I made a right turn onto a gravel driveway that has a slight incline. My speed was less than 10 mph. At the top of the incline I attempted to make a u turn so I could park the car. Midway through the u turn the car's acceleration increased rapidly to high speed without my stepping on the accelerator. I pressed the brake 3 times. The brake pedal went to the floor without seemingly any effect. In seconds the car traveled across the front lawn, picked up concrete from and old cistern top and smashed into the front porch tearing it off it's foundation. Portions of the porch and tree limb fell shortly after the impact. The front tire was spinning at a rapid rate when I exited the vehicle. Extraction of the vehicle was not accomplished for 2 weeks while experts determined how best to accomplish the extraction. The car insurance company and 2 towing businesses declared the vehicle a total loss. The vehicle is with the towing company per car insurance company request. I don't know if it has been inspected. I did not see any warning lights. I was treated at the hospital for a slight concussion and significant head and facial bruising and lacerations as well as a wrist burn likely caused by air bag deployment.

About a year ago, I noticed multiple warning lights on my car on, including the traction control, abs, emergency brake, tire pressure indicator, etc. I took my car to the dealership where they diagnosed the problem as related to a faulty speed sensor. I have been reading consumer info on Mazda forums and it appears numerous people are impacted by this, which leads me to wonder if it may warrant assessment for a potential recall. My vehicle had under 35k miles on it when the issue first presented.

The Mazda radar cruise control (mrcc) system performs poorly (it does not hold a set speed well up and down hill; compared to our 2006 Mazda3), but the safety concern is the forceful, sudden, and not-always-predictable braking the vehicle does even if there is not another vehicle in front of us. It brakes, sometimes, for a vehicle in the right lane (we, in the left), and continues to brake unless we override the system. No problem--except if there may be someone following too closely, it could cause a collision. It seems to be related to the mrcc, as no "brake!" warning accompanies the unwanted brake application. I've contacted Mazda numerous times; they recommended taking it to the dealer. I have--they can find nothing wrong. All of this seems related to a mrcc update (ssp c5) that was done to smooth the mrcc's operation. It did fix the sudden deceleration and acceleration when following other vehicles, but now is worse in other aspects. I may be wrong, but it all seems to be a software issue, so it's not surprising that the dealer can find "no concerns". As to the date of this incident, it happens nearly every time we take the car on a highway or interstate.

The front radar sensor failed and doesn’t work anymore rendering the fcw and acc useless. There is a TSB out about water damage getting into the sensor. There are 2 warning lights on. They first appeared when I turned on my car one day.
Mazda dealer is refusing to update software per sspc5 to correct a safety issue relating to the mrcc (Mazda radar cruise control) unless I first agree to having Mazda connected services installed on my 2019 Mazda 3 sedan (csp06). When I purchased my new car Mazda connected services was not included in my car. I am now being coerced into having Mazda connected services installed into my vehicle before they will correct a safety issue regarding a known issue with the adaptive cruise control. My vehicle is still under warranty but unless I agree to having their telemetrics control unit (tcu) installed, they refuse to install the software to correct the dangerous adaptive cruise concern per sspc5.

Horn works on and off, car is unable to reverse (put the car in drive was able to go forward,turned the car off went to restart the vehicle and now the reverse gear does not work,transmission clutch gets stuck and takes off unexpectedly at a fast speed without pressing the gas.

When this feature is left on in my vehicle, it will detect curves, vehicles, and objects that are not there, and as a result, will attempt to turn my steering wheel (safety concern). I have had to keep this feature turned off to prevent it wrecking the car, because I could be on a straight road, and it will detect a curve that isn’t there and begin turning the wheel, which could result in a fatal accident if not corrected and disabled by the driver. It is available for inspection upon request. As the dealer has not driven the vehicle, they have not recreated the error, however if anyone were to drive the vehicle, they would be able to observe the malfunction. The dealer has been made aware on many occasions, though they never documented it and only suggested that I turn the feature off, which automatically turns itself on upon ignition. There are no warnings that there is a malfunction; you only know there is an error once an alert pops up thinking you’re missing a curve and attempts to turn your steering wheel though there is no curve. This issue has persisted over the life of the vehicle.
