36 Assist problem of the 2018 Tesla Model 3

Failure Date: 04/13/2024

The most recent software update v11. 1(2024. 3. 10 9d92fb944d7f) was downloaded automatically and I installed it. The next time I drove, the car made uncommanded and unwanted lane changes, apparently because I could "go faster" if I passed the traffic in front of me. I did not want to go faster due to congestion and aggressive drivers where I live. I then discovered that "full self driving" had been activated after the download and without my knowledge or authorization. I believe that this was an unsafe condition and wish to report it. I immediately turned "fsd" off.

Failure Date: 03/20/2024

With the latest update of v12. 3 fsd, the full self driving cannot predict the obstacles clearly, resulting in am incorrect left turn manuever, running over a central divider curb, and damaging tires. Not sure, what could have happened, if a human was standing there instead!.

38 Assist problem of the 2020 Tesla Model 3

Failure Date: 03/07/2024

My Model 3 intermittently has the steering wheel locked in position, which sometimes resolves temporarily after rebooting the car. I always get a steering assist reduced error on the car, regardless of rebooting. It also no longer has lane keep capabilities or autopilot, which involve the car automatically moving the steering. I have not gotten this inspected by a dealer or service center. The issue first appeared yesterday, after I tried to pull out of a parking lot, but I had trouble since my wheel would not turn. I will be going to a service center next week. After the errors began, I have not meaningfully driven the car, since locking of the wheel while driving is very dangerous. A handful of errors showed up on the screen (cruise control disabled, traction control disabled, stability control disabled, steering assist reduced), and I did not have any regenerative braking applying. I returned safely to my parking spot. After multiple reboots of the car (pressing both wheel buttons), I found that sometimes after putting my car into drive, my steering wheel was locked. Other times, I could turn my steering wheel without issue, but I would always receive the steering assist reduced error.

39 Assist problem of the 2018 Tesla Model 3

Failure Date: 02/13/2024

I got a recall notice from Tesla and Tesla is still in safety violation and is refusing to fix the issue, so I need your help to force Tesla to fix their abusive ways. The recall 23v-838 is related to "when autosteer is engaged". Autosteer/autopilot/fsd now disables even when not engaged, alleging "improper use" when it wasn't even being used. It also disengages autosteer/autopilot/fsd while driving if it detects "improper use" (even if it's detection system itself is buggy), forcing the car to slow down like in the middle of the freeway! when the battery is low and there's only a mile or two to go to a charger, the software will slow down and force the car to pull over to get a tow, taking on average 2 hours to get a tow truck to go 2 miles, rather than allowing the driver to reach the charger with the remaining battery it shows. Tesla keeps changing the battery percentage for when it shuts down. It used to be 0%, then 3% and recently 6%. This randomness creates "range anxiety" and distrust in the battery percentage being a useful indicator. Tesla should use 0% as "shut down" and leave the rest of the battery as "drivable" to reach a charger to have an accurate indicator. It also flashes alerts on the screen to "pay attention to the road", which is ironic to have a screen display to distract attention to the screen. . . Rather than pay attention to the road. Tesla took away my autosteer/autopilot improperly, and no one in the service center or support at Tesla knows how to fix this bug or give it back to me, so I can drive safely like with lane centering. I paid $10,000 for this "safety" that Tesla takes away at random, claiming they can do whatever they want because they've now labeled it beta software! this is an abusive practice the NHTSA should force Tesla to stop doing and give the power back to responsible drivers like me. Tesla abuser names include gilbert, raquel and tara. Teslas also don't detect cars in blind spots and still phantom brakes.

40 Assist problem of the 2020 Tesla Model 3

Failure Date: 02/11/2024

When driving in "autopilot" mode which uses adaptive speed control, the car suddenly slows down without any warnings. This happens even when there is no vehicle in front of the car and therefore slowing down is not expected. This has happened several times while driving on freeways. If it weren't for my immediate reaction, serious multiple-car accidents would have happened. My reaction has been to quickly disengage the "autopilot" system and manually accelerating to avoid rear collisions. The other issue is the car suddenly steer the wheel and drag the car to the side even when there is no reason to do so. In such cases, I have to forcefully steer the wheel to the opposite direction to avoid collision with other cars moving on either side, or parked cars on the streets.

41 Assist problem of the 2019 Tesla Model 3

Failure Date: 01/30/2024

Ever since Tesla updated my vehicle to 2023. 44. 30. 8, the autopilot has become effectively unusable. It now nags and beeps at me whenever I am not looking straight ahead. It is ridiculous that Tesla has ruined one of the best aspects of my car due to (ostensibly?) NHTSA pressure. Please allow their previous driver attentiveness system with the steering wheel to be reinstated. It is so miserable to have my car nanny camera flip out on me every time I look anywhere other than straight ahead. At the very least, every other manufacturer should be forced to have just as annoying a system if my Tesla had to be ruined. Its wrong to punish every Tesla owner because some people misused autopilot.

42 Assist problem of the 2018 Tesla Model 3

Failure Date: 01/29/2024

The new software update forced as part of the autopilot recall is terrible and the system needs to go back to the way it was. They adjustments make the feature useless. There are many unnecessary warnings and notifications, all of which destroy the driving experience and actually make the car less safe. Bring back the old system.

43 Assist problem of the 2020 Tesla Model 3

Failure Date: 01/27/2024

I am writing to provide feedback on the recent mandated Tesla full self driving software update (recall). When I keep both hands on the steering wheel, I now get constant warnings. Specifically, even when I drive with both hands firmly on the wheel at 9 and 3 o'clock and maintaining what I feel is adequate control, the system nags me with disruptive alerts. It tells me to apply pressure and flashes visuals that my hands are not properly positioned, despite both remaining clearly in contact with the wheel the whole time. I typically drive responsibly with proper grip, yet these frequent notifications are frustrating and stress-inducing. The sensors seem to be calibrated too sensitively if they are detecting grip issues when both my hands are correctly and securely holding the wheel. Prior to the mandated update, I didn't receive nearly as many warnings. The update was not helpful and makes driving more stressful and less safe if anything. I understand the goal is to enforce safe driving, but the current warnings are excessive even when drivers are maintaining proper hand positioning. Perhaps the regulations could be relaxed so alerts only activate when grip is legitimately inadequate, rather than routinely throughout normal driving. Please let me know if any further details on my experience with both hands on the wheel would be helpful. I would be happy to provide additional information to aid review of this system. Thank you for your consideration.

44 Assist problem of the 2020 Tesla Model 3

Failure Date: 01/16/2024

Dear NHTSA, I am contacting you regarding campaign #23v-838 and the recent Tesla software update, which I believe has compromised the safety features of my Tesla Model 3. For over two years, I’ve trusted my Tesla’s capabilities, notably its autopilot feature. However, the latest update has introduced a wheel detection system that’s overly sensitive and disruptive. Previously, my driving experience was smooth, responsible and safe, with hands on the wheel and autopilot assist for when I needed to make blind lane changes, allowing me to look over my shoulder briefly at the blind spot while the wheel kept straight without rear ending anyone. Now, with this mandated update, I’m forced to make frequent, forceful adjustments to the steering wheel. I can’t look away from the front of the car even briefly to check my surroundings. This constant intervention and forbidding drivers from looking around to check for safety is distracting and dangerous, leading to abrupt autopilot disengagements and brief rapid ways in which the car behaves as the car disengages and throws you into manual when you weren’t expecting it. This isn’t just about inconvenience; it’s a safety hazard. I’m now more focused on maintaining autopilot than on the road and surrounding conditions. This undermines the very purpose of the feature. As someone who’s always vigilant about my surroundings, especially at intersections, I find this update counterproductive. It forces me to turn off the feature, leaving me without the assistance that I’ve come to rely on for safer driving. I urge you to review this matter with utmost priority. The balance between driver engagement and the autopilot system’s safety has been significantly disturbed by this update. Necessary adjustments are needed to prevent potential accidents and ensure that this technology continues to enhance driving safety and efficiency. Thank you for addressing this critical issue.

45 Assist problem of the 2022 Tesla Model 3

Failure Date: 01/10/2024

Something occurred with the steering, automatic braking, or lane keeping assistance on 01/10/2024 at approximately 6:35am while I was driving my 2022 Tesla Model 3 long range north in the carpool lane just south of the 106th off ramp in sandy utah. The roads were a little slick so was proceeding cautiously at around 65 mph when the car turned to the left slightly, then immediately to the right, which caused it to do a 180 degree turn in the same lane, I wasn't sure what was going on as the car felt like it was driving itself. Before I knew what was happening, the car then veered to the east across three lanes of traffic while driving backwards, it then stopped just as it was struck in front of the wheel on the front drivers side by a two trailer semi-truck which spun the car around. Luckily I was conscious so could slowly limp across the last couple lanes to the right side of the road where I called 911. I've driven all kinds of cars in all kinds of conditions and in many countries in the nearly 32 years I've been a licensed driver and have never lost control of a vehicle, I'm sure the steering or lane assist was trying to intervene so must've somehow inadvertently caused the crash! it's only through the grace of god that I'm still alive to tell about it! also, none of the airbags went off even though the impact from the semi was very jarring!.

46 Assist problem of the 2021 Tesla Model 3

Failure Date: 01/03/2024

Autopilot feature is now unstable as I get constant nags if I look away for a few seconds to check my surroundings or check my navigation. I have been locked out of the feature multiple times over the past week forcing me to make navigation changes while manually driving.

47 Assist problem of the 2020 Tesla Model 3

Failure Date: 01/01/2024

I am writing to bring to your attention a concerning issue regarding the recent update related to recall alerts in my vehicle, which has inadvertently led to an increase in distraction while driving. I received a software update for my vehicle, which included new features aimed at enhancing autopilot alerts. However, instead of improving safety, these updates have unintentionally resulted in an increased level of distraction while driving. Issues identified: frequency of alerts: the frequency of recall alerts has become excessive, leading to a constant stream of notifications while driving. This has proven to be highly distracting and has compromised my ability to focus on the road. Alert presentation: the visual and auditory presentation of the alerts is intrusive and often occurs excessively during driving. The alerts are not sufficiently streamlined to ensure minimal disruption, causing a significant distraction. Impact on driving behavior: the continuous alerts have forced me to divert attention away from the road, leading to an increased risk of accidents and compromising overall road safety. Recommendations: as per most other vehicle brands that benefit from self-driving technologies, like Cadillac and Ford, they are not affected by this recall, and do not make the driver continually look at the display for information regarding to the alert. Intelligent alert timing: incorporate a Smart algorithm that considers the driving context before issuing an alert. This can prevent alerts during critical driving moments, reducing distraction. Comprehensive testing: conduct thorough testing of software updates before release to identify and address any unintended consequences, such as increased distraction. Before this update, it was a lot less distracting to drive this vehicle, due to the limited notifications. Since this update, I've found myself removing my eyes from the road because of the increased notifications. A reversal of this recall is in need.

48 Assist problem of the 2018 Tesla Model 3

Failure Date: 12/26/2023

I am writing to report urgent safety concerns with my 2018 Tesla Model 3, software version 2023. 44. 30. 5. 1, after the NHTSA recall on autopilot functionality. I have owned my vehicle for over 4 years and have never had concerns with the adas / auto-pilot / auto-steering functionality until the NHTSA required changes in December 2024. While driving on I-405 in garden grove, CA, my vehicle while in adas / auto-steering / autopilot made repeated and unusually persistent attempts to change from lane 4 to lane 3. I had to manually cancel each attempt, despite my speed being set to match traffic flow. This issue never arose before the software update for the NHTSA recall. Moreover, the vehicle consistently signaled for driver attention, despite my full attention to the road, a concerning new behavior post-update. The need to frequently override these undesired maneuvers forced me to disable the adas autopilot/auto-steering. This puts me and my vehicle occupants at risk, as I believe the adas system, functioning correctly pre-recall update, safely maintained distance from vehicles ahead and did not make unintended maneuvers to change lanes. These issues suggest the recall modifications adversely affect autopilot functionality, creating safety hazards. This is especially alarming given the potential for serious accidents due to unexpected lane changes and false driver attention alerts. I urge the NHTSA to reconsider the recall modifications to Tesla's auto-steer/autopilot adas features. It's crucial that recall adjustments do not jeopardize vehicle and vehicle occupants safety. A comprehensive review of the recall's impact is needed, potentially leading to an amendment or reversal, to ensure the safety of Tesla users and other road users.

49 Assist problem of the 2018 Tesla Model 3

Failure Date: 12/25/2023

Dear NHTSA representative, I am writing to express my deep concerns and seek resolution regarding the latest version of autopilot enforced in the recent Tesla recall, identified under campaign id #23v-838. I am a Tesla owner that’s deeply invested in the safety and functionality of my vehicle. Recent reports indicate that the mandated autopilot update has rendered the car unsafe and nearly undrivable with autopilot engaged. Disturbingly, users have reported false warnings, causing drivers to divert their attention from the road and react to incorrect information. This poses a significant safety risk to drivers, passengers, and others on the road. I wish to bring to your attention that the autopilot feature in my vehicle is currently usable, and I am hesitant to compromise its functionality based on the reported issues with the mandated update. It is disconcerting that a safety recall mandated by the NHTSA could potentially make my vehicle less safe for me and my family. I urge the NHTSA to conduct a thorough investigation into the safety implications of the autopilot update and to consider the concerns raised by Tesla owners seriously. Thank you for your prompt attention to this matter. I look forward to a swift resolution that prioritizes the safety and satisfaction of Tesla owners. Sincerely, a Tesla owner.

Since NHTSA forced Tesla to update their driver attention monitoring, I can no longer change the radio station without the car screaming at me. This is jarring and unsafe as it surprises me when there is no danger present! NHTSA’s requirement for Tesla change this behavior for “safety” is unsafe! allow Tesla to roll back this change now!.

53 Assist problem of the 2023 Tesla Model 3

Failure Date: 12/21/2023

So thanks to your interference my autopilot on my Tesla is broken. . . You guys think you are making our cars safer by getting your controlling poorly educated stupid hands into Tesla systems that are designed to be more safe. On paper Tesla autopilot is factually proven to be more safe than a human driver, these reports are publicly available perhaps your team was too lazy to read them. My car disengaged autopilot randomly while driving with 2 cars behind me and freaked out and told me I was not paying enough attention to the road just because I reached to unlock my phone screen. . . Not even pick it up off the wireless charging pad. . . Just unlocked the screen. So thanks to your agencies interference and forcing Tesla to levy this bogus "recall fix" on us I almost got rear-ended. . . In the car I work so hard to pay for, the car that I love because it keeps me and my family safe because guess what we are humans and not robots. Just as you do and everyone in your agency we all have to look at our phones briefly while driving because we live lives where we have to make sure those we love are ok or make sure our phone is charged for when we reach our destination. You are going to do exactly the opposite of your agencies supposed mission of keeping us safe. You will kill people with this stupid "recall". Get your crap together and tell Tesla to reverse autopilot back to its normal safe operating parameters and stop banning people from autopilot otherwise there are going to be a lot more wrecks and deaths. I'm not saying you should be taking duck selfie pictures for social media with your phone while in autopilot on a rural back road. . . That person deserves to be a victim of modern natural selection. But what your doing is over-controlling and you have no right to dabble in things you obviously don't understand. Your agency should be ashamed of itself, you are about as efficient as our government is at managing money. . . Fix it back.

54 Assist problem of the 2023 Tesla Model 3

Failure Date: 12/20/2023

Subject: safety concerns regarding recent Model 3 autopilot update to whom it may concern at the national highway traffic safety administration (NHTSA), I am writing to urgently express my concerns about the latest software update for the Tesla Model 3’s autopilot system, which, in my view, significantly compromises safety. As a Model 3 owner, I have generally found the vehicle and its features to be reliable and safe. However, the recent update has introduced an overly stringent hands-on-wheel detection mechanism that is not only inconvenient but also potentially hazardous. The new update requires frequent and often forceful interaction with the steering wheel to assure the system of driver presence. This change is drastically different from my previous experience, where I received only one hands-on-wheel violation in two years. The current sensitivity of the system disrupts the smooth operation of the autopilot, leading to frequent and abrupt disengagements. I have found myself struggling to maintain the system’s activation, inadvertently causing the vehicle to exit autopilot mode multiple times. This issue goes beyond mere inconvenience; it actively detracts from driving safety. The need for constant and sometimes aggressive adjustments to satisfy the system’s requirements is distracting and can lead to erratic vehicle behavior. The irony is stark: a system designed to enhance driving safety and ease is now a source of potential danger and stress. The unpredictability and over-sensitivity of the updated system could lead to dangerous situations, especially on highways or in heavy traffic, where sudden disengagement of the autopilot can be particularly risky. As a driver, I now find myself more focused on keeping the autopilot engaged than on the actual driving conditions and surroundings, which is surely contrary to the feature’s intended purpose. Please fix this issue. It is bad for everyone.

59 Assist problem of the 2023 Tesla Model 3

Failure Date: 10/18/2023

Summary: fsd malfunctioned, failed to disengage, causing loss of steering control, thus causing car to steer into wrong lane and resulting in an accident. Tesla service center refused to inspect unless car is fixed. Car was sent to crash champions ( autobody shop) and was quoted $35k to fix. Car was deemed as totaled by Tesla insurance. No response from fsdbeta@Tesla. Com I suspect that the latest fsd software update has caused my car to lose control. Time of incident: 19:28 pm location : wildwood Ave, sunnyvale Ave. Entering from lawrence expressway. Weather condition: clear night, well lit. Low traffic. What happened:? my car has been driving in fsd on lawrence expressway and it was doing everything correctly. Approaching that upcoming tight bend, the fsd slowed down the car, and I took over by steering the steering wheel to the right to deactivate fsd and taking the tight turn, right before the white rows of triangles on the street. At this moment, the steering wheel forcibly turned to the opposite direction (I. E. Left). I tried to correct the steering wheel by turning right even more, but the steering wheel wasn’t responding to my input for a split second. At this moment, my car is now on the wrong lane of wildwood Ave and my car finally responded to my steering input. I had to swerve in the opposite direction to avoid the incoming traffic and ended up brushing against an SUV, followed by an impact on the curb, before finally slowing down. Seeing that there was incoming traffic from the back, I drove down to a residential street to park my car for safety reason. The SUV driver called the police to handle the situation, we had our information and testimonies recorded down. I have already followed the safe practices of driving with fsd according to the manual, but in this situation, I am not certain which technology is responsible for causing my car to override my manual steering input. This led to me to question the driver safety systems in this car.

Over several occasions, this Model 3 Tesla has engaged the brakes heavily while using the autopilot, lane-assist function. This has always happened while going highway speeds. The braking is very abrupt and without any apparent reason (nothing in the front or beside the car). I do not recall this occurring unless the autopilot function is enabled. I am able to disengage the system and take control, but it's an alarming and unsafe glitch in the software.

Louisiana 425 north. Inside lane. Yellow line remained solid on my left. There wer no striped lines for the car to follow to continue straight as we approached on full self driving. At highway speed the car veered into the turn lane. I immediately took control and prevented encountering the embankment. Dangerous situation. Tesla did what it was supposed to do. Louisiana highway engineers not so much. I have never encountered this in any of the other six states I have driven in. Tesla fsd will always stay in the lane if there are striped lines for it to follow on the outside of the lane,eg, when passing exit ramps. Thanks.

I was driving on the garden state parkway in new jersey in the right hand lane. I was using basic auto pilot and needed to pass a pick up truck in front of me. I disabled auto pilot and went into the left lane. I re-engaged auto pilot and the vehicle took a hard right and hit the truck. I heard the audible chime once it made contact with the truck indicating that auto pilot disengaged. My car went back into the left lane and I took control. We moved to the shoulder and waited for state police to arrive. Luckily no one was injured. A police report was filed.

The automatic windshield wipers came on and would not shut off. The car was clean, the day was sunny, and the camera lenses were free of dust or anything else that could cover them. While driving along, the wipers started and would not shut off. I went into the main monitor/screen that has the wiper settings but I was not able to activate the wiper/off button, so the wipers kept working. Pressing on that button would not activate it. I was in autopilot at the time, I. E. , cruise control + lane keep assistance. I have fsd/full self driving, but have never used it since it is still in beta testing. I’m using Tesla software # 2023. 7. 10 I checked the various Tesla websites, and you can turn off the wipers only if you disable the adas features. After I turned off the adas, I was able to activate the wiper/off button. But this is a terrible situation, the adas needs to be activated, but if I activate, it wipers will begin working whenever they feel like it, regardless of the weather outside. This is clearly a software problem with the Tesla adasn needs to be corrected. I don’t want to be driving along with auto pilot on, and then the wipers will simply turn on. Any help you could provide would be greatly appreciated to correct this dangerous situation. There are countless hundreds of complaints like this on the various Tesla websites about spontaneously functioning wipers when the weather is bright and sunny.

Failure Date: 04/10/2023

An incident occurred in April of 2023 while running fsd beta 11. 3. 6 firmware. I have been unable to retest this location on more recent software, so this concern may have been mitigated with a subsequent update. As seen in the video, the yellow lane marking on the left transitioned from the roadway up onto the movable center median. Once on the bridge, the left repeater camera and the center display visualization both illustrate the lane centering was definitely biased towards the left side of the lane, a possible indication the vision-based lane centering was miscued by the yellow line now appearing on the barrier instead of the road surface. This left bias brought the vehicle uncomfortably close to the barrier, which resulted in a manual takeover. It is difficult to judge based on the low resolution of the repeater camera view, but my estimation is that the tires were approximately 12 inches away from the protruding bottom lip of the barrier. .
