1 Vehicle Speed Control problem of the 2022 Toyota 4Runner

Failure Date: 06/05/2025

This had happened a year ago. I was driving and my anti collision brake just started to make a beeping noise and was telling me to call a Toyota dealer. And before that the anti collision brake would go off when there was nothing in front of me. A few months later my cruise control and lane assist also stopped working. I took it in for a diagnostic and they couldn't tell me the real problem and that it would be 1500 to actually know what the issue is. I have never been in an accident in this car and its annoying that I bought it because of the safety features that I now cant use.

3 Vehicle Speed Control problem of the 2018 Toyota 4Runner

Failure Date: 03/21/2025

While my father was exiting his 2018 Toyota 4runner sr5, the vehicle unexpectedly started reversing fairly quickly, despite no foot being on the brake. He was dragged by the vehicle while attempting to stop it outside the car, and it continued moving backwards then down a hill before crashing into a fence, causing significant damage to both the car and the fence. This vehicle should not have rolled back at this speed, and it appears to be related to a potential design flaw in the vehicle’s brake-shift interlock system or automatic reverse functionality. This incident is concerning, and after reviewing discussions in various forums, it seems that similar issues may not be isolated to this particular vehicle and could point to a larger defect in Toyota vehicles. I am very disappointed that there are no built-in safety features that could have prevented this incident. It is extremely concerning that the vehicle shifted into reverse while the driver was in the process of exiting. A vehicle should not be able to reserve this quickly or move into reverse unintentionally without the driver’s control. This poses a serious safety risk to drivers and bystandards, and I strongly urge the NHTSA to investigate this issue and determine whether a defect or design flaw contributed to the incident. Implementing proper safeguards could prevent similar accidents in the future.

4 Vehicle Speed Control problem of the 2010 Toyota 4Runner

Failure Date: 12/06/2023

Car keeps running out of gas while driving, due to faulty readings on the gas gauge and electronic readout. I have been driving this car for 11 years and never ran out of gas until running out 3 times in the past 2 months. Today was the 3rd occurrence, gas gauge showed over 1/4 tank and 85 miles to go until empty. Previous incident was similar with 1/4 tank and showing 65 miles to go. (pictures attached). The miles to go reading seems to be increasing with each incident (1st incident showed 25 miles, second incident 65 miles, then 85 miles today). No low fuel indicator light or warning has appeared on any of these 3 occasions. This is a very dangerous situation potentially causing loss of life as drivers have seconds to navigate their vehicle off the highway, while quickly losing speed and loss of steering. No indication that fuel is low, while instead providing incorrect (inflated) dashboard data. Vehicle is going into the shop tomorrow. Found multiple reports of this same incident on 4runners. . 6 Vehicle Speed Control problem of the 2022 Toyota 4Runner

Failure Date: 08/13/2022

On numerous occasions, while driving in a straight line with our adaptive cruise control on, our 2022 Toyota 4runner will automatically actuate the vehicle's brakes when approaching a slower or stationary vehicle that is either to the left or right, but not directly in front of our vehicle. This sudden speed reduction has almost caused any following vehicles to almost collide with the back of our 4runner. It has happened to us at speeds between 30 mph and 65 mph. When it happens, one has to quickly tap the brakes to deactivate the adaptive cruise control then get on the accelerator pedal to avoid a collision with any following vehicle. We have taken our 4runner into two separate Toyota dealerships on three separate occasions to have this problem diagnosed and repaired. The first dealership told us repeatedly that there is no issue. The second dealership pulled data from our vehicle and made an adjustment to the 4runner's forward camera system. Unfortunately, the day after this adjustment was made our 4runner irrationally applied the brakes when we were driving 35 mph on a straight road with no other vehicle in front of us. The problem is not fixed. We are now waiting for a response from Toyota corporate.

7 Vehicle Speed Control problem of the 2017 Toyota 4Runner

Failure Date: 04/19/2022

Text sent to Toyota. Copy to NHTSA. Hello, my name is lee lohman. I am writing to both Toyota and NHTSA with what I perceive as a safety problem. I have owned three Toyotas including a 2017 4runner. I have been pleased with them except for one characteristic they all shared. The turn signal sound volume is just too low. My son's f-150 and my wife's Audi a4 both have very clear and distinct turn signal sounds that I can clearly hear over normal conversation or the radio playing. The Toyota does not. Why? in a nation with aging baby-boomers, whose hearing is diminishing, it would seem obvious that an adjustable turn signal volume control (with a minimum level) would be a much valued enhancement to any vehicle. When you consider that many of the electronic features of modern cars are never used, it strikes me as odd that no vehicle manufacturer has addressed this obvious problem. I have sent a copy of this text to the national highway safety commission and several Toyota competitors in the hopes that someone will realize the potential for a very useful vehicle enhancement.

9 Vehicle Speed Control problem of the 2003 Toyota 4Runner

Failure Date: 09/01/2021

The contact owns a 2003 Toyota 4runner. The contact stated while his wife driving 60 mph with the cruise control activated, the accelerator pedal was depressed, and the vehicle rapidly started to accelerate. The contact stated no warning light was illuminated. The driver depressed the brake pedal to stop the vehicle and parked on the side of the road. The driver was able to restart the vehicle and drive to her destination. The contact stated that in October 2021 his wife experienced a similar failure. The contact also stated that on 11/8/2021 while his wife driving 55 mph and depressing the accelerator pedal, the vehicle accelerated quickly. The contact stated no warning light was illuminated. The driver depressed on the brake pedal with both feet and shifted gear to neutral to slow the vehicle. The driver stated she saw smoke coming from both front brakes. The drive was able to park on the side of the road. The driver stated that the brake pedal seized on the floorboard. The driver stated she applied the parking brake to release the brake pedal. The driver was able to drive to her residence. The contact stated that his wife had experience the failure on 3 different occasions. The vehicle was not diagnosed or repaired. A dealer was not contacted. The VIN was associated to NHTSA campaign number: 11v113000 (vehicle speed control) and NHTSA campaign number: 03v146000 (fuel system, gasoline). The manufacturer had not been informed of the failure. The failure mileage was approximately 240,000.

10 Vehicle Speed Control problem of the 2006 Toyota 4Runner

Failure Date: 12/06/2020

I experienced unexpected acceleration twice. Once was a month ago after starting the 4 runner in the garage. The rpms increased and as I put it in reverse the car accelerated stopping only as a pushed hard on the brakes. Today, 12-6-20, while the car was in motion and as I pulled into a parking spot, the vehicle again accelerated stopping only when I forcefully applied the brakes.

11 Vehicle Speed Control problem of the 2003 Toyota 4Runner

Failure Date: 11/11/2020

I was at a complete stop. Right after I let my foot off the brake, the car lunged forward violently. It was a traumatic experience. I don't know if this was an isolated incident, will it happen again or is this common? also, shortly after I took ownership of my car, the paint started to peel on the rear hatch below the license on the left, where there are somewhat sharp angles. Toyota repainted the rear hatch, excluding the spoiler. I just noticed two rust spots above the rear window. It appears water is seeping in from above the black strip and ponding on thr bottom of it (above the window), resulting in rust. I am wondering if this is common or a problem because the black strip was removed to repaint, or wasn't sealed as well by the factory?.

12 Vehicle Speed Control problem of the 2017 Toyota 4Runner

Failure Date: 07/01/2020

Truck shimmy's whenever I drive over 50mph. Front tires are damaged from cupping. Maintenance has been done as well as tire alignment and rotation. Dealership keeps saying nothing is wrong with it.

13 Vehicle Speed Control problem of the 2015 Toyota 4Runner

Failure Date: 05/13/2020

I came home from work and reversed my 4runner into my garage as I normally do. The next morning, my 4runner starts and the engine sounds normal but it won't move when I shift the gear from park to drive. I stepped on the gas while leaving the gear in drive, the rpm moves but the 4runner stayed in the same spot.

16 Vehicle Speed Control problem of the 2012 Toyota 4Runner

Failure Date: 09/18/2019

Approximately 7:45 am on 9/18/19, my son while coming home from work, had an accident. I was not with him at the time. When he showed up at home after the incident, he explained how he was unable to stop the vehicle before it collided with the vehicle in front. He articulated that when he applied the brakes, it was as if he vehicle sped up. Remembering years back during a presidential campaign, this was an issue with some other Toyota's. The verdict was the place mats interfered with the pedals. When I released the vehicle from the tow lot, I checked and this was not the case as the floor mat was secure. I have driven Toyota vehicles since the early ninety's and have owned five. Checking for recalls as a result of this I found multiple incidents on record. The panic braking system surprised me the first time I experienced it. Low speeds (25mph or less) I thought a true asset but because of the quick stopping, higher speeds worried me. Never needed the panic brake but once. Some how, I believe the communication (ai) with the panic, anti-lock, braking system, and the way it communicates speed and conditions is the culprit. The insurance company is in possession of the 4runner and I still have the title. Please contact me to advise how to proceed. They seem anxious to get the title but I am animate to know if conditions or vehicle defects played a part in the accident. When I mentioned this to the insurance company, they avoided the issue.

17 Vehicle Speed Control problem of the 2010 Toyota 4Runner

Failure Date: 03/04/2019

03/04/2019- I was driving down a dry road in santa fe, new mexico traveling at approximately 35 to 40 mph. A gradual turn, and I stress gradual, came up and upon turning the wheel to take the slight curb the vsc/ anti slip braking kicked in and the computer sensed I lost controlled at which point it took over and locked the brakes up. This action taken by the computer took away all of my control, straightened my car out sending me off the road into a dried up river bed totalling my truck completely. Air bags deployed, front end destroyed, thank god I wasnt seriously injured, but trust me when I say it couldve been much much worse. This problem needs to be addressed. Ive read multiple, almost identical stories involving the same problem with the same year. This is dangerous, unpredictable and has left me vehicleless in a financial bind. I will also mention this wasnt the first time this happened unwarranted. This just happened to be the first time it caused serious issues.

22 Vehicle Speed Control problem of the 2015 Toyota 4Runner

Failure Date: 05/05/2018

I have a 2015 4runner (VIN: jtebu5jr5f5276737) I purchased brand new. It currently has less than 50k miles on it. A few weeks ago it started periodically going into limp mode while driving at highway speeds. My two toddlers were in the vehicle the first time this happened; I almost lost control since the tractional control disabled concurrently. I took the vehicle to multiple mechanics who confirmed the diagnostic code p2445 (secondary air injection pump). I was shocked to hear this because the vehicle is relatively new and exceptionally well maintained. I looked online and the failure of this component appears to be a systemic issue with Toyota 4runners and tacomas. The fact that this component failed aftera short duration in such a sudden and violent way is very dangerous. I submitted a formal complaint to Toyota and they denied knoweledge of this issues despite hundreds of folks reporting identical problems on their 4runners & tacomas. For a nominal fee per vehicle Toyota could fix this issue, which also has environmental implications, but instead they are being disengenous and denying this major systemic safety hazzard.

23 Vehicle Speed Control problem of the 2015 Toyota 4Runner

Failure Date: 02/03/2018

Not sure. Wife was driving 2015 4runner into parking lot at low speed and it jumped a tree. Not at all saying it is a Toyota issue - just to be on safe side reporting. Air bags did not deploy. Understanding is it just ended up there when she was parking. Epic photo - all the same - worth documenting. She advised she was stopped or just stopped prior to the tree-climb. Pax side was daughter and taking her in for medical today due to complaints of pain and weakness. Police came but did not issue report.

24 Vehicle Speed Control problem of the 2016 Toyota 4Runner

Failure Date: 12/11/2017

Throttle control system failure on 2016 Toyota 4runner. While driving the vehicle the orange service engine amd trac orange loght illuminated. Once that happen d the throttle body became totally non-responsive. No matter how much I pressed the throttle it was non-responsibilve. I was able to safely pull off the road. I had to restart the vehicle twice, on the second restart but the warning lights remained on but throttle appeared to be functioning normal. The dealership was able to replicate the issue and advised the vehicle was not safe to drive and provided a loaner. The service tech and service advisor explained how rare this is especially on a 12 month old vehicle with under 30k miles on it. They ordered a new throttle body and we'll see if it fixes the issue. Given Toyota's recent history with sudden accelleration I thought I should report this just in case.

25 Vehicle Speed Control problem of the 2015 Toyota 4Runner

Failure Date: 10/18/2017

When accelerating through a turn from a stop, if for any reason you lift off the accelerator pedal and then reapply pressure to the accelerator pedal, no acceleration takes place. It is if you are not pressing the accelerator at all, no matter how hard you press. This has almost resulted in several collisions because the car is no longer accelerating and is just coasting. To correct this condition, you have to completely left your foot off the accelerator and reapply pressure. Sometimes you have to do this more than once. Very dangerous. I reported this to my Toyota service rep, and he said his does the same thing. It could be the throttle position sensor, or four wheel drive software, or . . .

26 Vehicle Speed Control problem of the 2016 Toyota 4Runner

"takata recall " also sticking gas pedal.

27 Vehicle Speed Control problem of the 2001 Toyota 4Runner

Failure Date: 05/19/2016

We were told today that our 2001` Toyota 4-runner has frame rot that is inconsistent with the age of the vehicle and the condition of all the other parts on it. Frame rot that constitutes a danger to our safety when driving it. The repair technicians advised us to determine if it was part of Toyota's frame rot recall with the tacomas and other vehicles because the frame rot rust was consistent with the pattern of damage they had seen on other recalled vehicles. When we looked up our 4-runner in the recall information, supposedly Toyota hasn't included our truck in the recall. We were advised by our auto repair specialist that the recall covered damage in trucks that were the same age and overall condition, so it's not as though the rest of the truck is eaten up by rust and we just happened to complain about the frame rot. The frame rot pattern found on our 4-runner is consistent with the frame rot found on recalled Toyota trucks and inconsistent with the rest of the vehicle's stability. Because this is such a dangerous situation, and because the frame rot is a known problem, it's important for other consumers to know that regardless of their care of the vehicle, the frame may rot at a rate that is inconsistent with the rest of the vehicle, and it costs lots of money to repair. 4-runner owners should have their vehicles checked for premature frame rot, and Toyota should do the right thing and fix the frame rot in the 4-runners. Just thought that after we were made aware, so should other consumers and safety organizations.

28 Vehicle Speed Control problem of the 2006 Toyota 4Runner

Failure Date: 12/03/2015

Sitting stopped to pull onto a very busy road. Car engine went crazy, had foot on brake already. Car engine rpm off the chart. Our car was shaking and bouncing. No there was no floor mat stuck no foot on gas. It was scary! my husband shifted to neutral keeping his foot firmly on brake. No change, he shifted to park and turned the car off. In a few minutes we started the car seemed fine drove straight to Toyota rented a car and said fix it. I do not feel safe and we are scared to drive the car. I thank god we were parked. Just fyi our auto has about 67000 miles and is in perfect condition. We have all maintenance done at dealer and last year did extra repairs couple of thousand dollars of recommended work, struts anything they said needed. Then we thought the leather looked a little worn so we had it replaced. Consume r reports says 4runner is one that you could get 200,000 milles. We were hoping for maybe one hundred. Don't care how old our well maintained auto is this should not happen!.

29 Vehicle Speed Control problem of the 2015 Toyota 4Runner

Failure Date: 04/25/2015

Approaching stop lite (40mph. ) applying brakes, accelerator went up to 3000 rpm,s with brakes applied. Pumped brakes finally rpms came down. There is nothing blocking accelerator(floor mat). Separate incident same day backing out of parking spot went into traction control mode with brakes vibrating and clicking on dry pavement.

30 Vehicle Speed Control problem of the 2005 Toyota 4Runner

Failure Date: 03/01/2015

The contact owns a 2005 Toyota 4runner. The contact stated that while attempting to park, the vehicle abruptly accelerated and crashed into three unoccupied vehicles. The vehicle was not taken to the dealer for diagnostic testing. The contact did not sustain any injuries. A police report was filed. The manufacturer was not made aware of the failure. The contact mentioned that the vehicle was repaired under NHTSA campaign number: 06v253000 (speed control) several years ago. The failure mileage was 49,000. The VIN was not available.

31 Vehicle Speed Control problem of the 2010 Toyota 4Runner

Failure Date: 02/01/2015

Vehicle randomly and suddenly revs itself both from a stopped position and while driving. When driving downhill and I take my foot off the gas the vehicle will continue to rev itself around 3000 rpm then when I start to travel uphill it will down shift to about 1500 rpm. This seems backwards. Tapping brakes does not cause it to downshift as needed, does not seem right to apply brakes and vehicle continues to rev itself higher nor does it seem right to take foot completely off the gas pedal and the vehicle continues to rev up. The only way to get it to idle is to apply pressure to the gas pedal and speed up which is dangerous when already going downhill. When travelling in a parking lot it rev itself up and will jump forward. From a stopped position such as in my driveway or at a stoplight I will press the gas and it will not go into drive but it will rev up to about 3500. None of these issues can be duplicated by the dealership of course. At one point they finally changed out some type of component in shift mechanism and that worked for about a week then the problem started again. There is never a code indicating there is anything wrong although the trac light will randomly come on and not turn off. I have been driving for 30 years and know when a car is not driving properly. This is my 2d 4 runner. First was an '03 and had similar issues but since was less than a year old I finally convinced the dealership to replace the transmission at Toyotas cost. This one was purchased used and has more miles on it but it seems to be the same problems as my original. The only way I have been able to get dealership to believe me is to ask the service manager to drive my vehicle for several days then they are like OH I see what you mean. Another incident was on a snow packed road. Travelling 10mph when the vehicle downshifted I lost control.

32 Vehicle Speed Control problem of the 2004 Toyota 4Runner

Failure Date: 01/13/2015

2004 Toyota 4-runner. Consumer writes in regards to accelerator pedal recall. The dealer informed the consumer that since he had interior package, including a sport accelerator pedal, the pedal could not be modified. The consumer was informed the silver would have to be stripped off the pedal and then modified, but the silver part of the pedal would not be replaced. The consumer would then have the silver sport package on all the other pedal interior pedal, but would have a thinner, black, accelerator pedal. The consumer chose not to have the recall performed.

34 Vehicle Speed Control problem of the 2004 Toyota 4Runner

Failure Date: 08/09/2014

Driving on the interstate in north carolina vsc light turned on, tsc light turned on, abs light turned on, brake light turned on. Car was drivable for a short period of time then braking became extremely difficult. Limped home with my 3 year old in her car seat and luckily was able to do so without incident as given time of day and locaiton from home, very light traffic made travel possible. Several times though I contemplated stopping and calling a tow service. Took the car to the dealer, it's a $3000 part the abs module and $700 installation price.

35 Vehicle Speed Control problem of the 2007 Toyota 4Runner

Failure Date: 07/29/2014

Over the past 9-12 months I have owned the 4runner, I have noticed the floormats slide up behind the brake and acceleration pedal when I am constantly switching between the two (eg. Heavy traffic, long trips, etc. ). I usually am able to pull them back down at a stoplight or stop sign; however, on Tuesday, July 29th, I was involved in an auto accident that was not only in heavy traffic, but while on a 200+ mile trip as well. I believe that the floor mat caused my car to have limited braking ability in addition to possible increased acceleration. The NHTSA's findings seem to be consistent with this. Additionally, the complaint with NHTSA id number: 10384563 states that "I pushed on the brake. . . But the car continued accelerating" and then "Toyota inspected the vehicle. . . And said they adjusted the mat" is one of many that have findings consistent with the malfunction that caused my wreck.
