Just days after taking delivery of our first Tesla (2022 Model Y, second-hand ), we experienced a dangerous incident. While autopilot was engaged, the vehicle failed to slow down or properly navigate a curb near a roundabout. Despite our immediate manual intervention, the front wheel was damaged, resulting in loss of steering and a repair bill exceeding $7,000. After the incident, we were surprised and alarmed to learn that Tesla’s autopilot is widely known to be unreliable and risky in urban settings. Yet, Tesla imposes no restrictions or clear warnings on its use in such areas, even where the vehicle’s software could easily identify elevated risks. Autopilot handles highways reasonably well, following lanes, making turns, and maintaining distance from other vehicles. This creates a misleading impression that it will perform just as effectively on city roads. However, city roads are far more complex, and the brief transition from autopilot to human control is often insufficient to prevent accidents. Our experience painfully illustrates this. What concerns us most is that Tesla includes autopilot as a standard feature but fails to adequately educate users about its limitations. There are no effective restrictions to prevent the system’s use in high-risk scenarios, nor are there built-in warnings when the system is engaged in unsuitable driving environments. This creates extremely dangerous situations for customers. Every life is valuable and deserves proper protection. In addition to the physical damage, the emotional toll after the incident has been significant. What feels especially unjust is Tesla’s response. The company has shown indifference, refusing to take responsibility or provide any compensation for an incident caused by clear system design flaws. We are submitting this report in the hope of preventing similar accidents and receiving the compensation we rightfully deserve. On Friday 10/11 at 10:45 a. M I was in traffic on day street in moreno valley, California. This was the third time I experienced a sudden unintended acceleration with my Tesla. I would have hit the car in front of me, but I was able to swerve into an island on my left to avoid the car. The car went over the curve and stopped. I am unsure what happened but after my prior experiences, I knew how to react. Had I not reacted swiftly, I would have hit the car in front of me. There were no warning messages. I have an appointment with Tesla on wed 10/26. When this happened the first time, they claimed there was nothing wrong with the car, but I think they are not looking hard enough. I am aware of a voltage problem that other Tesla owners have experienced that can lead to sudden unintended acceleration.
Multiple, random occurrences of hard braking at highway speeds when there was no object or obstruction ahead. In the 2-1/2 years we’ve owned the car this has happened on two lane and multiple lane highways, generally at speeds over 50 mph. Also, the windshield wipers randomly deploy even when turned completely off. They generally work when it’s raining but the delay function is also randomly not in sync with the settings or actual rainfall.
The issue I encountered was with Tesla's Smart summon feature. While using the feature, the car crossed the parking line and crashed directly into a car parked beside it. Happening so quickly I could not react in time. All of the cameras and sensors were in working order and it seems to have been a software related issue. The safety of anyone around the vehicle could have been at risk thankfully no one was hurt. The vehicle has since been inspected by insurance and damages to both cars were repaired. There were no warnings of any kind prior to the collision. I have a video from the on-board cameras of the entire incident.
The front triple camera glass was found to be dirty on the interior part of the glass. I had alerts of camera blocked/blinded and also I had several instances of emergency corrective maneuvers including braking, lane assists and forward collision warnings. Tesla cleaned the interior of part of the glass.

The proximity sensor alerts do not work with Tesla vision and resulted in a minor collision with adjacent vehicle while manually parking. With the removal of the ultrasonic sensors from the alert system, Tesla is not reliable anymore for distance warning and is responsible for accidents to to the false sense of confidence provided by that system.
Yesterday I drove 1 shift on uberx in chill mode, very carefully and tried to save as much energy as possible. Here’s what happened. I left the house with 100% charged 274 miles range not 303 advertised I track everywhere I go with everlance gps. After a couple rides, I noticed the cars augmented reality for auto pilot wasn’t seeing the environment properly and like usual the safety features were disabled, but I am unable to record it responsibly while driving my passenger on uberx not uber black. When I got to a red light with no passenger, I recorded on my iphone the Tesla failing to see about 35 to 40 vehicles over the course of 45 seconds and also fails to see a black man walking slowly across the crosswalk just 2 feet in front of the car but it can see cars 100 feet away. Meaning there is an intentional blindspot that would be in the collision warning zone that the car is purposely not seeing because the car is hacked. Directly after that I picked up a man from his house who also has a Model Y and brought him to the airport. He’s an augmented reality engineer that looks familiar to me from Tesla. Turns out someone stole his brand new Model Y from tampa Tesla, and brought it to a chop shop and went on a joyride, destroying the alignment, which he was given a large check for. He said Tesla was able to provide all the problems with his car with no issue. I told him all the problems with my car. Tesla has refused to even acknowledge after a dozen service appointment. Then I told him I have 100,000 gb of dash cam footage and secret recordings of the service manager telling me if I submit the footage to the fbi they would refuse service. Well, they already are refusing service. Also I drive a lot of federal agents and military personnel. When I got home, the car couldn’t account for 70 missing miles and it’s charging at one quarter speed. I reset the car three times. Now I’m trying to submit this on my computer, but the internet cut out! busted! shut down Tesla!.
Tesla took my car in for service again, fixed nothing again. This time they messed up my dashcam even though it is clearly marked with bright red notice stickers on both sides of the car that consent for audio and video recording is given by entering vehicle that is owned by me (not a lease). Car wouldn't charge yesterday, then I do the newest software update and when I get 3 miles down the street from my house I yield for an ems emt with siren and lights on in the middle of the day with perfect visibility and big surprise the car doesn't even register that I've not only left the lanes without lane keeping assistance but it also doesnt see the emergency vehicle so I have disabled fsd and am refusing to use ap because this is a death trap. When I complained to the fbi about it they tried to have the local pd basically swat my house at 4am and wake up my whole family. My wife and I are both fbi witnesses. I also received a letter from the irs claiming they changed my taxes from 2020 (filed by my accountant) claiming that I owe $132,021 (on less than $800 in gains) by next month or they will charge me $20,000 a month interest. Today I received a notice from my insurance company of 1 year claiming they are canceling my coverage because I "drive uber black" which isn't true I never have because the orlando pd wouldn't approve me to because the insurance agent lied about the coverage because ana (moore resources insurance) and Mercury insurance; was too drunk on new years day and has caused me to lose $30,000+ in lost revenue because she ruined my 4 month wait for uber black approval which has completely destroyed my financial ability to provide for my family. I received a call from a dot agent a couple weeks ago from washington DC but the call was dropped so he called me back on a cell and we haven't been able to contact each other since. Made it less than a mile from my house this time when the Tesla couldn't see the fhp (florida highway patrol) with lights on and perfect visibility. After the car couldn't drive me into oncoming traffic and put me out of my misery; I reset the car and then all the safety features were disabled again, the car couldn't see the solid white freshly painted lanes during the middle of the day with perfect visibility. On April 28th I was driving to work as usual in my Tesla Model Y , and I am on a local road driving around 40-45 miles speed. I was always maintaining enough stopping distance and on one of the local road, a driver before me stopped abruptly because of garbage trash can came and hit her car. I braked immediately and expected Tesla will stop before only but seems Tesla did not brake till last second and I ended up pulling to the right most side to avoid collision and I ended up hitting pole to save hitting any humans to the best of my ability to save lives. My biggest concern is why Tesla emergency brake did not apply for forward collision? what went wrong? as far as I remember I put the brakes and it did not stop. In a 1 minute clip I captured how the Tesla constantly misses collision warnings and/or assistance while all safety features are turned on because the car misses the first collision warning and then goes off early on the second scenario with the same car and driver in front of my passenger and I. It's dangerously inconsistent and this is the third passenger on lyft this week this has happened to and the passenger verified as a witness to the event in each example without any sort of coercion. Without the lidar the Tesla makes constant mis-judgements it never should have been removed. Last night I traveled down 429 west and had the same issue with the car reducing to 0mph like it has been for several months but this time I didn't let the car slow down all the way before canceling and leaving Tesla a nice explanation in my best forrest gump impression soundtrack was playing. I said "life is like Tesla fsd, you never know if your cars gonna stop on the highway". What's the point in the voice recording if the error isn't corrected? to make me feel like something is being done? everyone in person and online keeps telling me to just sell the car. Auto acceleration(forward) caused while parking and crashed into fence and utilities. Requested Tesla to diagonalize the cause of auto acceleration. Tesla couldn't handle my request well and they canceled and delayed my request, finally, they rejected my request as the car was salvaged.

During my driving shift on lyft my first ride was right next to me at the medical clinic when I accepted the ride but lyft sent me 10 minutes away to park in the middle of a median facing a memorial for a car crash victim (while playing stan by eminem about a fan being killed in a car accident after no one gets his letters(on Tesla radio which has been proven to be hackable and I have reported to NHTSA before)). She gives me directions; I accept the ride then drive 10 minutes back to where I accepted the ride in the first place. The gps location for lyft is over a mile off, after a couple minutes of scanning parking lots for people I've never met I find them at the health clinic (lyft had deleted her directions she sent me so I was going off memory from the phone call). We start talking and it turns out they are currently in a lawsuit against lyft. On the way to their destination (which was also wrong by several miles and required their directions) the husband and I start noticing really aggressive drivers around us then all of a sudden the car in front of us hit the brakes short notice and zero of the cars collision warnings and safety precautions worked because there is clearly no audible sound. I have the sounds enabled I also have since day 1 because that's why I bought this car, for safety. Luckily for me I said nothing until the passenger mentioned that they braked too hard, I did a good job reacting and they agree there was no collision warning once I explained to them that the Tesla is supposed to do that but it rarely works when it should and goes off all the time for overly dramatic reasons. It's like elon himself hacked my car and made it a clown car as described by himself in his own tweets. Upon the riders request I have released the entire recording to them via cloud server for use only with my written consent. They will be forwarding it to their attorney who is currently representing them so I would think twice about using 'kid gloves' on Tesla now on.
Tesla was in "full self-driving” (fsd) mode in a quiet residential area. Vehicle chose to make a left turn on a less than perfectly symmetric t junction. The car was on the thru street at the top of the t. Street had a middle yellow turning zone with yellow lines on the outer side and dotted ones inside. Camera was not filming at the time the crash began. But did start recording midway. Tesla may have other footage. In retrospect, while indicating it was intending to turn, I believe the car never entered the middle yellow zone, but instead proceeded directly to the middle of this unusually wide neighborhood intersection where no other vehicles were close by in any direction, although a few were approaching from a distance. It appeared safe to complete the turn, but instead the car feinted forward several times and hesitated to commit to the turn as cars gradually approached from both directions, but were still a safe distance away. As I waited for it to turn, the vehicle suddenly accelerated at an alarming rate into the turn, so fast that it slid off the road and over the curb into a wall causing extensive damage to the vehicle. There were no other cars involved and I was not injured, although the car suffered extensive damage and is still being repaired more than two months after the accident. My foot was definitely not on the accelerator throughout the accident and everything happened so very quickly that I had no time to brake or take other action. The oncoming car was quite far away and there was plenty of time to turn safely. If the car issued a warning (although I did not detect one) it would have been almost simultaneous to the amazingly rapid acceleration, leaving no time to react. My speculation is that in its confusion, the vehicle may have transferred nearly instantly from fsd mode to “accident-avoidance” mode, but only Tesla would have that information.
I've brought the car into the dealership 3 times and they swear they have gone over the whole car and there are no issues with the car but at the first stop light next to the dealership the car starts driving into oncoming traffic which I recorded on dashcam. The car keeps phantom braking (braking for no reason) swerving towards concrete barriers and brick walls, driving into oncoming traffic on roads its navigated before in the same and different conditions. Just yesterday the indicator noises stopped working so I was forced to reset the car while driving my whole family, this is very dangerous and far too common. I have been harassed by Tesla employees who refuse to find any solution. I have been requesting a refund for fsd since the week I bought the car. This is a huge scam and cover up and when you post videos of your car trying to kill you on twitter or ig (post elon) they shadow ban you. It is against their agenda. Car did not stop after I parked and continued to go forward and hit another parked a car even with my foot on the break. No warning light of any sor. This incident happened at 5:35 pm pacific time on Monday, September 12. I made an appointment with the service department and it took the car there on September 23, 2022, . They only asked me about the time this happened and they said you’re going to send it to engineers and will get back to you. They did even care to look at the car or inspect it. Since then, no one ever contacted me. I was lucky that this truck was in front of my car otherwise if there was anyone walking they would have been killed.
Was stopped at a light and the car rapidly accelerate on its own hitting the car in front of me. The air bags didn't deploy. I stepped on the break pedal but the car continued to accelerate, I have informed Tesla and opened a service ticket. A police report was filed for the resulting accident involving 2 other cars. I have also had instances in the recent past where while driving on the highway the car has rapidly slowed down / applied the breaks for now obvious reason. While on a cross country road trip, our 2022 Tesla Model Y has repeatedly slammed on the brakes at highway speeds with nothing in front of us. Most times have been cresting hills, but one time it was on straight road with, fortunately, no other cars near us.
My husband took ownership of a brand new Tesla y on Saturday 18 June from the Tesla loveland dealership in colorado. In less than 2 months of receiving the car, the electrical system ‘totally failed and ‘malfunctioned’ 3 times, while driving on busy roads/highways which placed my family’s life and safety at high risk of injury and death from a potential head on collision and crash from vehicles driving behind. The 3rd and most recent ‘malfunction’ occurred on Saturday 6 August at 12. 46pm when (with no warning from the car’s warning or emergency system) vibrated aggressively before completely stopping in the middle lane of a busy road. This potentially fatal ‘malfunction of the cars electrical system’ was on one of boulder’s busiest roads. I had turned onto a busy road (so was traveling at a very low speed) when the car started vibrating very loudly and stopped. There was no safety warning by the car nor any time to drive the car to the side of the road. This ‘malfunction’ and total outage put my life and my children’s lives at risk as we were trapped in a 4-lane carriage way and risked our safety to avoid oncoming traffic to get to safety. My husband called the police who created a safety barrier around the car while a tow truck arrived to tow the car to Tesla in loveland colorado. The incident put my family’s life at risk including my two young children (aged 8 and 6) who were passengers in the back seat. The first ‘electrical malfunction’ occurred on Thursday July 14 at 9. 30pm when the car’s electrical system totally malfunctioned and stopped on a busy road. The car was towed to Tesla in loveland. Several days later when Tesla technicians advised the problems were fixed and arranged for a Tesla technician to drive to us, the car malfunctioned and stopped on a busy road while the Tesla driver was in the car. It’s was towed again back to the dealership. This car is a lemon, unroadworthy and poses a death risk.
Driving down the road on cruise control and car will slam on the brakes and you almost get rear ended every time. Scary to say the least.

I was driving on the highway using autopilot, and the brakes engaged very suddenly without any warning signal nor was there anything in front of my car. (aka “phantom braking” event).
The car is experiencing phantom braking. The vehicle incorrectly detects objects and brakes vigorously dropping speed really quickly. If traffic is following we could easily get rear ended. This happened over 10 times in a 100mile journey.
While adaptive cruise control is engaged, the system seems to incorrectly interpret a potential head-on collision and brakes hard and usually plays the alarm sound. I've had this car around two months and have found that this usually happens on a two-lane highway while traveling at 50-60 mph and when a box truck or larger is approaching from the opposite direction. It's worst when the sun is bright and casts a shadow of the truck and trailer across my lane. But it occasionally happens at night and when conditions are cloudy. I have to keep my foot over the accelerator pedal and force the car to keep speed, especially if there is a car close behind me. Otherwise, I'm scared I'll cause the tailing car to hit me. It's not as bad if there is a car in front of me. If no cars are in front of me, and cars are behind me, I'll disengage adaptive cruise in order to avoid causing a collision. I have not had Tesla look into this, however, I have seen comments online that others experience this problem as well.
