On [xxx], I experienced multiple failures of core safety systems in my 2021 Audi Q7 that put my 17-month-old child at serious risk and exposed critical flaws in how the vehicle’s systems operate and record events. On [xxx], the pre sense automatic braking system suddenly engaged while I was driving approximately 35 mph on a clear, empty suburban road. There were no visible obstructions, no warning lights, and nothing recorded in the vehicle’s logs. This occurred less than 48 hours after the vehicle had received recall-related software updates and routine service at an Audi dealership. Prior to that service, the vehicle had no history of mechanical or safety issues. The following day, the keyless entry and anti-lockout system failed. I placed my toddler in his rear-facing car seat behind the front passenger seat and set our baby bag—with the key fob inside—on the rear seat. After closing the door and walking to the driver’s side, the car unexpectedly locked. The fob and my child were both inside. The outdoor temperature was over 90°f. I attempted to open the vehicle and then to break the window with tools, without success. After approximately seven minutes, I broke the driver’s side window with a sledgehammer and removed my child from the car. The car alarm eventually activated, but no lock-in event, key fob error, or warning was recorded. The vehicle was inspected by the dealership twice and Audi corporate engineers once. Audi confirmed faults in the emergency call module, comfort system, rear lid sensor, and parking sensors. However, they could not reproduce or explain the braking event or lock-in failure. Audi confirmed that the vehicle should not be able to lock with the key inside, and they do not know why it did and are unable to guarantee the safety feature will work in the future. The vehicle has not been driven since. The failure of both safety function and system visibility placed my child in danger. I am requesting a safety investigation. Information redacted pursuant to the freedom of information.

Twice, without logically-relevant symptom or warning, when the vehicle attempted to accelerate from a low speed in the middle of a busy intersection, the aeb incorrectly activated, suddenly stopped the vehicle, startled the drivers, and nearly caused accidents. Turned out, this was known behavior caused by a design defect software bug in the "door control modules" for which a software update had been released but never publicly announced. Audi charged us $315 for the software update, but since it is a manufacturing defect causing a critical safety issue, it should be covered by a safety recall, all owners of Audi vehicles with the affected "door control modules" should be proactively notified of the software update and should receive it without cost.

I am reaching out to raise a serious safety concern regarding the Audi pre sense feature in 2023 Audi models. This feature has caused sudden, unexpected braking on both freeways and internal roads, creating a significant hazard for drivers and passengers. There appears to be no permanent way to disable this feature, and its malfunction poses a critical risk of causing accidents rather than preventing them. I urge the NHTSA to investigate this issue and ensure that manufacturers provide a safe and reliable solution, including an option to disable the feature if needed.

Often brakes abruptly. The first time the car was set at 60mph adaptive cruise control when the car abruptly braked to almost a stop on a divided four lane hwy. I reported incidents to Audi cary & put my car in for service. They did not find diagnostic conclusion. After picking up my car the problem continues. While on adaptive cruise control set at 62 mph abruptly decelerates to 17, forcing driver to pull into right turn lane. Again on same afternoon car braked abruptly with another car following closely behind, the driver accelerated, there was a pause where braking stopped but car did not accelerate, then the car accelerated avoiding being hit from behind. The last two incidents I immediately call Audi cary to report the problem. I now journal all episodes of malfunction.
The auto brake suddenly braked hard and it was scary. The car reported a collision warning red light for a few seconds. Luckily, there was no car close behind me. While I was driving at about 70 miles per hour on the highway, there was no car in front of me to worry about. There was probably a car driving in the next lane. After the sudden brake, everything seemed to be back to normal. This type of sudden auto brake happened once in late 2019, on a local drive, I thought it was a falling leaf or something. But this time on the highway, there was nothing in front the car.

Car was parked, no cars or objects within approx. 40 feet (to rear). Started car and placed in reverse. Backed up about 3 feet and brakes activated automatically and fully. Warning lights flashed on the dash. Car was immobilized. Shifted to drive and pulled forward. Shifted to reverse and the same thing occurred again. Repeated two additional times with same result. On the fifth time we backed up normally and without incident. Did not ever shut the ignition off. It was raining at the time.
While traveling all alone in a 2 lane road on a rainy evening the brakes applied themselves violently. Same has happened when backing of my home garage and the car senses rain and the brakes violently. This is very distressing and the seatbelt pulls back very hard causing fear, confusion and pain.
The car's parking brake system is not engaging when a door is opened while the car's transmission is in "drive. " normal operation requires the parking brake to be engaged when the door is opened. Audi is aware of the issue and has issued a software patch to flash the car's firmware. See https://static. Nhtsa. Gov/odi/tsbs/2019/mc-10158462-9999. Pdf however, this is only done for cars in warranty. Since this is a safety concern it should be done for all cars as a car can roll when a door is opened and the parking brake is not automatically engaged as originally designed.
Forward collision engaged on a highway with no other vehicles or obstacles near. Brakes slammed on, and vehicle came to a complete stop in the roadway.
