The contact stated that while driving at approximately 40 mph, the front passenger tire had blown out. The contact stated that after pulling over, he noticed the tire was angled inward and detached from the wheel hub and that there was damage on the side wall. The dealer was not contacted. The tire was replaced with a spare while awaiting diagnosis and repairs with an independent mechanic. The tire was original equipment. The manufacturer was notified of the failure. I have primewell valera touring ii tires that have a date code of 0215 and they still have around 50% tread left. The side walls on all 4 tires are coming apart, they are split almost all the way around. These tires are on my daughter's car and I did not notice this problem because the splits where on the outside of the tire but facing the inside, I found them when I went to install the snow tires. I called primewell and they said there is no recall for the serial number on these tires and they would not help me at all. There is a recall on this brand and model of tire for the same defect and reading many complaints about this same problem the recall needs to be updated to cover more serial numbers.

The tire completely separately from the sidewalks on both sides. I heard a pop sound and within 15second I loss control of my car 2015 Mazda 6 I have pictures of the incident.

The tires were worn from the inside to the outside. Every 6 months she had to purchase new tires . The dealer was contacted, but no assistance was provided. The failure mileage was 13000. I leased my fully loaded 2006 Mazda a6 back in December of 2005. Ever since then, I have had trouble with the car. First, the driver side window was not going down. A few days after I bought the car, I realized that the turbo kept giving out on me. Then I came across a huge dilemma, where the limited slip differential, drive shaft, and all the bolts and hinges had to be replaced because of a bolt which was not torque enough. Then, the brakes kept making an annoying noise even without the brakes applied at low and high speeds. Subsequently, the tires wear and tear was horrible at 6,000 miles, to the point where I should have replaced them, and Mazda didn't do anything about it. Later I found out that the air pressure in the tire was too low. After that, the keyless entry/keyless start remote stopped working, and had to open the car manually, which got fixed after 2 attempts, and just replaced the whole thing. Now, there is a problem with the rear axle, and how it has to be replaced, because when the limited slip differential problem came across, they neglected to look or fix the axle from the start. Now they have the courage to tell me that I have to pay for the axle, and rent a car, and all the expenses, because they were negligent of looking at it from the start. They are blaming the axle issue on the tires, when they were bad to start with and the tires that were going bad were the front ones, not the rear.
Beginning in 2006, my 2003 Mazda 6 would randomly experience power failure while driving. This was both scary and dangerous. I would be driving, the car would completely loose power with no warning and all the lights on the dashboard would come on. I would have to pull over, turn my car off and back on, and then it would work fine till it randomly happened again. Obviously loosing power at a high speed is extremely dangerous for both myself and the other drivers. I took it to the dealership many times and they were never able to determine a cause. I brought my car to the dealership over 10 times for this behavior. Finally in August 2008, I was reading on a Mazda message board that this behavior is caused by a bad throttle body. There was even a Mazda service bulletin issued for it in 2006 (bulletins 01-016/06 last issued may 18, 2006). I called the dealership, had the throttle body replaced and have not experienced this behavior since. The service bulletin had serial numbers of bad throttle bodies listed on it, mine was one of them. I believe that this should be a Mazda recall, not just a service bulletin, as dealerships do not even seem to be aware of the problem and it the car is extremely dangerous if the throttle body fails.

While driving at 45mph consumer's vehicle was hit from behind, causing it to rear end another vehicle. Upon impact, both frontal air bags did not deploy. Driver sustained minor injuries. Prior to accident, left front tire kept loosing air. Dealer re-beaded it but didn't found anything wrong with the tire. Dealer told consumer to come back if tires still losses air.
