Brakes making honking sound when applied even when stopped. My brake actuator is failing and has been confirmed by the Lexus dealership. Noise and stiff breaks has been going on for a while. There is no recall, but Lexus put out lsb to dealerships may 15, 2023 and revisions earlier. This information should be provided to owners immediately, if the actuator fails, this is a safety issue.

2 Service Brakes problem of the 2021 Lexus RX

I was at the gas station, in line waiting for one of the three gas pumps to be available. I put my car in reverse to back up and pull around another car in order to go to the open pump in front. When my car was in reverse motion, I stepped on the brake pedal, but the car did not stop. I stepped on the brake pedal numerous times, but the brake did not function. My car backed into the front of the car in line behind me damaging both vehicles. The automatic emergency braking system did not activate, and there was no rear collision warning sound or message. I switched my car from reverse into park to stop its movement, but it was too late to avoid the collision. I had a passenger in my car who corroborates this description. Fortunately there were no pedestrians behind my car, so the only damage was to the vehicles. My vehicle has not been inspected by anyone since the accident, but will be soon. I have owned this car since April 2021 and previously had a few similar situations when moving in the forward direction. Previously, the brake problem occurred while moving forward at very low speed pulling into parking places. The brake failed to activate, and I had to throw the car into park to stop it, but this was the first time I had experienced this problem traveling in reverse. When this situation occurs, I am pressing on the brake pedal, but the car is not stopping and the brake pedal seems to go to the floor. On March 5, 2025 I took the car to a Lexus dealer and reported the brake failure problems. They inspected, road tested, and checked for codes, but could not identify the problem. I believe that the collision back up system should have prevented this accident, but it did not function. I suspect that there may be some kind of computer issue that prevents the brake from actuating. The car will be going in for repair soon, and I have requested that a complete diagnostic review be performed of this brake problem.

3 months before the accident the power steering light came on twice and went off. The day of the accident the brake light came on when I started the car that day. It disappeared when I was put in drive. I was entering onto the highway to merge with the traffic. Another car waiting for the light to turn green so he could enter also. My car didn’t stop because my brakes failed and the collision avoidance system didn’t work either. No sounds nor warnings. I have photos of the message that was displayed on the dashboard. Tried sending photos but it didn’t work.

The contact owns a 2025 Lexus Rx350h. The contact stated that while coming to a stop, the brake pedal and accelerator pedal were both depressed simultaneously. The contact stated that there was a loud sound coming from the engine compartment. There was no warning light illuminated. The contact forcefully depressed the brake pedal to stop the vehicle. The vehicle was towed to the dealer but was not diagnosed or repaired. The manufacturer was made aware of the failure. The approximate failure mileage was 119.

The dealer is aware this vehicle has a brake actuator problem,and when I contacted them to see if it had a recall for this specific problem,I was told to call Lexus corporate to get them to honor the repairs at their cost. They told me they would not repair the car at their cost because the extended warranty support program ended 4-28-2018. This is a very unsafe problem and they should honor the repair free of cost to the consumer.

There are multiple recalls on my vehicle and I am experiencing all of the recalls inverter failer vehicle stalls out alot. Crank shaft pulley is leaking oil and belt from the power steering maybe detached causing vehicle to stall. Inverter assembly causing the vehicle to stall and become non drivable. Floor carpet cover and retention clips. And my brake master cylinder leaking fluid causing major break problems. Every item I mentioned is recalls for the vehicle I have.

The contact stated while driving 35 mph on a rainy day, the brake pedal was depressed; however, there was an abnormal squishing sound coming from the brake pedal, and the vehicle failed to stop as intended. The contact stated the vehicle went through the stop light and up onto a curb before the vehicle stopped. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. Additionally, the contact stated that the failure had occurred previously. The vehicle was taken to the dealer; however, the dealer confirmed the abnormal squishing sound when the brake pedal was depressed. The vehicle was not repaired. The contact was informed that the manufacturer had declined to repair the vehicle. The manufacturer was made aware of the failure and a case was filed. The contact was informed that both actuators had been replaced previously. The contact referenced technical service bulletin: l-sb-0005-21; however, the contact was informed that the repair was not covered under warranty or any special programs. The failure mileage was approximately 31,600.

When I start to coast to come to a stop the engine will rev, even though the energy flow screen shows the gas engine is disengaged and running only on ev. The engine will rev, even with me using the brake. But the ev light is off and the rpms are rising. Eventually the ev light will come on and the gas engine will cut off and the vehicle will switch to ev mode. My mpg has dropped to 36. 2, so something is not right. When I was at the dealer for my 10k service, they told me this was the car "shedding" the extra energy, but the battery was not fully charged, so I'm perplexed and frustrated.

Below is the email that I sent to both the manufacturer and the dealership: I hope your christmas eve was better than mine. My son and I were driving up to the poconos christmas eve to go skiing and I noticed the sonar detection light come on. When we were driving back from the poconos, the transmission would not catch around the second or third gear. The rpms would shoot up. The dash / warning lights lit up. I pulled over to the side of the road and I called Lexus dealership, service center, and road side assistance. However, due to the fact that it was christmas eve, the offices were closed. I managed to make it to my parent’s home, but it was a very dangerous ride on rt. 80. The odometer stated that we were going 50 mph, but we were crawling and the rpms were 4000. The rpms at rest were always over 1000. Additionally at times, the gear would not shift into second or third and/or bottom out. Obviously the transmission is beyond malfunctioning. Below are pictures including: 1. ) check your engine: a malfunction in the electric control of the engine, throttle or automatic transmission has been detected. . . 2. ) a malfunction in the intelligent clearance, sonar system has been detected 3. ) traction control turned off 4. ) secondary collision break system malfunction, 5. ) system malfunction 6. ) drive-start control malfunction. Brake actuator failed with exact symptom in [xxx] repaired in dealer and the same symptom comes back within 2k miles. It seems a design flaw that affected many Lexus Rx hybrid and Toyota highlander hybrid cars, and likely due to air being trapped in the brake lines or brake actuator components. Air in these system is a major safety hazards. Please help to make it a recall and ask Toyota to come up with a solution that really solve this issue. Similar experience from many other owners. I am writing to formally express my serious concerns regarding the brake actuator on my Lexus vehicle, which remains unrepaired despite being under warranty. This issue poses a significant danger to both myself and the public, and I am seeking an urgent resolution. I have been a proud owner of my Lexus Rx450h 2022, and the vehicle is currently under warranty. Recently, I visited the Lexus dealership in roseville, California, where it was recommended that the brake actuator be repaired due to its faulty condition. Despite this recommendation and the warranty coverage, the repair has not been carried out. The malfunctioning brake actuator is a critical component of the vehicle's braking system. Its failure can lead to inadequate braking performance, significantly increasing the risk of an accident. This issue not only jeopardizes my safety but also endangers other road users. It is imperative that such a critical safety concern is addressed promptly to prevent potential harm. Vehicle does not stop properly due to defective part. Lexus has replaced two actuators and refuses to replace another one as it cost 3200. This is a serious safety issue that can cause physical harm or even worse death.

The accident happened when I was using the "brake hold system". It is designed to use when the needs to stop briefly. When the brake hold button is touched while the driver's foot is on the brake pedal, the driver do not need to step on the brake even though the shift lever is in d. After I finished paying for a car wash, I touched the brake hold button. Then the vehicle suddenly accelerated forward and ran at a high speed. I was pushing the brake pedal, when I touched the button. I was startled and attempted to stop the vehicle with all my strength by braking the brake pedal. It didn't stop and went over the cub and accelerated even more and it crossed the both lanes and hit the cub on the other side. It climbed up the bank wall and it finally stopped. The police and the ambulance came and took me a hospital. I later found out the vehicle was towed to a nearby junk yard. I believe that the new high-tech system known as brake hold malfunctioned and caused "unintended acceleration". I believe that it certainly is extremely dangerous. It has not yet been inspected by the Lexus mechanics. There were no warning signs whatsoever before the unintended acceleration. Btw, I have used the function before I. E. I am writing to raise a serious concern regarding a issue with my 2018 Lexus Rx450hl involving the brake actuator. Since purchasing the vehicle at 63,000 miles, I have noticed hissing, clicking, and squeaking noises when applying the brakes. As the mileage increased to 83,000, these noises have worsened, becoming unbearable. In addition, I am experiencing a shaky driver-side mirror and a rusting hood latch, both of which present safety concerns. For a vehicle under 5 years old , I believe these problems do not reflect the quality I expected, particularly given the premium price I paid. According to Lexus' technical service bulletin (TSB) #l-sb-0032-23 rev, issued in October 2023, which addresses squawk or knock noises from the engine compartment when the brake pedal is pressed or released, affecting models like the Rx450hl. My vehicle exhibits symptoms similar to those described in the TSB. The vehicle was regularly maintained by the previous owner at Lexus of thousand oaks and woodland hills. Despite this, the major part like brake actuator has failed prematurely. Given Lexus' reputation as a luxury brand, it is unacceptable that such a critical component would fail before the vehicle is 5 years old or has reached 100,000 miles. The combination of the brake actuator defect, the shaky mirror, and the rusting hood latch is disappointing, especially for a vehicle of this caliber. When I initially contacted Lexus customer service, I was also dealing with rusting parts and a failing brake booster. Lexus of cerritos provided a repair estimate of around $5,000 for brake actuator. I was advised to reach out to Lexus customer service after my request for a good faith repair was denied due to the inability to obtain the complete maintenance history. However, I have since confirmed with the previous owner and Lexus that the vehicle was regularly serviced. I am also aware that Lexus has issued brake actuator-related recalls for several models.

1. At [xxx] on [xxx], I was pulling into my gym's parking lot, when I stepped on the break to park my car, instead of stopping, the car accelerates rapidly and crashed through the other side of the sidewalk where there were light pole and cement walls, throwing me forward and backward with seat belt on, I was in total shock! 2. My insurance sent a field inspector to the body shop 2x in hope to repair my car but ended up declaring it a total loss. 3. Lexus said they would send a field tech to get the edr (event data recorder) data from my 2018 Lexus Rx350 within 30 days, but the results won't be available for 30-45 days after that. Lexus's slow response to this accident concerns and disappoints me b/c it was a serious crash that Lexus should treat it seriously and promptly. 4. The disbelief of a 'young' car with only 12297 miles could have issues like this puzzled me and drove me to do a lot of online research, then I came across this site [xxx] which contains an article titled '10 common problems of a Lexus Rx350'. Problem #5 described exactly what had happened to 'one recurring problem this car has is that the system malfunctions severely. This typically happenes when the driver is trying to park the vehicle. Reports paint the same pictures repeatedly: the driver is pulling into a parking lot, they push the break pedal, and instead of stopping, the car acelerates rapidly, which results in a crash. " based on this article, it seems that Lexus should have issued a recall, but no! 5. The strange thing was that no airbags were exploded/activated, yet the vehicle was damaged to beyond repair, it makes me wonder if there was an issue with the airbags too. If this incident had occurred at an intersection, I dare not imagine the outcome. . . I could have been killed or critically injured along with other innocent people on the road. I’ve approximately the last 45 thousand miles, Lexus dealers have replaced the brake actuator and other components of the brake system on this vehicle at least 6 times due to noise issues. Lexus has stated that this is not a safety issue, I have an open ticket with Lexus, but yesterday I had to slam on the brakes and did not get nearly the response that should have come right away. There are no warning lights, as far as I can tell the traction control didn’t engage, and eventually the car suddenly started breaking much harder, but it was very delayed l, and disproportionate with any additional force applied. Lexus knows the brake system is an issue in this model to the point where they are covering the system for 10 years or 150,000 miles, but to say that it isn’t a safety issue seems short-sighted. This system works in conjunction with the radar sensors adaptive cruise control, and I’ve had. This car has also had a number of seeming electrical issues, with every door lock actuator replaced at least once, some of them more than once, and the infotainment system shuts down and reboots randomly or freezes.

Lexus has admitted a manufacturing problem with the pumps and has a service bulletin (l-sb-0010-18) and a safety recall on the 2017's (jld j2d). The recall should be extended to the 2016's. I started to notice that when in reverse or coming to a brake at low speeds, the brake pedal and car starts to vibrate/stutter. I have driven hybrids for the last 5 years and never had this issue before. It is very strange that the car vibrates when coming to a stop and it is on going. Hydraulic brake booster pump abs actuator has been on a service bulletin around 2015 with a definitive timetable. I have had numerous notifications with warning lights that are indicative of the booster pump failure for more than 10 years and every time I bring it to the Lexus dealer they said it is my battery. They replace the battery and reset the computer and it repeats itself over and over. My main concern as this should have been a recall and not a service bulletin; an abs brake failure is a significant defect with possible fatal outcome. The problem has been well documented with numerous Lexus and Toyota customers hybrid vehicles going back to 2005. My vehicle is 2007 Lexus Rx 400h with 78,000 miles. I am afraid to drive the vehicle with the issue unresolved.

Car turned on and no driving yet. All vehicle alert signs popped up. This is brand new car. Warning signs including: anti-lock brak system advanced ultrasonic detection parking braking system pre-collision system blind spot monitor these signs are constantly popping up without any hazard condition. Lexus keeps refusing to do thorough exam because they keep stating there is no issues with the vehicle.

Lexus 2019 Rx350 driving down hill with adaptive cruise control on tries to keep speed set at 72mph but as going down hill car gains speed so adaptive cruise control applies brakes to slow down. After 1-2 mile of downhill driving the car starts to vibrate severely until adc is turned off allowing brakes to cool. Brakes seem normal as they slow car down without the vibration. Engaging adc starts the whole process over again while going down hill. Adc does not cause vibration if on normal level or slightly downgrade sloping road.

I am financing a 2006 Lexus Rx400h hybrid and the other day I went out to start it up and the light and everything came on but would not display ready and would not start. Check hybrid system with red triangle was on dash display along with several other warning lights, also when I put the key in the ignition it will release in the acc position. Where I am purchasing the vehicle will not fix it saying my VIN is not part of the recall list and the Lexus dealership to me it was to late to have the recalled parts replaced for free. What do I do?.

The accident happened on 12/28/22. At 8:50 am, I was picking up a friend and was leaving the parking lot. My friend got in and and then opened the door to lock it again . My foot was on the gas pedal trying to leave the parking lot. The car jumped and went to the opposite side. I turned the steering wheel to keep it from hitting an object but could not get any control on the car. The car kept accelerating and I had to put the car back into park to stop the car. The acceleration was so much that the car jumped the curb and stopped by a wall. The air bags opened up.

In about 2016, abs light went on and I took it for repair at an independent auto shop for $400. A few months later,received a notice from Lexus of abs issues so after speaking to Lexus, USA and sending a copy of receipt,I received a credit for having already paid to fix it. Fast forward to Feb 2023 and abs, brake,etc. . Lights all go on. Received estimate from southbay Lexus for over $5200 to fix abs actuator assembly and hoses and pump kit, etc. . Found out today online after turning it in for repair at Lexus southbay that there was primary coverage and secondary coverage offered for this exact issue and a safety/recall letter was supposed to be sent out starting in 9/2020 to owners of Lexus 2010-2012 Rx450h vehicles. I never received that letter, so I missed that deadline to receive an inspection/service. Was I and my family in danger? after basic coverage expired at 48k miles, primary coverage expired in 12/31/21 and secondary expired for owners of 10 yrs or 150k miles or less whichever comes first for this exact issue. I don’t drive much, especially since covid started so my 2010 Lexus as of today only has only 73,124 miles. Less than 1/2 of 150k miles for secondary miles. I was never informed that there would further issues with abs system nor this extra primary and secondary coverage was available.

Abs actuator failure causing wheels to lock up in wet conditions along with loss of control. . I nearly crashed into pedestrians when it failed. Suddenly check abs light came on along with abs system malfunction. . . I could've been killed or killed someone. Lexus will not cover the repair though they did for customers under 50k miles and under warranty which is completely criminal. Any Lexus of Toyota highlander with abs assembly part # 44510-48080 needs to be recalled and replaced at Toyota and Lexus immediately. They want $4000 to fix the system knowing they sold a critical safety part that malfunctions.

Lexus 2018 Rx450hl is known to have issue with brake actuator. A faulty brake actuator causes the car to fail sudden stop. It’s definitely a safety issue. Lexus have technical service information bulletin(tsib) for this issue since 2019. They never contact the owner to replace the faulty part. So if the car is under warranty they will fix the problem with no cost. But if the warranty pass the threshold, they are charging the costumer 3. 5k to replace it. It’s an absolute horrible business practices. This part is a manufactured defect and it’s a safety issue.

I purchased a 2017 Lexus Rx 450h fsport last year at carmax. About 6 months ago, I noticed that every time I pressed the brakes, there was a chirping sound. I figured it could be dust in the calipers. As the months went by, the chirping sound has gotten louder and now it affected the parking brakes. I researched this problem and many people online have also complained that they are hearing this chirping sound or what the dealer calls a "brake squawking" sound. I received a letter from Lexus saying to bring my vehicle in for a possible brake booster and fuel pump failure. So I brought my car to the dealership. Since it's there I complained about the chirping sounds that it's making every time I step on the brakes and when the parking brake is applied. I told the advisor at Lexus that a lot of people online have also complained about this problem and it should be a "recall". He said that it's not a recall. To make the story short, the dealership didn't find anything wrong with the brake booster and the fuel pump. What's causing the chirping or squawking sounds was the "brake actuator". I was quoted $4,327 to have these parts replaced because my Rx was already past the 50,000 miles warranty. I brought my Rx to the dealership at 55,483 miles. It was a great thing that we purchased the extended warranty at carmax called the max care which I only had to pay a $100 CO-pay. Amazing! it took about 2 days to have this completed. Now, the chirping sound has disappeared when I press the brakes and when the parking brake is applied. I truly believe that this should be a recall by Lexus and be covered. It shouldn't matter how many miles the customer has. This is a safety issue. The brake actuator shouldn't fail this soon. I have a 2007 Lexus gs350 with 225,000 plus miles and I never replaced the brake actuator. I don't know if even has one. The bottom line is when I press the brakes on that car, there were never any chirping sounds.

It was found during inspection of front break calipers that rubber dust boots on 3 out of 4 pistons (2 pistons per caliper) were torn apart. Chunks of cut out rubber boots were found inside the pistons. The car never had break work done since the time it was manufactured. The brake pads on both front wheels are still factory original. The only way for dust boots to get damaged is to have improper installation during car assembly process where rubber boot is not tacked fully inside caliper. The issue would result in premature wear of piston seals dust boot supposed to protect. Wearing off seals would result in leak of break fluid and potential sudden loss of break functionality. The workmanship issue is likely to be impacting other vehicles assembled on the same factory floor by the same employee.

The contact owns a 2012 Lexus Rx350. The contact stated while driving 70 mph, the abs warning light illuminated, and the abs seized. The contact was able to coast to the side of the road. The vehicle was restarted and functioned as intended. The vehicle was taken to the dealer; however, no cause for the failure was found. The contact stated that the failure persisted. Upon investigation, the contact related the failure to NHTSA campaign number: 14v054000 (service brakes, hydraulic, electronic stability control (esc); however, the VIN was not included. The manufacturer was not made aware of the failure. The failure mileage was unknown.
