1 Vehicle Speed Control problem of the 2024 Toyota Prius

Failure Date: 02/10/2026

The way the accelerator pedal sensor is positioned allows for water infiltration from wet carpet / tracked in snow and water. The component is available for inspection upon request. Once sufficiently saturated the sensor fails, and ceases function during vehicle movement. The problem is reproducible but has not been confirmed as a pattern of failure by the dealership. As far as my recollection, the manufacturer hasn’t inspected the part. P2138 is the associated check engine code and the vehicle enters limp mode during travel. The check engine light (and subsequent problems) first appeared while in traffic on 2/10/2026.

Entering the highway the car suddenly learched and stopped excelerating, creating an unsafe situation, and went into a slow mode. It limped along. Caused cars issue around having to slow merge and driving only 10-15 miles per hour. The dealer could find no codes and said it could be an air bubble in fluid line??? no warning lights.

On [xxx] at [xxx] I was backing out of a driveway when my 2016 Prius v5 accelerated on its own. When I applied the brakes, nothing happened. I hit a curb, which flattened the front passenger tire. In order to get the car to stop, I took it out of gear and put it in neutral. The car finally came to a stop. My speed while backing out of the driveway was less than 5 mph. It accelerated to about 15 mph. I was told that because the tire flattened due to the force of impact with the curb, Toyota considered it an accident. Toyota had me complete an "unintended acceleration" form so they could investigate the situation. A case number ([xxx]) was assigned. I was also told that I needed to contact Toyota's brand engagement center at (800) 331-4331. Toyota assigned engineering analysis associates (eaa) to perform a thorough inspection and read out the event data recorder (edr) of my vehicle at findlay Toyota. Anne chapman with eaa did the inspection and sent a copy of the report to me on 09/15/25, the day of the inspection. She suggested that I keep the copy because I may need it if I needed to contact an attorney. My insurance company, usaa, sent an adjuster to examine the extent of the damage. Findlay collision center completed the repairs on 09/26/25, which cost $3,433. 92. The accelerator nor the brakes were addressed. On 10/09/25, Toyota provided a response to the incident: they were not at fault. They claimed operator error. Unintended acceleration and unresponsive brakes have been an issue with Toyota's in the past. At that time, instead of fixing the problem, they told dealerships to educate their buyers on how to take the car out of gear. Approximately one week after picking up my car from the repair shop, the vehicle accelerated on its own again. Fortunately, the brakes worked this time. My name is [xxx] ,I am filing this report on behalf of my daughter [xxx] the driver of the vehicle that involved with the unintended acceleration on the 2009 Toyota Prius . On a good clear morning on [xxx] . My daughter drove the 2009 Toyota Prius to work- when she enter to the parking lot at the speed round 5 mile/hour . Just when she was making a left turn on to the entrance gate with her left foot on the brake pedal . On a sudden the vehicle rpm unintended rev up too high and the vehicle had an unintended accelerating to move forward aggressively causing lose control of the steering . Also she could not brake even though her left foot was on the brake pedal and pressing hard to try to stop the vehicle . . This dangerous incident causing her Prius to bumped into the electrical gate panel control box . The accident caused some damages to her Prius and to the entrance gate control box though no body was hurt . But it got us through the terrible experience because it could kill some one for sure . My daughter been driving this commute car daily in the past 5 years and she has the skill of 20 year driving with good record . This Prius of her still have all oem equipments with no add- on parts-,no modifying part on any systems what so ever- and no extra floor mat cause we been warned before about the possible of unintended acceleration causing by aftermarket floor mat to any vehicles. . We feel unsafe after the mystery accelerating problem on Toyota vehicle that could take people lives . So the mystery unintended accelerating on car is real . I am currently checked and find out her Prius has 2 incomplete recalls (un-notice to us ) but none of them related to the unintended acceleration problem . We would like the NHTSA to take this problem seriously to save people life . I will included photos of this incidents and willing to provide all information if needed to address this problem, again to save lives . Cruise control not working. Traction control light active, brake light active, and light active the cause was diagnosed by Toyota of paris. TSB for c1391 code abnormal leak in accumulator. The corrective action is to replace brake booster assembly with master cylinder and brake booster pump assembly.

Car was driving fine one day and then while driving down the interstate I had the pre collision system malfunction error, brake light came on, cruise will not operate, abs light is on. After much research it seems Prius v has a common issue with brake booster faulting.

As I was pulling my car into a parking space with the brake applied lightly, the car suddenly lost braking and lurched forward when the front wheels touched the curb. This caused the front of the car to jump the curb and hit a tree a foot or two away. My foot was on the brake during this and the car did not brake automatically, or engage any other sort of safety measure. This was terrifying and I could have easily hit a pedestrian or injured myself and the passengers of my vehicle on impact. I completely lost control of the vehicle's braking due to this issue. Since the incident happened today, my vehicle has not yet been inspected. No warning lamps of any kind had been activated. The car was nearly brand new. Upon further investigation, there are numerous reports of similar events happening to Toyota hybrids over the years. A simple google search for this turns up many reports. These reports center around sudden loss of braking from the regenerative braking system and a "lag" before conventional braking is applied that causes the forward lurch due to the momentum of the car. Many others on online forums seem to also suggest running into this issue specifically when the wheels hit a bump. This sounds exactly like what happened to my car, causing it to lose control despite my having the brake applied. It is insane that such a problem has existed for years in the braking system without being fixed by Toyota.

When I am coming to a stop or need to put on my brakes my car will accelerate at a jump ahead pace as if to take off instead of slowing down and stopping. Regardless whether I’m turning a corner or I’m continuing straight when I have to put on the brakes it speeds up instead.

9 Vehicle Speed Control problem of the 2022 Toyota Prius

Failure Date: 01/14/2025

The contact owned a 2022 Toyota Prius. The contact stated that while idled forward into a parking space at the local car wash, the brake pedal was depressed to stop but the vehicle erroneously accelerated forward and crashed into a brick wall where it came to a stop. During the crash, the air bags did deploy and no injuries were reported. The vehicle was destroyed and later towed to an undisclosed location. No police report was filed. The cause of the failure was not determined. The manufacturer was notified of the failure. The local dealer was not contacted. The failure mileage was 25,000.

10 Vehicle Speed Control problem of the 2006 Toyota Prius

The light up display for the odometer, fuel gauge, and speedometer does not always light up. When it is not lit, the cruise control and any warning and informative alarms/lights also do not work. This is a well-known issue among owners of these cars, related to the capacitors on the circuit board for the display being defective, and there is extensive documentation of it online. It may also have had a technical service bulletin released for the issue, but I'm not entirely sure. The light goes out intermittently and is not always consistently out. I did mention it to the service personnel when I took it in to get recalls repaired recently, and they didn't seem surprised or unaware of the issue. I did not ask them to diagnose or fix it, as I know I can get it done for a better deal elsewhere.

11 Vehicle Speed Control problem of the 2018 Toyota Prius

Failure Date: 10/31/2024

The contact owns a 2018 Toyota Prius. The contact stated that while in bumper-to-bumper traffic, the vehicle suddenly accelerated. The brake pedal was applied but failed to respond. The driver crashed into the front passenger side of the Lexus. The vehicle continuously moved forward. The contact was eventually able to pull over safely. The pre-collision malfunction warning light illuminated. The brake malfunction light was flashing. The air bags did not deploy. There was no reported fire or injury. A police report was filed. The vehicle was taken to the local dealer, who could not determine the failure. The vehicle was not diagnosed or repaired. The vehicle was then towed to an autobody center where it remained. The manufacturer was contacted, but no additional assistance was provided. The approximate failure mileage was 39,000.

Car was diagnosed at a Toyota dealership with a brake booster issue reading code 20te07 after my vehicles brakes suddenly would not work and the car continued to jolt forward. The brakes wouldn’t stop the car. According to Toyota, my vehicle is three months outside of the warranty period but under the covered mileage so we are left to repair this defect ourselves. This was a very dangerous situation and could have cost us and our children our lives instead of just the $4800 repair cost. This has happened to both of our Prius’s so this is not a rare issue. Toyota needs to have a plan for these parts failing suddenly and they should all be recalled for safety in my opinion. The experience was very scary and we had zero control over the vehicle when the brakes suddenly went out. Where do I join the class action lawsuit?.

Heading westbound on I-70, taking exit 29 off ramp from highway, I drove over uneven pavement. My traction control, abs and brake lights illuminated, the brakes suddenly felt mushy. As I approached the stoplight at the end of the exit ramp, I questioned whether I would be able to stop. Scared me immensely. Luckily no cars were near me. I now do not have abs, traction control, stability control or regenerative braking.

14 Vehicle Speed Control problem of the 2021 Toyota Prius

Failure Date: 09/14/2024

The contact owns a 2021 Toyota Prius. The contact stated while driving approximately 35 - 45 mph, the vehicle independently accelerated. The brake pedal was depressed but failed to stop the vehicle immediately. The contact swerved around the vehicle in front and ended up on the opposite side of the roadway. The contact attempted to avoid oncoming traffic and swerving around all the other vehicles and ended up in a ravine. The contact's husband depressed the start/stop button from the passenger’s side to turn off the vehicle, enabling the vehicle to come to a stop. The contact restarted the vehicle and continued to drive the vehicle to the dealer at slow speeds. The dealer was unable to determine the cause of the failure. The vehicle was not repaired. The manufacturer was made aware of the failure by the dealer. The failure mileage was 56,000.

I was driving the vehicle around 55 mph when a check hybrid system warning popped up. About 30 seconds later the vehicle went from being in drive to b (brake mode) and the vehicle began to slow at a rapid pace with no warning. Could potentially cause an accident as this occurred with very little warning.

* I was pulling into a parking space at a slow speed. When I applied the brake the car accelerated causing the car to scrape the car next to the space. The car then stopped. * the acceleration was out of my control and the brakes did not stop it. Fortunately there was no person in front of me. * the tow truck operator who came to get the car experienced the same issue when pulling into the bay at the dealer. I do not know about the manufacturer during their inspection as they did not provide me with that information. * Toyota had an inspection performed and that report was submitted to Toyota on August 20. We just received notification from Toyota that they found no problem with the car. * no warning lights noted.

My car accelerated significantly on its own while I was pulling into a parking stall in a private parking lot. It caused my vehicle to jump a curb and accelerate up a dirt divider and push over a bush that was planted in the dirt divider. Depressing my brakes did not decelerate the car, so I pressed the park button to eventually stop the vehicle. I’m not sure what speed it was traveling when it accelerated, but I could smell burning rubber at the end of the incident. I’m not sure when the smell first occurred.

See all over the internet that 2010 Prius have a recall for inverter. I called the dealer they said no recall with my VIN. I was told that a new inver is almost $5000 dollars. I need help I have bought 3 Toyota already. Please help me with good will or warrenty the inverter like you have so many other people with 2010 Prius. The 2013 pruis has developed the notorious brake booster fault triggering the brake light abs light and traction control light to stay illuminated. Toyota has many complaints about this brake booster issue and needs to recall and fix this issue as they have done on other Toyotas. This is leaving owners with a $2500 repair or having to take a big risk driving these cars with a big prevalent brake issue that Toyota needs to be held accountable for.

Total malfunction at highway speeds. Very unsafe, the car completely shut off. This may have resulted in a highway fatality due to collision as the vehicle was in the fast lane. Ntsa please do your due diligence and investigate the faulty, defective hybrid system as I believe the faulty inverter recall is much more larger than the original recall.

Took vehicle for state inspection on 2/13/2024, failed inspection because of brake warning lights. Slip indicator, electronically controlled brake warning light, brake warning light, anti-lock brake system, and low tire pressure lights are all on. I have looked into it, and it seems the brake actuator/booster used to be under warranty, however since this is a 2013 in 2024 with almost 210k miles on it, it no longer qualifies for the original recall.

The conditions listed in Toyota customer support program zjb for the brake booster and master cylinder occurred after the 10 year in service date on my vehicle. Brake, abs, and traction control warning lights turn on intermittently and when this happens brake response is poor and cruise control shuts off. When applying the brakes an audible tat tat tat sound can be heard from the front of the car that sounds like the abs being triggered. You should have Toyota recall impacted cars and fix the issue as outlined in the TSB and support campaign.

On 10/16/23 I was turning slowly into a parking spot and my car suddenly accelerated without my pressing the accelerator. I slammed on the break and the car stopped. It was frightening because I could have hit someone or something but luckily the car stopped before that happened. I believe that this was a malfunction of the accelerator. This has not been checked or reported to anyone yet but I will be calling Toyota to have it checked. There have been no warning messages.

I waited about 20 minutes and I turned the car back on.

I entered a large, strip mall parking area and was driving at the vastly reduced speed one would use in such a situation. My intention was to park my car to enter a store in the mall. I turned a corner to park on the side of the building (parking options were limited). As I entered a parking space (moving at the speed one would drive to do so; essentially incredibly slowly; moving forward into the space at almost 0 mph and already applying the brake). Suddenly and with no warning, the car violently lurched forward at a vastly increased speed. I pressed down on the brake harder to try to stop the vehicle, but it propelled into and over the concrete curb stop and into the thick brick wall in front of me. There is considerable damage to the front end of my vehicle as a result. I was horrified at this dangerous situation. My safety was definitely at risk. I feel very fortunate that I wasn't seriously injured (and that no one, either a pedestrian or another driver, was involved in the accident). The airbags didn't deploy and I was of course, wearing my safety belt, which kept me from any severe injury. I do feel some physical injury effects this morning however (sore neck, shoulder, back; a slight headache). I'm not aware of any warning lamps, messages, etc. Going off, but I must say everything happened very quickly. I've reported the accident to my insurance company & filed a claim. Given that the incident happened on the evening before the July 4th holiday, my insurance company scheduled vehicle towing (flatbed) for my car. It's my understanding that it's being stored at the tow company location & will be delivered tomorrow (Wednesday, 7/5/23) to the certified body repair facility recommended by my insurance company and chosen by me. An insurance adjuster will be doing their evaluation, likely in the next few days, at the earliest tomorrow morning. I've also advised the insurance company of these details & told them of my safety concerns and fears.

Abs unit failure (may also be called abs control module actuator and pump assembly). Three mechanics, including a Toyota dealership, is unable to reset the system to correct. The pcs system cannot be disabled which, in turn, means cruise control is no longer operable.

The contact owns a 2006 Toyota Prius. The contact stated that while driving approximately 35 mph the steering wheel erroneously steered to the left and then to the right causing the contact to loose control of the vehicle and crash into a curb. The vehicle then suddenly experienced unintended acceleration and drove across two lanes and into oncoming traffic and collided head on into a van. During the crash the contact sustained multiple fractures of the spine, pelvis and ribs and was transported to the hospital. The vehicle was destroyed and towed away. No police report was available. The cause of the failure was not determined. The manufacturer and local dealer were not notified of the incident. The contact indicated that the vehicle had also experienced a similar failure previously. The VIN was not available. The failure mileage was 77,000.

I was driving in my neighboorhood and as I went up the wall bridge my acceleration stalled and my abs light, check engine light, and check hybrid system light came on simultaneously and started beeping. At the same time, I lost my braking ability completely. I had to swerve off to the grass in the hopes of not hitting turning traffic at the stop sign. Luckily, it stopped before doing any major damage to anything. Thankfully, I was close to home and was able to put it in my driveway safely. The check engine light then cut itself off, but my brakes remained completely nonfunctional. If this had happened on the highway or a busy road I'm sure it would’ve been much worse.
