I was driving and going about 50mph and there was no other car near me. Suddenly the car slammed on its brakes. Dropped to probably 10mph. My mom and I were both wearing seatbelts but our heads were thrown forward. My neck and shoulder are sore and stiff today now. There were dark skid marks in the road ahead of us and I believe the car saw those and broke because of them. If there had been a car behind us, we would have be rear ended. There were no warnings beforehand. When the car broke, lights flashed and there was also an alarm. The vehicle has not been inspected by police or manufacturer.

This is a complaint regarding a malfunction of the Tesla full self-driving (fsd) feature, which resulted in damage to my vehicle and may potentially pose a significant safety risk to public. On 07/02/2024, my newly purchased Tesla [model-y 2024] was parked next to a pillar in a parking garage. After ensuring that the full self-driving sign was on, I activated the fsd feature. However, as soon as the function was engaged, the vehicle instantly took off and made a right turn. This sudden and unexpected action caused the right side of the back passenger door to scrape against the iron guardrail on the pillar and caused damages to the right back passenger door and a portion of right back fender. This action happened so sudden that did not have the chance to react and take manual control. I contacted Tesla to report the issue and seek assistance, but after +4 weeks of contacting and restless following up with Tesla service teams, Tesla stated that the software was functioning properly and denied any responsibility for the malfunction, and they closed my case without offering any resolution or further investigation. Luckily, this incident happened in a confined parking garage. It could have resulted in more severe damage or injury had the circumstances been different. Additionally, Tesla's dismissal of the issue and refusal to accept responsibility is deeply concerning, especially about the reliability and safety of their full self-driving technology. I sincerely request your office to investigate into this incident, and I hope you could advise any appropriate action to examine and address this issue. I attached a few photographs of where my vehicle was parked before the fsd was turned on, and the damages on both my vehicle and the guardrail at the time of the incident. I am more than happy to provide any further information or clarification. Thank you for your attention to this matter. I look forward to your guidance and assistance.

The Tesla accelerated by itself while I am driving in a heavy traffic. To avoid hitting the front car, I braked several times, which didn’t work at all. It still accelerated, instinctively I steered wheels to the right front and hit a small hill. Airbags are out. I struggled to get off the car and shouted for help. 911 was called and emergency ambulance took me to er. Luckily I am alive, it could be much worse that I would tremble when thinking of that crash scene again. This happened on 6/8/2024 12:41pm. I am waiting for the report from the police and the next steps from Tesla insurance.

This is my fifth Tesla, so I. Am very familiar with the self driving feature which is mostly good. However, recently I got a Model Y and with in 24 hours I was rear-ended on the freeway! I had the car in full self driving mode (fsd)! for some reason it just braked hard in the middle of my driving however, traffic was backing up. I injured my lower back and neck and I'm now having to go in for therapy and schedule a cat scan. Luckily the person behind me was fine. After the incident I downloaded the zip disk which records incidences. The car had documented pre and post accident! but the actual incident there is no record. . . Almost like it got wiped out!?!.

Unknown. Drove car in front of garage. Car came to a full stop. About to put car into reverse from drive in order to back into the garage, when car suddenly accelerated forward. Turned wheel sharply to the left in order avoid hitting neighbors fences. Vehicle traveled in an arch and collided with a tree next to an fence and stopped.

Incident date: February 12, 2024, at 1:00 pm location: [xxx] vehicle model: Tesla model 3 VIN: [insert VIN of the vehicle] license plate: [insert license plate number] description of the incident: on [xxx], while driving westbound on [xxx] at a speed of 35 mph, [xxx] experienced a sudden and dangerous malfunction of the Tesla model 3. Despite the driver's best efforts to maintain control, the vehicle autonomously shifted from the innermost lane to the outermost lane, jumped over the concrete sidewalk, and fell into a trench. Fortunately, no other vehicles were involved, preventing what could have been a fatal accident. The police report filed with the suwanee, GA police department documents the incident, and the car was subsequently towed to a body shop, where it was declared totaled. It is essential to note that the auto-drive feature was disabled, and no subscription to the self-driving feature had been purchased or renewed. This raises serious concerns about a potential malfunction in Tesla’s system, and it is imperative that this issue is thoroughly investigated. Attempts to resolve: since the incident, numerous attempts have been made to contact Tesla for a root cause investigation and resolution, but Tesla has failed to provide any substantive response or investigation. Below is a history of our efforts: 1. March 3, 2024: incident claim submitted during a call with raquel. No claim number received. 2. March 14, 2024: called Tesla customer service and spoke with anthony. Case was assigned to duluth, GA service center, but no case number was provided. 3. April 15, 2024: spoke with christian, and later with manager andrew. An email was received with reference id [xxx], but no further communication or action has been taken. Vehicle warning icons are blocked by the drivers right arm. In some driving positions, the current vehicle speed is also blocked. Recommend shifting the icons toward the center of the monitor.
