1 Forward Collision Avoidance problem of the 2023 Chevrolet Silverado 1500

Failure Date: 09/05/2025

The contact owns a 2023 Chevrolet Silverado 1500. The contact stated that while pulling into a parking space and turning the steering wheel to the right, the vehicle surged forward going up onto the curb, and the contact depressed the brake pedal; however, the vehicle failed to stop. The contact continued to depress the brake pedal however, the vehicle continued driving approximately 5-10 mph. The contact shifted into neutral(n); however, it was too late, and the vehicle crashed into another parked vehicle before coming to a stop. The contact stated that during the failure, the braking system had malfunctioned while depressing the brake pedal. No injuries were reported. A police report was filed at the scene. The vehicle was taken to the local dealer who was unable to determine the cause of the failure. The contact stated that one week before the failure, at almost the same spot, while driving at an undisclosed speed, the forward collision avoidance warning light illuminated and remained illuminated without another vehicle being nearby. The contact turned off the vehicle, exited the vehicle and inspected the vehicle before re-entering and restarting the vehicle, and the forward collision avoidance warning light remained illuminated. The contact turned and restarted the vehicle and the warning light was no longer illuminated. The manufacturer was notified of the failure. The failure mileage was approximately 11,000.

2 Forward Collision Avoidance problem of the 2022 Chevrolet Silverado 1500

Failure Date: 09/15/2023

The contact owns a 2022 Chevrolet Silverado 1500. The contact stated while driving approximately 35 mph, the vehicle in front came to a stop and the contact depressed the brake pedal however, the pedal went to the floorboard. The contact stated that the forward collision avoidance system failed to activate as needed. The contact stated no warning light was illuminated. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed and was not repaired. The manufacturer was informed of the failure. Failure Date: 08/15/2022

The contact owns a 2022 Chevrolet silverado. The contact stated that while her husband was driving at approximately 25 mph a vehicle in front came to a sudden stop, the contact stated that the vehicle is equipped with front collision avoidance equipment. The husband crashed into the rear end of the vehicle. The contact's vehicle had damage to the front bumper which was crushed following contact with the other vehicle's bumper and trailer hitch. The other vehicle suffered light scratch damage to the trailer hitch and bumper. The contact stated that the collision avoidance light was illuminated. There were no injuries, both vehicles were drivable. Additionally, the contact stated that the auto-high beam feature had stopped functioning. The contact had not taken the vehicle to a local dealer or independent mechanic to be diagnosed. The vehicle had not been repaired. The manufacturer had been informed of the failure. The failure mileage was 11,105.

5 Forward Collision Avoidance problem of the 2003 Chevrolet Silverado 1500

Failure Date: 07/11/2016

265/65/16 michelin ltx m/s2 tire, these tires were new and purchased on 6/21/16, on 7/11/16 less then one month I had to brake hard to avoid an accident. The 3 rows of treads in the middle of all four tires became severely separated and worn rendering all four tires as junk causing the pickup to shake badly while going down the road. To make matters worse michelin didn't want honor any warranties, claims, etc. , including the michelin plus guaranty. This is where if you are not 100% satisfied within 30 days you can get a replacement free. After one braking incident all the tires was junk. I would not recommend these tires to anyone. The tread pattern of these tires is the direct cause. I also have pictures.
