1 Vehicle Speed Control problem of the 2024 Mazda CX-5

Failure Date: 08/11/2025

The contact owns a 2024 Mazda Cx-5. The contact stated that while his wife was entering the garage, at an undisclosed speed, the vehicle suddenly accelerated; the brake pedal was applied but failed to respond, and it crashed into the refrigerator. The contact was concerned that the failure could have occurred while children were at play. No warning lights illuminated. There were no reported injuries, airbag deployments, fires, or police reports filed. There was a tire imprint on the garage floor. The contact called the local dealer, who scheduled an appointment. The vehicle was not diagnosed or repaired. The manufacturer was contacted, who opened a case and confirmed there was no recall coverage. The failure mileage was approximately 10,000.

2 Vehicle Speed Control problem of the 2019 Mazda CX-5

Failure Date: 07/14/2025

For the 3rd or 4th time since I have own this car I experienced a loss of acceleration when attempting to make quick acceleration while attempting to merge with traffic. Acceleration is fine around town where quick acceleration is not appropriate. But the first attempt to merge with flowing traffic the car will slowly accelerates to about 30 mph and no more regardless as to position of accelerator pedal. On all occasion this bad behavior has this first drive of the day - going to work in the a. M. There is often a warning message indicating the safety radar is not working due to limited visibility which is a very common warning in the a. M.

4 Vehicle Speed Control problem of the 2024 Mazda CX-5

Failure Date: 05/01/2025

Straight to the point, I backed the vehicle up, pushed the brake peddle in and shifted into park. The engine begun to increase in rpms and I could hear the engine load increase, maybe about 2,000 to 5,000 rpms higher than idle or first start building vacuum idle, as if I was pushing on the accelerator. I literally looked down at the peddles, as I was just doing a quick spot change in my driveway, the accelerator did not appear to be stuck or in contact with the aftermarket winter weather plastic floor mat. I decompressed the brake peddle again and shifted the car from p to d and the engine went to its normal baseline idling rpms and sound. I put it back in p and turned it off, no issue. I have noticed this problem before, but I only noticed it during this past winter season and blamed it on my winter boots and/or myself instead of the vehicle. I am going to guess this has happened in total about 4 times, including the most recent incident that I just wrote about. The Cx-5 is connected to Mazda's network/ data service or whatever it is called, because I like my cars clock to maintain correct time, which it won't or can't do if I prefer my privacy from Mazda. So Mazda should be able to see what happened on their end. It happened in the late afternoon to early evening. I do also use 'auto hold' daily and cruise control w/out radar active.

5 Vehicle Speed Control problem of the 2025 Mazda CX-5

Failure Date: 12/04/2024

1. What component or system failed or malfunctioned, and is it available for inspection upon request? the vehicle exhibits trembling while idling and previously showed erratic behavior during hard stops. Possible issue with the engine control software or powertrain system. The vehicle is available for inspection upon request. 2. How was your safety or the safety of others put at risk? the trembling at idle and inconsistent braking behavior compromise vehicle stability and driver control, especially in heavy traffic or emergency situations. These symptoms raise serious concerns about a potential underlying defect. Mazda has issued recent software-related recalls involving loss of drive power—though not for my model—which heightens my fear that similar risks may exist. I should not have to wait for a catastrophic failure or breakdown to validate my safety concerns. By then, it may be too late. I do not feel safe driving the vehicle. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? yes. Mastria Mazda in raynham, MA acknowledged the issue and performed a software update. While braking behavior improved slightly, the trembling at idle and other concerns persist. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? no formal inspection has been conducted. I mailed a detailed letter to Mazda’s corporate office in hiroshima in July 2025 and received confirmation of receipt, but no response or inspection followed. I also mailed a letter to quirk Mazda, the dealership where I purchased the vehicle, and received no reply. When I attempted to discuss the issue the service manager at quirk Mazda dismissed my concerns and stated it would be “impossible to recreate” the issue. No one from Mazda or its representatives has contacted me to inspect or address the problem. 5. No warning lamps or dashboard messages appeared. The trembling and braking issues began shortly after purchase.

6 Vehicle Speed Control problem of the 2020 Mazda CX-5

Failure Date: 10/31/2024

I was in the drive thru at a restaurant and had my foot on the brake. After a couple of minutes of sitting with my foot on the brake, I gently pressed the accelerator to slowly move forward. All of a sudden the car sped up on its own like I had floored it. The speed of acceleration threw my head back and the car whipped around the front of the building as that's where I was steering. I braked and it thankfully stopped, but if a car had been in front of me I would have slammed into it. I could have hit a pedestrian. This was very scary and its an obvious safety concern. Mazda needs to look into this issue as I see there are other reports on this website and on the internet by numerous people for the same issue of sudden acceleration by the vehicle on its own.

7 Vehicle Speed Control problem of the 2020 Mazda CX-5

Failure Date: 09/18/2024

The vehicle accelerated on its own, while I was parking. My foot was on the brake when the vehicle just took off. It continued to accelerate until a building stopped it. I continued to brake but the vehicle did not stop. Once the vehicle collided with building, saw on the digital read out the "brake system malfunction" and then camera malfunction. The vehicle was towed from the site and the insurance adjuster has sent me word that the vehicle is a total loss.

8 Vehicle Speed Control problem of the 2021 Mazda CX-5

Failure Date: 08/22/2024

Will driving on the interstate, using the adaptive cruise control feature, vehicles were slowing down to a stop ahead and my car accelerated instead of slowing down. I then pressed the brake and simultaneously, the car breaks locked down (automatic braking). Unfortunately I wasn’t able to avoid an accident as I rear ended the vehicle in front of me. My vehicle has been turned over to the insurance company to assess damage. I’m sure it can be inspected by NHTSA. Mine and others safety were at risk due to the malfunction of the cruise control accelerating instead of stopping. It caused an accident. Luckily no one was extremely injured. I have neck and back pain from the collision that will hopefully heal in a few days. Time will tell. But this could have been much worse had I not reacted as quickly as I did. This could cause a fatal accident. The care isn’t drivable so it has not been reproduced by anyone. I have notified the insurance company about the issue and it is my hope that they will inspect this. There were no warning lights or messages displayed prior to the incident.

9 Vehicle Speed Control problem of the 2019 Mazda CX-5

Failure Date: 06/01/2024

Went to drive home from work one night, car had been normal until this event. Car suddenly began to state that it was overheating and throttled my speed down to 20 mph. I was in the middle of traffic going 40-50 at this time. Turns out there is a vsb regarding a coolant leak that can occur and break. Nothing about how you are suddenly governed to 20 mph.

10 Vehicle Speed Control problem of the 2023 Mazda CX-5

Failure Date: 04/23/2024

Slowly and cautiously entered parking lot of a small strip mall. Proceeded to a parking space located in between two parked vehicles. Maneuvering into parking space I removed foot from gas pedal toward the break pedal; before foot made contact with break pedal the vehicle accelerated on its own surging over the cement parking bumper, over bushes, through the grass, down the curb and into the street. I jammed on the breaks. Lifting foot from break pedal I did not touch the gas pedal in fear the surge might happened again. Instead I allowed the vehicle to coast as I guided vehicle to the curb. In all the commotion, while trying to control vehicle safely I noticed the dashboard transmission indicator lights flash from park to drive to neutral. Safety issues for me and family at risk because we had no control over the acceleration of the vehicle.

11 Vehicle Speed Control problem of the 2018 Mazda CX-5

Failure Date: 04/18/2024

The issue started happening [xxx]. I was driving home and the car went into limp mode. Several errors came on the heads up display and on the dashboard. At first I could not go faster than 31mph, then it dropped down to 17mph, then to 7mph by the time I got home. 1. When accelerating from a stop light, car goes into limp mode and won’t accelerate almost stalling in the road. You must pull over, turn off the car and wait 5 minutes so error clears so you can drive again. 2. Car goes into limp mode when going up a hill 3. Between 46-48 mph, car hesitates when accelerating 4. Between 56-60 mph, car hesitates when accelerating 5. Error codes received per lake car care: p2299, c1001, b1303 I brought the car to lake car care and they replaced the spark plugs and coils as the codes said it was a misfire on cylinder 1 & 2. This cost me $ 947. 57. This did not fix the problem. The mechanic todd took the car for a ride and it went into limp mode for him also. He could not figure the issue out after having the car for a couple weeks so suggested I take it to Mazda in clermont. Mazda has had the car twice for over 2 months now and cannot figure it out. They replaced the abs module for $ 1284 which did not fix the problem. Replaced the injectors for $ 1426 which didn't fix the problem. They did a fuel compression test which came back fine and cleaned the catalytic converter which they said was clogged and this didn't fix the issue either. It is unsafe to drive this vehicle on the road as it does not accelerate so it is putting myself and others at risk. Mazda corporate has been called and case manager has been assigned but has been no help to me at all. Photos are attached of all error messages, text messages with Mazda, repair receipts, etc. August 9th, Mazda told me they are at a loss and don't know what else to do, I haven't heard from them since. Please help. I am at a loss and don't know what my options are. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

12 Vehicle Speed Control problem of the 2024 Mazda CX-5

Failure Date: 01/31/2024

I have a 2024 Mazda Cx-5 but this is in any model and year that has the adaptive radar cruise control. When you set the distance you want to follow the vehicle in front of you, you will go the speed you are going until you come to the space you have set for the vehicle in front of you. Your vehicle will slow to match that vehicles speed. If that vehicle slows down, you will slow down. What it doesn't do it turn on your brake lights so the vehicle behind you knows that your vehicle is slowing down. Sometimes if the vehicle in front of you slows down fast, so does your vehicle. You have no control how fast it does that, it just does it. Now if the vehicle behind you doesn't realize that your vehicle has done this, well it has been very close two times so far. I do not want my new car rearened. I did not know it did this with the brake lights and Mazda doesn't tell you that you have no brake lights when this happens. Why would they have a setup like this. It is like brake checking the vehicle behind you. I do not use my cruise control for that reason. It would be nice to be able to use it on a new car but I do not want to get hurt or get someone else hurt by doing so. I did contact Mazda and they know it does that. This is what they told me "case number 00171326      hello. My name is dominique. I am sorry that you have had some very close calls while slowing down while vehicles were behind you. As for the question you had asked about cruise control activating the brakes when your vehicle starts to slow down, it does not activate the brake lights. It is just like when you are driving without cruise control and you go to take your foot off of the gas pedal instead of using the brake to slow down, it would not activate brake lights either unless you actually mash the brakes, the same with cruise control.  thank you for being a part of the Mazda family.  " isn't that great. I am now a safely hazard or at least my vehicle is, and they are all like that. Who knew. Not me.

13 Vehicle Speed Control problem of the 2016 Mazda CX-5

Failure Date: 11/17/2023

11/19/2023 - about 8 pm. On November 17, 2023, I was parking at my condo parking lot and slowly drove the car into the parking space when all of a sudden the car accelerated on its own. I immediately pressed on the brake pedal and it stopped. I felt fortunate that nobody was on the sidewalk. The same thing happened about a month ago but I thought it was just something unusual but now that it happened again I am very concerned and I am going to call the maxda dealership to see what they say.

14 Vehicle Speed Control problem of the 2023 Mazda CX-5

Failure Date: 09/26/2023

At 2:00 on a clear afternoon I was slowly coasting into a right hand turn into a parking space, and lightly tapping the brake to finalize the stop, when the car accelerated hard forward over a curb and a sidewalk into the brick wall of a building. Airbag was deployed. My injuries were a nasty wrist contusion and bloody gashes where the airbag "door" deployed and I also had complete seatbelt brusing on my throat, chest and across my abdomen. This after traveling a mere 7-8 feet. At the time I was very shaken up and incredulous that such a thing could have happened logically, and conceded that perhaps I had made a mistake somehow. As I settled down later I became dubious, especially when I got behind the wheel of my rental car and 100% when I sat in my actual car at the wrecking yard. I am crystal clear I was depressing just the brake. Even if I had accidentally hit the accelerator instead of the brake (I did not) it would not have accelerated to the degree that it did. I am now scared of my car and am just hoping it is declared totaled by my insurance so I don't have to drive it again - any car but that one.

15 Vehicle Speed Control problem of the 2018 Mazda CX-5

Failure Date: 07/04/2023

Cruise control suddenly ceased operating and I believe that is disabling the automatic collision avoidance system.

16 Vehicle Speed Control problem of the 2017 Mazda CX-5

Failure Date: 02/14/2023

On February 14, 2023 around 3:30 - 4:30 p. M. (est), I drove to the local wawa. Upon entering the driveway of wawa at a very low speed and depressing the brake the car failed to come to a complete stop. I pressed down on the brake as hard as I good, but the car did not come to a stop until hitting the security/safety bollard in front of the wawa store. I did not get injured and no one else was injured. The front end of my car sustained over $4000 worth of damage. The insurance company repaired by car. I was asked by the insurance company what caused the front end damage. As I played the accident over and over in my head, I just could not see what I did wrong. My foot was on the brake. Also, I entered the entrance of wawa at a very slow/low speed. After a few weeks I took the car to the Mazda dealer to let them know the car accelerates and engine revs. Also, the car jolts forward while decelerating from about 20mph to 10mph. The car dealer/repair shop could not replicate the problem. Was told to bring the car back in, if problem continues.

17 Vehicle Speed Control problem of the 2021 Mazda CX-5

Failure Date: 01/18/2023

I experienced a sudden unintended acceleration incident. I pulled into my driveway and stopped the vehicle with my foot firmly on the brake since my driveway is sloped. I pushed the garage door opener remote when suddenly the vehicle accelerated and shot forward hitting the garage door. There was no brake action and all backup systems failed. There was no automatic emergency braking or forward collision warning. There were no warning lights displayed. No airbags were deployed. If the garage door had been open, the vehicle would have hit a concrete wall in the garage. The vehicle was taken to the Mazda dealership and a full circle inspection and diagnostic check. Mazda found no codes and everything was working as designed in vehicle.

18 Vehicle Speed Control problem of the 2022 Mazda CX-5

Failure Date: 01/09/2023

Unexpected acceleration ___ while stopped on a slight downgrade, rpm suddenly increased and vehicle lurched forward. Applying more pressure to brake stopped movement. I forced transmission into park. Took foot off brake, rpm still high. Tapped accelerator, rpm dropped to normal idle. Fortunately, there were no vehicles in front of mine and cross traffic was not present. Vehicle did not enter intersection. I drive single footed and shoe did not overlap brake and accelerator. The day was clear 37f, dry. Vehicle had made 3 short trips prior so engine was up to temperature, 35 miles total over 2 hours. Cruise control was not engaged and had not been used in two days. This issue has not been reported to dealer. There were no warning lights or other indications of problems. The vehicle has driven normally since. I have not been able to reproduce the issue. ____ a similar issue happened two months prior. Circumstances were not the same. Intersection was flat, engine was not up to temperature, removing pressure from brake pedal was enough to stop high rpm. Attributed instance to the shoes I was wearing. The running shoes were thicker and wider than I normally wear while driving. I believed I may have pressed both brake and accelerator without realizing it since reducing pressure corrected the problem.

19 Vehicle Speed Control problem of the 2021 Mazda CX-5

Failure Date: 11/16/2022

Vehicle drives very "rough" at low speeds less than 30 mph. The car feels like it is jumping and can't tell at what year it should be driving in. This is very noticeable when driving around residential areas. This is a common issue with the Mazdas with the skyactive engines. Dealership in raleigh knew about this problem and said that there is nothing they can do except possibly provide an update to the computer software. I have written to Mazda corporation regarding this and of course, no response was received. Mazda needs to acknowledge this problem with their special transmission/engines and fix this. I shouldn't be having these kinds of issues with a vehicle that has less than 13,000 miles.

20 Vehicle Speed Control problem of the 2021 Mazda CX-5

Failure Date: 09/28/2022

Upon exiting my local car wash, I drove my car around to the area used for vacuuming. I pulled into the parking space and when I applied the brake pedal to stop, the car suddenly accelerated and crashed into the wall of the building. I have the car wash security camera footage of this incident. It shows my brake light is on at the car's point of acceleration and my brake light is on at point of impact with the wall. If you need to review it, I can email it to you.

21 Vehicle Speed Control problem of the 2019 Mazda CX-5

Failure Date: 09/28/2022

After purchasing this vehicle I started noticing that as I accelerated from a stop the transmission would shift hard into 1st gear and 3rd gear. This is also occurring during deceleration as if the car brakes I'm stages due to the downshifting hard. The dealership has refused to look into the issue. They have refused to add the notes to my vehicle records in effort to avoid accountability.

22 Vehicle Speed Control problem of the 2022 Mazda CX-5

Failure Date: 09/25/2022

When coming to a stop, car accelerated and went over curb. From a search of the internet I found examples with this and other cars where people with larger shoes had the same problem of pressing the brake and unintentionally pressing the accelerator pedal at the same time. I read of one manufacturer that programed the car to brake if both pedals are pressed at the same time and ignore the accelerator. The same thing happened in my 2015 Mazda cx5. In that case I had it checked at the dealer. The technician found nothing wrong but was able to reproduce the problem when his boot pressed both the brake pedal and accelerator because both pedals were so close together. Since this would probably be an inexpensive software change, it would be prudent for a safety recall by manufacturers that have pedals that are close to one another.

23 Vehicle Speed Control problem of the 2015 Mazda CX-5

Failure Date: 09/05/2022

2015 Mazda Cx-5 with 91,000 miles on it. Automatic transmission has failed. When I attempted to pull out into traffic the vehicle would not shift into 2nd gear and instead jerked hard and the engine rpms revved up. I was not able to get the vehicle past 10mph. This failure with the transmission resulted in the oncoming traffic having to swerve and break to avoid a collision with me. The vehicle was taken to a Mazda dealership who did a diagnostic on it and found that the Mazda had an "internal transmission failure" and would require a full replacement. I asked if it would be replaced with the same exact transmission and was told "no, the defect was discovered by Mazda and has been correct for new transmission models. " the manufacture knows about this issue. Luckily I escaped serious bodily injury. The problem has been confirmed by the dealer as well as an independent 3rd party transmission repair specialist. There were no warning lamps, messages or other symptoms prior to this incident to give warning.

24 Vehicle Speed Control problem of the 2021 Mazda CX-5

Failure Date: 06/28/2022

I bought a 2021 Mazda Cx-5 from ramsey Mazda of des moines, IA. It offered comparable comfort & features. A selling point was the safety features touted by the salesman. Odd things occurred during the first few months but I attributed it to a learning curve. I saw some odd messages but I really did not think it was relevant until June 28, 2022. I was backing out from a parking space. The blind spot monitoring failed to indicate a car approaching from the right that then stopped in my blind spot. I hit the car behind me. Despite the fact, I understand personal responsibility, I never would have had the bsm worked correctly. I dropped the car off on July 15, 2022 to the ramsey Mazda service center after the bsm failure and a check engine light appeared. I already had the ramsey Mazda collision center look at the car and told them of the of the bsm failure. That man doing the estimate said he had heard of those failures before. On July 15, I dropped the car off at the ramsey Mazda service center to look at the bsm, a check engine light, the door locks and the hatch opening on its own. I tried to tell the service center that there was something severely wrong with the electrical system in this vehicle. I have a witness to its erratic behavior, but I was continually dismissed. There is more wrong now with my car than when I left it with the dealership. They kept my car for 19 days. During the 19 days, I continually had to call and check on the status of my vehicle. Ramsey Mazda was not and is still not initiating conversation with me or responding to voicemails. The manager of the ramsey Mazda dealership on hickman in des moines, IA (jd) is supposed to be handling this issue personally with Mazda corporate because it now “appears” to be affecting multiple systems in his words. Multiple issues include but are not limited to the bsm, seat belt notifications, power driver’s seat, camera system, cruise control system, entertainment system, & hands free communications.

25 Vehicle Speed Control problem of the 2018 Mazda CX-5

Failure Date: 06/21/2022

Module in the the car sofi and radio stop working at times car slow acceleration from stop to move.

26 Vehicle Speed Control problem of the 2016 Mazda CX-5

Failure Date: 05/09/2022

I own a 2016 Mazda Cx-5 touring with awd. I purchased it brand new from a dealer--no miles on it. It's been a great car until this sudden unintended acceleration problem (about 60k miles now). It's happened 5 or 6 times--twice when in a drive thru. It seems that if I am idling (not pressing on gas or brake) the car will sometimes suddenly accelerate. I have used suggestions to brake, then place car in neutral. That seemed to resolve the problem. However, recently I pulled up to my (rural) mailbox and the car tried to accelerate out of control. I had the brake on (was on a downhill) when it happened, so this was scary. I really had to press down more on the brake pedal, then shifted to neutral, then to park. The acceleration finally stopped. I did not observe tachometer readings during this or other events, but will pay more attention if/when this happens in the future. I do not drive with both feet, and as these incidents have occurred I have been able to maintain more composure and analyze what is going on. I am not getting my foot caught in the mat and I'm not mistakenly pressing on the accelerator. I've been to my local Mazda dealer (sport durst in durham) on may 13, 2022 and they were unable to replicate the problem, so could not repair or even troubleshoot it. The car was t-boned about a year ago, with some damage to the passenger front tire. Could that have anything to do with my problems? my partner feels this could be an issue with the throttle control sensor (hardware) or with the software managing throttle control (much more difficult to diagnose, he says). I wanted to ask: for anyone experiencing this problem, did replacing the throttle control sensor or diagnosing software problems resolve the issue? and, do you have any recommendations for me? since the car is (barely) out of warranty, (just over 6 yrs/60,000 miles) would filing a claim with Mazda be a good approach? would you suggest I just trade the car in?.

27 Vehicle Speed Control problem of the 2019 Mazda CX-5

Failure Date: 05/03/2022

Tuesday, 5/3, I was parking my car in the new leaf grocery parking lot in aptos, CA. I pulled into the lot and made slow right into an available space. I had my foot on the accelerator, tapped down on it gently to ease into the space. My car lurched forward at such high speed that I was unable to remove my foot from the accelerator. I swerved to the right as the car went over a partition and hit another parked car on the other side of the partition. My airbag did not activate. I've replayed the experience in my mind and tried convincing myself that this could have been a matter of human error on my part. I am beyond convinced that what I experienced yesterday was a case of sudden unintended acceleration. I am concerned and afraid to drive my car now. Had circumstances been different, someone could have been killed. Had I not swerved right, I was headed to crash into a building. I called 911 and since no one was hurt there was no police report. To address questions set-out above: speed control (sudden unintended acceleration) and air bag failed my safety and the safety of others was put at risk as of yet the problem has not been reproduced however there are a great number of reports related to unintended acceleration (however not acknowledged as a problem by Mazda) I plan to have my Mazda inspected by a local dealership service center there were no warning messages or symptoms in advance of the incident occurring I've owned my Mazda for 3 years and never experienced anything like this. I have never had a car accident to date.

28 Vehicle Speed Control problem of the 2021 Mazda CX-5

Failure Date: 04/16/2022

On April 16, 2022, around 6:45pm, I drove my new Mazda cx5 (@ 3500 miles) into our church parking lot and turned right down the first row of cars. It was light out and the weather was pleasant. I turned right into a parking space with my foot on the brake. The car was facing and perpendicular to a sidewalk that was about 10 feet away. It was separated by a grassy median. When I depressed the brake further to shift from drive into park, the car surged forward and accelerated. There was a pedestrian on the sidewalk in front of the car. I turned the wheel to the right at a 90 degree angle to avoid hitting her. I kept pressing the brake but the car kept accelerating until it crashed into a large tree. The airbag deployed. Both the pedestrian and I were transported to the hospital. The pedestrian had bruises on her leg and hip, but fortunately was not more seriously injured and was released from the hospital. I was admitted to the hospital for contusions to the heart. The car hood was crushed and the car was totaled. The incident was terrifying. The police estimated the car went from nearly a standstill to about 40mph in about 15-20 feet. There were no warning lamps or messages prior to the accident. I checked the NHTSA website after the accident and downloaded 16 similar complaints where low mileage Mazda cx5s accelerated out of control when drivers were attempting to park or stop.

29 Vehicle Speed Control problem of the 2018 Mazda CX-5

Failure Date: 03/01/2022

Unknown my vehicle stalls while I’m driving. My rpm tends to stay at one number when this happens. I was nearly struck by a vehicle while trying to merge onto the highway. This is very dangerous. There’s also I light that comes on my dashboard that seems to be indicating an accident or something of that sort.

30 Vehicle Speed Control problem of the 2019 Mazda CX-5

On interstate 90 while driving at 75 mph under cruise control I passed an 18 wheel truck on a curve. With no cars in front and two following, the car without warning braked heavily and control screen flashed warning brake. The car immediately behind braked and veered on the bearm to avoid me. I stepped on gas to override the brakes and avoid an accident.

31 Vehicle Speed Control problem of the 2015 Mazda CX-5

Failure Date: 01/14/2022

I was on my way home going down a windy back road going about 45 mph. There was a Jeep in front of me and the mail truck was in front of them. The mail truck went to slow down and take a right onto a different road and as the Jeep slowed down I applied my breaks and there was no response. This was not the first time this had happened in this vehicle but this time I panicked and I had to cut my wheel into the persons front yard to the right of us as I was continuously picking my foot up and slamming it down on the break and there was no response my car was just speeding up. I ended up going into the road that the mail truck was taking a right on and I hit the front of his truck and went flying into the woods and the trees were what stopped me. I now live with constant nerve pain everyday from this and suffer with a compressed lower back fracture that limits me daily from many things. Before this incident I had issues with the breaks and acceleration twice. I had taken it to Mazda and they fixed my “break switch” ? obviously that wasn’t the problem.

33 Vehicle Speed Control problem of the 2018 Mazda CX-5

Failure Date: 05/11/2021

Situation: driving uphill on hwy 120 on new priest road, big oak flat, CA, two lane mountain road. Slow car in front of me and a delivery truck tailgating. Slow car eventually moves aside in a turnout and I accelerate to open up space between myself and what I thought would be the truck but somehow the slow car returned to the driving lane ahead of the truck. I continue to accelerate to attempt to open up space between myself and the car behind me when I lost power, the car behind me moves to the opposing traffic lane to avoid rear ending my car that is running in degraded mode (accelerator has no effect, basically moving on idle power) and narrowly misses an oncoming car as I move towards the right edge of the right lane. Truck behind me has to brake hard to avoid rear ending me. There was no shoulder to pull over to at that location immediate location. I pull over a few hundred yards further at a safe location. Grade was perhaps 4% to 6%. Weather: hot 90 degrees speed: accelerating from 35 to 50 mph (guess) diagnostic lights: Smart city braking system failure warning, plus a lot of other warning lights lit including check engine light. Aftermath: I pulled safely over, stopped the engine, waited a while and restarted the engine and the car returned to it's normal driving behavior and completed my trip. Other information: this is the 3rd occurrence of this behavior. Each time I have had Mazda look at the problem. 1st occurrence they replaced the cylinder coil, 2nd occurrence they replaced a spark plug for misfire. 3rd occurrence, to be determined.

34 Vehicle Speed Control problem of the 2020 Mazda CX-5

Failure Date: 03/28/2021

I was parked at a fast food restaurant that had an outdoor kiosk for payment. I got out of my car to pay and got back in. After about a minute or two my car accelerated forward, going over a sloped curb and down a short low grade hill, hitting a pole. I was confused to how the car would go forward because I was in park the whole time.

35 Vehicle Speed Control problem of the 2013 Mazda CX-5

Failure Date: 03/04/2021

While driving my vehicle on the interstate the car lost the ability to accelerate. All of the dashboard lights came on and I had to coast to the side of the interstate. After restarting the vehicle I was able to drive it but I cannot take the vehicle over 30 mph now without it shuttering and having acceleration issues. This is a serious and life threatening issue that needs to be fixed free of charge. I have found other reports of this happening but I see no recall information. This needs to be fixed before Mazda has lawsuits filed against them. This is a manufacturing error and not a usage error. Please look into this.
