Sun glare is a well-documented driving hazard, yet Tesla’s y model’s visors are too short to offer sufficient coverage. This compromises driver safety and forces customers to seek aftermarket solutions for a problem rooted in design. A responsible manufacturer should address this defect directly. Driving home as sun was setting, I was completely blinded by the sun. It was terrifying as the visor was too short to stop the sun glare. I was lucky I didn’t hit something or somebody plus my own safety.

The vehicle comes with a software feature that automatically shifts the car into park "to prevent rollaway". It does it at random times when we are driving and has multiple times almost caused an accident because my vehicle will suddently stop when I am in the middle of a road or pulling out onto a busy road. The most recent incident was on March 11 2025, I was pulling out from a parking lot, taking a right onto a very busy road. The software unexpectedly shifted the vehicle to park, stopping it completely while I was halfway in the road, with other cars approaching me at 45-50mph. In a panic it was difficult to shift back into drive quickly, and nearly caused an accident that could have been fatal. I called the manufacturer and was uanble to get through to an agent, so I sent an email and have not yet received a response.

Upon approaching a railway crossing, the signal was green, indicating it was safe to proceed. However, instead of maintaining a controlled speed, the car suddenly accelerated while attempting to cross. Before my spouse could take control, the vehicle struck the boom barrier, which was either lowering or had already lowered at that moment. The incident happened so suddenly that she did not have enough time to react and override the system. Fortunately, no injuries occurred, but the vehicle sustained damage to the front bumper, windshield, and frame. The unexpected behavior of fsd at a railway crossing raises serious safety concerns regarding its decision-making. Safety concerns: I have been a long-time appreciative user of fsd, but this incident has shaken my confidence in the system. I now fear using fsd. Given the serious nature of this incident, I request an urgent review and response regarding my concerns, as well as coverage for the necessary repairs. Request for investigation & cost coverage: access to vehicle logs: I request access to the vehicle's telemetry and camera recordings to determine why this happened. Fsd system review: please investigate whether this was an fsd-related miscalculation and what corrective actions are necessary. Cost of repairs: as this incident was caused by an unexpected fsd action, I request Tesla to cover the repair costs for the vehicle and any associated damages. The car horn is a speaker and is extremely quiet during normal operation. When in rain or snow, the horn speaker disables itself completely leaving the driver unable to utilize the horn which I believe is a hazard to drivers.

Description for NHTSA report: I am reporting a significant safety concern with the new Teslas (starting with the 2024 models) that do not have a traditional physical horn. Instead, the horn sound is generated through the car’s external speakers. This design poses a severe safety risk, especially in rainy conditions or when the speakers are wet, as the horn becomes inaudible or significantly less effective. Additionally, the volume of the speaker-generated horn is not comparable to the loudness of traditional horns, making it insufficient for alerting other drivers and pedestrians in emergencies. This lack of an actual horn is extremely dangerous and has already led to situations where the horn was not effective. I urge the NHTSA to investigate this issue as it compromises vehicle safety.

With the new Teslas (not sure about all but I know on the Model Y ). There is no way to reduce or turn off regenerative braking. Which on snow covered roads makes the car want to go into a skid. Only way to get around it is to force the car in to nutral ( also not great but better than the other option). Then use brakes so it slowed all wheels evenly.

Hello , I just bought the new Tesla Model Y 2024 in the October and all of a sudden all the driver assistance programs are not working, including the rear camera(reverse camera) I took it for the service on December 4th and they fixed issue on December 5th. After few days on December 13th the same issue happened again and Tesla service center is saying I need to wait till February 20th 2025 for the repair. All cameras are not working in the car, it does not show any backup camera, no forward collision warnings or no lane change assist Tesla is not considering this as a safety issue I am including a chat summary with the service center via pdf file. I have included previous repair invoice also when the same issue happened on 4th December.

Ap4 computer failure has led to navigation, cameras, blind spot monitoring, lane departure and other systems being inoperable. Tesla has stated they don’t have the parts to fix and have pushed out my service repair time twice already. Issue reported on December 4th, since then no time table for repair has been given. Tesla keeps stating the car is operable, leaving out the facts it’s dangerous to drive due to small rear windows and blind spots based on the cars design to work with operable cameras that give the drive clearer angles to view.

All cameras not working with latest software update - told need new ap4 computer on 2024 Model Y which is 2 months old. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.

Components failing - reported as central computer. This made the vehicle turn signals and backup cameras fail as critical components. These two elements were/are completely non-functional. Additional less critical safety functions that have been disabled include blind spot monitoring and cameras, both forward and reverse emergency braking, and pedestrian alerts. My safety was put at-risk due to camera failures while driving and backing out of parking spaces. These vehicles are designed with poor and minimal visibility due to their cameras compensating for visibility shortcomings. You cannot adequately see proximity to other vehicles or pedestrians when cameras are not active. This particular impacts reversing when backing in and out of parking spots since the rear windshield is so small and poorly angled. Drivers have reported that the cameras for them are failing mid-drive leading them to not know where other vehicles were next to them and making unsafe lane changes thinking their warning systems were active. The vehicle component has not been inspected by anyone as there was thankfully no actually accident or incident due to the failure. Tesla has not inspected the vehicle due to no availability for 3+ weeks and they will not consider human contact to expedite review. There are now 100s of community reports of the same defect on online communities all showing failures on same-day or within 24 hours of each other. There were no warnings prior to the failure. The symptoms simply stopped working. It required a phone call to Tesla support to even receive an accurate error code. Tesla is claiming a "short" in the main computer system of the vehicle. This occurred for myself and dozens of others all within several hours of each other in the early morning hours of Friday November 29th 2024. Suspicion is that that this was due to a software update from Tesla.

I noticed that now Tesla uses windshield mounted cameras for everything in front of car including fsd and forward collision. My Model Y was made in fremont, California. Problem is windshield glass is not cleaned before assembly to car. Either not a prescribed step by Tesla during assembly or overlooked by assembly workers. I noticed after I picked up my new Model Y from Tesla that there is haze and layer of residue on windshield glass. As if glass are sitting in bulk in factory and assembled to car without cleaning. I was able to clean windshield inside with glass cleaner and microfiber cloth. Problem is windshield haze and layer of residue cannot come out of front facing camera module. This is causing all functions to degrade that rely on these windshield mounted cameras. I get random "one of cameras occulted" error during full self driving fsd. Car cuts out during summon. Haze can be seen on app cameras during summon, while rear camera and fender cameras show clear view. This will get worse with winter now as fog also sets on windshield. Cleaning windshield glass behind camera module requires professional disassembly. Maybe it only affects Model Y made in fremont, CA or more.

Issue description: recently, I discovered that the glassroof of my Tesla Model Y has developed noticeable cracks. This issue is perplexing and unacceptable, especially considering the vehicle has not been subjected to any conditions that could cause such damage. The sudden appearance of these cracks suggests a potential manufacturing defect or poor assembly quality, both of which are serious concerns for vehicle safety and integrity. Another warning popped up stating “power reduced - exiting and re-entering vehicle may restore operation”. Has a yellow icon of a turtle. I was driving and going about 50mph and there was no other car near me. Suddenly the car slammed on its brakes. Dropped to probably 10mph. My mom and I were both wearing seatbelts but our heads were thrown forward. My neck and shoulder are sore and stiff today now. There were dark skid marks in the road ahead of us and I believe the car saw those and broke because of them. If there had been a car behind us, we would have be rear ended. There were no warnings beforehand. When the car broke, lights flashed and there was also an alarm. The vehicle has not been inspected by police or manufacturer.

This is a complaint regarding a malfunction of the Tesla full self-driving (fsd) feature, which resulted in damage to my vehicle and may potentially pose a significant safety risk to public. On 07/02/2024, my newly purchased Tesla [model-y 2024] was parked next to a pillar in a parking garage. After ensuring that the full self-driving sign was on, I activated the fsd feature. However, as soon as the function was engaged, the vehicle instantly took off and made a right turn. This sudden and unexpected action caused the right side of the back passenger door to scrape against the iron guardrail on the pillar and caused damages to the right back passenger door and a portion of right back fender. This action happened so sudden that did not have the chance to react and take manual control. I contacted Tesla to report the issue and seek assistance, but after +4 weeks of contacting and restless following up with Tesla service teams, Tesla stated that the software was functioning properly and denied any responsibility for the malfunction, and they closed my case without offering any resolution or further investigation. Luckily, this incident happened in a confined parking garage. It could have resulted in more severe damage or injury had the circumstances been different. Additionally, Tesla's dismissal of the issue and refusal to accept responsibility is deeply concerning, especially about the reliability and safety of their full self-driving technology. I sincerely request your office to investigate into this incident, and I hope you could advise any appropriate action to examine and address this issue. I attached a few photographs of where my vehicle was parked before the fsd was turned on, and the damages on both my vehicle and the guardrail at the time of the incident. The Tesla accelerated by itself while I am driving in a heavy traffic. To avoid hitting the front car, I braked several times, which didn’t work at all. It still accelerated, instinctively I steered wheels to the right front and hit a small hill. Airbags are out. I struggled to get off the car and shouted for help. 911 was called and emergency ambulance took me to er. Luckily I am alive, it could be much worse that I would tremble when thinking of that crash scene again. This happened on 6/8/2024 12:41pm. I am waiting for the report from the police and the next steps from Tesla insurance.

This is my fifth Tesla, so I. Am very familiar with the self driving feature which is mostly good. However, recently I got a Model Y and with in 24 hours I was rear-ended on the freeway! I had the car in full self driving mode (fsd)! for some reason it just braked hard in the middle of my driving however, traffic was backing up. I injured my lower back and neck and I'm now having to go in for therapy and schedule a cat scan. Luckily the person behind me was fine. After the incident I downloaded the zip disk which records incidences. The car had documented pre and post accident! but the actual incident there is no record. . . Almost like it got wiped out!?!.

Turned wheel sharply to the left in order avoid hitting neighbors fences. Vehicle traveled in an arch and collided with a tree next to an fence and stopped.
