At restaurant on 6/1/25, backed out of parking spot. Drove about 30 ft to parking lot exit. Turned onto side street to go to light and turn on main street. About 3 or 4 car lengths from exit to light. Side street sloped down about 20-30 degrees. Brakes would not stop van. Not sure if brakes slowed down van as going slow. However, brake pedal pushed hard down to floor but did not stop van. If hit gas pedal van would have accelerated and engine raced causing acceleration and more damage. No damage to back of vehicle I hit. Very minor damage to my van. Tried yesterday and today to replicate (in isolated locations and with no other vehicle near) the van’s conditions at time of incident. I will be contacting dealer. There is no recall. Do not know if anyone else had this problem or if dealer can find any issue. Notifying NHTSA to advise of this potential systemic issue. I’ll follow up with results of interaction with dealer.

The brake system warning light is on. With this, it is showing 5 different issue messages when the car is running. I've taken to the dealer and they are saying there is an issue with the multipurpose front camera. My car is on 39,900 miles.
2021 Odyssey exl the electric parking brake 'issue' warning comes on while driving and collision mitigation warning / lane assist switches off and flashes in console; when this happens the car continues to run but the dashboard warning lights comes up for parking brake / collision warning and or lane assist. It gets cleared after I turn on/off the car couple of times and drive for approximately 5 miles. Those features had to be turned on manually, once the car is in parked mode, again. The issue started in the car at around 48k miles. Never had any issue with it before that.

The problem first appeared in may 2025 when multiple system failure lights illuminated simultaneously. After confirming the battery and alternator were in good condition, I took the van to Honda of cleveland (2701 s lee hwy, cleveland, TN 37311) for a diagnosis. Their service department confirmed that the adas front camera (36160-thr-a91) had failed, which disabled the following safety systems: adaptive cruise control, automatic emergency braking, forward collision warning, lane departure warning, and lane keeping assistance. I asked if this is a wear component (expected to need replacement over time) and was informed that it is not. The quoted cost to replace the camera and calibrate was approximately $1,400.

Without any notable prior messages or symptoms prior to the failure, on [xxx] after dropping off the kids at school and then turning on my 2019 Honda Odyssey ex-l (28,170 miles), the "brake system" light was on and the following 5 dashboard warning lights/messages turned 1) adaptive cruise control problem, 2) lane keeping assist problem, 3) collision mitigation system problem, 4) road departure mitigation system problem, 5) auto high-beam problem. The advanced driver assistance systems no longer works, and it becomes unknown to the driver whether the "brake system" light is being triggered only by the collision mitigation system problem, or an additional problemch as the anti-lock brake system not working. While no accidents occurred, the warning lights add unneeded driver anxiety, which can contribute to a potential safety impact. The problem has been confirmed by the san leandro, CA Honda dealer to be error code u3000-49. Dealer diagnosed problem as "multipurpose camera / millimeter wave radar internal circuit failure" and recommended to replace the multipurpose camera and perform aiming. Dealer advised that safety features will not work. The diagnostic fee was $179. 10, and the repair fee was quoted as $2456. 55 the vehicle or component has not been inspected by the manufacturer, police, insurance representatives or others. Vehicle is indicating all sensing system not working such as lane keeping assist system, collision mitigation braking system, road departure warning system, adaptive cruise control and others. Honda installed a cheap product on this vehicle when it was originally manufactured and is double dipping by selling us in a 7 year period another set of the same cheap product . Dealer is quoting me $3,139. 80 to replace a radar and a camera both at the same time.

My radar and multipurpose camera need replacement.
Multiple warning lights: 1. Acc- adaptive cruise control 2. Lkas- lane keeping assist problem 3. Collision mitigation system problem 4. Road departure mitigation system problem 5. Auto high beam problem safety issues: with almost all of these not working the risk of avoiding a possible accident is greatly increased. An independent service center read errors for the can. No prior warnings, they just came up. I have replaced the battery and the issues were not resolved. An inspection is possible.
Took the van to the dealership to fix a recall for the fuel pump. A few days later, error messages popped up saying there are problems with the adaptive cruise control, collision mitigation system, lane keep assist system, and brake system, among others. I test drove it from arcadia to pasadena on both the freeway and the sidestreets without any problems. We took it back to the dealership who assessed it and told us that the multipurpose camera was bad. It would cost us $2100 to get it fixed. We refused. No one was hurt, however, the dealership told me that there is a safety risk to my family if I did not repair it. Attached is the invoice for the last visit during which the dealership gave us the estimate for replacing the multipurpose camera.
Front facing camera went out on a 2020 model. Several warning lights appear related to lane assist, front collision warnings, and automatic emergency braking. These safety features are important, and Honda should issue a recall. The camera is around $800 to replace, plus another $800-$1000 to install and calibrate. Car is only 5 years old, and there have been other camera/electrical related problems, and many recalls. Please hold Honda accountable to fix their declining reliability issues.
Upon starting the car, multiple alerts came on the dashboard reporting faults with multiple systems. There was no damage to the car, it had been parked safely and appropriately prior to starting. Dealership inspected the issue and reported the camera/sensor system in the windshield behind the rear view mirror is faulty. This faulty camera now causes multiple systems off line as the sensor is no longer reliable. This is a safety issue as this sensor assists with split second support to the driver to prevent collisions, leaving your lane or speed adjustment with cruise control.
Took it to certified Honda garage to solve another issue and now there is an issue with the front facing camera system. - adaptive cruise control - lane keeping assistant - collision mitigation system - road departure mitigation system - auto high beam system.
Over 8 multiple safety warnings turned up on dashboard on many occasions while driving. Also, vehicle fails to issue a code on obd and Honda rep claims they’ve never seen anything like this after showing them video proof of multiple occurrences. I’ve seen many complaints online. My wife drives this vehicle with young children and doesn’t feel safe while driving it because of it stating multiple safety warnings.
The entire advanced driver assistance system failed. My vehicle has not been inspected. The entire adas does not work upon startup.
Within 6 months of purchase (no accidents or related issues), the car starts firing off a wide range of alerts on the dashboard about failures. These include collision mitigation braking system (cmbs), road departure mitigation (rdm), or adaptive cruise control, auto high-beam problem, and lane keeping assist problems. There are many reported issues like this online and it’s a clear failure of on Honda’s part to provide working safety features that they advertise.

We bought this van with a clean bill of health as far as accidents and single owner prior to us. The car had a documented maintenance history but after 5k miles of driving it the active cruise control began to through all kinds of errors. I researched this issue and find that many more Honda owners with active cruise control have the same issue. A few customers say that they had their front camera replaced and calibrated for a cost of about $1,600. 00 only to have the error show up again. I am writing this message to report that in my case I have found a work around. It’s not a permanent fix but points to Honda’s computer throwing a false positive error that is costing a great deal of money to Honda owners. On our van when this error happens, I force a diagnostic scan followed by a forced computer reboot and the warning lights are fixed an all equipment back to working. It’s horrible to read that Honda charges $1,600. 00 to replace perfectly good parts in all their line without taking the time to truly diagnose their issues with acc errors and do good to their customers. I hope this helps reach someone who can further investigate this issue with Hondas. The front facing monocular camera has failed and my 2018 Odyssey has less than 55k miles on it. Without the operation of this camera you cannot use the cruise control and the dash lights up with all kinds of warnings (lane mitigation failure, acc failure, brake light continuously on). This is a known faulty safety feature that is far to common for the newer Odyssey models. This is ridiculous that Honda has put this system into their vehicles, in particular the Odyssey and there needs to be a service bulletin to repair this issue or recall completely. It is obviously a faulty part and needs to be resolved.
The present complaint alleges as follows: 1. Honda has a defective design in the advanced driver assistance systems (adas) of all Honda Odysseys, models 2018-2023. 2. Namely, Honda has placed the radar behind the large Honda emblem on the front grille. This causes the radar to give a "radar obstructed" warning, which disables all advanced driver assistance systems in the car (automatic emergency braking, adaptive cruise control, forward collision warning). 3. The "radar obstructed" warning cannot be reset by restarting the vehicle or disconnecting the battery, requiring recalibration of the radar at the cost of $800 each time since it takes several hours to do (per dealership). 4. The official Honda Odyssey forum online has countless complaints on the same issue without resolution by Honda. 5. Honda has acted in bad-faith towards their customers since this defect impacts the vehicle's advanced safety features for which buyers incur additional expenses, endangering the lives of passengers. 6. Honda has acted in bad-faith towards their customers by designing a system that requires continuous and substantial expenditures without resolution because it is an engineering problem, I. E. , the placement of the radar behind the large, thick plastic h emblem obstructs the radar. 7. I hereby ask that the NHTSA investigate this issue and order Honda to offer a solution in the form of a recall that permanently addresses the problem.

The Honda sense front facing camera has went out on my 2018 Honda Odyssey. The camera is currently still installed in my van and if it is necessary to replace to keep it from draining the battery I will request that the faulty part be provided to me. This camera being faulty causes the loss of cruise control, adaptive cruise control, auto high beams, lane keep assist and emergency breaking. This issue is also causing a drain on the battery, which impacts other items due to low voltage. My van is currently at the dealership and I am being told that I will have to purchase a new camera which will be around $1500. The following lights are on; breaks, acc and lkas. These items first appeared within a few weeks of having the rear camera recall completed. Based on my research this is a common problem and just like the rear camera recall, Honda should have to replace this camera as well. The date provided is the date it was officially diagnosed by Honda, the lights appeared in 10/2024 and all of the functions were lost around that time.
The multifunction forward looking camera in the top of the windshield developed an "internal fault" and either needs replacement or these safety features will fail to work. These safety features allow me to avoid accidents. The problem has been confirmed by the dealer. There are warning lamps that state that these features are no longer working.
The warning for these adas keeps coming on without any reason.
I am reporting a serious safety concern regarding my 2019 Honda Odyssey, which poses a risk to me and my family. The issue is intermittent, making it difficult for the dealership to replicate. I proceeded with the repair in December 2023. However, the same issues returned in January 2025. This defect poses a severe safety risk, especially when driving at higher speeds or in traffic. Since the dealership has been unable to diagnose the issue due to its sporadic nature, I am requesting an investigation into this matter to determine if other owners are experiencing similar safety risks. The Honda monocular camera is apparently causing malfunctions and or warning lights. This is impacting many Odyssey owners according to my research. This should be addressed by Honda and reimbursement or repair covered as a recall.
All alert lights came on suddenly (acc, collision mitigation, road departure, auto high beam, etc). Took the vehicle to the dealer for a diagnosis, and found out there is a faulty multipurpose camera in the front near the windshield. Upon research, this seems to be a common problem for Honda vehicles manufactured between the years 2018-2019. Total cost of repair and labor is over $2,000.
The camera projector needs to be replaced according to the Honda dealership. Too many Honda Odysseys are having similar issues. Come thing is battery replacement or vehicle is located in warmer climate states. This camera mount is too sensitive to voltage swings or warm weather which is a poor design by Honda. It is a major safety concern not being able to use key features in the elite and touring models such as adaptive cruise control, lkas, and the other 5 warnings that won't go away. This needs to be resolved by Honda so that vehicle owners can get back on the road safely. The cost to replace the camera and calibration is ridiculous ($1700 - $2000). This will be the last Honda vehicle we ever buy if Honda doesn't setup and correct this issue with thousands of Odyssey vehicles. This a major issue and Honda needs to recall and correct this issue before a major lawsuit comes their way to due to this poor design.

I have a 2023 Honda Odyssey van. The van has been modified for me to drive from my wheelchair. The steering wheel has been locking up eight times when driving. It won’t out let me go straight or turn left or right. This happens when I come to a stop. It happened twice when driving. The wheelchair modifications company southern mobility said they fixed many times but it still have the steering wheel lock up. Now they are telling me it’s the Honda’s fault. Honda said it’s the van modification fault. Honda said they can look at it for $150 a hour. I am very frustrated and very concerned. Multiple dash lights warnings (lkas, forward collision, lane departure, etc. ) came up. Brought it to the dealership and was told that this is an electrical camera issue that is overheating, and there is no preventative or ways to keep it from happening. It would cost an upward of $2000 to fix it. This should be part of the recall since it seems like it’s happened quite frequently with the other Honda Odyssey.

New windshield wuth calibration August 2024, new battery 10/28/24. Issue started 10/25/24. A little over 90,000 miles on vehicle. Started vehicle in parking lot and all warnings came on. After new battery, warnings cine on and off at random.
