The first failure occurred when he had taken the vehicle to an independent mechanic for a tire rotation. The contact was informed by the mechanic that several lug nuts had to be broken off since the lug nuts would not unscrew after multiple attempts. The second failure occurred while the contact was on vacation and the front passenger’s side tire had run flat. The contact stated that the wrench that came equipped with the vehicle could not unscrew the bolts on the flat tire. The contact sought outside assistance to remove the passenger’s side lug nuts. The contact stated that the lug nuts presented a great danger and could leave a motorist stranded in the event of an emergency. 2000 Jeep Cherokee. Unit #1 was traveling eastbound on the ohio turnpike when the left front wheel came off. The wheel traveled across the roadway and bounced over the concrete wall and struck unit#2. According to the police report, 1 of 5 bolts were missing from the right front hub, 1 of 5 stud bolts were missing from the right rear hub and the right rear tire was bald and exposing steel belts.
