1 Equipment problem

On Jan 5, 2026 I was driving locally at about 40-45 mph. All of a sudden 3 indicator lights came on abs, brake, and brake system warning light. There were no noises, but the brakes felt different. They did stop when needed. I left space between myself and others just in case, did not want an accident. Called my regular mechanic. They advised to not drive the car, but to bring it into the shop at 8am the following day. On Jan 6th, the mechanic stated it was beyond their expertise. It was towed to the local Toyota dealer for repairs. It ended up being the brake booster and pump. Upon investigation online, we saw this was a recall, but we were not notified by Toyota.

We were driving on a city street about 25-30mph and the car jerked suddenly and the lights on the dashboard illuminated and the "check hybrid system" message came up as well. The car was able to coast to a nearby parking lot. We attempted to drive the car home and the car stalled in the middle of the street. We were able to push the car off the road and then get it towed. The presented a potentially very dangerous situation as the car had a complete failure while driving in the middle of the road. The Toyota service department ran a diagnosis which returned dtc p0a94. All of this indicates that the car should be covered under the customer confidence program 23te03 however that program seems to only cover the 2010-2015 Prius and 2011-2017 Prius v models and not the Prius c. There are several reports of this same issue happening on the Prius c as well though so it doesn't make sense that it is not covered.

Cruise control not working. Traction control light active, brake light active, and light active the cause was diagnosed by Toyota of paris. TSB for c1391 code abnormal leak in accumulator. The corrective action is to replace brake booster assembly with master cylinder and brake booster pump assembly.

While driving my 2014 Prius v it stalled. I turned it off and restarted it. The yellow car symbol with an exclamation point lit up. I turned the car on and off again. The light went away and I was able to drive home. I was unable to get any obd 2 codes on my personal reader. I changed the 12 v battery and thought this had fixed the stalling. Then it stalled again and repeated the same process. Though we were able to drove the car I had it towed to a dealership for safety reasons. The dealership pulled codes, cleared them, drove the car 20 miles, and were unable to reproduce the codes. I was told to return when the codes returned. It stalled again and now I am afraid to drive this car.

My 2014 Toyota Prius c experienced a complete hybrid inverter failure. The part number of the inverter is the same as the one covered under Toyota’s inverter recalls (such as recall j0v), yet my VIN is not included. The failure created a serious safety concern, as the vehicle lost power while in motion. Toyota’s recall appears to be too limited in scope and may need to be expanded to cover Prius c models that share the same faulty part. I am requesting that NHTSA investigate this issue, as it appears to involve the same hardware defect acknowledged in other Prius models.

2014 Toyota Prius: while driving at high speed the engine stalled unexpectedly, steering and braking were not effected. Was able to safely coast to a stop in the breakdown lane and after a few minutes, restart the engine and proceed driving. Another event occurred on the entrance ramp onto an interstate highway. I was able to pull over safely and the car restarted after a short delay. This seems to be a very dangerous condition. Toyota dealer service said they could not determine the cause and they know of no recall. I found online a reference to NHTSA recall # 20v369000 that referred to "unexpected hybrid power system shut down. " that is what happened to me.

Got rear end on 08. 21. 2023. Was able to drive car to work next day. Once I got home the abs, traction control, and brake light went on. Using obd2 scanner. Code 16-17 right--rear wheel speed sensor electrical noise or intermittent interruption, code 16-17 left-right wheel speed sensor malfunction, 61-01 modulator-control unit initial ig low voltage, 65-33 brake fluid level switch stuck on, 83-19 ecm/pcm relation failure, ct1203 ecm/ev communication circuit, c1345 linear solenoid value offset learning undone, c1392 zero point calibration of stroke sensor undone. Tow car to Toyota and they recommend to replace brake booster with master cylinder assembly. Ran me $2968. 41 bill which adjuster claim wasn't cause by rear engine damage. . Read more...

At a stop light, when the light turned green, I took my foot off the brake and pressed the gas pedal but the car did not advance forward. Instead it was as though the car was in neutral and it began to roll backwards because I was on a slight slope. I attempted to get the car to move forward by engaging in shifter into drive, but nothing changed (still as though it was in neutral). I heard a whirring sound but couldn’t tell where it was coming from. The only way I could get it to drive was to turn the car off and restart. This happened twice before I could get where I was going. Fortunately it didn’t result in an accident but I’m afraid to drive the car now.

I was driving home from work on 10/14/22. The traffic was moving very slowly because there was a wreck ahead and emergency crews were on the scene. I was having to break often while moving at a slow speed. All of the sudden multiple dash warning lights came on the vehicle and the vehicle no longer responded to my breaking in a normal manner. I pulled off of the hwy and drove to my local dealership on a side road at a slower than normal speed. I told the dealership to check my car for the recall I remember getting in the mail a while back that mentioned the car trouble I was experiencing. I was told that the recall did not apply to me because my car was over 150,000 miles. The local dealership indicated they could not assist me with having Toyota honoring the recall past the specified mileage. I contacted the national Toyota number and was told the problem was included in a "customer support program" not an actual recall from Toyota and that Toyota would not pay to fix my vehicle since it was over 150,000 miles. I see that some people have filed a class action lawsuit against Toyota in this matter. I was told the brake booster needed to be replaced and also a break pump assembly would be required with an estimated cost of $2,500. The car is not safe to drive as it is right now. Toyota is aware of the problem and I guess put the "support program" in place to try and get around a forced government recall and also in turn put a cap on the mileage out of the reach of many vehicle owners. I would like the government to make Toyota actually recall the vehicles that have this problem and pay to fix my vehicle with this issue.

Blizzard pearl white paint started peeling off the car's body around the top areas of the 4 side doors (passenger & driver sides). There seems to be an active recall on this from Toyota for the same paint color and type, but the Prius model is not included in the active recall for this same condition. It has become a hazard such that such paint chips can be ingested by children. I took it to the santa monica, CA Toyota dealership and it was verified by the dealership representative there that this was a condition caused by a defect in the original paint with exposure to sunlight.

Going 70 mph coming down a steep hill on a freeway in solid traffic, the cars ahead in my lane suddenly stopped. I had to slam on the brakes. My foot was on the brake and my hands on the wheel. When the car ahead started moving, I put my foot onto the gas pedal but the car would not move. All that I saw on the dashboard was a light green long arrow pointing left (not the turn signal higher on the dash). I managed to roll into the left shoulder. I never turned off the car but at that point put it into park. (before that I had not touched the gear shift. )called 911. Called them again 10 minutes later as no one had arrived. She asked if I could drive the car. At that point I put the car into drive and managed to drive a short distance in the shoulder. (while I was driving before the sudden stop I was not in cruise control as there was too much traffic. ) I managed to drive home without any difficulty. My 2014 Prius had a loss of power and check engine light came on and msg check hybrid system appeared while started in the morning and drove out of my garage. That day I went to san francisco Toyota and they did a diagnosis and told me that my hybrid system battery was bad and need replacement and quoted $3600. I asked that if they can do the recall because I was 90% sure that the problem was due to the recall, but they told me that they cant unless the error code clears out by replacing the hv battery. I drove my car back home and the next day I went to a hv battery specialist and found out that my hv battery is in good condition and doesnt need replacement. Now I am 100% certain that the problem is due to the recall but Toyota is not fixing the issue they are saying its the hv battery which is not. The Toyota of s. F not only diagnosed my car wrong they tried to rip me off by spending $3600 on a hv battery that doesnt need any replacement! on top of that they are refusing to do the recall.

The car would not accelerate as expected. It behaved as if the fuel injector was not working properly. We were not able to keep a consistent speed and the motor started to have a rough idle sound. The hybrid system stopped working, the icon of the batteries turned purple and would not recharge when stopping. We took it into the dealership, the service person they said it was a software and emissions miss-fire issue. It will take a part to be replaced and software updated. Toyota sent out a memo to them stating this might happen and this is how to fix it. It seems since they are warning service staff to be aware of this that maybe they should take care of it before it happens and not have me pay over $900 and the stress of it happening on the freeway and then having me get it fixed.

Dear NHTSA - my name is anand; I reside in CA. 1. I have a 2000 Toyota camry & a 2014 Toyota Prius. 2. The 2000 Toyota camry has the day time running lights as a standard feature. 3. The 2014 Toyota Prius does not have the day time running lights as a standard feature. 4. In states like CA & I bet in other states - day time running lights & head lights getting turned on with limited sun light should be made mandatory. 5. This is more of a safety feature than an option. 6. It will help prevent accidents, it will help compliance with the law where applicable when it is raining & head lights are mandatory 7. It will also save anyone from getting traffic violation tickets 8. Can you all pls look into this & help review to make this as a mandatory feature? thanks in advance anand.

The edr report clearly showed that the brake oil pressure changes did not correlate with the progressively increasing vehicle deceleration. The issue is occasional, rare and cannot be reproduced every single time. This leads to disparity in assumed braking distance from routine experience by driving the vehicle and actual braking distance at the time of failed braking which supposedly may be as a result of inverter issues that are responsible for exclusively regenerative braking at low (<50 mph) speeds.

Hi my name is amjad dawoud. Toyota headquarter case # 1806041860 incident day July 6, 2018 I had a big technical problem with my 2014 Toyota Prius v where it caught on fire because one of the valve lifters fell into the engine head and broke the engine block while driving. When that happened, the oil began to leak into the muffler and the engine and hybrid wiring immediately caught on fire. Many of my colleagues who also bought the same vehicle for taxi business use in san francisco all have a similar issue as mine. A few of theirs caught on fire, and a few others mentioned to me that the head gasket for their same Toyota vehicle blew out and each of them had to replace it at around 120,000 miles. I believe that’s why they don’t manufacture these same vehicles anymore because of the defectiveness in the engine. I opened a case with Toyota manufacturer back in July 2018 and they sent a specialist to inspect the vehicle after the fire. He found the piece that fell inside the engine and that damaged the engine. The Toyota specialist admit to me that the engine of my 2014 Toyota vehicle is very defective and must be covered under warranty. My mechanic replaced the engine for me with a used one with 28,000 miles. This entire case was documented under my name with Toyota motors manufacture. I contacted the highway safety and I tried to file a claim against Toyota at the time, but I never got a follow up on my case after I left various messages. All they said was that they will investigate but never gave me any information regarding the engine fire situation or honor any warranty. I hope this incident will be investigated by your department and have this vehicle under recall. Thank you.

�takata recall � I believe there is a major problem with Toyota Prius v they make it only for five years, they stop making it after 2016 because they realize there is so much problems in the engine, many car this model the engine get in fires, head gasket had to be replaced at 125,000 mile. Hybrid battery it go bad at 120,000 miles where is most Prius hybrid battery go for 250,000 mile minimum. My Prius be in gin GA on fire because there is a piece of metal from the engine fell down in the engin block and the engine break down with a big hole and most of the engine oil came down in to the muffler and my car got on fire. And that's happened I was driving on the freeway and there is no check engine light came on as a warning when that piece of metal from the valve lifter I believe fill down in to the block.

We are driving home in the 55 south with my family I had fourth month old and 2 year old and my wife all of sudden the car was stalled in the carpool lane. It was scary part me what can do. I need to react fast. We where thankful that we pulled out safely. The next day I went to the dealership to inspection and they take a while to check so I left my car for two days. And they said that five different codes and I'm gonna pay 3k just too take it apart and plus labor and parts. I tell my story what happened they want me too pay. The was well maintained and that day all the lights where on the dashboard. I called the Toyota headquarters on costa mesa. But they said my warranty where expired. So cannot helped me. But for family safety I just wanted to say if just fixed the defects on the car or just recalled. I still had the car and able to drive. I filled this today because I happen again.

The check engine light came on 11 months after it had previously come on and supposedly fixed (April 2017). Now I am told it is an evap problem again ( evap pump and purge flow valve weakness). The recommendation is replacement. A internet search revealed that this same problem in australia caused a recall. This is a $950. 00 replacement and my vehicle has only 57700 miles. This is shameful.

Inquired re law suit related to inverter failure and recall. Learned my warranty was extended to cover possible failure of inverter. No repair was made. Other party contends part failure is dangerous. Toyota says it is rare and car will limp home. I say extended warranty is insufficient reaction and portends dangerous failure. I say fix the inverted. My inverter has not yet failed. Power drop in traffic is dangerous.

The hybrid system on the involved vehicles contain an electric vehicle (ev) fuse which may malfunction if the vehicle is repeatedly operated in ev mode under high-load driving conditions, such as during a long hill climb. If the fuse malfunctions, warning lights and warning messages will illuminate. In some cases, the vehicle can be driven, but with reduced power. N other cases, the hybrid system could shut down resulting in the loss of motive power. Power steering and braking will not be affected. Loss of motive power while driving at higher speeds can increase the risk of a crash.

I was traveling on an interstate highway using cruise control and going about 72mph. The Prius suddenly shut down. At first I thought I had bumped the cruise control so I tried resetting it but nothing happened. I stepped on the gas and nothing happened I continued to slow down. Made it to the side of the highway where I sat wondering what had happened. After shutting the car down I restarted it and it seemed to be ok so I continued my trip. Never forgot the episode.

I was driving on a freeway off ramp when the power just stopped. It did not slow but stopped and I managed to pull over. The warning signals all came on and so I could not look up the problem. Since then they say there is a problem with the inverter but the dealer will not fix it even though we are under the original warranty as well as buying an extra certified platinum warranty for $2000!.
