1 Warning problem

Failure Date: 02/27/2026

The camera module failed in our van and gave us a u3000-49 code which caused the vehicle to lose adaptive cruise control (acc), lane keeping assist system (lkas), collision mitigation braking system (cmbs), and road departure mitigation (rdm) functions on the vehicle with a cost to fix from Honda of $ 2,475. 56.

2 Warning problem

Failure Date: 01/17/2026

We turned the car on and got an acc, lkas, fcs, ahl errors due to the front sensor and the camera failing. This is an issue that many Honda owners are dealing with. The issue will not allow you to use cruise control and will disable 90% of the safety features we bought this vehicle for. The car drives fine but the safety of those in the car is affected by this error and needs to be recalled due to the number of complaints of this issue. There were no errors or warning signs prior to this happening.

3 Warning problem

Failure Date: 01/16/2026

Front camera is known to go out on these vans causing acc, lkas, collision mitigation, road departure mitigation system, auto high beam, transmission problems and more.

4 Warning problem

Failure Date: 10/21/2025

All the systems are not working this is a very low mileage vehicle and has not been involved in any accidents. They are many owners reporting the same issue. This is a safety issue as it shuts off many of the safety features of this vehicle while driving. Honda claims it’s normal.

5 Warning problem

Failure Date: 10/08/2025

Several of my warning lights came on related to safety features such as brake assist, lane assist, collision mitigation. Brake warning. The source has been identified as a failure with the multipurpose camera unit.

6 Warning problem

Failure Date: 10/01/2025

Automatic emergency braking, forward collision warning, and lane departure warning systems stopped working simultaneously without warning. Upon further investigation, failure of the forward-facing camera is the likely root cause. Based on conversations in internet discussion forums [xxx] , [xxx] ), this is a common failure. There was a recall for the Odyssey to fix a wiring harness issue that could affect the back-up camera. I believe loss of the automatic emergency braking, forward collision warning, and lane departure warning systems is much more serious, with potentially catastrophic consequences. Since evidence suggests failure of the forward-facing camera happens frequently, I believe this issue should be treated with the same gravity as the back-up camera and a recall should be issued. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

7 Warning problem

Failure Date: 09/30/2025

Mass safety system failure due to front projector camera/ wiring harness. Noted systems below failed: adaptive cruise control auto emergency braking forward collision warning lane departure warning lane keeping assistance auto high beams all above system are not operational.

8 Warning problem

Failure Date: 09/12/2025

I have 6 error lights on my dash board. Road departure mitigation system problem, auto high-beam problem, collision mitigation system problem, lane keeping assist problem, ada pat I’ve cruise control problem. My car was rear ended from an insured from utica national insurance group. After my car was rear ended these error message came up. They fixed the car but refused to fix to have the lights removed because they said that these error lights do not come on after a rear end accident.

9 Warning problem

Failure Date: 09/01/2025

On September 1, 2025 the front windshield camera on my 2018 Honda Odyssey failed, part #36160-thr-a03-m1, resulting in the loss of many of my vehicle’s safety features including: adaptive cruise control, emergency braking, forward collision warning, lane departure warning and lane keep assistance. The camera is available for inspection. Now when I start the vehicle it displays warnings that none of the above safety features are functional. I contacted 2 local Honda dealers, bellingham Honda and burlington Honda in washington state. Both dealers said they could not fix my car since the camera is not available, since it is not in stock and on national back order, with no date of when a new camera would be be available. I have also contacted 2 online Honda parts distributors and they also say the camera is in national back order and not available, with no date of when it would be available. The result is that my vehicle has most of it’s safety systems inoperable and it can not be repaired, since a new camera is not available, and Honda cannot provide me a date when the part is available and my vehicle can be fixed. I see on the internet the front camera failure is a common occurrence in 2018 Honda Odyssey’s and request NHTSA investigate make Honda recall these vehicles to fix the safety problem.

10 Warning problem

Failure Date: 09/01/2025

The front-facing camera responsible for detecting surrounding traffic is malfunctioning, which in turn compromises several critical safety systems. These include adaptive cruise control, lane keep assist, blind spot monitoring, automatic emergency braking, forward collision warning, and lane departure warning. All of these features depend on the proper functioning of this camera. The vehicle has never been involved in a collision, and the camera was neither abused nor damaged. The failure occurred suddenly and without warning, triggering multiple warning lights on the dashboard. While the vehicle remains operable, all advanced driver assistance systems are currently disabled. It is my contention that Honda has engineered a defective camera system that requires a substantial out-of-pocket expense for vehicle owners to repair. I believe this cost should be borne by Honda, not the consumer. Accordingly, I am requesting that the NHTSA investigate this issue and require Honda to implement a recall that permanently addresses the problem.

11 Warning problem

Failure Date: 08/25/2025

Unknown, though others who have reported this problem are saying the front camera and it's calibration are at fault. Currently, 5 systems (3 reported + 2 more) are under warning on my vehicle due to this unknown issue and the dealership wants a ridiculous amount of money to fix even though it appears to be very common and frequent as these vehicles get more and more miles on them.

12 Warning problem

Failure Date: 08/15/2025

The windshield-mounted front camera assembly (part number 36160?thr?a81) that powers lkas, acc, collision mitigation, auto high beam, and other safety systems has failed on my 2018 Honda Odyssey. When the camera fails, multiple critical safety systems become disabled at once, posing a significant risk to driver and road safety. " supporting evidence from other owners: Honda Odyssey owner posted in a parts-related forum: > “the Honda garage wants $1. 5k to replace the front camera, but I’m wondering why it catastrophically failed after 3. 5 years and 30,000 miles!” a documented complaint filed with NHTSA highlights a very similar situation: > “all Honda sensing technology is not working… acc, lkas, collision mitigation, auto high beam… many warning lights appear on the dash… diagnostics… indicated the issue may be related to a manufacturer defect… dealership… said the monocular camera was defective… the contact… replaced the defective monocular camera, … once the repair occurred, all of the messages and lights… cleared and all of the safety systems… are now working correctly. ” costs: owners have reported paying approximately $750 for the camera part and another $500 for calibration, totaling around $1,250. We have even been quoted $2,500 to replace the front camera. Safety risk: the failure disables multiple driver-assistive safety systems simultaneously, increasing risk of collision — especially since these systems (acc, lkas, collision mitigation) are intended to help prevent accidents. Conclusion / call for action: given the frequency of similar reports, recurring nature of the front camera failure (36160?thr?a81) in 2018–2019 Odyssey models, the high cost of repair, and the safety-critical systems that fail when this component fails, I believe this qualifies as a design or manufacturing defect. I respectfully request that NHTSA investigate this issue and consider issuing a recall for affected vehicles.

13 Warning problem

Failure Date: 08/13/2025

1. Vehicle information year / make / model: 2018 Honda Odyssey touring mileage at time of incident: approximately 123,952 miles purchased new or used: used 2. Incident summary on June?13,?2025 upon starting the vehicle for the first time that day (vehicle is garage kept, year round, at home when not in use), problem codes for the auto high beam, adaptive cruise control (acc), lane keeping assist system (lkas), collision mitigation braking system (cmbs), and road departure mitigation (rdm) showed on the display and all features failed to function. 3. Detailed description of failure & safety risk (chronological) component/system affected: I have not taken the vehicle to the local Honda dealership as yet as I plan to schedule an appointment during the week of 17 August. After researching this issues it is found that many other consumers have experienced the same issue and confirmed that it was the adas front-facing camera and radar sensor failure. Due to the failure this causes the following safety systems to cease functionality: auto high beam, acc, lkas, cmbs, and rdm. Vehicle is available for inspection upon request. Safety risk: all affected systems are critical for maintaining lane position, speed, collision avoidance, and visibility. Not having these critical safety features creates a heightened risk of collision or unsafe maneuver. Inspection: only an authorized Honda dealer will inspect/diagnose the issue. There has been no inspection from the vehicle manufacturer, law enforcement, insurance, or independent service.

14 Warning problem

Failure Date: 08/11/2025

I in August 2025, I experienced a sudden and complete failure of the forward-facing windshield camera in my 2018 Honda Odyssey, which disabled multiple safety-critical systems. After having new tires installed at discount tire in preparation for a road trip, I started my vehicle and was immediately confronted with multiple dashboard warnings, including: •“road mitigation system problem” •“auto high-beam problem” •“adaptive cruise control problem” •“lane keeping assist problem” •“brake system” I took the vehicle to a Honda dealership. They diagnosed dtc u3000:49 — internal failure with multipurpose camera unit and informed me the only repair option is to replace the camera and perform camera aiming. The quoted repair cost is approximately $2,000. The dealer explained that this failure is “common in hot climates. ” if true, this represents a serious design flaw. The Odyssey was marketed as a family vehicle equipped with advanced safety systems, yet a single component failure suddenly disables nearly every major driver-assist safety feature. This is a serious safety defect for several reasons: 1. Safety systems suddenly fail without warning. One moment the systems are working, and the next they are completely disabled. A driver relying on lane keeping, collision mitigation, or adaptive cruise control would be placed at immediate risk. 2. Known widespread issue. The dealer admitted this happens in hot climates, suggesting Honda is aware of the problem but has not addressed it. 3. Unreasonable repair cost. A vital safety component should not have a built-in failure mode tied to environmental conditions, forcing owners to pay thousands out of pocket for safety-critical repairs. 4. Loss of federally encouraged safety features. Forward collision mitigation and lane departure warning systems are strongly promoted by NHTSA itself. Their failure removes protections drivers expect when purchasing the vehicle. This issue appears to be a common defect in Honda Odyssey vehicle.

15 Warning problem

Failure Date: 08/07/2025

All of the warning lights associated with advanced safety features came on on the way back from a visit to local store. When checking the codes there is a the u3000-49 code indicating an issue with the mono camera or radar unit. These warnings do not allow for the use of any of the safety features affected. The code cannot be cleared from the system either.

16 Warning problem

Failure Date: 07/07/2025

I recently replaced a battery on the 2018 Honda Odyssey. When I turned the car on, the dashboard showed 6 issues: adaptive cruise control problem; lane keeping assist problem; collision mitigation system problem; road departure mitigation system problem; auto high beam problem; and the brake system light was on. I paid a mechanic to clear out the code and the was told that the front sensing camera needs to be replaced. Without a replacement, all these errors will continue to show up. I search online to find out if other people have had this issue because it was just too coincidental that a battery replacement broke the front camera. I found posts by many people who own the 2018 Honda Odyssey having similar or near identical issues that I have had. The cost to replace the camera and have it recalibrate it is going to cost $1,800. 00 as estimated by a local Honda dealer. The Honda dealer told me that they have seen this problem over and over again.

17 Warning problem

Failure Date: 07/01/2025

Brake system light, and other warning lights including adaptive cruise control, lane keeping assist, collision mitigation, road departure warning, and auto high beams: a common issue from internet which likely be caused by the camera/sensor issue (near the rearview mirror).

18 Warning problem

Failure Date: 05/01/2025

It seems the front dash camera failed to operate which caused all safety systems that rely on it to fail.

19 Warning problem

Failure Date: 04/07/2025

Adas system failure. An audible alert would sound and the following problems would scroll across dash display: auto high-beam problem; adaptive cruise control problem; lane keeping assist problem; collision mitigation system problem; road departure mitigation system problem. Also, "brake system' illuminated in red near bottom of display. Diagnostic at Honda dealer indicated, "u3000-49 multipurpose camera unit internal electronic failure". To repair, the quote for the unit itself was $1201. 34 plus tax. Calibration was an additional $660. 00. I found the unit online at a better price and so purchased it and had the dealer install. My total cost for the unit and installation/calibration was $1. 496. 74. Even though it is a 2018 model I would have expected this camera unit to last much longer. It's an expensive part! my hope would be for Honda to issue an extended warranty and I'd be eligible for compensation on this repair. If needed I can provide paperwork. Please advise. Thank you.

20 Warning problem

Failure Date: 01/24/2025

The front facing monocular camera has failed and my 2018 Odyssey has less than 55k miles on it. Without the operation of this camera you cannot use the cruise control and the dash lights up with all kinds of warnings (lane mitigation failure, acc failure, brake light continuously on). This is a known faulty safety feature that is far to common for the newer Odyssey models. This is ridiculous that Honda has put this system into their vehicles, in particular the Odyssey and there needs to be a service bulletin to repair this issue or recall completely. It is obviously a faulty part and needs to be resolved.

21 Warning problem

Failure Date: 11/20/2024

All alert lights came on suddenly (acc, collision mitigation, road departure, auto high beam, etc). Took the vehicle to the dealer for a diagnosis, and found out there is a faulty multipurpose camera in the front near the windshield. Upon research, this seems to be a common problem for Honda vehicles manufactured between the years 2018-2019. Total cost of repair and labor is over $2,000.

22 Warning problem

Failure Date: 11/18/2024

The camera projector needs to be replaced according to the Honda dealership. Too many Honda Odysseys are having similar issues. Come thing is battery replacement or vehicle is located in warmer climate states. This camera mount is too sensitive to voltage swings or warm weather which is a poor design by Honda. It is a major safety concern not being able to use key features in the elite and touring models such as adaptive cruise control, lkas, and the other 5 warnings that won't go away. This needs to be resolved by Honda so that vehicle owners can get back on the road safely. The cost to replace the camera and calibration is ridiculous ($1700 - $2000). This will be the last Honda vehicle we ever buy if Honda doesn't setup and correct this issue with thousands of Odyssey vehicles. This a major issue and Honda needs to recall and correct this issue before a major lawsuit comes their way to due to this poor design.

23 Warning problem

Failure Date: 11/10/2024

Unknown but many Honda Odyssey owners are reporting the same issue which most likely caused by defective multipurpose camera at the front windshield. This create alerts and deactivates safety features: adaptive cruise control, emergency braking, lane keep assist, collision mitigation, auto-high beam, road departure mitigation, and brake system.

24 Warning problem

Failure Date: 11/05/2024

Driving vehicle on the road, the suite of driver assistance system suddenly failed including lkas, front collision warning system, adaptive cruise control, lane departure as well as brake system light. I took the car to the dealership which indicated the front camera had catastrophically failed. This issue seems to be a recurring issue in all models of Odyssey minivans. The issue is well documented in online forums. It should be a safety recall from Honda compared to other recalls on same vehicle. Dealer quoted me $2000 to fix this! Honda - fix this and issue a recall before there is a lawsuit and someone gets hurt.

25 Warning problem

Failure Date: 11/01/2024

The monocular front window camera is malfunctioning. This front window camera controls the following safety features: -lane keep assist (lkas) -collision mitigation -adaptive cruise control -lane departure mitigation -rain sensor windshield wipes -auto high beam -automatic emergency braking in addition to compromising these safety features when the camera fails, false warnings are repeatedly provided on the dash of the vehicle, sometimes in a singular and sometimes in a cyclic manner.

26 Warning problem

Failure Date: 10/31/2024

Multiple dash lights warnings (lkas, forward collision, lane departure, etc. ) came up. Brought it to the dealership and was told that this is an electrical camera issue that is overheating, and there is no preventative or ways to keep it from happening. It would cost an upward of $2000 to fix it. This should be part of the recall since it seems like it’s happened quite frequently with the other Honda Odyssey.

27 Warning problem

Failure Date: 10/11/2024

Upon inspection from Honda dealership, internal failure of multipurpose camera.

28 Warning problem

Failure Date: 09/28/2024

Multipurpose camera failure. Warnings appeared upon failure after being parked in the shade on a moderately warm day ~80 degrees. Warnings with chimes reappear after being cleared, causing distraction while driving. Safety systems are inoperable without expensive camera replacement. Systems disabled as a result include adaptive cruise control, collision mitigation system, lane keeping assist, road departure mitigation, auto high beam system.

29 Warning problem

Failure Date: 09/21/2024

The front monocular camera (36160-thr-a03) completely stopped working. It is behind the rear view mirror, behind the front windshield. Errors displayed on dashboard indicating lane keep assist, forward collision warning, brake mitigation system, adaptive cruise control, and brake system lights turned on. (99% of the time). On occasion, transmission problem, blind spot detection, and rear camera cross traffic detection (radar disabled) would also go on (3% of the time). While driving the vehicle on the freeway, the dashboard went completely black for about 3-5 seconds, and I could not see my speed, fuel, odometer, and everything else. It came back on, but that really concerned me. I immediately opened a ticket with Honda corporate. (repair diagnosis request: 14552995). The root cause was not identified. However, some speculate a failing battery (low voltage) can trigger it. In my case, I replaced the battery first when the symptoms first started, but the issue did not resolve even after driving it 500+ miles. The oddest symptom is something I could not explain. Every Saturday (like clockwork because kids soccer practice), between 12pm-1pm local time (eastern for me), the issue would go away completely and the camera (and all assistive features) would work completely normal. As soon as I power off/on the vehicle, the issue came back immediately… until the next Saturday. This happened 3 times, 3 weeks in a row. Makes me think there’s a software/firmware glitch. Honda did a goodwill with me and paid 50% of parts and labor. Honda (dealership) confirmed it was a part failure, and replaced the part and all issues went away. Given the front camera issue also affected the rear camera on occasion (cross traffic radar failure), and that part was already recalled, my opinion that when this part fails, it has greater implication to other safety features/electrical system. Due to this also being cost prohibitive, I suspect many will opt to not to fix it.

30 Warning problem

Failure Date: 09/18/2024

Diagnostic is u3000- "multipurpose camera unit/millimeter wave radar internal electronic failure. Multipurpose camera internal failure. Camera is overheating and hot to the touch. " Honda is saying this is separate from the recall already issued on the camera and will be a $2k repair. Multiple other owners of other 2018 Honda Odysseys reporting same failure. Van running fine, then all of a sudden warning panel lit up with the following warning messages: adaptive cruise control problem lane keeping assist problem collision mitigation system problem road departure mitigations system problem auto high-beam problem local reputable auto repair shop said it is not safe to drive due to the extremely high temperature of the camera unit.

31 Warning problem

Failure Date: 09/05/2024

Acc,lkas, collision mitigation system, road departure mitigation system, auto high beam warnings. All read problem and do not go away.

32 Warning problem

Failure Date: 07/10/2024

My vehicle started to show warning: collision mitigationsystem problem/ road departure mitigation system problem/ adaptive cruise control problem / lane keeping assist problem/ auto high-beam problem. I took it back to Honda dealership and was told the multipurpose camera is damaged and needed to be changed. They asked if I have a vehicle accident and I told them I have not been in one. I don’t understand why this just starts up all of a sudden and have seen other people complaining of this same problem.

33 Warning problem

Failure Date: 04/19/2024

- monocular camera failure. It is available for inspection upon request. - multiple dash warning lights are on and several safety features, including forward collision warning, lane departure warning, lane keeping assistance, adaptive cruise control, and automatic emergency braking fail to work. Dealer states that the monocular camera cannot be disabled because it is a safety feature. - the problem has been confirmed by a dealer. The dealer stated the problem was not caused by consumer usage or accident, but was an internal failure of the monocular camera. - no additional inspections have been performed. -there were no warning lamps, messages or other symptoms of the problem prior to the failure, which first appeared in April 2024 at ~104,000 miles on the vehicle.

34 Warning problem

Failure Date: 03/20/2024

The forward warning system will say brake without anyone or anything in front of you. It will then slam on the brakes. This is very dangerous and has almost resulted in multiple collisions. We have took it to the dealership numerous times and it has not been fixed. I have called american Honda and they will not fix the issue. I get the run around that it is outside of warranty. It was took to them under warranty but was never fixed. The local Honda dealer wants to just try swapping out cameras at a price of $2,500. They do not know if it will even fix it. Now the system has numerous warning lights related to the adaptive cruise, lane keep assistance, forward warning system and lane departure warning. Again I have called America Honda and local dealership. They do not know how to fix it. This is a major safety hazard and they should be forced to repair it under a recall.

35 Warning problem

Failure Date: 02/21/2024

All warning lights related to the Honda sensing driver assist features come on and the assist features are inoperative. Vehicle still drives fine but even normal cruise control is no longer an option. Some warning notifications pop back up while driving even after dismissed which is rather distracting.
