On August 15, 2026, at approximately 5:00 pm, the vehicle was being driven with full self-driving (supervised) engaged during light rain. As the vehicle approached traffic ahead, it did not slow or brake as expected. The driver recognized that the vehicle was not slowing and attempted to intervene manually by pressing the brake pedal multiple times. The driver reports that despite these attempts, the vehicle did not slow or respond to the brake pedal as expected, and a collision occurred. There were no known warning lamps or warning messages indicating a braking-system problem prior to the incident. The vehicle has not yet been inspected to determine the cause of the reported braking behavior. The incident has been reported to the manufacturer, and a request has been made to preserve and review available vehicle data surrounding the collision, including braking inputs, vehicle speed, full self-driving status/actions, warnings, diagnostic information, and safety/event data. The vehicle is available for inspection. At this time, the cause of the failure to slow as expected is unknown.

Tesla will not repair under warranty which requires disassembly of the camera housing. Front camera housing has been redesigned by Tesla but they are not retrofitting this fix for existing Model Y and have not issues a safety recall. This camera housing defect presents a risk to my safely and the safety of others.

The vehicle attempted to proceed through the intersection without yielding. I was fully attentive with my hands on the wheel and had to instantly apply heavy manual braking and steering intervention to prevent a collision with an oncoming vehicle that had the right of way. This failure of the automated driving system to recognize fundamental traffic control devices constitutes an immediate and severe collision hazard.

On the day after taking delivery of my 2026 Tesla Model Y, fsd reversed into a visible stationary garage pillar while backing into my garage. The following day, while fsd was attempting a left turn at an intersection, the vehicle nearly entered the opposing-traffic lane before making several hesitant steering corrections. I disengaged fsd and completed the turn manually to avoid a potential collision. Both incidents occurred within two days of delivery. Tesla has opened a service case, escalated the matter to its engineering team for review, and I have requested preservation of all available vehicle data. I am submitting this complaint because these incidents raise concerns about the safety of fsd behavior.

I was operating my Tesla using full self-driving (supervised) from my home to a coffee shop. The vehicle completed the entire drive under fsd without any manual intervention. After arriving at the destination, fsd automatically entered the parking lot, selected an available parking space, and initiated the parking maneuver on its own. I did not manually select the parking space or start the parking maneuver. During the automatic parking maneuver, the vehicle failed to detect a fixed square concrete bollard located within the parking space and continued moving until the front bumper made contact with the bollard, causing damage to the vehicle. No collision warning, parking collision warning, or automatic emergency braking occurred before impact. The vehicle remained under fsd control until the collision. The incident occurred in daylight with good visibility. The obstacle was a permanent concrete bollard, not a temporary object or a low parking curb. Dashcam footage of the incident has been preserved and has been provided to Tesla insurance. I believe Tesla should review the vehicle telemetry and fsd logs to determine why the system failed to recognize the fixed obstacle during the automated parking maneuver.

The vehicle's indirect tpms showed a "pressure sensor fault (may clear next drive)" without issuing any "low tire pressure" alert. On July 20, 2026, while driving, the rear right tire lost pressure and disintegrated over a 10-minute drive. Despite severe tire failure that triggered multiple safety system alerts (traction control, stability control, automatic emergency braking, and lane departure avoidance disabled), the vehicle software failed to issue a critical "do not drive" command or explicit "flat tire" warning. Furthermore, the official Tesla app automated response advised that "most alerts may resolve automatically on your next drive," encouraging continued driving. This total absence of critical emergency alerts and misleading system guidance created an extreme, life-threatening road hazard and resulted in severe tire and wheel destruction.

The contact stated that while driving at an undisclosed speed with the full self driving mode activated, the vehicle struck the garage wall, damaging the rear passenger’s side door. No injuries were sustained. The contact stated that the dealer had previously provided mobile service to replace the camera and that a second dealer, Tesla white plains (250 tarrytown rd, white plains, NY 10607), had performed a hard reset on the vehicle due to phantom braking; however, the failure persisted. The dealer was made aware of the failure but canceled an upcoming appointment and informed the contact that there was no failure found. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 13,001.

7/17/26 about 8pm my Tesla Model Y backed into a wall under fsd (full self driving). The vehicle was placed into fsd to assist in backing the car from a narrow street into a garage space. Wall on one side and another SUV parked on the driver side. Driver was monitoring the vehicle maneuver forward and backward attempting to clear the wall so it can back into the space. The vehicle appeared to be close to the wall and I anticipated the vehicle to inch forward but instead the car went backwards into the wall and caused damage to my vehicle. The vehicle should have stopped and asked the driver to take over or alarms/chimes. The fsd system failed to recognize that the right rear would not clear the wall and still proceeded to reverse and cause damages.

This is what happened: while full self-driving (fsd) was engaged, the vehicle approached an intersection and appeared unable to correctly follow the roadway. Instead of following the intended road path, the vehicle continued forward, drove over the curb, and struck a road sign. The impact knocked the sign down, and the sign subsequently struck the vehicle, including the roof glass. The roof glass shattered and small pieces of glass entered the cabin, exposing both the driver and front passenger to a safety risk. Vehicle data obtained after the incident shows that the vehicle was in active_fsd leading up to the collision. Fsd changed from active to unavailable approximately 1. 5 seconds before the crash algorithm wake-up. Significant braking occurred only very close to the collision, and no automatic emergency braking intervention was recorded during the incident window. The data also shows multiple substantial steering movements shortly before the collision. Accelerator-pedal data does not establish driver acceleration as the cause of the collision. The available samples show an accelerator position of 2. 0% shortly before the collision and 0. 0% in the next recorded sample. The driver applied the brake shortly before impact. There were no prior warning lamps or messages indicating that fsd was about to become unavailable before the vehicle departed the roadway. The vehicle has been inspected/repaired following the collision, and vehicle data relating to the incident has been obtained and preserved. Police responded to the collision, and the incident was also reported to insurance. A police report was filed, and the case number is 2026-07160174. I have not yet received the formal cr-4 crash report. Following the collision, I experienced significant psychological distress and trauma-related stress symptoms as a result of the incident.

Yesterday evening, my wife and I had friends over to our house, and we were excited to show them our new Tesla. We offered to take them on a short drive around town using the full self-driving (supervised) feature. At approximately 8:00 pm, we entered the address of our local sam's club as the destination. As the vehicle approached sam's club, it entered the parking lot through the gas station entrance, which appeared to be the expected route. However, instead of proceeding to a parking space, the vehicle unexpectedly continued driving and attempted to make another loop toward the gas station entrance. At that moment, an SUV was exiting the gas station. The Tesla steered toward the SUV and then continued toward a brick wall where there was no roadway. It appeared the vehicle was attempting to drive into an area that was not a valid path. I immediately disengaged full self-driving, took manual control of the vehicle, and reversed to avoid what could have been a collision with both the SUV and the brick wall. The occupants of the other vehicles were understandably upset because the situation appeared unsafe. This was a very dangerous situation that placed my wife, our friends, and me at risk. Had I not intervened immediately, the incident could have resulted in a collision causing property damage or personal injury. I am reporting this event so your engineering team can investigate the behavior and improve the full self-driving software. I hope this information helps identify and correct this issue to improve the safety of the system for all Tesla owners and the public.

On June 7, 2026, while full self-driving (fsd) was engaged, my 2025 Tesla Model Y drove itself forward out of a parking spot and struck a stationary landscape rock directly in its path. The vehicle did not slow, brake, or steer to avoid the obstacle before impact. The rock was a fixed object roughly 20 inches tall, clearly visible in the vehicle's forward (windshield) camera from a complete standstill, before fsd began moving the car. It stayed visible as the car advanced and dropped into the camera's near-field blind zone just before contact. The vehicle struck it with no apparent detection or avoidance. This is a serious safety concern beyond the resulting property damage. An obstacle of this height is in the same range as a small child or a crouching person. An automated driving system that fails to detect and respond to a fixed, clearly visible object in its direct path presents a significant risk of injury. The dashcam footage (windshield camera) is available and shows the obstacle visible from a standstill. The vehicle's front bumper camera feed is not saved to dashcam, so only the manufacturer's internal logs can confirm whether the obstacle was detected and how it was classified. I reported this to the manufacturer's service team and requested a log review; they declined to confirm detection and directed me elsewhere. No warning lamps or alerts appeared before the impact. While traveling on highway 16 at approximately 60 mph, the vehicle approached the right-side white lane line. At that point, I experienced a sudden steering intervention that turned the vehicle sharply to the left. The steering input was unexpected and occurred while I was manually controlling the vehicle. Following the steering intervention, the vehicle crossed the centerline into the opposing lane of travel and continued off the roadway into the ditch on the opposite side of the road. The incident resulted in damage to the vehicle. Road conditions were dry. Weather conditions were safe. Lane markings were clear. I am requesting that Tesla preserve and review all vehicle telemetry, event logs, steering-assistance records, and any other relevant data associated with this incident. Specifically, I request an investigation into whether lane departure avoidance, emergency lane departure avoidance, or any other driver-assistance feature activated and contributed to the steering maneuver. On April 11 2026, I was involved in a serious accident involving my 2026 Tesla Model Y. At the time of the incident, I was returning home from my daughter’s event in downtown and had just entered my residential driveway. As I approached the driveway, I applied the brakes and slowed the vehicle. While momentarily stationary or near-stationary, I spoke to my wife, who was seated in the front passenger seat, advising her to secure the food she was holding due to the uneven surface of the driveway. Upon removing my foot from the brake pedal, the vehicle suddenly and unexpectedly accelerated without any input . The vehicle proceeded forward and collided with my garage door. Notably, no collision warning alerts were issued, and no automatic emergency braking system engaged prior to or during the impact. Following the initial collision with garage door, the vehicle failed to stop and instead continued to accelerate into the garage, striking a refrigerator. The impact resulted in substantial damage to the refrigerator and caused extensive structural harm to the property, affecting the garage, adjacent living space, floors, and interior walls. The severity of the collision triggered deployment of the front airbags. At the time, my wife was seated in the front passenger seat, and my [xxx] daughter, along with her friend, was seated in the rear of the vehicle. Emergency services were called, I have forehead bruises and a burn on my hand from airbag deployment. My wife is advised physiotherapy for severe back pain and my daughter has since experienced ongoing emotional distress, nightmares. I am dealing with car and home claims issues for damages I am seeking a formal investigation into the cause of this incident, specifically addressing: 1)the unexpected and unintended acceleration of the vehicle 2)the failure of the vehicle to stop after the initial collision with the garage door 3)the absence of forward collision warnings and failure of automatic emergency braking . I am submitting this complaint regarding a newly leased 2026 Tesla Model Y premium rwd, which I took delivery of on February 6, 2026 from Tesla watertown, MA. I am a returning Tesla customer and entered this lease after test driving the same model on January 24, which did not exhibit any issues. Immediately upon driving the vehicle off the lot (approximately 12 miles), I noticed a persistent rattling noise from the front suspension area. The noise occurs over normal road conditions and has been present since delivery, significantly affecting the quality and value of the vehicle. On February 9, Tesla watertown evaluated the vehicle and later replaced both front upper control arm (fuca) mounts. On February 10, before repairs were completed, multiple safety systems suddenly disabled while driving, including regenerative braking behavior, traction and stability control, lane safety features, and automatic emergency braking, creating a dangerous situation. I returned the vehicle immediately and was provided a loaner. I picked up the vehicle on February 13 and was told the suspension issue was resolved, but the noise remained. I then sought a second opinion at Tesla peabody, where a master technician conducted a ride-along and confirmed the condition. He verified that the latest fuca parts had been installed and the front structural “pencil braces” had been removed per Tesla guidance. I was informed the vehicle falls into a known noise category affecting 2026 rwd Model Y vehicles and that there is currently no repair or engineering solution available. This defect has existed since delivery and remains unresolved after multiple service visits and component replacements. I have contacted Tesla regional leadership and customer support multiple times without response.

On January 20, around 9:35 am, phoenix time, I had the car in self-driving mode for a left-hand turn at the intercession of camino real and river road in tucson, arizona. A real-time view shows that it’s a tricky and dangerous left-hand turn. For the past 3 weeks, the car navigated it well, waiting until it was perfectly safe to do so. Today, however, the car moved quickly and unexpectedly into the center of river road, narrowly escaping a head on collision with a west-bound car, and then paused, squeezed in between west and east-bound lanes when I tapped the brake and took the wheel. Everything happened so quickly. I made the left turn into the east-bound lane, but, looking back, I don’t know how an accident didn’t occur, as traffic was still moving in east-bound lanes rapidly. There must have been enough distance between two cars at just the right time, that nothing hit me.

While driving on a residential street at night with full self-driving (supervised) engaged, the vehicle suddenly applied automatic emergency braking without any visible obstacle, vehicle, pedestrian, or hazard present. The braking was abrupt and unexpected and did not correspond to traffic, road conditions, or driver input. No collision occurred, but the sudden stop caused a whiplash effect to occupants, creating a risk of injury despite the absence of an external hazard. The driver immediately disengaged the system after the event. Tesla later reviewed vehicle data and confirmed that an automatic emergency braking event occurred while fsd (supervised) was active. Tesla identified the software version as fsd v14. 2. 1 and documented the incident as unexpected emergency braking, with the date, time, location, and environmental conditions recorded.

The vehicle continued forward by inertia and collided with a roadside curb, resulting in wheel damage. Attached dashcam video clearly shows: • fsd active throughout the event • no alerts or chimes • violent steering oscillation/hesitation • driver's foot slamming brake pedal with no deceleration • impact despite timely braking this incident matches the ongoing NHTSA investigation pe25-012 regarding fsd intersection hesitation and failure to respond to driver input.

When using cruise control (traffic aware cruise control and autosteer) the car acts dangerously and stops for no reason. I've been driving on open roads with no traffic in front of me on regular roads, the car will slam on the brakes and it causes the people behind me to need to slam on their brakes as well. I feel like I'm going to get into an accident constantly. . . I was on the freeway and no one around in front of me and the car slammed on the brakes to almost a stop on a 70 mph speed limit road. Today the same thing occurred in high traffic on a 55mph road, luckily the driver behind me wasn't to close and was able to stop in time . . . There is no option to use regular cruise control or disable enough features to prevent this from occurring. . . It's incredibly terrifying. If they can't do traffic aware cruise, there should be the option to do regular cruise. . . And it shouldn't require passing Tesla $8000 for full self driving to be able to drive your car at a speed without the that of being rear ended. This feels like extortion. . . I've submitted feature requests to Tesla, talked to their bot about fixing this and I've currently been on hold for almost an hour without any response. This dangerous tech is unacceptable!.

Yesterday on October 1st I was driving car recently last few days my car usually go more right side but I thought this may be due to keep in lane car feature or something so I didn’t bother much but yesterday when I was driving at around 11:13 am I didn’t know I reciived notifications in car about 1. Automatic vehicle hold disabled 2. Tracking control disabled 3. Stability control disabled 4. Break pedal regenerative breaking unavailable 5. Lane departure avoidance feature unavailable 6. Automatic emergency breaking unavailable and I see from y car pe drive vidio recording in 11:14 minute one my car hit curb and it damages my both side wheel and tire so badly damaged and broke like out of both pieces came off but didn’t flat my tire and when this happens I was trying it break but it didn’t break instead in lost all function so it was kind going fast and lost all control and with lot of work my car wind on side and then completely turn to other side this all happened in few seconds and was very scary I wasn’t far from Tesla service centre and I have wheel protection and tire protection plan so I took straight to Tesla service centre and I ask them about using my that plan and after some time they said based on damage and they check video they said this is collision and at that time I wasn’t aware what to do and this and that and I wasn’t fighting and I didn’t know about video or notifications all untill they show me and ask me to file insurence claim for collision and luckily I took picture of notification picture in car at that time and then I end up following insurence claim process in hour of something it took me and then went in car to take Tesla flash drive and my stuff I saw they put vehicle in service mode and all notifications were cleared and I even tell them luckily there wasn’t car near by or highways and no one on street and it avoided big incident but all this failure due to impact or this impact due to failure I want investigation because I suspect.

While traveling northbound with the flow of traffic in the middle lane of a surface street, the vehicle initiated a sudden right turn onto a freeway on-ramp at excessive speed and without adequate steering input. This resulted in the vehicle colliding with the left-side barrier. The vehicle is currently in the custody of state farm insurance and can be made available for inspection upon request. Dashcam footage of the incident has been preserved. How was your safety or the safety of others put at risk? the unexpected maneuver caused the vehicle to lose control and strike the barrier, placing both the driver and other motorists at risk. The driver had no time to react to this unexpected action. The driver sustained a whiplash injury along with left shoulder and right chest pain. If another vehicle had been present in the adjacent lane, a collision could have occurred. Has the problem been reproduced or confirmed by a dealer or independent service center? no. The issue has not been reproduced or confirmed by a dealer or service center at this time. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? the vehicle has been inspected by state farm insurance for collision damage. A request has been made to Tesla to preserve and provide all electronic data logs, event data, and over-the-air communications. No law enforcement or ems responded at the scene. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? no warning lamps, messages, or alerts were displayed prior to or during the incident. The maneuver occurred suddenly and without any indication that corrective driver action was needed.

I was using the full self driving feature of the vehicle. At an unusual intersection, there was a flashing do not turn right light because of a train. The car turned right ignoring the do not turn sign and headed for the railroad tracks. I had to slam on the brakes because the car was not going to stop. 30 Automatic Emergency Braking problem

24 hour after taking delivery, operating on full self driving on a residential street at 25-30 mph, the car struck a landscaping trailer parked on the right. The tailgate of the trailer was down and the car struck the left rear upright post of the trailer causing significant damage to my car. No automatic breaking occurred: the car struck the trailer at full speed. The collision alarm sounded just as the vehicle struck the trailer, not before. I requested vehicle status info form Tesla, but the video stops well before the collision and I can't decipher the rest. But I can send it to you if it would be helpful,.

My Tesla Model Y was in full self driving mode when it turned into a garbage truck, leading to a total loss to my car. There were no warnings from the Tesla system that could have helped me prevent the crash. Thankfully, no one was hurt, but my car is not longer usable.

32 Automatic Emergency Braking problem

Failure Date: 08/19/2025

I was in a parking lot and activated autopilot while the car was next to a stationary pillar near a parking space. Immediately after activation, the vehicle failed to detect the pillar and drove directly into it. The impact happened so quickly that there was no practical opportunity for human intervention to avoid the collision. There may or may not have been a warning, but even if a warning was displayed, it occurred too late to allow the driver to react before the impact. The system did not provide effective parking collision warning or apply automatic emergency braking. This incident caused damage to the driver-side door and mirror. My safety and the safety of others were at risk because the system failed to prevent a collision with a fixed obstacle. No inspection has yet been performed by Tesla service.

33 Automatic Emergency Braking problem

I was provided a Tesla Model Y juniper demo vehicle for a test drive on August 3, 2026 and the incident took place in it around 6:00 pm (±45 min). Full self-driving (fsd) was engaged. I had placed an order for a new Model Y on August 2 and took delivery on August 10 (different VIN). During the demo, fsd initially performed well: it autoparked into an apartment parking spot and exited once correctly. However, the second time, the system made a dangerous maneuver. It activated the left indicator and accelerated rapidly as if entering a roadway, but instead turned left directly into a pole. The pole struck the rear door. I braked, but the system’s sudden move left no time to react. A reasonable human would have exited slowly and cautiously, but fsd acted abruptly and unsafely. My pregnant wife was in the passenger seat, and both of us were frightened. This incident raised serious concerns about fsd’s ability to handle low-speed, high-precision situations. The car should have detected the pole and stopped, yet it did not. If a child or pedestrian had been nearby, the consequences could have been severe. This also makes me question whether fsd could make dangerous high-speed maneuvers. At the Tesla showroom, we were told fsd drives “better than a human,” which misled us as new customers. This was the first car I purchased, and I was unfamiliar with fsd’s limits. Only after the incident did I realize how risky it can be. In my new Tesla, although offered a free fsd trial, I have chosen not to use it. Tesla’s claims administrator, fleet response, has since sent me a repair bill of $7,611. I was never asked to sign any waiver, never shown liability terms, and was told demo drives are insured. This incident shows serious safety concerns and misleading sales practices regarding fsd at Tesla.

34 Automatic Emergency Braking problem

Vehicle make/model/year: 2026 Tesla Model Y (launch series) incident date: [xxx] incident location: [xxx] system/component involved: automated driving systems / autopilot / steering what happened: while driving my new 2026 Tesla Model Y on autopilot, the vehicle suddenly turned off the main road near [xxx] , and steered toward the curb. I immediately pressed the brake, but due to the vehicle’s speed, it still hit the curb, causing damage to the front right tire, wheel well liner, and triggering multiple system error messages. Shortly after, I witnessed another Tesla model 3, also using autopilot, make the exact same wrong turn at the same location and crash into the curb. No injuries occurred, but this appears to be a mapping or autopilot software issue that can cause vehicles to leave the roadway unexpectedly, creating a serious safety hazard. Tesla is preparing the incident data, but this report is being filed to alert NHTSA of a potential autopilot safety defect in this area. 35 Automatic Emergency Braking problem

Failure Date: 05/31/2025

Phantom braking on the highway. We were the only car and without warning the car slammed on the brakes. There was nothing for the vehicle to hit in the road or cross traffic. It did this 3x in 35 miles. This has happened both while just cruise control and when the full self driving was on. We are too worried to use either of there is any traffic around us. We have not been to dealership yet. Fast beeping when it hits the breaks. Have not seen any alarms.
