I was driving on the highway and suddenly there was a loud explosion and the sound of hundreds of time things or debris hitting my car. Thankfully I did not hit the brakes or lose control of my vehicle, but I easily could have! I then saw little pieces of glass falling off my car from my rear view mirror. I pulled over to find that it was my sunroof. There was no impact from anything, the glass was pointed upward as if the explosion came from inside. This was incredibly scary!.

Around 11 am on sat. , may 2, 2026, I was sitting in a very long line in starbuck's drive thru when my 2025 Lexus Rx hybrid suddenly and uncontrollably accelerated--like an uncontrollable bucking horse. Brakes didn't work. I couldn't stop my out of control car, and the multiple computer sensors also didn't stop the car. It was like the throttle was engaged because of a computer glitch. In an effort to avoid hitting the car in front of me, I immediately tried to swerve, but still hit the car, then jumped the exceptionally high curb and ended up in the grass below and almost into the busy road ahead of me. It was terrifying. The car in front of me hit the car in front of her. The exact same issue happened to me on June 18, 2025 when my car unintentionally accelerated--also in a very tight space and going at 2 mph. There was no accident at this time because my car's computer stopped me from plowing into the building immediately in front of me after unintentionally accelerating. The next day at the recommendation of the person who sold me the car, I took my new Lexus to the service department. They gave me a loaner and told me to contact Toyota's/Lexus' brand engagement department in order to start the review, which I did. The review was conducted and ended around July 9, 2025 . On mon. , may 4, 2026 I took my car back to the Lexus dealer where I purchased, but was not given a loaner, which I requested, and was told I would need to wait several hours to have this inspected. I then took my car to boyce body werks. A state farm agent examined my car and initially approved almost $24k worth of damages. Boyce body werks is saying when they disassembled my car that there was more damage underneath my car. I also notified Toyota/Lexus about the same issue that happened one year prior. They also refused to give me a loaner, and I am now forced to pay my rental fees. Boyce body werks--a Lexus certified dealer has multiple pictures of the damage. My car is still under warranty.

I was not made aware of the recall on the rubber oil cooler hoses. The rubber hoses malfunctioned and all the oil leaked out on 4/13/2026. I paid to have fixed. Lexus kept the hoses. This could have ruined my engine, caused an explosion or fire. When the warning light came on I stopped driving. Lexus would not discuss the issue. I am concerned I was never made aware of this issue. I bought the vehicle in 2012 at the dealership and all service has been done there.

I bought a cpo Lexus 2025 Rx 350 in Jan 9, 2026. This car was lightly used in less than 3 months and right now it's around 7685 mileage. However, just with 3 months I purchased this car, the a/c stops cooling and the dealership said this is due to the condenser was damaged by small road stones where the small road stones can easily pass through the grille. - we have noticed that there are multiple reports of Lexus/Toyota suvs with condensers easily punctured by normal road debris. - dealers acknowledge it as a common pattern. - loss of a/c can affect defogging/visibility and is risky in extremely hot weather, e. G. Causing hard breathing, etc for driver and passengers in the car. This issue needs to be fixed by Lexus/Toyota.
The hatchback window shattered without any known cause. It appeared to have been hit from inside the vehicle but no one was in or around the vehicle when it occurred. It was 82 degrees outside and the vehicle was parked in the shade. Lexus told me there were no known issues regarding the window on other Lexus vehicles.

Vehicle was parked, engine off, and unattended. An internal electrical fire originated in the overhead/sunroof wiring area. Damage includes melted wiring harness near the sunroof, burned center console, and soot throughout the interior. The fire originated in a constant-power electrical circuit and spread downward into the cabin. There were no aftermarket modifications, no external heat sources, and no objects in the vehicle that could have caused ignition. Ambient temperature conditions were normal and insufficient to cause this damage. This represents a serious safety defect because the fire originated spontaneously within the vehicle’s electrical system and could have resulted in a full vehicle fire or injury if occupants were present. The failure appears to involve factory-installed wiring/components and is not consistent with normal wear or external factors.

While driving westbound on route 70 in ohio at highway speed, we suddenly heard what sounded like an explosion and immediately felt exposure to outside air within the vehicle. The event was abrupt and very startling while in motion. After safely stopping and inspecting the vehicle, we observed that the driver-side rear door window had shattered. The damage included a pronounced irregular opening located approximately in the middle of the window toward the rear edge, with complete fragmentation of the surrounding tempered glass. We inspected the interior of the vehicle and surrounding area for evidence of a rock, debris, or other projectile but did not locate any object that could have caused the damage. This unexpected window failure created driver distraction, potential risk of injury from glass fragments, and sudden loss of window integrity while driving at highway speed. This report is submitted so that any potential pattern of unexpected automotive glass failure in this vehicle model can be evaluated.
Odometer reading in vehicle doesn't match information on carfax, zilocar, or title reading. Also sellers may have titled jumped vehicle. Recieved call from owner on title I recieved and she said that she had sold vehicle on 2/4/2026 to couple. I purchased vehicle on 2/13/2026 with her name on title. Odometer was correct mileage to carfax when she sold to buyers, yet changed when I purchased vehicle with about 150,000 mile differebce.

I was just driving regular and the Rx350 started to jerk and apply the breaks by itself and slip and go side ways and the car behind me almost hit me because my Lexus started to stop hard and fast . Im pressing the gas but it’s not accelerating. The engine wanted to stop and very dangerous I’m scared to even drive the vehicle. The 12. 3-inch display of the instrument panel is blank at startup. Operating the vehicle with this display blank reduces the ability to see certain malfunction indicators. If malfunctions occur while driving, continuing to drive could lead to an increased risk of a crash or an increased risk of injury in the event of a crash.

I took my vehicle to Lexus of louisville on 2/2/26 for recall campaign number: NHTSA 25v744000 (Lexus internal 25lb06. My backup camera does not work at all but I was told my VIN is not part of the recall even though my year, make and model is. Lexus charged $250 to diagnose the problem and the repair is $1311 for a new camera. The part is on backorder. I drive on average 25,000 miles per year. Not having a back up camera is a safety risk for myself and everyone around me as I am in and out of a minimum of 12 parking lots per day. My service rep, john said to diagnose the issue they replaced my camera with another one on the lot to confirm that was the issue. I have attached pictures of the problem. I do not believe this is a coincidence and hope Lexus will stand up and recognize the larger issue here.
My 2024 Lexus Rx 350h experienced a catastrophic 12v battery collapse (2. 00v), disabling the electronic 'e-latch' door system and rendering interior handles non-functional for standard use. While a manual override exists, it is not intuitive for emergency egress. Additionally, the electronic parking brake and shift-by-wire systems remained physically locked, preventing the vehicle from being moved to a safe location during the power failure. I am reporting this as a failure of primary safety and fail-safe protocols. Dealer did not consider this a a problem because he said I “needed to read the manual before I ever drove the car”.

2025 Lexus Rx350h purchased new on 09/04/2025. The sunroof portion of the panoramic sunroof exploded, sounded like shotgun blast. Then we heard some wind noise but realized none of the windows were open/affected. I started to open the inside electronic sunscreen but noticed glass shards so I immediately closed it back up. We were greatly startled and I could have swerved/loss control of the car, thankfully I did not. Also, if the interior electronic sunscreen would have been open at the time of explosion we could have been cut from falling glass shards and would have probably lost control of the car. We were crossing the old tampa bay on the howard frankland bridge (I-275) heading to tampa, FL airport. There are no overpasses on the bridge so no falling debris was possible and we did not see any air-born road debris that impacted the roof of the car. There were no other dents etc. In the roof or car body indicating an impact. When I took the car to Lexus to get the sunroof replaced the service representative stated it was the third replacement recently. When I picked the car up a couple days later I spoke with the service manager about my extended warranty coverage and the lack of evidence of an impact from road debris. He stated warranty would not cover the replacement and Lexus would not cover the replacement even on a 4 month old vehicle.
I want to raise concern about the safety issue and health issues relating to the new led headlights on cars now. In the morning and evenings, I regularly encounter these extremely bright headlights. It's a big distraction and my driving visual field is impacted by these lights. I would like to see this addressed and resolved! please advise. The SUV pulls hard to the right when accelerating and has almost driven me off the road. It does it at random times and its super dangerous.
I purchased the 2022 Lexus Rx 350 used a couple of months ago. While driving down the highway the instrument panel went blank. I was terrified. I continued to drive not knowing what speed I was driving. The instrument panel went panel came back minutes later. Another incident occurred when the car was parked in my driveway. I turned on the engine and the instrument panel was blank. No numbers and no information on the instrument panel. As I drove down the street slowly the instrument panel returned to normal. A third incident occurred when occurred as we were driving on the highway. The light on the instrument panel began to darken. My husband was driving and I was in the passenger seat I was able to access the light app on the dash and restore the light to the original setting. This car is not on the recall list. It should be because this is a matter of life or death. The car had 59000 miles on it when we bought it so it is not under warranty. I contacted Lexus and they refused any help until I take it to a Lexus dealer and have it diagnosed. I don’t drive it daily because I have other vehicles but I would like to be able to drive it and have full use of the instrument panel. The problem repeats itself.

The moonroof spontaneously shattered while driving on the freeway at night. This limit the speed to less than 20 mph since the moonroof continued to shatter at a higher speed.

While we were driving, our sunroof just exploded! there was no-one around us. There was a loud bang, glass was flying and the wind was so loud we couldn't hear each other. If my wife would have been driving, we probably would have wrecked because she screamed and through her arms in the air. We exited off the road, inspected our vehicle and took pictures. The only damage we could see was the front section of our sunroof was shattered. We drove to the nearest dealership where they took pictures and ordered a new sunroof. I might add they couldn't guarantee the replacement would be under warranty, which I find shocking. My wife and I are on vacation and we had to leave our car in another state along the way and rent a car.
Dashboard crack in 2024, that started on passenger side near air vent, expanded recently causing the "passenger air bag off" alert to light up and stay on continually. If I were to have a passenger at the time of an accident, the air bag might not deploy, which would be a safety issue. This could possibly lead to a liability issue, so I have not had any passengers but that isn't sustainable, so I contacted the dealer. I was told that the previous recall for cracked dashboards ended in 2017 and that it was cosmetic only, not safety related, but that the part was available for $964. 50, not including labor, tax, etc. I feel that a safety related recall should be issued, the previously known dash cracking problem didn't end when their recall did, the fact that my vehicle is primarily garaged preserved the interior integrity, I don't believe I should be penalized for my dash not cracking 8 years earlier. While driving on the freeway the sun roof "exploded" shattering glass out onto the highway. The noise was loud and frightening. Upon pulling over I observed most of the sunroof was gone with fragments of glass remaining along the edges of the roof. I believe this to be a major safety issue. If the ceiling cover had been open, the passenger in the rear seat could have been seriously injured. No doubt flying glass is a danger to any cars following. The vehicle was inspected by the Lexus dealer who declined to cover the damage under warranty. My insurance (csaa) agreed to pay the cost of replacement. The glass shop which did the work indicated this is a manufacturing defect and indicated they replace 5-6 sun roofs a week.
Lexus Rx sunroof exploded on it's own. I just left my driveway and was the only vehicle on the road. There were no projectiles, the sunroof glass simply exploded and glass flew into the air. If I was driving at 60 mph, the exploded glass would have become a projectile and could seriously hurt a bystander. They were no warnings. I filed a complaint with Lexus reference # 250820001020 but their response was that this was not a mfg issue. I pointed out that they are two class action suits on Lexus, but they insist that this is not a recalled part. This is a safety issue with glass flying at 40 to 60 mph in the open air.
I was driving and pressed the button to close the sunshade on the panoramic sunroof when I heard this like a grinding noise. The cloth sunshade stopped moving. Then I heard a pop noise, and the sunshade came down, and it hit me more on the back of the neck. Then I heard something fall on the floor behind me in the back seat area. I couldn't pull over right away, stopped at a store, and there was a black, plastic and metal piece that was on the floor in back. The sunshade itself was partly hanging off the ceiling where it came from. It didn't hurt but of course it startled me. This happened on Friday going to work 7/18/2025. I haven't used the sunroof for a while because it has been too hot but Friday I wanted some natural air, and opened it up. Researching I see that it is supposed to be all one piece, and everything which includes the entire panoramic sunroof to be replaced which is close to $3,000. I found online that a lot of people have the same problem for years 16-22 on the Rx. I found out where the dealers are telling people excessive heat melts the glue holding these adapters in place, and it eventually give way and break. Most people including me park outside, I don't have a garage, and it's summer, this is defective, and it should be covered my Lexus. Since it just happened yesterday, the vehicle wasn't inspected by nobody, there were no warning signs that this was happening either. The safety issue is when it hit me on the back of the neck while I was driving. Thank you for your time.

My 2021 Lexus Rx 350 (mileage approx. [19,000], has developed a severe mold contamination issue caused by hvac condensate drain failure. This issue is a known design defect covered by Lexus technical service bulletin l-sb-0020-21, which identifies blockage or poor drainage in the hvac system that allows water to enter the cabin. As a result, water intrusion under the carpet has led to mold growth, making the vehicle uninhabitable and unsafe to drive. The cabin now contains visible mold and musty odor, raising serious health and air quality concerns. I brought the vehicle to Lexus of route 10 (whippqny, NJ), who acknowledged the water intrusion but have refused to repair it under warranty or goodwill. Instead, they are pressuring me to file a personal insurance claim, even though the issue stems from a known factory defect. The vehicle has now been out of service for weeks, and I am driving a loaner vehicle while mold continues to worsen in mine. Lexus corporate stated that they "no longer goodwill" these repairs, despite the ongoing safety and hygiene risks. There has been no recall or warranty extension, and I believe other owners may be similarly affected by this unresolved defect. This is a serious safety issue due to the potential for mold-related health problems, interior air contamination, and possible electrical damage from long-term moisture. I urge NHTSA to investigate whether this hvac defect poses a broader consumer risk, and whether Lexus is avoiding its responsibilities to repair or disclose this issue to other drivers.

I experienced a sudden release of toxic fumes from the vehicle between the steering column and instrument panel area on June 29, 2025, while driving. The fumes struck my face, caused immediate bitter chemical-like taste and made me feel physically ill and extremely concerned for my safety. The fumes were strong enough to leave a lingering taste described as “bitter and green” with initial symptoms of headache, lightheadedness, nausea, vomiting green substances, eye irritations, nose and throat irritations. The local auto dealership inspected the vehicle, no mechanical defect was identified, and they noted that they cannot reproduced the event that led to the release of the toxic fumes. However, the vehicle manufacturer took possession of the vehicle for 1 week to conduit further inspections.

I bought my car on may 30, 2025, and on June 3, 2025 while driving, the driver’s side rear window behind the passenger’s seat shattered. It sounded like an explosion. No cars were around at the time of the incident. Luckily, the window stayed intact.

The inside driver door handle broken.
I was trying to park behind a car. Slowed down the car. Just about 20 feet behind the car, the car suddenly started to accelerate. The speed was super fast. I had to step on the brake some hard to stop the car. Fortunately the car stopped just in time. No crash happened.
When I was parking my car 20 feet behind a car by the street , the car suddenly accelerated. I stepped hard on the brake and fortunately stopped the car about 3 feet from the car in the front. No crash happened.
