Second generation Toyota Prius (model years 04-09, an estimated 669,700 vehicles, Toyota highlander 06 ~66,200 vehicles (and Lexus models~196,000 vehicles) have a known defective brake power booster known as brake actuator assembly. Toyota published TSB 0032-16. The brakes fail. Toyota denied to remove the known faulty part in 2016 because the part had not established symptoms yet and vehicle was over 175,000 miles. 5/24/2019 while in motion on interstate 77, the yellow anti-lock brake system abs, the electronic variable speed control vsc (traction) , the electronic controlled brake alert lamps illuminate. The hybrid propulsion system fails because the power brakes do not regenerate electricity to charge the hybrid battery fuel cells. Turning the hybrid vehicle into a plain gas powered vehicle with faulty brakes. The power brake system creates a thumping vibration sound due to the pressure inside the actuator is not retaining/holding pressure. I asked local o'reily's to scan vehicle, reading codes brake actuator failure. Reported to Toyota dealership 5/30/2019 with yellow warning lamps engaged and with knowledge of codes. Charged $150. 09 to verify codes that I retrieved in 3 minutes and estimate $2938. 47 to replace known faulty part. I contacted Toyota national -1906030924. Toyota needs to own this recall. Recall the faulty part known to fail in 2016. Toyota aware of part failure, to such extent, Toyota discontinued this brake actuator and redesigned for generation 3 2010-. Toyota has updated the replacement part with new part #s. How many vehicles are currently on highways with this known faulty system, endangering all vehicles in proximity? this is a national issue. A safety issue. Needs immediate attention, please. I purchased new updated part, drove to tampa, florida to have installed by professional installer.

Brake actuator failure. TSB released in 2017. My vehicle was over stated mileage limit (150,000), and was not showing any ill effects of the 2017 Toyota repair at no cost. I am driving along us 77 and on this 4 lane major highway, the power assist brakes fail. Now 204780 miles 5/2019 and the emergency brake, abs,vsc and brake warning lamps illuminate and the power brakes are no longer functioning. Codes c1256 and c1391. The dealer estimate is $2938. 47. Toyota has a TSB from 2017, and without repair, the vehicle is useless. A major part failure, safety recall needs to be issued. This is the most popular most sold hybrid vehicle line, and has a known system failure. Known since 2017. This is an unsafe vehicle with a brake actuator failure.
The brake accuator on the generation 2 Toyota Prius has become famous for failing. Toyota ran an extended warranty, however, dealerships ignored the problem and instead fixed other issues without actually going after the accuator until after the extended warranty expired. Now my car's accuator for the brakes is bad, as indicated by warning lights and alarms. I fear that my 10 year old car will have a brake failure. The repair is estimated at over $2500 for a defective Toyota part. This is unfair that Toyota made a bad part that consistently fails in this model and that they do not fix all of them with a proper recall, or exclude drivers after not relaying important information. I was driving my car down the street when all the lights came on warning me that my brake system was failing so I left the car on the side of the road and will not drive it further.

Brake accumulator failure resulting in not being able to stop properly. Brakes began pulsating and making sounds under passenger seat. Electric pumping sound became more and more prominent over time. Brakes failed and warning lights came on, resulting in inability to stop properly, and hitting the curb in a parking lot. This is obviously a major issue with the Prius. Service manager admitted they see it all the time. When I asked why we were not notified in advance, they said a 'hidden warranty' existed, but you have to wait for the problem to happen. This problem happened twice in our 2005 Prius. Thank goodness we did not have any accidents as a result of the brake failure. Brakes are the most important system in a car, and without them, well, you can't stop. Please work with Toyota to notify owners of this dangerous defect in their Priuses from the year 2004 on.
There was brake actuator failure in my car which due to part failure. Toyota did not issue a recall but instead a warranty enhancement. To me, this seems ridiculous since Toyota did not notify owners and I was not aware of the problem until I experienced the problem and luckily was not in an accident. We misses the cut off for the warranty by a few months. This unexpected expense seems is a huge financial burden to my family for a safety issue I feel Toyota should morally cover. Are they waiting for someone to be injured due to this problem?.
The brakes on my 2015 Prius totally failed and I had to swerve to avoid crashing into the car stopped in front of me. I've been driving for more than 30 years and have never experienced anything like this. It was terrifying. To wit: I was driving about 20 m. P. H. On city streets in long beach, California, in April 2019. As I drove over a bridge, traffic was coming to a stop at a red light 200 yards in front of me. I drove over a narrow metal plate--there are several on the bridge--and hit the brakes, but my car didn't slow. A warning light flashed--a stability control light, I believe, due to the metal plate. I pressed the brakes to the floor as my car rolled downhill on pavement. My car did not slow at all. I looked in the driver side mirror and saw that the lane to my left was clear, so I swerved into it with my foot on the brakes. The car slowly came to a stop well beyond where I should have stopped had my brakes worked properly. The incident was so terrifying that I had to pull over to collect myself. My brakes worked properly after that. A few days later, I took my car to penske Toyota in downey, California, where I bought the car, and told a service agent what happened. I asked him to fix the problem. After service technicians inspected the brakes that day, I was told the brakes appeared to be fine and, barring a recall, nothing could be done. We have a 2008 Prius hybrid. **brake acentuator has failed** when the brake pedal is applied, the vehicle continues to move and then the vehicle suddenly stops (without notice). I took the vehicle to the dealership and was informed of the recall. Appreciate your support. Due to a class action suit against Toyota for this problem, they extented the warranty. Apparantly I fall out of the date specified. This is a problem where thousands of vehicles are on the road with an identified issue. Toyota does not want to assist me in any way. As a veteran with 100% disability, I am requesting for this issue to be reopened immediately, and assist owners correct a extremely dangerous safety problem. I'm constantly hearing a barking/ purging noise coming from the brake accumulator pump whenever I do hard braking. Whenever I hit potholes or rough roads, the car would still rolls during braking as if the abs is getting triggered. I was quoted by Toyota dealer of an estimate of $3000 to replace the brake accumulator pump and booster. I'm afraid that the brakes would fail while driving.

Toyota extended the abs module warranty for a known issue, but we were never notified therefore it expired. Now the abs accumulator has failed and Toyota wants $3-4k for the repairs even though this is a common problem from factory that can cause brakes to fail.

Brakes completely failed and I rear ended a car. I was stopping normally on a city street. According to dashcam I was traveling at about 25mph. The car slowed to about 12mph when the brakes went down to the floor and the car accelerated.

I was exiting exit 3 of interstate 91 in new haven CT and was trying slow down and stop but when I applied the brakes there was no response. I pumped the pedal several times but the brakes still failed. I steered the car to the left to avoid directly colliding with the vehicle in front of me and wound up scraping it and crashing into into a barrier. I have since learned that this has been an ongoing problem with the 2010 Prius and am outraged that the company has been willing to gamble with my safety and that of other Prius owners. I do not have the ability to scan documents but the claim number is allstate 0523873875 and the CT state police incident report is 1800537795.

Brakes failing, getting a c1391 code, leak in accumulator. Toyota has a warranty enhancement for this issue on the 2010 model but it has not extended to 2011 or 2012 models. I contacted local dealer and national representatives and was refused any assistance from Toyota. This issue happened twice over the last two days, both times while vehicle in motion on a city street. Warning lights came on and almost no stopping power.

I was working as a delivery driver lucky for me I just turned in to a gated community and had to stop. Went past gate tried to break and couldn't stop tell I floored the break petal. It took so long to stop if I had been doing 45 mph I'd never stopped in time looks to be the brake actuator failed I see this a lot with the Prius. Something needs to be done. I could have goten seriously hurt and it's a big safety flaw.
Did an oil change to the car on 9/14/18. Driving to key largo Saturday 9/15/18 morning and while driving (almost there) when the abs, anti skid, system failure lights came on. Took it to the Toyota and the service manager had it scanned for the codes. Came back as brake actuator failure and they told me the car was not safe to drive because the brakes failed. Same exact thing that happened to my 2012 Prius which I had to pay to get fixed. This time I was told by the assistant service manager that Toyota seems to be aware there's an issue so they would probably cover the cost of the part $3500 but I'm responsible for the labor which they quoted at $1500!!! something is clearly wrong. I don't think I should have been responsible for the money paid to get the 2012 fixed nor should I have to pay for the 2014 now. My car is at the Toyota until I can come up with the money. . . Sounds like a class action lawsuit !! also, interestingly enough, both my 2012 and 2014 were roughly at the same mileage when this part failed.
The brakes temporarily disengaged sometimes when braking. At around 100,000 miles, the brake accumulator would constantly make noises while driving and after shutting the car off. Eventually the brake accumulator and brake booster failed completely at 117,000 miles, costing around $4,000 to repair.
Tl-the contact owns a 2010 Toyota Prius. The contact stated that while driving another vehicle came in front of the vehicle, the contact quickly depressed the brake pedal, the vehicle started to skid, the contact had to led off the brake to straighten the vehicle to stay in her lane. The contact stated that the anti skid brake failed. The vehicle was not taken to dealer. The contact mention that she had taken the vehicle to an independent mechanic for an oil change before the failure and was informed the vehicle was under a recall. The contact called fred anderson Toyota of asheville, 777 brevard rd, asheville, nc 28806, and informed them of a recall on the brake buster pump assembly, the dealer informed the contact that the vehicle was over the mileage for the repair. The manufacturer was made aware of the failure and provided a case number. The failure mileage was approximately 197,000. Aw.
The brake actuator has failed and Toyota wants $3000 to repair it and I see hundreds of people with the same issue. I only have 137000 on the vehicle.

The abs/brake/skid/warning triangle on. My 2011 Prius iii is 121k mile. This went on/off for a few days then it was on the whole time. The brake seems to loose the hydraulic power brake. I feel like the brakes feel weird whenever hit small bump, brake just failed for a second. I went to Toyota dealership they said it's the actuator that need to be replaced and it's expensive $2-3k. They said it need to be fix soon because brake might fail at any time. There seems to be a warranty enhancement for the 2010 and older but not 2011 and newer. Seems like Toyota still have the same issue with newer model, not fixed yet. Please bring this issue up for safety reason.
Toyota has a known issue with brake actuator failure. There was no notice sent to let owners of these vehicles know that their vehicle could be subject to these failures. My vehicle's instrument panel revealed this issue while driving home from church. Vehicle still operational, but could lose the ability to apply brakes at any time.
I took the 2007 Prius to the mechanic and he new what the problem was right away. The brake actuator was failing and that it would cost over three thousand to fix. He said that you may loose the brakes if it fails totally and advised me to sell the car . Toyota knows about this problem in there 2nd gen Prius. The 2005 Toyota has a known brake actuator problem. Toyota issued a TSB/extended warranty which ended in Dec 2017, but did not notify owners and did not do a recall. It is expensive and only the dealers can do the work. Owners are left with the dilemma of spending about 2/3 the value of the vehicle to keep it safe to use, lose a lot of money by junking the vehicle, or continuing to use the vehicle when the brakes may fail at any time.
Brake actuator failed when car was backing up in a parking lot; car made jerking action and stopped on its own; all warning lights lit; warning to take it for service immediately - which was done after consultation with the dealer (driven there).
I've counted at least 30 brake booster/actuator complaints on this site year to date for the same issue on this model year car. Our issue is the same as everyone else. Just under 150k miles, brake, abs and traction control lights came on my daughter's car (we are original owners) as she was driving home from work. Non responsive brakes, only rear brakes engage if you push the pedal to the floor of the car. Took to dealer who diagnosed dtc c1391, accumulator leak malfunction. Our car was not included in original warranty enhancement (manufacture date of 1/2010), so price quoted was $4814. 66. Our daughter cannot afford, so we called Toyota warranty who offered us $2007 as a measure of good will, but no more. This is a serious safety issue, beyond ridiculous that some cars with the same defect had the warranty enhancement program available which paid for the entire repair, yet most others like us were denied the fix and are facing astronomical repair bills that many cannot afford, nor anticipate on a car such as a Toyota. The brakes failed while my 22 year old daughter was driving - that could have gone so very wrong.

I was backing into a parking space and applied the brakes. The brakes failed and my vehicle climbed the curb, crossed the side walk, and crash into a metal fence on the other side of the side walk. The fence stopped the vehicle. After having the damage to my car repaired and a body shop, I brought it to a mechanic who determined that the car's brake actuator had failed, leading to a potentially dangerous crash.
Serious issue with the Prius. Brake actuator/brake booster fail. The braking is seriously compromised because of this problem. The dealership says it is known to Toyota, but they will not offer any assistance. When the brake/abs/trac lights are reset the brakes function perfectly for a short period of time and then the lights/problem returns.
Brake activator unit failed after 4 months of reporting the issue to Toyota. It follows the conditions indicated on Toyota wen zg1. Vehicle was in motion at highway speed when braking malfunction occurred. Vehicle was controllable and allow for secure stop. Brake response was sluggish and hard on the foot.
In bumper to bumper traffic the brakes failed to stop the car when approaching a traffic light. It bumped the vehicle in front at a low speed without injuries or damage.
My brakes failed 4/3/18 while I was in traffic. I was driving on a city street during rush hour. The car in front of me braked in traffic and I had plenty of brake distance but as my car slowed almost to a complete stop it suddenly felt as if the engine revved and propelled the car forward even though the brake was fully engaged. The only thing that stopped the car was that it ran into the car in front of me causing damage to both cars. I told the two officers that arrived about the problem with my brakes but they ignored me and issued a $165 ticket for "following too closely". The accident became a lawsuit and a rep from my insurance contacted me for details. I also told her about the problem with the brakes and once again was ignored. This past Saturday (4/14/18 about 2 weeks after the first brake failure) I decided to go to the grocery store. After shopping I got into my car, hit the power button and put the car into reverse, my intent being to leave my parking space. I kept my foot on the brake since I saw a car approaching from my right in the parking lot. My car was stationary, but I had my foot on the brake and my car started to move in reverse, even though I had the brake pedal fully depressed. It just drove itself, in reverse, out into oncoming parking lot traffic. This time I was better prepared and since the car was moving from a complete stop, it wasn't going very fast so I pumped the brake pedal and that seemed to "reset" it and the brakes came back. This time I was able to stop the car before colliding with anyone. I have always loved this car but now I no longer feel safe driving it and unfortunately, I cannot afford to buy a new car. The stress alone is causing me to lose sleep and focus throughout my day. Every time I have to get behind the wheel and the worry that I may end up hurting someone else. This is a serious safety hazard.
The brakes failed to work due to a bad brake aviator pump. Thankfully it happened backing out of the driveway at low speed and I was able to stop using the emergency brake.
Brake actuator failing (warning lights on, clunking sound when depressing brake, increased force to apply brakes) two months after extended "service campaign" ended. Had a recall been issued I would have been notified there was a technical problem that needs to be fixed. A known technical issue that affects a vehicle's brakes and anti- locking braking system should result in a recall.
