While driving at approximately 25 mph on a local street in seattle, wa, my Tesla suddenly engaged the brakes without any apparent reason. The vehicle was not in autopilot or full self-driving mode, and my foot was actively pressing the accelerator pedal at the time. The braking was abrupt and forceful, causing a sudden deceleration. The weather was clear with good lighting. There were no obstacles, vehicles, or road hazards in my path that could have triggered the braking. The unexpected stop was shocking and could have led to a rear-end collision, as there were vehicles behind me.

I was driving the car on supervised self drive and going home around 9 pm. When I reached my house I tapped the break to disengage the fsd and take over so that I can pull into our driveway. After coming to a stop the car all of a sudden just took off on its own at almost 60 mph on a residential street. I was repeatedly pressing on the brake to try to control the car. But it seemed like the car had a mind of its own and was not responding to me at all. Approximately 8 seconds (based on Tesla dashcam recordings) later after travelling a distance of approximately 700 feet the car hit 2 parked cars on the street, which eventually activated the brake and brought the car to a stop at the front yard of a house across the street from where the parked cars were hit.

The fsd is not fully matured and I have tried multiple times and all times and the experience is weird and very hard braking and no control on parking lots. So not sure why this is called fully self driving capability by Tesla? I have paid 12k for this capability in my car and unfortunately it's not fully functional. Two incidents: 1) non-accident: my wife was slowing to make a right turn at a stop light and the steering locked up for about 3 seconds, where she wasn't able to regain manual steering control. We did not have full self-driving mode at the time. Luckily, she was only going about 10 mph, and was able to brake before running into oncoming traffic. 2) accident: our son was driving on a country road, along a section with a slight curve. We had a free trial for full self-driving mode during the previous month, but it ended two weeks before the accident, so he could not have had it in self-driving mode. It appears that he didn't make the curve, but as evidenced by a single long tire skid mark, continued almost perfectly straight, across the oncoming lane, and ran into a redwood tree. The car rolled and ended up upside down, against the bank back on the right side of the road, with the lithium batteries on fire. Our son and his passenger sustained severe injuries and neither has a memory of what caused the accident. Naturally we suspect the possibility that the steering locked up again. But it also seems likely that the aeb and abs were not functioning correctly, as if they were, there should have been several short skid marks for the tires on both sides of the car, rather than only a single long skid mark. Additionally, there's no shoulder on that road, and there is a clump of bushes that come very close to the white line, so it's possible that a steering auto-correct initiated the incident. We have not yet been able to recover edr or log data from the vehicle.

I was performing a 3-point turn on a residential street. While reversing during the turn the automatic emergency braking enabled and slammed on the brakes, bringing the car to a complete halt instantly. This caused my head to hit the headrest with enough force that I have headaches and brain fog the next day, indicating a concussion. I was 10 feet from the nearest parked vehicle when reversing; the aeb kicked in erroneously. This is not the first time that Tesla's aeb has erroneously and dangerously engaged while I was driving.

On November 12, 2023, approximately 3pm, my family and I were traveling eastbound on miramar road, near camino santa fe road, san diego, when we experienced an immediate and sudden braking of our vehicle (2022 Tesla, Model Y), for no apparent reason. There were no warning lights or symptoms that would have exhibited any kind of problem with the vehicle. This sudden braking caused my wife, the driver, to panic and swerve. We were traveling at the posted road speed of about 50mph. Fortunately, there was no traffic and no other vehicles in the vicinity or there might have been an accident. We have not yet reported this incident to Tesla, the police, or anyone else. I am conducting my initial research. Coincidentally, my daughter just experienced a similar incident this morning in her 2017 Tesla, model 3. She will file her own report. After hearing of the problem happening to my daughter I decided we need to file immediate reports and contact Tesla today.

I was driving south bound on hwy90 at about 78mph using navigate on autopilot. Without warning the car swerved into a left turn lane and then slammed on the brakes when the turn lane came to an end. This caused a number of personal belongings to be damaged. Thus happened as I was on my way home from a service center visit that stated the autopilot systems were all functioning correctly. Tesla was notified immediately a d has not responded to my complaints.

The vehicle has suddenly applied automatic emergency braking on numerous occasions when there is no obstruction in the road or potential emergency situation. This often occurs multiple times per trip on longer trips (multiple hours of driving). It seems to happen regardless of whether autopilot is activated or just cruise control. It seems much more common on hot, sunny days when highway mirages are present but it occasionally happens due to some shadow conditions. None of these situations have resulted in compromised safety but that might have been different if there had been a vehicles immediately behind us. The brakes are applied without warning and the vehicle decelerates rapidly.

There was an update last night on my Tesla Model Y. I've noticed two episodes of phantom breaking after this update this morning when I was driving to work. There was a truck in the next lane but the car hit the brakes all of a sudden on the highway when the autopilot was on active. It was at a curve on the highway. I'm assuming the car thought that the truck was moving into my lane leading to sudden activation of brakes. This could have been dangerous. Fortunately there was nobody behind me and we were safe.

Many occurrences of phantom braking have occurred since I purchased the vehicle several months ago. I was driving on I-5 in California with the cruise control set at 70mph, then changed lanes from left lane to right lane with about 5-6 car links from the semi truck to the rear in the right lane. The emergency braking feature engaged with no vehicles in front of me in either lane for at least 500 yards. The semi truck locked up its breaks to avoid rear ending me. I applied the accelerator as quickly as possible and when I did the semi was approximately 3 feet from crashing into the rear of my vehicle.

When I drive a Tesla Model Y under self-driving mode at night inside yellowstone np, there is another car on the opposite side with a high beam turned on, and just near that car there is a bison on the road, the self-driving computer fails to detect the bison in the beginning, and too late to do emergency break, the car hit the bison after passing that car. The bison died but driver is safe, the front right head light part and right repeater camera was missing. It's very hard to see the bison in the dark when the other car turns on the high beam, for both humans and cameras, the self-driving without radar or lidar is not able to detect the obstacle at the blind spot. I want to report this accident as a reference, the car's self-driving software helped me avoid serious injury. The Tesla fsd(full self-driving or autopilot) is great but I still hope there's more sensor such as radar or lidar can be added to the car for better night vision self-driving. The car deemed a total loss and there is a video from the front dash cam record how this accident happen.

When using Tesla's cruise-control, traffic aware cruise control or enhanced auto-pilot the vehicle severely brakes without any reason. This happens every time the vehicle is taken on the highway. It is sometimes reproducible because it happens every single time at specific geographic locations. Most often it is not reproducible. On the highways it has put me in danger more than once. The vehicle would brake while going at 75mph and slow down rapidly all the way down to 45mph. Other vehicles following at safe distances have to veer away quickly or slam on their brakes to avoid an accident. Everyone inside the car experience a whiplash from their seat belts. I opened a service ticket with Tesla. I was told that they ran a remote diagnostics and found no hardware issue in the car. The ticket was closed by Tesla after they pointed out that enhanced auto pilot is a beta version. I gave them very specific examples of how to reproduce this issue. No on came to drive the car before closing the ticket.

My wife and I were on the highway 44 in the Model Y travelling at around 79 miles an hour. There was no other vehicles close to us or in front of us. This was about 1:30 pm and there was no clouds in the sky. The autopilot had been engaged and for no reason, the car started to emergency brake. This was very scary since it was very abrupt and if another vehicle would have been following too close we would have been hit from behind causing damage and or causing death to us both. The vehicle has done this numerous times since we have owned it. It was reported to Tesla and I had a service scheduled to have them look at it. They claimed they looked at the logs and did not see any issues, which I find hard to believe, I asked for the logs and they refused to provide them to me. They claimed I would not understand them, although I explained that I am engineer and I have 25 years of experience with such things. Trust me, I can read a safety log! they claim they will fix with a software update, but when asked for an ecd or eta they refused to give me that information.

While driving between Lincoln nebraska and denver using the driver assist system in my Tesla the car braked for no apparent reason. This occurred 3 times in my 8 hour drive. I would be driving at 80 miles an hour with nothing around on a sunny day and all of a sudden the vehicle would hard brake.

Since I received this awful vehicle it has never properly identified emergency vehicles. Sometimes it confuses neon/led signs with emergency lights and smashes on the brake for emergency vehicles that aren't there. Other times it slams on the brakes on the highway when behind an emergency vehicle and the reflective tape is lit up from my own headlights bouncing on the awfully hard and cheap suspension on this 'performance' vehicle. Within the last week, while driving cross-country on interstate highways at speeds of 65-75 mph, car repeatedly (estimate 10-15 times over 1800 miles) experienced “phantom braking,” automatically engaging the brakes in the absence of any sort of hazard whatsoever (no cars or other objects nearby, fortunately; clear, sunny weather). This occurred when full self-driving was engaged and also when cruise control without fsd was engaged.

Same issue I've been reporting several months. This morning I downloaded yet another software update, got in my car, drove to chick-fil-a on a Sunday because Tesla has not fixed this fatal braking bug on the highway for a non-existent round-a-bout so I use that for the car to navigate to. The car tries to get on the state road instead of the highway so I cancel and take over and drive onto the highway/expressway myself. After a couple miles the fsd finally takes over in a 70mph zone and reduces my speed to 45mph and stays at 45mph while passing several 70mph signs it clearly sees in its own 'Tesla vision' on the map in ar(augmented reality). I cancel and speed back up to 70 before the next overpass where the car puts its hazards on, pulls into the slow lane, says it's executing a round-a-bout. Comes to a complete stop on the highway with hazards while in the travel lane, cars whipping past me on the driver side. Then the car starts yelling at me to grab the wheel which I've never let go of during this entire event so I shake the wheel vigorously left and right and the car does not process that I'm holding the wheel (this is so Tesla can commit fraud when they blame it on the driver and buy more time because NHTSA rules aren't up-to-date on software patches and NHTSA needs to just recall all fsd because Tesla has displayed inability to repair) then to add the cherry on top while the car is stopped on the highway, yelling at me to pay attention while I'm shaking the wheel; I press the door open button and my door opens into the fast lane; allowing the (inattentive driver) to unconsciously fall onto the highway in the fast lane at 0mph while not in drive. I have been reporting these vulnerabilities for months specifically the bluetooth radio vulnerability that was just proven in a hacking competition where Tesla awarded a new car and $350,000 for a vulnerability I was reporting 6 months before. This isn't another case of the auto pilot being engaged and the driver not paying attention. I was driving, in the rain, on a normal road surface, with both hands on the wheel. The car attempted to steer away and out of my lane when there wasn't an obstacle or any other hazard near me. My hand went from the 10 and 2 o'clock positions to the 11 and 3 o'clock position before it straightened back out. Because of this abrupt movement that the car made, the rear end of the car lost traction and spun around on me before the car came to a stop. Fortunately, no one else was hit during this accident. By looking at the dashcam video, the car does show brakes were applied before spinning out of control but this wasn't by my input. I feel that the car made a descision for me that I had no control of. The car was not on auto pilot but yet still steered for me. Accident report indicates no issues with road surface, no speeding, and no citations.

On Saturday, 3/4/2023, I stopped at a supercharger lot to charge my car during a trip. As I entered the lot, I noticed that the road conditions were poor, almost like a gravel road. To be cautious, I drove slowly towards the three empty parking spaces in the middle of the lot, intending to park in the middle spot. I noticed that the road conditions were even worse in front of the parking spot, but it looked wide and not too deep, so I didn't think it warranted serious concern. I carefully maneuvered the car around to prepare for backing up. I've been using the hold braking mode, which allows for one-pedal driving, for more than five months. Just as I lifted my foot off the accelerator, the car jerked forward from a small pothole. I still expected the car to come to a stop, as my foot wasn't touching the pedal. I made a comment to my passenger that I was puzzled why the car was still moving as I looked down at my foot far above the pedals. Suddenly, there was a really loud sound (like revving if I remember correctly) that I had never heard before, which surprised both me and my passenger. We exchanged a puzzled look just as the car suddenly jumped forward at full power and hit the car in front of us before I could do anything to stop it. I believe that this was a software issue related to overcompensation of losing traction control on both sides as the car came to a stop. It's important to note that I did not purchase Tesla's full self driving and was not using any driving assistance features at the time of the accident. In addition, I believe you can request more data from Tesla. I do not have access to the camera recording of interior scene. Luckily, both me and my passenger as well as the two people in the front seats in the other vehicle (also same white Tesla Model Y) were not injured. I can't get Tesla insurance to even investigate the accident. After the accident, I noticed that the Tesla app reported that automatic emergency brake is disabled (only occur).

I was driving my Tesla Model Y on 02-26-2022 with my wife in the passenger seat. We were on our way to pick up our son and his two friends from the movie theater in Lincoln square north, bellevue square, washington. As we entered the Lincoln square parking garage approximately around 6 30pm and turned into the narrow passage leading down to the several floors of parking lots at approximately 5 miles an hour, the Tesla on its own, and to my alarm and surprise suddenly picked up tremendous speed and went careening down the parking driveway and slammed to a stop into the concrete boundary of the pillar on parking level p2. The distance travelled from start to finish was to the best of my very rough guesstimate 60ft to 70 ft. My repeated attempts at slamming the brakes did not stop the car. The airbags deployed. •with the impact the car was completely totaled •the front right wheel broke free and hit the rear fender of a car parked in the parking garage in p2. - no one in the parking garage was impacted my wife was in great pain and very shaken. She was helped out of the car by a good samaritan. Lincoln square security reached us almost immediately. We called 911 and the fire brigade and police came to the accident site within minutes. The police inspected the vehicle and filed a case report. Post the accident, I took my wife to the er. She was badly bruised with the impact of the air bags and seat belt restraint. We continue to get treated to manage the pain and the bruising. The experience was a horrific one and my wife has been badly impacted …it will take time for her to begin driving again. We have not been contacted by Tesla regarding this accident as of March 9, 2023.

The contact owns a 2022 Tesla Model Y. The contact stated that while in autonomous self driving mode and in manual driving mode with the adaptive cruise control engaged with speeds set to 70 mph or higher, the vehicle experienced phantom braking. There were no warning signs illuminated. The failure was self-corrected, but then recurred. The failure had occurred approximately 9 times. The contact had not taken the vehicle to the dealer. The vehicle had not been diagnosed or repaired. The manufacturer had not been made aware of the failure. The approximate failure mileage was 10,977.

Driving both east and west on 429 near the wekiva river results in the car coming to a complete 0mph stop in a 70mph two lane highway with barriers on both sides and no signs or lights. Then the car starts changing lanes on the highway going about 15mph while cars are whipping past me at 70+mph with the horn on. The last 8 or 10 updates have not solved this issue I've been very alarmed and done everything in my power to avoid using the software and have demanded a refund many times, this is simply a deadly scam. The fsd contract is a license to kill and silence your attempts to alert the public like a good samaritan; it must be shut down until proven safe. Each time they say they fixed it, each time I go out and risk my life and they lie, and lie and lie and now the stories of people dying from this exact scenario are starting to pile up (pun intended). The car is still confusing its sea level or altitude positioning on the gps for the round about with stop signs below the sunpass highway. Fact of the matter is I have over 15,000 gb's of footage and there are hundreds if not thousands of scenarios where the car needs to be corrected. I am a professional driver and have software development experience but we cannot allow a private corporation to use human american citizens as lab rats and throw them aside when their software that is not safe yet kills the very people trying to improve the system. Not to mention innocent by standards who have never seen the inside of a Tesla nevermind an arbitration contract. This is all completely unethical. Rented the vehicle from hertz for a holiday road trip of 3100 miles. Experienced multiple instances of "phantom braking" where the cruise control would slam on the brakes with nothing in front of the car within the range of vision. The car would rapidly slow from the posted speed limit to under 45 mph and then about 5-10 seconds later resume as if nothing had happened. This problem happened to me several times per driving day, and seemed to be more prevalent when cresting a rise in the highway. A couple of time times, this happened with a car behind me, and the sudden braking could have caused the vehicle behind to rear-end the Tesla. In heavy traffic it is easy to see how this phantom braking could cause a chain reaction accident. At this point, I think that Tesla needs to permanently disable their cruise control in all Model Y vehicles.
