36 Blind Spot Detection problem of the 2023 Tesla Model Y

On [xxx] at [xxx] on [xxx] I experienced a sudden malfunction of steering control of my Tesla Model Y, followed by a rightward movement initiated by vehicle automation and potentially amplified due to the steering malfunction. The vehicle then collided with an 18-wheeler truck on the right lane and came to a stop on the middle lane. I was driving past tracy CA on [xxx] towards modesto on a personal trip to merced with my daughter, [xxx] , in the front passenger seat to drop her off. Fortunately, there was no casualty or injury from this incident. We are fortunate enough to have survived physically unscathed. A difference of few seconds could have led to significant injuries or impairment. Pictures attached show the steering wheel position turned to the left at about 160 degrees, while the front wheels turned to the right at about 20-30 degrees. For a right turn it’s about a 220-degree position, which is not possible by a single swipe of the steering wheel. Please note, the front wheel position in below picture cannot be justified either way. The steering wheel and front wheel remained frozen in this position when high voltage battery supply got disconnected at crash. This proves the turn of the wheel, thereby the movement of my vehicle to the right was not caused by my steering wheel control. Vehicle automation engaged related to this safety incident- advanced driver assistance systems such as autopilot, lane assist, collision avoidance assist and other safety features were active during this incident. There were no audio or visual warnings related to lane departure, forward collision or blind spot on the vehicle dashboard. The vehicle was declared a total loss by the insurance carrier, Tesla insurance and currently in possession of their salvage vendor copart, title express. Tesla service refused to do any inspection of the components involved in this failure while the vehicle was at the body shop, carstar chilton autobody, san carlos, CA. 37 Blind Spot Detection problem of the 2024 Tesla Model Y

Failure Date: 07/02/2024

This is a complaint regarding a malfunction of the Tesla full self-driving (fsd) feature, which resulted in damage to my vehicle and may potentially pose a significant safety risk to public. On 07/02/2024, my newly purchased Tesla [model-y 2024] was parked next to a pillar in a parking garage. After ensuring that the full self-driving sign was on, I activated the fsd feature. However, as soon as the function was engaged, the vehicle instantly took off and made a right turn. This sudden and unexpected action caused the right side of the back passenger door to scrape against the iron guardrail on the pillar and caused damages to the right back passenger door and a portion of right back fender. This action happened so sudden that did not have the chance to react and take manual control. I contacted Tesla to report the issue and seek assistance, but after +4 weeks of contacting and restless following up with Tesla service teams, Tesla stated that the software was functioning properly and denied any responsibility for the malfunction, and they closed my case without offering any resolution or further investigation. Luckily, this incident happened in a confined parking garage. It could have resulted in more severe damage or injury had the circumstances been different. Additionally, Tesla's dismissal of the issue and refusal to accept responsibility is deeply concerning, especially about the reliability and safety of their full self-driving technology. I sincerely request your office to investigate into this incident, and I hope you could advise any appropriate action to examine and address this issue. I attached a few photographs of where my vehicle was parked before the fsd was turned on, and the damages on both my vehicle and the guardrail at the time of the incident. The issue I encountered was with Tesla's Smart summon feature. While using the feature, the car crossed the parking line and crashed directly into a car parked beside it. Happening so quickly I could not react in time. All of the cameras and sensors were in working order and it seems to have been a software related issue. The safety of anyone around the vehicle could have been at risk thankfully no one was hurt. The vehicle has since been inspected by insurance and damages to both cars were repaired. There were no warnings of any kind prior to the collision. I have a video from the on-board cameras of the entire incident.

Took Tesla's 30-day free trial offer of full self-driving (supervised) for my Model Y. Encountered several concerning situations: 1. Autopilot failed to detect a parking barrier arm, causing acceleration towards it; fortunately, I intervened in time. 2. Merged onto the highway in the wrong lane, prompting honks from vehicles behind me. 3. Wheel hit the side road curb when making a right turn, damaging the hubcaps and tire on the right rear side. Unsure if Tesla will take responsibility for repairs, but I feel it's important to share my experience to highlight that autopilot may not be as advanced as expected.

The back camera of the Tesla Model Y does not provide necessary image quality to identify any objects/persons of cross traffic when reversing the vehicle. In one incident I was about to hit a person behind the vehicle as there was no proper cross traffic alerting and no proper camera view. When I approached Tesla for support they identified the problem and determined it is by design and would not be able to help much. As per the images uploaded, there is clear distinction of camera quality between side camera and back camera. Also the images taken when the vehicle is in stop mode making the back light illuminate otherwise the images would be much darker. Also I want to point out that the back camera functions good during day time and the issue happens only during night.

When their is a car in my blind spot the car does not beep or try to pull the car back unless you are basically hitting the car next to you. It also does not beep or show me space out side my range of view to prevent backing into another car. My car (Tesla Model Y) has had non-function blind spot warnings and backing up warnings since we purchased it in Feb 2023. There have been multiple times when I have not seen other cars and almost ran into them (they were able to swerve). On this Monday night the other car was not able to swerve because there was someone in the lane next to her (3 lane road) so we crashed. My car is available for inspection upon request but someone would have to come to my home because the car is not drivable and is waiting to get into the Tesla body shop.

On 9/4/23, I encountered a serious malfunction with my Model Y. The issue in question involved the vehicle crashing into a wall on the top level of a parking structure while using the “Smart summon” feature in Tesla’s mobile app. The “Smart summon” feature allows you to use the Tesla app to retrieve the vehicle and have it move to your location (within 12 meters) while you are standing outside of the vehicle. This malfunction rendered the vehicle unsafe to drive and significantly affected its performance. I immediately contacted Tesla's customer support and was advised to take my vehicle to the nearest Tesla service center or certified repair service center. The Tesla service center located in west covina, CA, performed a diagnostics inspection and technicians said that due to the “Smart summon” feature being a beta version, they do not take responsibility for the damage caused. I explained that the “Smart summon” feature was first released in 2016 and that it’s original feature design is for the vehicle to stop completely if it detects any objects in its path of travel. I also raised the question on what if it was our children or any other pedestrian that could have been injured. I would like to emphasize that my Model Y is only 1. 4 year old, purchased in April 2022 and it has been meticulously maintained according to Tesla's recommended guidelines. It is perplexing and disheartening to learn that a significant malfunction, seemingly unrelated to any neglect or misuse on my part, would not be covered by Tesla's warranty. I understand that warranties have limitations, but I firmly believe that the malfunction I experienced falls well within the scope of what should be covered. This situation has placed an unexpected financial burden on me, as the estimated repair costs are substantial at almost $8,000. It also raises questions about the reliability and durability of Tesla vehicles. I kindly request your time and assistance in the matter.

Vehicle visualizations degrade over a few days following a firmware update and eventually show nothing on screen. Features such as auto wipers, traffic-aware cruise control, auto turn signals and blind spot indicators do not function. There is no warning to the driver in the car that these items are not working. While driving I am unaware if other safety features are not working (emergency braking for example). Manufacturer admits this is a firmware bug and has promised a repair multiple times. The service team has provided a temporary fix to manually reset the self driving computer each time this happens (approximately every 3 days) which requires a recalibration of the cameras rendering these features useless.

Backing up in a parking lot with direct sun behind the car, lost sight via the mirrors, dashboard camera and parking assist and collided with parking bollard approximately 3' high causing extensive damage to the left, rear quarter panel and wheel well. Parking assist didn't beep, blare or other as it does and has for our yard waste bins and garden cart at the edges of our driveway (backing up in the dark). I believe the fact that cameras only and sun exposure is a bad mix and was sure glad we weren't in fsd and going forward. I think that removing the radar function via software and or removing the connector plug (at a service visit) is both unsafe and a fraud against the purchaser of such a vehicle and radar systems should be reinstated for the consideration of safety whether going backwards or forwards. Wanted to make the complaint first as we also can't get a service appointment for this incident until the end of July. Happy with the overall vehicle but this safety function is fallible and unsafe.

Hello it's me for the 500th time. I was sitting at a light when a new song came on that's actually a movie clip from pulp fiction directed by quentin tarantino about hitmen that kill people that speak out, like whistleblowers; for their boss. So I'm sitting at the light and the cars behind me are all glitching between 3 lanes but nobody is moving and my volume is up and it starts 'yelling' "storing dead niggers aint my fucking business!". This was soon after almost being ran off the road by a white Cadillac escalade that I called in and reported to the local pd. I was rear ended by a white Cadillac escalade in 2019, I almost died and received death threats while leaving a competitors business. And I'm permanently partially handicapped at 31 years old. About 1 mile down the road my car started encountering 'ghost cars' that show up on the gui of the screen but aren't actually there in real life lol. Just so I'm clear the car is basically threatening me after Tesla says there's nothing wrong with it so I disabled my $12,000 fsd and will be keeping this car in the garage from now on. There are multiple issues: 1. When trying to back up slowly, or move forward slowly, the car goes automatically on park "to prevent roll". I am intentionally trying to slowly back out of parking, and the car keeps stopping 2. When I try to change lanes, in many cases the car gives a solid "beep" which is equivalent to me almost crashing to another car. But there are no other cars! 3. The car keeps giving forward collision warnings when there is no immediate danger to collide with anything, and in most cases there is nothin in front.

Bought 2023 Tesla Model Y on 29-Dec-2022. Noticed that when I am backing up the car, even when there is another car right behind, or any pedestrian crossing there was no warning. At first I thought some settings is turned off. Later, when I was changing lanes in freeway, no lane departure alert. Also no blind spot chime came on even though that option was turned on in settings. Almost I was about to crash into another car which was in my blind spot. I started googling and learnt that Tesla has removed ultrasonic sensors from oct2022 onwards and they are working on Tesla vision which is a camera based system. But Tesla vision is not ready as of 08-Mar-2023 when I am writing this. How is the 2023 Model Y manufactured without ultrasonic sensors and no Tesla vision passed basic safety test? I feel threatened every moment I drive the car without these basic safety features like rear object detection while backing up, lane departure alert, curb sensor alert, blind spot warning. I feel like I am driving a 2000 model old car which doesn't have any of these features. How does NHTSA deem Tesla cars manufactured after oct22 safe on roads? The Tesla Model Y blind spot warning system does not accurately detect the position of cars in the lanes next to the car. The display shows the car as behind the vehicle while it is actually at the rear quarter panel. There is only an audio warning if the turn signal is activated. The car is in the blind spot yet the system does not warn the driver. I took the car in for an adjustment to cameras and Tesla said they are functioning as designed. Tesla removed the radar sensors that aid in parking at low speeds. This was intended to be replaced with Tesla vision. Tesla vision functionality is not yet available. Although a low-speed collision with an object may not result in injury to a person, it is a safety feature that has been removed. The windshield wiper's speed cannot be adjusted or even started on the stock. To change speed or turn on the wipers you must look away from the road, locate the control panel and then select the desired speed. This clearly is a safety hazzard.

Tesla gave me appointment to bring in the car on 11/28/22 then emailed me informing appointment pushed back to 12/8/22 and today emailed informing appointment pushed back again to 12/12/22. The car is not drivable without these safety features as it poses the risk of accident that could harm driver and other on the road. Tesla is not responding at all for providing a replacement vehicle, while this car is still under warranty.

In Tesla’s from Model Years before 2021, the side “repeater cameras” that are rear facing suffer from a light-leak of the blinker. There are three small holes in the pcb circuit board that allow light from the blinker to leak into the camera at night. Tesla subsequently fixed this problem, first by applying a sticker over the holes in the circuit board, and later by redesigning the part entirely. But this “light leak” problem exists on hundreds of thousands of cars. There are two problems with this: 1) Tesla has now changed their “autopilot” and “full self driving” software to a Tesla vision system that only uses cameras for the autopilot features, including lane keeping and lane changes. They no longer use ultrasonic sensors or forward looking radar, even though it is still installed on these cars. This reliance on vision makes it impossible for the Tesla side facing cameras to see when the blinker is turned on. This can result in unpredictable behavior when changing lanes or turning at night with autopilot engaged, including unpredictable or erratic driving where the car abandons a lane change mid course and swerves back into the lane. 2) even when autopilot is not engaged, the blind spot monitoring in this car consists of a pop up window on the center screen showing the side view camera when the turn signal is engaged. At night, with the blinker on, the driver sees a bright light flashing intermittently in the side view camera. This is very distracting and potentially hazardous to the driver. Tesla has refused to replace the cameras on my car under warranty because they state the cameras are “neither a defect of materials nor workmanship” and are an “expected condition based on the inherent capability of the camera design”.

Hi, my main concerned about having a Tesla is not having warning beeping sound when reversing. I almost ran over someone because Tesla rear-view camera doesn't provide virtual alert/audio when getting too close to an object or if a pedestrian decides to walk behind your vehicle while you are about to reverse or pull in reverse into a parking space. I feel this should be a mandatory monitoring system for all Tesla when reversing, just like BMW or Acura, you do get a beeping warning sound when there's something there or when vehicle is getting too close to any objects. I started a drive from my house. I entered a destination into the nav system, it plotted a route. I then proceeded to start my drive. When I tried to backup, the backup cameras did not display any video, I waited over 1 minute and there was still no video. I then proceeded with my drive. The car would not start the cruise control, autopilot or fsd functions. Whenever I tried to engage the systems the car would state that they are unavailable. At this time, I also noticed that the nav system was not updating the display, it continued to display the same image that showed that I was at home in the driveway. After 15 minutes, I pulled into a parking lot to video the problem. First, I put the car in reverse and recorded that the cameras did not display video for over 30 seconds, I then started to drive and recorded that the navigation display was not updating. The display that usually has the lane markings and displays the other vehicles was also not working, there was no information other than a picture of my car with a totally blank gray area around it. I have contacted Tesla about the problem, but they keep saying that they don't detect any problem with the car. I sent them the video and am waiting to hear back from them.

Several times, after over the air software updates, several components of the electrical systems would stop working, including gps & cameras, resulting in no cruise control, no back up camera, no navigation, no blind spot warning/heads up collision warning/lane departure warning. Essentially, all safety features, cruise control and navigation fail when gps and cameras fail. In all cases, the reset options provided such as powering off the carf, powering off the screen etc did not help and instead the 12 v battery had to be disconnected, sometimes more than once, before car would reset to a safe state. This requires removal of a panel, removal of cabin filter, removal of cable to battery. This is a time consuming process and could theoretically void warranties. We are waiting for the next software update to see if replacement of a control board by Tesla this week fixed the issue.

While driving along on an interstate highway at night, the frame rate for the display system including speedometer and other vehicle information drastically dropped. This resulted in limited situational awareness of what the car was doing. Updates for lane changing, blind spot detection, and lane departure warnings were delayed/lagged as a result of the reduced frame rate. This did not impact how the vehicle drove, or activation of turn signals, and the lower frame rate did not result in any flickering of the display. A display reset was required while driving at highway speeds to regain full situational awareness.

The blind spot warning creates a visual indication on the media control unit display in the center of the dashboard, but no aural warning or indication when looking at the side mirror. In effect, this requires the driver to look forward and towards the center of the vehicle to gain any value of warning, directly opposite the threat of a vehicle in the left side blind spot. This issue manifested itself while I was driving my vehicle on a 6-lane divided arterial road and was attempting to change lanes towards a left turn lane. I observed a vehicle approaching on my left while I had my turn signal activated (and saw the vehicle indicated on the mcu), and once the vehicle had passed I began to move towards the left lane to find a second vehicle rapidly approaching and pass that I had not previously seen. An aural indication that the vehicle detected another passing vehicle would have alerted me before I began changing lanes and had to perform an abrupt evasive maneuver to avoid a collision. This issue has been raised with Tesla numerous times based on comments in various online forums but has not yet been addressed by the company.

Hello, Tesla has released an update that will turn on the side camera when the blinker is activated. At night, the blinker can blind the camera making it difficult if not impossible to see any object. I'm concerned that this can cause the driver to turn into another car at night.
