Issue description gps freeze: the navigation system remains stuck at one location even when the vehicle is moving. Park assist not available. Fsd not available. Rear camera shows black screen, completely disabling reverse visibility. These issues occur intermittently, often lasting for days, and then resolve themselves unexpectedly. Unfortunately, when scheduling service, the problem often disappears by the appointment date, making diagnosis inconsistent. However, the failures have already occurred more than three times, and each time they severely impact both safety and usability. Safety concerns lack of rear camera visibility creates a direct safety hazard during reversing. Disabled park assist and fsd compromise Tesla’s advertised safety features. Gps malfunction affects navigation and system reliability.

2 Blind Spot Detection problem of the 2020 Tesla Model X

Failure Date: 06/20/2025

On or about 20jun2025: Tesla driver assistance features - after the most recent 2025. 20. 3 softball update and installation my safety feature of the blind spot monitoring doesn't chim when a car is in my blind spot the car will show red in the font display but the sounds are now inoperable.

3 Blind Spot Detection problem of the 2021 Tesla Model X

Failure Date: 06/02/2024

This complain is regarding the recent recall regarding fsd Tesla Smart summon failures. Incident date June 02, 2024 car was parked in a parking lot outside a movie theater. I was less than 20 feet away from car with clear visibility, and tried Smart summon. Car pulled out of the parking making an acute turn damaging driver side door, and front bumper of car parked next to it. Tesla was contacted immediately regarding this malfunction of Smart summon and requested them to stop offering this service to owners as it is dangerous. They took 3 months to conduct an internal investigation but refused to share written results. Instead read a generic statement from their website. Still not received a copy of the investigation. Tesla also refused to take take financial responsibility for the damage and their false claim. Smart summon is dangerous and Tesla should not be allowed to advertise.

5 Blind Spot Detection problem of the 2022 Tesla Model X

Failure Date: 01/17/2024

Ever since a recent software update, when turn signal is activated, right fender camera feed appears on screen but is completely black instead of showing a visual of the right blind spot. Due to this other features such as autopilot, automatic emergency braking, and likely most of the adas functions are also disabled. Due to the vehicle's poor natural visibility, there is a high reliance on the camera systems functioning properly. Not having a blind spot camera endangers turns and lane changes, especially if one has become accustomed to their usage. With most if not all the adas features disabled, we are returning to a 1990's level of auto safety. The problem remains unresolved despite a recent 2 week stint in Tesla service. Since this is most likely a software issue (possibly a regression introduced with the recent rear camera recall fix) there is no recourse except for Tesla to issue a bug fix. The car was returned to me without resolution. I'm not the only party experiencing this problem. 6 Blind Spot Detection problem of the 2022 Tesla Model X

In April, 10 2023 approximately 6:10 pm I had an traffic accident. My vehicle was parked at paid parking spot next to the pedestrian road (street parking) and I was approached from the back of the car and I recognized the bus approached from my backside of the road. I have no idea somehow, the front driver side door suddenly opened when I approached to my car about 4-5 feet away from the trunk, even though I did not pressed any door open button in my Tesla smartphone app. As a result, the opened door had collided to the la metro bus backdoor. I would like to NHTSA will investigate this faulty safety issue. Thank you.

7 Blind Spot Detection problem of the 2017 Tesla Model X

Failure Date: 12/22/2022

Continuation of complaint 3 of 3 though this occurred 12 times over 6 months and nearly causing 6 accidents - tell me if you need me to make 6 separate complaints - 12/8/2022 - vehicle key fobs stopped working (still able to start vehicle from the Tesla mobile app) and the screens completely locked up while driving multiple times nearly causing multiple accidents. It's also likely that the key fob failure was a result of the malfunctioning computer. Cleared computer cache. 12/23/2022 - battery fault (vehicle may not start warning) bms_w035, front passenger safety restraint fault, buggy and unsafe screens; still no information from Tesla regarding the recall for the malfunctioning screens and computers. Tesla stated that they could not "see" any of the faults in the vehicle or in their server, though my wife took photos. 1/16/2023 - service request scheduled for warranty/recall repair for unsafe screen hazard that was resetting while driving, vehicle towed to Tesla; 14 Jan 2023 Tesla cancelled the service request and input their own request that describes a customer inquiry for an infotainment (computer) upgrade and that we've declined the repair. Based on this, I am unable to download the invoices because they've jammed up the system to further impede proving their negligence. Currently, Tesla has locked up the app so that I cannot create service requests for the warranty/recall issue, they're failing to repair the vehicle, they're failing to communicate beyond threats of daily charges and towing, and it is impossible to reach someone at the service center though I've been able to describe the issue on multiple occasions to the Tesla phone answering service folks. Tesla is out of control and has placed my family and others in harms way due to their negligence to inform and accomplish their warranty and recall requirements. I respectfully request your help in holding Tesla accountable so others are not longer put at risk, and so their vehicles get repaired.

8 Blind Spot Detection problem of the 2017 Tesla Model X

Failure Date: 12/14/2022

This is a follow on complaint due to this occurring 12 times over the past 6 months, 6 of those times nearly resulted in catastrophic accidents. I wrote Tesla via certified mail, per their owner's rights requirements in Dec 22 and Jan 23, stated the problem and requested repair, replace, or repurchase. I gave Tesla ample opportunity to repair the vehicle. Tesla now refuses to repair and is failing to adhere to their own warranty, recall, stated owners rights, TX code, and nthsa guidance (recall and likely codes due to breach), and they're now threatening me and behaving negligently by asking that I take their malfunctioning and hazardous vehicle back out on the road. Our vehicle has been out of service 2 months total and 1 month currently over multiple corrective action attempts for a recalled item. Service visit invoices versus reality, plus dates in and out of service do not include additional vehicle down time while waiting for service visit time slots - we purchased the vehicle on 7/23/2022 - I do not have access to records prior to this date, though I am certain this vehicle was plagued by the recalled issue under factory warranty. I firmly believe, based on Tesla's actions under my ownership, that they will do anything in their power, including blocking or changing vehicle history so they can avoid their responsibilities. 7/23/2022 - key fob battery replaced; invoice failed to include service center employee hard resetting the computer system and faulty ac system, plus trim piece identified as needing repair 8/10/2022 - hvac and trim corrected; invoice failed to include issues with computer and technician described that some of these require resets sometimes, but failed to provide info about the recall and the nature of the failures. I described that the screens would unsafely reset on their own and the technicians failed to provide any further information. 9/16/2022 - vehicle failed to charge at high speed charging and buggy/malfunctioning screens included.

When the car automatically started to "precondition" for supercharging while driving to the supercharger location, the binnacle lit up like a christmas tree. Warnings included, autopilot unavailable, brakes faulty, navigation unavailable and low tire pressure. In addition, all camera related functions such as back up camera and blind spot warning froze. The behavior has repeated itself without warning shortly after I supercharge the car each and every time.

While driving on an interstate at night traveling at 70mph the computer/console froze navigation and wouldn’t respond to touch commands. Then everything on the car went dark. No screen, no signal lights, nothing. Luck is the only reason I did not have an accident. This was terrifying and dangerous and I and others could easily have gotten hurt. My car had no lights and other cars couldn’t see me as I changed lanes (without being able to signal) to pull in the emergency lane. The car has had connectivity issues starting at 1 year after purchase (about one month ago). I am sometimes unable to play music and the computer screen reads “no connectivity”. It may be a coincidence that the free 1 year trial of premium connectivity just ended at the same time these issues began. I now pay $10/month for the service. Right before the screen went dark I had been on the phone, which was docked in the charger. This all just happened and I have an appointment in 10 days at the Tesla service center.

The mcu (media control unit) and the driver's dash goes out while driving under normal driving conditions. When the mcu and dash goes dark/dies, the turn signals don't work, the rear view camera goes out, the blind spot warning and lane departure warning is disabled and the speedometer doesn't display. Basically I was driving in the dark. It was a very dangerous situation. Fortunately I was driving and not my wife who is the primary driver. This is no longer a technical safety concern but rather a safety issue for us and for others. I brought it in for service 2 times and the outcome was: we're aware of the bug and working on an over the air fix but no eta. This is not an isolated issue since it occurred many times captured below: Jun 22 - 8:33 pm pst Jun 30 - 8:32 am pst Jul 2 - 12:32 pm pst Jul 8 - 5:58 pm pst Jul 13 - 8:09 am pst Jul 16 - 6:46 pm pst this is similar to the situation that occurred in the collapse of the condo in florida. In that case: 1. They were aware of the issue 2. They knew they had to fix it 3. But they didn't take responsibility nor accountability. They should have evacuated the building instead of killing 100+ innocent lives. In our case: 1. Tesla knows there's an issue 2. Tesla knows it needs to be fix 3. Tesla refuses to take immediate action until something drastic occurs. The car is practically brand new with only 5k miles on it. I have videos to capture this frightening ordeal.

After software update blind spot monitoring no longer exists. This resulted in a few near miss accidents on the highway. When I raised the issue with service they said it is operating as designed.

After I put my car in park mood . . Within few second car started taking off speeding over the grass /plants area of parking lot in to the office unite ( meta/ glass door ) shattering glass was all over broke through frame work of the office unite all the way to the examination chair of dr office . . Had there be any worker / patient/ dr in the unitr someone would have been dead or injured . . I am scared for my life to drive this car around after police & fire department arrived at the scene I was informed by firefighters that code they were called in to a house near me for another Tesla speeding through the garage to the living room of the house I believe there is defects in these cars as most of complaints file 2020 has reported same issue . . I received my car in the middle of pandemic where no body walked me through this car so I had any idea there has been injury and death reports as well . .

Failure Date: 10/11/2020

On the beginning of October 11, my reverse camera stops functioning. At night, when I park my car in the garage, my front screen (behind steering wheel) was blank. Shortly after, my car suddenly self-accelerated while I was parked. This resulted in the car smashing through my garage wall, causing loads of damage in the room opposite of the wall. Some damage included: a broken wooden bar, shattered glasses & liquor bottles, and damages to the front of my Tesla. This car was newly purchased, with a milage of simply 40 miles.
