Car Stall Problems of Smart Fortwo

Today I received a third (3rd) recall notification for my 2009 Smart four two coupe rear engine insulation mat. The notice states the current mat could increase the risk of fire. Each time I have contacted the dealer (55 miles distant) they have informed they are out of stock of this item. I've asked them to notify me when the item is in stock. I have never been notified. Could you please look into this apparent fire hazard. This has been going on for almost 2 years. I would be happy to install this easy fix if they would send me the insulation pad itself.

I was driving eastbound in the number 1 lane of California highway 24 about 1/4 mile before the tunnel road exit. I heard a noise coming from the engine are of the car, looked at the instrument cluster and noticed that the engine light had illuminated, and experience a sudden loss of power. I pulled off of the highway at the tunnel road exit and stalled on the off ramp. I attempted to restart the engine twice by turning the key. I saw smoke in my rearview mirror, exited the vehicle, and saw flames coming from underneath the vehicle. I called 911 and gave the dispatch operator my location. By the time the highway patrol and local fire department arrived, my car was totaled. The attached photos tell the story of the damage.

Tysinger replaced the fuel tank on 08/30/2018. The car has had problems with the fuel tank since I purchased the car. The car has been making a loud bang and then there is a strong smell of smoke afterwards. This has happened at least 45 times since I purchased the car, I thought the problem would go away, but it has not. The car stalls at red lights and during stop and go traffic, there is a strong smell of smoke after it stalls.

The contact owns a 2014 Smart Fortwo. The contact stated that while driving at various speeds, the key would move from the run position causing the vehicle to stall. The contact also stated that the brake and the accelerator pedals were extremely close to one another. The contact stated mentioned that when switch from the brake pedal to the accelerator pedal, his foot would get caught under the brake pedal. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failures. The failure mileage was 1,000.

The contact owns a 2010 Smart Fortwo. While driving approximately 7 mph, the vehicle stalled. The contact was able to restart the vehicle and it resumed normal operation. The failure occurred four times. The vehicle was taken to a dealer, but was not diagnosed or repaired. The manufacturer was notified. The approximate failure mileage was 37,900.

The contact owns a 2008 Smart Fortwo passion convertible. The contact stated while driving highway speeds, the vehicle stalled without any warnings. The vehicle was taken to the dealer where the technician diagnosed that the engine wiring harness was touching the body of the vehicle causing a short circuit. In addition, the contact mentioned that the engine warning lamp illuminated. The vehicle was taken to the dealer where the technician diagnosed that one of the cylinder valves burned causing the failure and needed to be replaced. The manufacturer was made aware of the failure. The vehicle was not repaired. The failure mileage was 39,457 and the current mileage was 48,500.
