Intermittent electrical failure with the front monocular camera confirmed via dtc codes. Upon inspection, camera is hot to the touch and all adas systems on the vehicle cut on/off with warning lights while driving. Was going down interstate utilizing acc, system cut off throwing warning codes, then disappeared, then came back on. System continues to have intermittent issues indicating a failing camera. Even after 1-2 minutes of idle - camera is extremely hot when touching module underneath decorative garnish.

The front windshield camera completely failed. No longer have emergency braking. Honda said known problem and want to charge $2000 to fix.
The front monocular camera assembly (Honda part number 36160-thr-a81) on my 2019 Honda Odyssey elite failed and is available for inspection upon request. When the failure occurs, the vehicle displays multiple warning messages including “collision mitigation system problem,” “adaptive cruise control problem,” “lane keeping assist problem,” “road departure mitigation system problem,” and “auto high beam problem. ” as a result, several driver-assist safety systems are disabled, including automatic emergency braking, adaptive cruise control, lane keeping assist, and road departure mitigation, which reduces the vehicle’s active safety features and increases the risk of a collision. The warnings appear simultaneously on the dashboard while driving and remain active until the vehicle is restarted. The condition has been reproduced multiple times and has been confirmed during diagnostic inspection by a service technician. The camera failures are very common and are a design defect. The design was changed in later years but there was not a recall for the 2019 models. This is a very expensive fix and should be covered by a recall.
The camera module failed in our van and gave us a u3000-49 code which caused the vehicle to lose adaptive cruise control (acc), lane keeping assist system (lkas), collision mitigation braking system (cmbs), and road departure mitigation (rdm) functions on the vehicle with a cost to fix from Honda of $ 2,475. 56.
System is starting to fail by alerting me to clean windshield. Windshield and camera have been cleaned, still getting multiple warnings that system is shutting off safety features during my commutes.

All Honda sensing / adas safety warnings appear at the same time while driving or shortly after starting the vehicle. When this happens, all driver assistance systems are disabled, including: •adaptive cruise control (acc) •lane keeping assist (lkas) •collision mitigation braking system (cmbs) •road departure mitigation •automatic high beam when the failure occurs, the vehicle displays multiple safety system warnings and the entire driver assistance system shuts down, leaving the vehicle without any adas safety features.
Failure of camera unit leading to all safety systems requiring it becoming non operative. Well known issue and widely reported.
For the past 6 months the vehicle shows warnings regarding lane keeping assistance problems, cruise problems, braking problems,. The warning can show up in the middle of driving, or right at start. Sometimes they go away while driving.
We turned the car on and got an acc, lkas, fcs, ahl errors due to the front sensor and the camera failing. This is an issue that many Honda owners are dealing with. The issue will not allow you to use cruise control and will disable 90% of the safety features we bought this vehicle for. The car drives fine but the safety of those in the car is affected by this error and needs to be recalled due to the number of complaints of this issue. There were no errors or warning signs prior to this happening.
Front camera is known to go out on these vans causing acc, lkas, collision mitigation, road departure mitigation system, auto high beam, transmission problems and more.
Multiple warning lights, all for the safety system: acc, automatic emergency braking, etc. I am told by the dealer that this is due to a faulty forward camera. Looking at the NHTSA site and other forums, this is a very common issue and should be addressed.
While driving, the vehicle will suddenly jerk taking control of the steering. As similar defect has been noted on other models. Took to the dealership and they said it is the automatic gear box.

I am submitting this complaint to report a serious safety defect involving my Honda Odyssey equipped with Honda sensing technology. During normal operation and without any accident, impact, windshield damage, repair, or environmental event, all Honda sensing safety systems suddenly and simultaneously stopped working. The disabled systems include collision mitigation braking system, adaptive cruise control, lane keeping assist, road departure mitigation, forward collision warning, and related camera- and radar-based features. Multiple warning messages appeared, and the vehicle effectively lost all advanced driver-assistance and collision-avoidance capabilities at once. This occurred without warning and without any action on my part. This failure is extremely concerning because Honda sensing features are marketed and relied upon as critical active safety systems intended to help prevent or mitigate crashes. A sudden, total loss of these systems materially reduces vehicle safety and places occupants and other road users at increased risk. A safety system that can fail entirely and unexpectedly—absent any triggering event—indicates a potential systemic defect, not routine maintenance or wear. I am concerned this issue may affect other Honda vehicles equipped with Honda sensing and may be related to a common failure in cameras, radar units, software, or vehicle electronics. I respectfully request that NHTSA review and log this complaint, investigate this failure pattern, and determine whether a defect investigation, service bulletin, or recall is warranted in the interest of public safety. Went to pull into parking lot, adas system starting going crazy. Throws transmission malfunction error. Headlights won’t work - they turn on and off or go extremely dim. Van keeps jerking and losing power. Code reader showed u3000-49 electronic failure. Van drove normal, no issues until this code popped up. Issue started 01/08/2026.
The front-facing camera stopped working, and all associated adas systems no longer functioned (adaptive cruise control, automatic emergency braking, forward collision warning, lane departure warning, lane keeping assistance). Several errors needed to be cleared every time the vehicle was started. Our Honda dealership determined that it was a failed multipurpose camera (code u3000-49), and that the entire camera unit needed to be replaced and re-calibrated.
My multi-purpose camera system has gone bad after only 5 years. Collision mitigation, cruise control, brake system, lane keep assist, among other safety features are failing because of this.
Vehicle was driving normally down the highway then proceeded to shutdown without warning. Suddenly all of the trouble lights appeared indicatinging engine, transmission, hill assist etc cut on without warning. The van would not drive above 55 mph when turned back on.

In December, out of the blue suddenly had rotating list of warning on dash panel (lane keeping assist problem, collision mitigation system, departure mitigation system problem, auto high-beam problem, adaptive cruise control problem) plus brake warning system light is on constantly and brake hold not working. At first this was intermittent but now on constantly when starting and driving car. Only 55k miles on my Odyssey. Dealer said he had 3 of these in the last three months. Said it was multi purpose front camera failure. Cost over $1500. Since huge safety issue putting not only my family but other cars/people at risk, we are paying to repair, however we just found hundreds of people complaining about this same issue. This should be a Honda recall. We did nothing wrong to cause this safety issue and this part should not fail in 5 years. I don't have the final invoice yet but I can upload later if required.
The camera for these sensors has gone out and it appears to be an extremely common issue. It makes all safety features un-usable.

The multipurpose camera unit has failed resulting in loss of adaptive cruise control, lane keep assist, automatic braking and other features related to the mcu. There are approximately 6 warnings when starting the car of features not working, and 3 lights that remain lit on the dash including "brake system". This is the second time this issue has arise. The first time was replaced by the dealer under warranty.
The front facing camera has stopped working. Anytime we turn the vehicle on we get multiple warning lights (lane assist, cruise control, bright lights, braking, etc) aren't working.
While driving with my family, my vehicle experienced a sudden and total failure of the front-facing multi-purpose camera mounted behind the windshield. This component is the primary sensor for the Honda sensing suite. The failure caused multiple critical safety systems to become inoperative simultaneously, including the collision mitigation braking system (cmbs), lane keeping assist (lkas), and adaptive cruise control (acc). These systems are essential for crash avoidance and driver assistance.
Suddenly, all the lane keeping assistance, auto beam headlights, forward collision warning, adaptive cruise control came on. When I took it in for diagnosis, they claim that the camera assembly monocular no longer works. From searching forums, it appears this is a known issue. This is a safety issue and should be recalled.
The warning messages keep popping up regarding the lane assist system, adaptive cruise control not working and dealer states that the camera needs replacement and this is common issue for most Honda cars so should be recalled.
The front camera that regulates the adaptive cruise control failed. Now it does not allow for any of the safety features to work. I’ve gone on multiple Honda forums and a lot of people have the same issue.

The multipurpose camera failed showing lot of error codes. This affects the braking system putting my family and kids in risk. Honda is asking 2000$ to fix the camera.
Several warnings lights appeared on the dash at the same time regarding the accident safety features including: adaptive cruise control, automatic emergency braking, forward collision warning, lane departure warning and lane keep assistance. None are currently working. The warning randomly appeared. It's associated with the camera behind the windshield and the radar detection behind the front emblem. Problem confirmed by dealer. Components have not been evaluated by anyone.
One light on dashboard lit up, started as "blind spot info system problem", light would go on and off for couple of years. Now every light on dashboard lights up, including lane assist, blind spot infor system problem, forward collision warning, adaptive cruise control problem, rear cross traffic warning, lane departure warning.
Driving on the interstate with cruise control. All of a sudden the car braked (no one in front within eye shot) and cruise control stopped. I had to tap the brake manually then accelerator to get the car back under my control. Ever since then I get acc, lkas and other warnings when the car starts up. It seems like the front camera catastrophically failed while driving and almost caused the car to come to a complete stop on the interstate. It was very scary.
