1 Warnings problem of the 2019 Mazda MAZDA3

To whom it may concern, I was on a back road late in the evening not far from home [xxx] and a small deer ran into the road. I needed to brake to avoid hitting the deer. After breaking, a brake system malfunction warning message and I-activesense system malfunction warning illuminated on my dashboard. I brought the car to a Mazda dealership the next morning. The technician determined that the brake fluid level sensor became stuck, requiring replacement of the brake fluid reservoir and the brake fluid level sensor. What failed: the repairs include replacement of the brake fluid reservoir and the brake fluid level sensor. This is the link to the dealer report [xxx] . No one other than the dealer has inspected these parts to my knowledge. There was no accident of any kind. I personally felt unsafe after these parts failing and reached out to Mazda customer care to request investigation of the issue. The case manager just called me back today to say they could not help me. Mazda has denied warranty coverage, claiming the parts are not covered. However, I really do feel this is a critical safety system component that failed after normal emergency use with no collision or impact. Why I feel this is a safety concern: - the brake warning system became unreliable after a single emergency stop. - my car only has 42k miles and is in excellent condition and is fully dealer serviced. - a driver could lose the ability to receive proper brake system warnings if this failure occurs again. - a critical brake sensor should not fail under normal driving conditions, and its failure could lead to delayed driver response in an actual brake fluid loss scenario. It makes me worry for the future. For your consideration: I believe this represents a safety-related defect that should be investigated, as it affects the reliability of the brake system warning indicators in the 2019 Mazda 3. 2 Warnings problem of the 2018 Mazda MAZDA3

The collision warning light and lane assistance light come on frequently. It used to just be when the weather was under 40 degrees and the car was cold. However, it now happens all the time. It seems to go on and off for no reason. Additionally, the infotainment system keeps switching radio stations or ghosting without anyone touching it. It also drops bluetooth connectivity constantly.

I took my vehicle into rbmc dealership due to the vehicle was experiencing electrical issues and had an open recall / still does. The dealership said I can’t have a dash cam plugged in to a charging port. Fcm / ldw / e-break staying on / throwing error codes = c1001 fsc failure type 97 & u3003 epb control module power supply failure type 16. They gave me back the car the battery failed the next day and when I called them they banned the VIN of this car to ever return to them. ( the battery was under factory warranty) the issue has only gotten worse and removed my dash cam as they demanded and now I can’t get the car fixed and the issue has gotten worse then taking the car to this dealership less then 30 days ago. They also did not perform the recall when the car was there. The car now has the fallowing codes b13d3 / c1001 / u3003 & possibly more codes because the whole dash is flashing auto light / fcm/ ldw and break. My 73 year old grandma can’t even take the car in to get fixed the refuse to touch it after it had issues when they gave it back to us and it’s under a extended warranty and was a cpo and has a extended warranty through Mazda. Car only has 31k miles on it and is dangerous for her to dive and myself. Issue has been on going for over a month.

Vehicles center console display screen suffers from ghost touches. The display registers inputs from the touchscreen with no one touching the display this is the same issue that is on the 14-16 versions of this vehicle. The display self navigates causing a substantial distraction and increasing the likelihood of a crash. As well on a few instances self navigates through the menu and sometimes enables or diables adas system features collision prevention etc.

7 Warnings problem of the 2021 Mazda MAZDA3

Failure Date: 09/21/2021

The error code "b14fo-76 forward sensing camera deviation - wrong mounting position" was discovered, and verified, by a safelite auto glass service center when the 2021 Mazda 3 was in for a repair (stone chip) to the windshield on September 21, 2021. There were no warning messages nor lights to alert the driver as to the improper installation/mounting of this device. There has been no known modification to nor obstruction of the fsc -- its condition is now as it was received new from the dealer. Improper mounting ("wrong mounting position") of the forward sensing camera affects multiple safety systems of this vehicle, which affects the safety of the vehicle's passengers as well as everyone's safety around the vehicle every time it is driven -- there's no telling what injury and damage may occur from an errant un-commanded driving tasks such as emergency braking and faulty lane departure corrections.

8 Warnings problem of the 2020 Mazda MAZDA3

Failure Date: 03/13/2021

I was driving my vehicle straight ahead on a two-way city street with no other vehicles in front or on my sides. I was traveling at a speed of 40 mph. The two-way street is separated by a concrete divider and trolly tracks. A "brake!" alert appeared on the display screen followed by beeps. Suddenly the brakes were engaged bringing the vehicle to a complete stop causing the vehicle behind to come to a screeching halt. In a few seconds I was able to drive off.
