36 Warning problem of the 2020 Tesla Model Y

Failure Date: 04/03/2023

On several occasions, my car will either I) slam on the brakes when in assisted driving for no reason, ii) start beeping for a lab departure when I have no even made the slightest move to switch lanes, iii) windows will roll back down when I try to roll them up (not sure if that’s related to the fsd issues). I paid an extra $15,000 for fsd and not only does it not work, but it seems to have created unsafe driving conditions in my car.

37 Warning problem of the 2022 Tesla Model Y

Failure Date: 03/25/2023

I was stopped at a light, and I as I continued my trip after the green. I had put the car in auto pilot. As I approached the curve there deer to my left that got my attention. The vehicle didn't follow the road, nor did it announce with any warnings, or lane assists that the vehicle was departing the lane. Impact was made into a broken guard rail. Guard rail impaled the vehcile from the left side into the cabin impacting my left leg causing serious injuries.

38 Warning problem of the 2022 Tesla Model Y

Failure Date: 03/07/2023

My Tesla Model Y reports that cameras are blocked or blinded nearly every trip. This happens day or night, and seems to be related to condensation in front of the camera. There is no other condensation in the vehicle; and there is nothing I can do to remedy this. I have brought it to the service center multiple times. On the last visit, the manager informed me that this is a known/common issue and that there is no current repair available. When this occurs, it either disables autopilot which can be alarming/distracting, or stops it from being enabled. Assumedly multiple safety features are disabled while the vehicle is in this state. I can provide additional documentation if required. The image supplied is from the last trip to the repair center.

39 Warning problem of the 2022 Tesla Model Y

Failure Date: 01/24/2023

"full self driving" without explanation stopped working while I was using it. I tried to park the car, walk away, lock it; I even rebooted the car by holding both scroll wheels on the steering wheel as I've been directed in the past. Nothing worked. I noticed during my shift that "auto pilot" still worked on the highway so I was using it for a couple miles successfully. Then when the time for the exit came the car took the exit normally but then when it switched to "fsd" the car stopped steering and drove me directly towards a drainage ditch next to the off ramp and all steering assist and lane keeping was turned off all at the same time (while traveling at 50mph on i4). I was really scared luckily I never took my hands off the wheel but anyone with lesser reaction time would have crashed and possibly died. I got this on dashcam, pulled over to calm down (mcdonalds near mco), then drove back to prove it happened with the iphone as Tesla has directed me to do on many occasions to "prove it". So I got it all on video you can see on this link here since NHTSA only allows low quality short videos less than 10mb like its the 1990's. The more I report Tesla and tell my riders about the child slaves in the congo mining the cobalt in the batteries the more frequently the car tries to kill me. This is a cover up not a coincidence. . 40 Warning problem of the 2023 Tesla Model Y

Failure Date: 12/08/2022

When I purchased this vehicle, I was told that all adas features would be available once the cameras calibrated (approximately 50 miles). After 100 miles my wife bumped into a wall in our garage. I opened a ticket and Tesla said the adas features are unavailable until a software update is sent. The sales person who I spoke to were not aware of this. This is extremely dangerous. Drivers are reliant on warnings from features like this and to have the features off without a warning could get someone run over.

41 Warning problem of the 2021 Tesla Model Y

Failure Date: 11/29/2022

11/29/vehicle begins to flash warning alerts (e. G. , traction control disabled, lane departure warning unavailable), then power steering stopped working, and brakes pulsed violently when used (almost like abs was active). Software update was available shortly thereafter, assumed this would remedy the situation. 12/5/same problems recur (loss of power steering, odd braking behavior, vehicle alerts). Vehicle was towed to Tesla for service. Tesla was able to id the alerts in the vehicle history but could not identify the root cause. Took the vehicle one extended test drives, but was unable to cause the problems to recur. Tesla is unable to identify any further actions to prevent recurrence.

42 Warning problem of the 2022 Tesla Model Y

Failure Date: 11/24/2022

The lane departure warning goes off frequently and at random times when there is no issue with it at all. The windows are constantly malfunctioning and turning down when there is nothing obstructing the path of the window. The tail lights will go out at random times and will only be notified by other drivers particularly at night making the vehicle unsafe to operate and unreliable.

43 Warning problem of the 2021 Tesla Model Y

Failure Date: 06/11/2022

Several instances of emergency braking applied without warning or obvious need during multiple driving sessions. This results in an unexpected rapid vehicle deceleration. Fortunately no other vehicle was following close enough to represent an imminent safety hazard though it is easily plausible this could happen. Typically the issue occurs when traveling uphill or around a bend and encountering traffic in an oncoming lane, structures (bridges) or dark shadows. The unexpected emergency braking has also happened when adaptive cruise control or autopilot is engaged while overtaking large vehicles on the left (tractor trailers in the right lane). I have also noticed several erroneous lane departure warnings despite turning off these notifications on the dashboard. They are more a nuisance than safety hazard so far but would like to completely silence them entirely. These issues have yet to be reproduced or inspected by the dealer, police, insurance or other investigator. No warning lamps or indicators were active at the time the braking or lane departure warnings were observed.

44 Warning problem of the 2021 Tesla Model Y

Failure Date: 05/31/2022

Recently, I had a long distance trip and and Tesla Model Y braked the vehicle at highway speed when it was on advanced cruise control 3 or 4 times with obvious reasons. I had to push the acceralator pedal to speed up in order to avoid rear ended collision in a bumper to bumper traffic.

45 Warning problem of the 2021 Tesla Model Y

Failure Date: 05/01/2022

Currently and noticing other reporting the inaccurate location of the gps in the car. At times the car gpe will drift into areas not associated with the cars correct location. If the car is in adaptive cruse or Teslas autopilot the car will maintain the posted speed limit for that gps location or a delta of the users defined (+ - 10 or 15 mph). This cause a car to abruptly reduce the speed if not within the posted speed limit for that area. For example a car in autopilot or adaptive cruse going 70mph on the highway, the gps will start drifting into areas outside of you location and abruptly reduce the speed to assumed location. This is even more troublesome if autopilot is engaged where the car will try to change lanes as directed by the gps.

46 Warning problem of the 2020 Tesla Model Y

Failure Date: 04/09/2022

Four times in the last six weeks (three while driving) the car has lost all power. My screen either freezes or goes completely black so I have no speedometer, the blinkers do not work, I cannot control the windows or open the doors without the manual override. This usually lasts for 3-5 minutes, sometimes longer - and then the computer resets and everything reloads. Tesla is telling me it is a known issue and I have to wait until they figure out the firmware issue and get the next update. They were not at all concerned about the safety of the car and gave me no suggestions on how to continue to drive safely. The date entered below is the most recent occurrence.

47 Warning problem of the 2021 Tesla Model Y

Failure Date: 02/01/2022

The vehicle driver assist system frequently has phantom braking in situations it should not be, especially when there are vehicles behind putting us at risk for a collision due to unexpected braking behavior of the car. This happens especially in setting of 2 lane roads with oncoming traffic, and guaranteed with a truck in the vicinity. On major highways it is less so, but does still happen occasionally. On local roads even if straight and no obstruction the automatic braking without notice occurs when least expecting. Also the lane departure system disables automatically for the remainder of a drive if you veer off a lane even once and didn't take the corrective action the vehicle thinks should have happened. That's not how a safety feature should work. . . It should remain on regardless. As much as I do enjoy this car, these quality issues definitely make you second guess and keep a closer eye on how you drive to avoid accidents if the system decides to not work as advertised.

48 Warning problem of the 2021 Tesla Model Y

Failure Date: 01/08/2022

Vehicle constantly experience phantom braking. Will randomly brake for shadows, vehicles in the opposing lane, and when it feels like it. Chance for rear collision is higher due to autopilot.

49 Warning problem of the 2021 Tesla Model Y

Failure Date: 11/22/2021

On Monday November 22 at around 6:15 pm cst, I was driving my Model Y and had Tesla's full self driving beta software engaged. A right banked curve on a two lane road in a rural area came up with a vehicle coming around the curve the opposite direction. The Model Y slowed slightly and began making the turn properly and without cause for concern. Suddenly, about 40% of the way through the turn, the Model Y straightened the wheel and crossed over the center line into the direct path of the oncoming vehicle. I reacted as quickly as I could, trying to pull the vehicle back into the lane. I really did not have a lot of time to react, so chose to override fsd by turning the steering wheel since my hands were already on the wheel and I felt this would be the fastest way to avoid a front overlap collision with the oncoming vehicle. When I attempted to pull the vehicle back into my lane, I lost control and skidded off into a ditch and through the woods, causing significant damage to the vehicle. I was able to retrieve the dashcam footage of this incident and it is attached here. I did analyze the footage myself briefly. You can see the vehicle leave the lane, and within about 10 frames, I had already begun pulling back into the lane before losing control and skidding off the road. Since this video is recorded at about 36 fps, this would mean I reacted within 360ms of the lane departure. I understand it is my responsibility to pay attention and maintain control of the vehicle, which I agreed to when I enrolled in fsd beta. I was paying attention, but human reaction does not get much faster than this and I am not sure how I could have otherwise avoided this incident. I am thankful this was the outcome rather than a front overlap collision with the oncoming vehicle, which would have likely resulted in bodily injury. Tesla has confirmed that fsd beta was engaged, per the attached correspondence, but insists that this incident and the resulting accident are my responsibility.

50 Warning problem of the 2021 Tesla Model Y

Failure Date: 01/27/2021

Vehicle made a sudden right turn when it was on auto pilot and because of that there are minor cosmetic damages to wheel and tire. Manufacturer doesn't accept any fault when it was humanly impossible to control the vehicle because of sudden turn.

51 Warning problem of the 2020 Tesla Model Y

Failure Date: 11/26/2020

My car is a 2020 Model Y, delivered on June 27, 2020. This fall, I've had an issue crop up that looks like it's been addressed in the past (at least for models s, x and 3 before March 31, 2019). The problem is that on several occasions (specifically on 11/26/20) while driving on city streets and with humid and cool weather (here in iowa) I've had a report on my display indicating that the b pillar camera is obscured. This, of course, would disable auto drive (including auto lane change) but I'm also concerned that this blind spot may mean that lane change protection during traffic aware cruise control (I. E. * auto drive) will not work, or not be very effective, either. The only formal reference I can locate for this problem is service bulletin sb-20-12-002 but the remedy proposed there has apparently been implemented on all production vehicles starting on March 31, 2019. Well, that solution for this design defect clearly does not work. On November 26, 2020 (the date of the first occurrence of this issue on my Model Y), I requested a service appointment. This was based on the seriousness of the problem given the display warning I received which requested that I stop the vehicle and clear the obscuration immediately. Upon stopping, a glance at the b pillar camera location showed that the issue was one of condensation on the inside of the protective glass over the camera (see attached). From online forums, it's clear that I'm far from alone in having this issue. My service appointment date was originally set for December 17, 2020 but has been canceled as of today. The service center handling my requests is saying that this is "normal" operation of the vehicle and, while the camera may be operating as designed, I disagree that it should be working this way since it removes blind spot awareness from the vehicle's software system and the occurrence is random.
