When the vehicle was parked, the middle row side air bag deployed and particles from the bag caused injuries to the child occupant.
Consumer was involved in a head-on collision, impact speed over 40 mph in which neither driver's side nor passenger side air bags deployed, causing injuries to three occupants of vehicle.
Consumer was driving at 30 mph and hit another vehicle head-on, and driver's side air bag didn't deploy.
Drivers side under seat airbag impact sensor was not mounted firmly to floor. It was just dangling from the wiring harness, effectively disabling the airbags. Dealer looked at seat pedestal and determined that the wrong seat mount was installed at the factory. Dealer is installing the correct seat mount. I am not convinced that the passenger side airbag wiring has been done correctly either.
