As the driver approached a red light, there was a loud noise. He planned to get through the intersection, pull over to investigate. When the light turned green, he accelerated and the front left wheel popped off the axle and rolled away. The truck fell and hit the pavement. Upon investigation it was determined that three lugs broke off simultaneously and were not located at the scene. Two lugs remained but broke off when the vehicle attempted to move. The vehicle had four new tires installed on 12/27/12, and oil change and new brakes installed on 12/31/12. At the time that work was done, the mechanics did not observe any issues with the lugs. The wheels are alloy aftermarket installed by the dealer before the vehicle was sold. The vehicle had just exited the highway when the incident occurred. The policeman on the scene said the driver would not have survived if this happened five minutes earlier when he was still on the highway. The contact owns a 2006 Toyota Tacoma. The contact stated that after experiencing a flat tire. He attempted to use his lug nut wrench which would not remove the lug nuts as needed. Another vehicle came along and offered some assistance with a larger lug nut wrench. The vehicle was later taken to an independent mechanic who advised him that the lug nuts had expanded making it difficult to use the wrench. The vehicle was repaired by the tires being replaced. While rotating the wheels on my 2006 Tacoma pick up truck the lug stud snapped. I was using a torque wrench and the stud snapped at less than 85 ft-lbs of torque.
: the contact stated while driving at 40 mph, the front passenger wheel came off the vehicle, causing the vehicle to leave the road and strike other vehicles parked on the side of the road. The police filed a report at the scene. Both the driver and passenger received injuries, although both were wearing seat belts. The vehicle was deemed a total loss by the insurance agency. In the month of June I took my truck into a reputable service center for our states required state safty inspection. During this inspection the tires must be removed for the brake inspection. While the inspecter was placing the tires back onto the truck, two of the wheel studs broke on the front hub. He brought them in to show me the problem and stated that this happens on almost every 2000 - 2003 Toyota pickup they work on. He even showed me that the stud wasn't cross threaded and removed the stud from the lug nut using a pair of pliers. The stud came right out with little pressure. The studs were replaced with new studs from a locale auto parts store. The old studs were thrown in the trash.

I would like to make you aware of a problem I recently have had with a 2000 Toyota Tacoma pick-up. I have 15000 miles on the truck and recently on a long road trip I received a flat due to a nail on the highway. In changing the tire I snapped 2 lug studs. Now I have owned truck for 20 plus years and have never snapped a lug off a wheel. I had it serviced and was told it was due to too much torque and still find this hard to believe. I cant imagine that I could give this that much torque with a tire iron to snap off the studs. I feel it is poor quality steel or a failure in the steel design itself. I am very concern for future lugs breaking when traveling and fear a major safety hazard. Please look into this matter and advise me if you have received similar complaints and what can I do to protect my family and me. Thank you.

Date of purchase Toyota of somerset reported this was a second buyer. They reported a lady had owned the vehicle. Through carfax I found me to be the 5th owner. They did not have the warranty information in with the manual information. They did not supply this at time of purchase and has not offered upon request. They said they would honor the service. Upon the reverse coming out there was disagreement and the service manager, mr. Turpin and mr. Johnson were very adamant and refused to honor the mfg. Warranty on the drive train. They first asked $700 and then negotiated for $450. Which I refused. Needless to say we do not have any correspondence but I still have to drive without reverse in my vehicle. This is why I did not report the lug nuts breaking and causing my wheel to fall off and causing damage to the truck. Toyota of somerset does not offer assistance and accuses abuse.
Consumer was driving on the highway when the right wheel came off. But, brake pads and rotors were still attached.
