obvious warning lamps, messages, or other symptoms appear. The user doesn't know these federally-mandated safety features are inoperable until they put the car in "reverse" or attempt a lane change.
None of the cameras work in the Tesla. Unable to use any safety features. No reverse camera. Any adaptive cruise control, safety features are all disabled. Two months to get it repaired and serviced.
My car slammed on the brakes on the highway this morning at 75 mph. A box truck was following me but thankfully he didn't hit me. I was in what Tesla calls traffic aware cruise control (tacc) at the time and quickly brought the car out of tacc and hit the accelerator to get me back up to speed so I didn't pose a danger to other drivers on the highway. I dont know exactly how much my car slowed, but it was enough to throw me forward in my seat. There were no vehicles in front of me at the time. This was a classic example of "phantom braking", which is talked about a lot in the Tesla community. This is my 2nd report of phantom braking to the NHTSA.
