Lexus 2018 Rx450hl is known to have issue with brake actuator. A faulty brake actuator causes the car to fail sudden stop. It’s definitely a safety issue. Lexus have technical service information bulletin(tsib) for this issue since 2019. They never contact the owner to replace the faulty part. So if the car is under warranty they will fix the problem with no cost. But if the warranty pass the threshold, they are charging the costumer 3. 5k to replace it. It’s an absolute horrible business practices. This part is a manufactured defect and it’s a safety issue.

I purchased a 2017 Lexus Rx 450h fsport last year at carmax. About 6 months ago, I noticed that every time I pressed the brakes, there was a chirping sound. I figured it could be dust in the calipers. As the months went by, the chirping sound has gotten louder and now it affected the parking brakes. I researched this problem and many people online have also complained that they are hearing this chirping sound or what the dealer calls a "brake squawking" sound. I received a letter from Lexus saying to bring my vehicle in for a possible brake booster and fuel pump failure. So I brought my car to the dealership. Since it's there I complained about the chirping sounds that it's making every time I step on the brakes and when the parking brake is applied. I told the advisor at Lexus that a lot of people online have also complained about this problem and it should be a "recall". He said that it's not a recall. To make the story short, the dealership didn't find anything wrong with the brake booster and the fuel pump. What's causing the chirping or squawking sounds was the "brake actuator". I was quoted $4,327 to have these parts replaced because my Rx was already past the 50,000 miles warranty. I brought my Rx to the dealership at 55,483 miles. It was a great thing that we purchased the extended warranty at carmax called the max care which I only had to pay a $100 CO-pay. Amazing! it took about 2 days to have this completed. Now, the chirping sound has disappeared when I press the brakes and when the parking brake is applied. I truly believe that this should be a recall by Lexus and be covered. It shouldn't matter how many miles the customer has. This is a safety issue. The brake actuator shouldn't fail this soon. I have a 2007 Lexus gs350 with 225,000 plus miles and I never replaced the brake actuator. I don't know if even has one. The bottom line is when I press the brakes on that car, there were never any chirping sounds.
It was found during inspection of front break calipers that rubber dust boots on 3 out of 4 pistons (2 pistons per caliper) were torn apart. Chunks of cut out rubber boots were found inside the pistons. The car never had break work done since the time it was manufactured. The brake pads on both front wheels are still factory original. The only way for dust boots to get damaged is to have improper installation during car assembly process where rubber boot is not tacked fully inside caliper. The issue would result in premature wear of piston seals dust boot supposed to protect. Wearing off seals would result in leak of break fluid and potential sudden loss of break functionality. The workmanship issue is likely to be impacting other vehicles assembled on the same factory floor by the same employee.
The contact owns a 2012 Lexus Rx350. The contact stated while driving 70 mph, the abs warning light illuminated, and the abs seized. The contact was able to coast to the side of the road. The vehicle was restarted and functioned as intended. The vehicle was taken to the dealer; however, no cause for the failure was found. The contact stated that the failure persisted. Upon investigation, the contact related the failure to NHTSA campaign number: 14v054000 (service brakes, hydraulic, electronic stability control (esc); however, the VIN was not included. The manufacturer was not made aware of the failure. The failure mileage was unknown.

While driving in local conditions (traveling 20 mph) the brakes stopped working partially (about 90% of braking power was lost) and a message on the dash board show check vsc, after pulling aside it turning off the engine. I inspected the brakes in the engine and at the wheels for leaks. No leaks. Upon restarting the car all was normal again.

Electrical system malfunction began (to my best calculation & recollection) in summer 2021 with the first of four battery malfunctions in one year. Each time, aaa discovered low battery response and recommended battery replacement. This time around, battery was replaced about two weeks prior, due to battery failure/aaa reporting would not accept charge. The new battery failed (8/3) and the starter broke while attempting to charge. Mechanic repaired starter and battery accepted the charge. No problem, right? wrong. This driver drove about 30 miles and while driving, the electrical system shut down altogether. With I believe all warning lights lighting up, extremely dangerous situation. No breaking, no steering, no warning. Fortunately, it occurred in a suburban residential area as opposed to the highway. Second tow to my mechanic and now, after a full week, no diagnosis, though the car has been running in the shop. This is an intermittent problem. I suspect the alternator.

The sole occupant driver drove into a slightly elevated driveway and stopped. The driveway is maybe 5 degree incline. The shift select lever was placed in park and the driver visually confirmed it was in park. Driver sat in the car for 12 seconds before exiting the vehicle with the engine running and ac on. The driver exited and was standing outside of the vehicle with both feet on the ground for 6 seconds reaching back inside at which time the vehicle began to suddenly roll down the driveway causing injury to the driver. This seems to be a failure of the parking system of the vehicle causing serious injury to the driver. The vehicle has not been inspected yet and investigation is ongoing. It is unknown if there were any warning lamps, messages or other symptoms of the problem prior to the failure, but there was no prior service work performed on the near new vehicle. Vehicle brakes are inconsistent. The abs system also locks up periodically, and fail to engage at other times. Extremely dangerous to drive. They are also very noisy. I contacted both the local Lexus dealership and national number, but they refused to fix them under warranty. Lexus did offer an extended warranty in 2016 or so, but since I was the second owner of the vehicle, I never received notification of the extended warranty that went through Dec. 31 2017. In September, 2020, I could no longer deal with the brakes, so I bought a new abs actuator from the Lexus dealership and installed it myself. It was straight forward and took less than an hour to install and bleed the brake lines per Lexus instructions. Less than 16 months later, the abs system is failing again. Lexus told me that the part warranty is only one year, and refused to cover the expense to fix.
Car displaying parking brake malfunction and and cruise control malfunction intermittent when using the parking break, seem that TSB (t-sb-0168-19) was open for camay from 2018 to 2020 exhibiting the same issue but not for this car. This a safety concern since parking brake becomes disable, this issue resets after restarting the car again.
I was driving up a hill, turning on to a dirt road. I had my foot on the brake as I was going down hill, I bounce from the crevice on the dirt road. My car started going faster, I kept my foot on the brake trying to stop, but it did not stop. I avoided hitting a parked car and slammed in to a k-rail. My car would not stop.
My awd check system keeps going on and off every few days. I took it to mavis who gave me codes and stated that there is a TSB found to the issues my vehicle read. I took it to Lexus of brooklyn who basically disregard the recommendations just so I javelin to pay them again to diagnosis whay was already diagnosed.
The contact owns a 1999 Lexus Rx300. The contact stated while driving approximately 35 mph, the brakes failed and could not be depressed to stop the vehicle. The contact was able to drive the vehicle to the side of the road and stopped by shifting into neutral. The contact stated the "check engine" warning light illuminated intermittently. The contact had the vehicle towed to an independent mechanic, where it was not diagnosed and not repaired. The contact read of a related NHTSA campaign number 99v012000 (service brakes, hydraulic). The manufacturer had not been informed of the failure. The failure mileage was approximately 249,000.

Unknown suspect brake actuator failure. Brake noise when stepping on pedal at a stop.
There seems to be a barking noise when pressing the brakes even when engine is not on. Local shop could not diagnose the issue. I heard this could be the regenerative system that is expensive to repair. Braking seems ok but I am concerned as I thought there were some recalls on brake booster for this year but they say not on my car.

2008 Lexus Rx 400h faulty abs actuator yes my car is available for inspection upon request . This is a known problem on this vehicle there is 100s of complaints on same issue with the same cars Lexus is neglecting responsibility for this faulty and very dangerous hazard , this part has been faulty for years with out my knowledge and now it’s gotten worse all the lights related to my brake system is on and the brakes seem to have a mind of there own they break real hard at times and times I have to slam on them to break . Yes I have had the car checked and the dealer told me that there was a extended warranty issued on this part but that it’s expired. They also gave me a quote $2,100 just for the part no labor that’s crazy, this is not a extended warranty matter this should be recalled there is thousands of people driving this cars with out knowing that there breaks could give out anytime . I hope that Lexus considers the seriousness of this matter and not wait on till lives are lost do to neglecting. They took effort years ago in extending the warranty on the part but there is thousands of owners like my self that were not aware or notified of this extension , also this is a malfunction that is not noticeable and can happen at anytime especially while you driving .

The contact owns a 2019 Lexus Rx350. The contact stated while his wife, was reversing in the driveway she hit the retaining wall the driving alarms failed to engage and alert. The driver depressed the brake pedal however failed to respond and depressed to the floor board. The emergency braked failed to engage. 2007 Lexus Rx400h,,,couple week ago ,,,my sister drive this car on the highway,,,suddenly the check engine, abs, traction control, vse, ect light come on and the vehicle when brake will pull to right also brake grab immediately. Bring to the tech and get a code c1391 abnormal leak in accumulator,,,that mean replace the whole abs unit assembly bring to Lexus dealer on 9-30-2021 and still get the same code c1391 and pay $175. 93 for diagnostic and cost repair $3,573 (this condition may be caused by a small internal brake fluid leak inside the brake actuator) call Lexus head quarter and turn out this is only basic warranty,,,,,no recall for safety,,,, because no one dead yet research online turn out a lot of problem about this: (2006 - 2007 GS 430) (2007 - 2011 gs450h) (2008-2011 GS 460) (2006-2008 Rx400h) (2007-2013 ls460) this only Lexus,,,,more about Toyota but Lexus still ignore about this safy.

Toyota and Lexus are aware of the fact that what is called break actuator assembly, part number 44050-48191 (for Toyota highlander hybrid 2006-2007, and new part number for Lexus Rx400h 2006-2007) and 44510-48060 (old/original part number for Rx400h 2006-2007) are faulty resulting in the break system failing creating an extremely dangerous situation. Toyota extended the warranty for the part, but this extended warranty does not cover all vehicles, as it is not a recall. (the extended warranty has a maximum 150k miles or 10years clause but this is beyond the point regardless. ) the part and repair costs more then $3000 at a Toyota/Lexus dealer. Toyota/Lexus is not offering a recall to avoid paying for the large number of cars out there with this well known issue which essentially renders the cars not drivable, unless the owner pays out of pocket what is close to half the current value of the car, for a defective item that is failing consistently in all these cars. Toyota/Lexus should recall the part, rather than just offering an extended warranty. This is a very well known problem, for which people have complained (including in NHTSA) for years, which forced Toyota/Lexus extend the warranty, but this is not enough as it does not cover all vehicles that it should cover.

July 2021 at 69568 miles on 2008 Lexus Rx350 brake booster was changed by Lexus of orange park, florida, for $1652. 97. Situation/discrepancy: after normal stop, brake light would illuminate and brakes would mysteriously release and vehicle would start rolling forward! after several brake pumping actions vehicle would stop. Brake booster was leaking internally! corrective action: replace power brakes booster.
When I press on my brakes they make a loud roaring sound right away. Usually when brakes are about to go out its usually a squeaky noise then it will go to loud roaring noise. There has not been an indicator on the dash about any issues with the vehicle. I found it rather odd that the brakes made the roaring noise immediately. It didn't squeak at all. I would like for this matter to be looked into. There is no way there should be a brake system failure right away. The schrader valve has a crack and freon is leaking out of and the air is not blowing cold air.

Brake actuator failure reported by dealer because booster pump is running frequently. No info from Toyota and Toyota declined repair . Dealer indicates my vehicle is not under extended warranty. 117000 miles on my car.
While driving brake & abs warning lights displayed on the dash with the warning brake malfunction, took it to the shop they diagnose bad brake module. My brake pads & rotors and all scedule maintenance uptodate.

The car makes a "brake squawk" noise when the brake pedal is applied. The noise sometimes is unbearable. Dealer replaced the brake actuator. The noise came back after a few months.
I called the local dealer was told they can not help me. I call the Lexus brand engagement center line and was called back with an explanation why my vehicle is not eligible. Even though it IA exhibiting the malfunction. The deadline was December 31,2020. This last refusal to deal with a part that they are well aware is malfunctioning due to their failure to build a reliable product, as they have offered their own version of satisfaction for those lucky enough to have the failure occur timely. Mine did not know if the timely requirements and opted to malfunction less than a year after the established deadline. However, the letter they sent went to the previous registered owner. As a registered owner in the state of florida, it is my opinion that lexis could have easily located me through the help of the state of florida registration office. Which is what their site states is done. I purchased this car June 2020. Lexus/Toyota should have to fix because it is a known issue that they sent out customer support warranty extensions for previously. As of this week my vehicles braking ability has only 10% of what it had a month ago and is no longer safe to drive! I havent owned for a year as yet smh! it is now parked in my garage.
Car has been making a clicking noise during stops for a year. I chalked it up to something unimportant. On 2/20/21, the �check avs,� �check abs,� and �check brake system� lights all came on, and car lurched while backing out of my driveway. I was driving my wife and daughter at the time. Brought Lexus to my local mechanic on 2/22/21. After several days, mechanic returned the car to me, saying he could hear the clicking and, according to his research, it could be the brake actuator given Lexus had done a warranty extension on this part in 2016. He told me this repair would cost in the thousands of dollars, and advised me to contact Lexus. Spoke with Lexus brand manager on 2/25 who told me about warranty extension zlg for actuators in 2016, and told me that in order for Lexus to ascertain whether or not the car was experiencing an actuator failure, it must be inspected by a Lexus service center. Because I was told about the specific extension and asked to take it to Lexus to diagnose the issue, I assumed Lexus would cover the repair cost. I took the car to Lexus santa monica on 3/2/21 and was notified that the problem was related to the brake actuator, and that the cost to repair would be $3000 plus tax. I relayed to Lexus sm what I was told by Lexus corporate, and Lexus sm told me they would call Lexus corporate and see what they had to say about warranty extension �zlg. � ultimately, Lexus would not cover cost because the part was outside the extended warranty. I spoke with another Lexus case manager about why a warranty extension was offered as opposed to a recall, and he said I should lodge a complaint with NHTSA. I also spoke with his supervisor, who said Lexus would never pay for a part outside a warranty, even a faulty part given an extended warranty. Nobody in the Lexus chain of command can tell me if the car is safe to drive. I will support class action suit.
When driving I would have all the lights on my dash come on with a dinging sound and when I go to press on my brakes vehicle will not stop unless I press down hard on the brake this is really dangerous will cause a deadly crash and to replace this will cost 3000 dollars that I do not have I reall believe Rx 400h have a defeat with there braking system.
"check vsc" system with accompanying continuous beep noise, brakes become hard to push/activate. This started a few weeks ago, and would happen intermittently, and progressively longer in the dysfunctional brakes phase. I had an employee at auto zone clear the code 2 days ago, but the problem started again, intermittently upon vehicle start, but would go away shortly, with normally responsive brakes. This intermittent issue occurs when I am driving, therefore this is a serious road hazard as my brakes become significantly less responsive when the problem occurs. In other words, this occurs when stationary and when driving, intermittently.
Daughter was driving, conditions were dry, sunny fall weather. Avg speed 45 mph, light turned yellow, vehicle in front started to slow down, daughter applied brakes, pedal went to floor, no slowing down, repeatedly pushed brake and applied full force, car did not slow down, hit other vehicle, totaling the Lexus. Airbags did not deploy until after my daughter hit the steering wheel. Witness reported seeing my daughter trying to brake but the car not responding. No evidence of tire marks on the road.
