1 Engine And Engine Cooling problem

Failure Date: 03/01/2026

Head gasket failure. No check, engine, light or warning. My car was very well maintained regularly. After research, I found this to be a very common problem. Yes it is available for inspection. Upon request. The problem was confirmed by chemical test, ignition vapors going into engine coolant. Other symptoms were misfiring on cylinder two and cylinder four. Severe engine knocking.

2 Engine And Engine Cooling problem

Failure Date: 12/18/2025

Misfire code along with rough starts/light load. T-sb-0116-15.

3 Engine And Engine Cooling problem

Failure Date: 12/12/2024

The head gasket failed on my vehicle, and began to burn coolant and overheat my engine. Upon description of the issue, a dealership mechanic was able to diagnose the issue. The sudden failure of my vehicle due to overheating while driving was extremely risky, and I had no indication that the issue was a head gasket until a dealership mechanic was able to look at my vehicle after being towed. I had taken my vehicle to a third-party mechanic, and they could not tell me why my vehicle was burning coolant, but provided a temporary stop-leak fix that did not hold. The first appearance of the check engine light was about 9 months ago, was assumed to be the spark plugs, which I had replaced, but then my coolant light started coming on with the check engine light for about a week before the engine overheat incident. The vehicle has been inspected by a Toyota hybird mechanic. No other inspections have been done. My safety and other's safety was put at-risk as my steering, speed control, and speed monitoring systems immediately shut off once the system detected an overheat, making it difficult to pull over safely and slowly to address the issue/call a tow truck. I required physical assistance from another motorist to move my vehicle off the road to allow others to pass.

4 Engine And Engine Cooling problem

Failure Date: 05/03/2024

2014 Toyota Prius: while driving at high speed the engine stalled unexpectedly, steering and braking were not effected. Was able to safely coast to a stop in the breakdown lane and after a few minutes, restart the engine and proceed driving. Another event occurred on the entrance ramp onto an interstate highway. I was able to pull over safely and the car restarted after a short delay. This seems to be a very dangerous condition. Toyota dealer service said they could not determine the cause and they know of no recall. I found online a reference to NHTSA recall # 20v369000 that referred to "unexpected hybrid power system shut down. " that is what happened to me.

5 Engine And Engine Cooling problem

Failure Date: 08/29/2023

The car began to jerk, made knocking noises, miss fires on cylinder 1, 2 and 4, coolant leakages, oil leakages, compression issues and the check engine light keeps turning on and off. My safety has been put into play because, the car would jerk many of times as it would want to stop as I was driving. I would then have to pull over and call for assistance. Per the Toyota service advisor he recommended that I needed a new rebuilt engine kit. The extended warranty company stated that I was negligent with my car and denied my claim and refused to pay for the 10 grand motor and I had to tow my car to another shop. However, the car at this time is still experiencing the same malfunctions. The warning lamps are still showing at this present time and prior to the failure of problem. This car meets the "lemon law guidelines, however, I have been informed otherwise that I don't meet the criteria for lemon law due it being expired sometime last year in oct of 2024. As stated this is a lemon car.

9 Engine And Engine Cooling problem

Failure Date: 01/27/2022

My car stalled while I was driving it on 2 separate occasions. It is a 2014 w less than 70k miles: 1/2022 within seconds of 2 or more warning lights going on, the engine stalled. I was able to steer, brake and drive on the electric motor towards home. The electric motor drove another . 5 mile before it also died, leaving me with no power steering or brakes. I was on a slight incline and coasted most of the way off road. The car was towed to our local dealer, which said the intake manifold had failed. Dealer replaced im and related hoses and gaskets. 6/2022 I was driving on a busy interstate at high speed in the passing lane. The car started to lose power and decelerate. I maneuvered to the side of the road and up! the on ramp. Made it to roadside business on remaining battery power.

10 Engine And Engine Cooling problem

Failure Date: 11/07/2021

While driving on the interstate yesterday my Prius computer shut down entirely.

11 Engine And Engine Cooling problem

Failure Date: 10/27/2021

While driving, I received a warning "hybrid system malfunction". Within seconds, the vehicle powered off and the steering wheel locked. I had no control of the car and came to an abrupt stop in lane 1 of a two lane road. I was fortunate that it was late in the evening with few cars on the road, had I been on the highway during rush hour the outcome, due to loss of power and control (steering), could have been catastrophic and perhaps fatal for me or other commuters. I note that in 2014, Toyota notified NHTSA of a recall to, "rectify software in two of the Prius's electronic control units, one for the motor/generator and the other controlling the hybrid system. " further Toyota said, "in rare circumstances, it could shut down the hybrid system itself, resulting in loss of power and the vehicle coming to a stop". This is exactly what happened to me the other night. Toyota claims, in 2014, that "it has received no reports of accidents or injuries related to the problem. " this seems to be an extraordinarily amount of good luck as I could have been easily rear-ended the other night. Curiously, in this NHTSA data base, another owner reported an incident (11439720) on November 8, 2021, that is similar to the experience I had just days prior. And our vins are eerily close. Coincidence? not likely. Does the 2014 recall apply to my car and if not, why not given I experienced the exact same symptoms?.

12 Engine And Engine Cooling problem

Failure Date: 09/23/2021

The contact owns a 2014 Toyota Prius v. The contact’s grandson stated while driving approximately 15 mph, the vehicle failed to accelerate while the pedal was depressed. The contact stated that the check engine warning light was illuminated. The vehicle was taken to a local dealer where it was diagnosed with needing the head gasket replaced however, the vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was 163,335.

13 Engine And Engine Cooling problem

Failure Date: 12/06/2020

Unknown. I was basically driving on the freeway, when my car's engine suddenly stopped running. The car was automatically put into neutral and was not able to control the speed of the car, so I had to move to the side of the freeway. As I was sitting in the car, I started smelling burned wire and interior started to fill with smoke. I immediately turned the car off and got oit. As I was standing far away from the vehicle, fluid started leaking and it had caught into fire. The front bumper was completely burned off, as well as some of the engine and electrical parts were affected as well.

14 Engine And Engine Cooling problem

Failure Date: 06/11/2020

Originally, Toyota issued a safety recall in 2014 to address the inverter defect in model years 2010 to 2014, an attempt to fix the problem on about 800,000 cars in the u. S. By modifying software that controls the hybrid electrical system. It was intended to fix the overheating problems. The inverter controls the power flowing between the battery and electrical motors. The inverter boosts the battery from 200 - 500 volts. They inspected my car saying it was in excellent condition. After, the software upgrade I had major problems. My original Prius battery with around 65,000 miles stopped holding a charge. My 12 volt battery also died shortly after. And my power outlets in my got super hot and spiratually worked off and on. And my driver seat gets hot when I turn on car even though seat button is off. I am sweating all the time. Plus, I have skin conditions and asma. I talked to dimitri, and they tried to charge me 1200for unnecessary repair. Also, failed to call NHTSA as promised.

15 Engine And Engine Cooling problem

Failure Date: 02/19/2020

I turned on my vehicle and when I switched gears, from park to drive, it did not register that it was in drive. When I tried to drive away, there was no acceleration and lack of power. My battery was operating at almost max capacity.

16 Engine And Engine Cooling problem

Failure Date: 07/28/2019

City street . . . Pressed on gas after the light changed and it did not go forward and then waivered for awhile. I was very close to home. I do not know how it will work tomorrow . No warning lights were on. It has acted funny before like this on highway trips and then it wouldn't do it. Toyota dealers do not even know about them and it is annoying. It feels like a transmission slipping but has less than 70,000 miles. I thought maybe heat is doing it but now that it is out of warranty ie over 5 years old, I do not know what to do. I am unsure what causes this since no lights warn anything is wrong.

17 Engine And Engine Cooling problem

Failure Date: 07/15/2018

I was going down the highway at about 70 mph when all the sudden the Prius lost power. The Prius dropped down to about 55 mph. After a short period of time the power picked up. I got off the highway and took the back roads. It was a very hot day.

18 Engine And Engine Cooling problem

Failure Date: 07/06/2018

Hi my name is amjad dawoud. Toyota headquarter case # 1806041860 incident day July 6, 2018 I had a big technical problem with my 2014 Toyota Prius v where it caught on fire because one of the valve lifters fell into the engine head and broke the engine block while driving. When that happened, the oil began to leak into the muffler and the engine and hybrid wiring immediately caught on fire. Many of my colleagues who also bought the same vehicle for taxi business use in san francisco all have a similar issue as mine. A few of theirs caught on fire, and a few others mentioned to me that the head gasket for their same Toyota vehicle blew out and each of them had to replace it at around 120,000 miles. I believe that’s why they don’t manufacture these same vehicles anymore because of the defectiveness in the engine. I opened a case with Toyota manufacturer back in July 2018 and they sent a specialist to inspect the vehicle after the fire. He found the piece that fell inside the engine and that damaged the engine. The Toyota specialist admit to me that the engine of my 2014 Toyota vehicle is very defective and must be covered under warranty. My mechanic replaced the engine for me with a used one with 28,000 miles. This entire case was documented under my name with Toyota motors manufacture. I contacted the highway safety and I tried to file a claim against Toyota at the time, but I never got a follow up on my case after I left various messages. All they said was that they will investigate but never gave me any information regarding the engine fire situation or honor any warranty. I hope this incident will be investigated by your department and have this vehicle under recall. Thank you.

19 Engine And Engine Cooling problem

Failure Date: 04/14/2018

I actually would like to file a concern regarding the 2010 - 2014 inverter/component failures. This problem was brought to my attention by my dealer indicating that the motor/generator ecu could reset, causing the hybrid system to shut down, resulting in my vehicle stopping while being driven and subjecting me or whom ever is driving at risk of a crash. I called Toyota customer service and show little concern for this potential problem but just told me that I should verify with my dealer that the eoe-prius recall to refresh software in the inverter was performed. It was confirmed that it was performed on my car on 2/14/14 at san francisco Toyota and that they tended the warranty for 15 years, unlimited mileage. My dealer told me that some vehicles that have had the eoe-prius recall have still suffered inverter failure. Is Toyota going to wait for someone to die, before they will correct this problem. A very concerned pirus owner! im just wondering if I should sell my pirus before it (Toyota) kills me or someone I love.

20 Engine And Engine Cooling problem

Failure Date: 11/28/2016

Rodents chewed through majority of the wiring under the hood and build nests in the engine. This has happened twice and I have only owned the car for 5 months and cost me a lot of money for repairs which Toyota will not cover and said it is not their responsibility for using these soy based wires that attracts rodents.

21 Engine And Engine Cooling problem

Failure Date: 07/11/2016

Headlights and cabin lights failed to turn off when remote lock was activated. Lights were then turned off manually. Five hours later, in the parking lot, the car was nearly completely dead. Last time it happened, we left it and it recharged itself. This time, it did not recharge itself; it was completely dead the next day. We had to have it towed to the dealership, who "jumped" it and let it sit overnight. They say it now has no problem.

22 Engine And Engine Cooling problem

Failure Date: 08/04/2015

Within the last six months, and about four times, I had experienced rough/high idling during hot days and after driving over 80 miles; using air conditioning. (I work over 70 miles from home. ) I decided to take the car to the Toyota of redlands dealership on August 4, 2015, because the car was displaying the same conditions. While on the way to the dealership, and on the freeway, the car began to idle heavily. I merged to exit the freeway and while on the off ramp, stopped, the car completely shut off. The display read "check hybrid engine. Stop park the car safely. " I was able to restart the vehicle to move from off-ramp traffic and park it. The message on display was still there. I shut off the car and called the dealership. They recommended I have the car towed to them. When tow truck arrived, I turned on the car to position it for the towing bed. The message no longer displayed . The car stayed with dealership all day on August 5, 2015. According to the dealership, the display showed no message and there is no code to indicate a malfunction occurred. They were not able to diagnose the problem. I am keeping the vehicle at the dealership on August 6, 2015, for further evaluation. My concern is two-fold: 1. My safety, 2. If the system displayed a message, why does the system not produce a code for the technicians as a non-hybrid car would? this itself presents a serious issue.

23 Engine And Engine Cooling problem

Failure Date: 04/30/2015

Was driving at approx 20mph on a side road when the car completely lost power and went dead. All the dashboard warning lights lit up at the same time. Lost steering control (the steering wheel became very heavy and was unable to steer). Managed to coast to a stop a 100 feet away (since the car had absolutely no power). Turned off the power button, turned it back on and the car started running again. Took it to the dealer the next day. They ran diagnostics, found codes (listed below) which suggested that the Smart key lost communication with the car. They kept the car for an extra day, ran the diags overnight just to see if they can reproduce but, was told that they were not able to reproduce the problem. No idea what may have caused the issue. The Smart key was sitting on the center console. Below are the Smart key related codes that were recorded: u0100 - lost communication with ecm/pcm "a" u0142 - lost communication with body control module "b" u0155 - lost communication with instrument panel cluster (ipc) control module u0293 - lost communication with hybrid vehicle control system please help investigate this as this is potentially a very serious issue. By god's grace, there was no other vehicle on the road, was going at a low speed and thank god, this did not happen on a highway at highway speed. Thanks.

24 Engine And Engine Cooling problem

Failure Date: 01/05/2015

After parked for 20 minutes, the car is dead, had to jump start. Battery was replaced by the dealer, but the issue was not taking care of. At least 3 more times since the battery replacement, I found my car was again ded after parking, can not even open the door with remote. But all 3 times the car was able to power on after a few minutes on it's own while waiting for tow. . . Above that, the clock, trip odometer and temp settings were resetting on random times since 650 miles, as well as the radio which will turn on and off at random. Dealer replaced the radio but it still does the same. I was told that they (dealer) can't tell anything wrong with the car, and can't do anything about it, unless it's dead while it's in their shop, even after I showed them the photos and videos of the issues.
