The contact stated that while driving approximately 10 mph and making a left turn, the forward collision avoidance system activated ad the vehicle stopped independently. The contact stated there was no warning light illuminated. The contact stated that the braking function resumed, and the contact was able to continue driving. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the forward collision avoidance was functioning correctly and there were no failure found with the brakes. The vehicle was not repaired. Additionally, the contact reported that the vehicle had started to shake abnormally while driving at speeds under 40 mph. The contact stated no warning light was illuminated. The contact had not taken the vehicle back to a dealer or mechanic to be diagnosed. The manufacturer was not informed of the failure. The failure mileage was approximately 116,000.

2 Forward Collision Avoidance problem of the 2017 Toyota Corolla

Failure Date: 04/30/2024

The contact owns a 2017 Toyota Corolla. The contact received notification of NHTSA campaign number: 20v024000 (air bags). The contact stated that while driving at 15-20 mph, the forward collision avoidance feature failed to alert her of vehicles ahead. The contact swerved the vehicle, crashing into the bridge wall. The air bags did not deploy. Several unknown warning lights were illuminated. The contact suffered bruising and neck pain, but no medical attention was received. The contact's mother, who was a passenger, suffered body aches and pain. The contact stated that her mother passed away from an unrelated illness a month after the crash. There was no police report filed. The vehicle was able to be driven away from the crash site. Furthermore, the contact stated that the vehicle was equipped with unknown tires, and that the front driver's side tire and spare tire had a blowout four times within a year. The front driver side tire was replaced by a tire retailer, walmart, and an independent mechanic, but they were unable to determine the cause of the tire failure. The contact stated that after the crash, the front driver's side tire was flat, the rearview camera was inoperable, and the front passenger door handle had fractured and detached. The message "no rear visibility" was displayed. The vehicle was towed to the dealer, and the front bumper detached while being towed. The dealer informed the contact that the recall was not related to the failures. The vehicle was not diagnosed or repaired. Failure Date: 01/23/2023

The contact owns a 2022 Toyota Corolla. The contact stated that while driving at 10 mph, he depressed his brake pedal as he approached a red light; however, the vehicle failed to stop and enter abs mode. Due to the failure, the vehicle skid as he both hit the vehicle in front of him and the vehicle to the right of him. The airbags did not deploy upon impact and the forward collision avoidance feature failed to operate as needed. The contact hit his head on the steering wheel and later sought treatment by a chiropractor for pain to his lower back and neck.
