36 Vehicle Speed Control problem of the 2016 Toyota Prius

On September 2022 (at 46k) dealer did TSB related fix on my 2016 Prius. Pfa for service record. They replaced exhaust front pipe, which is suggested work from Toyota pfa for Toyota technical bulletin. T-sb-0135-19 at 51,747 on March 1st 2023 around 8 40 am, while we are driving on highway at 70 mph suddenly check engine light, coolant light, hybrid related lights were on vehicle lost the control, speed was dropping luckily we could safely stop the vehicle on the shoulder no vehicle hit us from behind. Sudden stop of engine while driving on highway at 51k miles is safety issue. Toyota dealer could not provide additional details except engine is over heating.

The abs, vsc/traction control, and brake system warning lights all came on at the same time in my 2010 Toyota Prius. I almost lost control of my car's braking ability. Afterwards, the hybrid system does not perform its regenerative braking/recharging any longer. And the cruise control mode doesn't work also. The dtc from the obd2 port is c1391 - abnormal leak in the brake fluid accumulator. I found out that this issue is widespread among gen 3 Priuses, but Toyota has not made this officially a recall campaign, but classifies it as a "customer support bulletin. " but the issue is dangerous enough that it should be a recall item. I moved homes in fall of 2018, and Toyota says it mailed out a letter about this brake issue and resulting campaign, but I never received the letter. I was totally unaware that this was a safety issue, that Toyota acknowledged it, and that this safety issue with an ongoing "recall" campaign with an expiration date. Toyota denies that this is a recall issue, and will not provide this service. I would like for NHTSA to reopen and investigate this issue, and make Toyota issue a general recall campaign to resolve this dangerous safety problem with the car.

I am currently having this issue with combination meter, which means the Prius dash board will not show any speed, fuel, p, r, d label and many more important information is not shown on display. I had complained to Toyota, case # 221219001515 on 19dec2022, I have spoken to danial n and manager alex s from Toyota. They both have denied solving this issue from Toyota as the warranty extension ztv has expired. Even I had provided them proof that Toyota knew about this problem, please see doc#pol-12-05 from Toyota regarding this. My Prius has 258000 miles, and it is 2008, it is high mileage as per their documents this will accrue in high mileage Prius with cold weather. I live in NJ, and it is getting cold and my car as high miles. Toyota refuse to take care of the problem and I as a consumer will have to pay over $1200 to get it fix as this is only done at Toyota dealership. Currently I am thinking of getting it fix at paul miller Toyota of west caldwell, 1155 bloomfield Ave, west caldwell, NJ 07006, 973-287-4050. What's sad is Toyota used to rank at the top of best customer service. I'm guessing most of the problems are just after the 9 years, calculated to guarantee fixing the problem to as few people as possible. There's no extension and Toyota won't help. I tried and was told all deals expired and to help one would mean they'd have to help all. We're on our own. Hoping that Toyota has released a solution to the dangerous and consumer related problem related to the failing combination meter. The previous ext. Warranty was only good for 9 years from date of first use which did not help many drivers. Super negligent and ripe situation for class action lawsuits not to mention many potentially fatal accidents on the roads. Toyota has denied fixing this problem even Toyota knows this is on going problem. As a government official please try to help me resolve this issue as this is a major concern for many millions of drivers which Toyota need to resolve this. Please.

Instrument panel cluster failure. Panel goes black while driving and lose power. Vehicle won’t start or shut off. My vehicle was getting serviced at the dealership and they couldn’t start the vehicle. I was told the instrument panel cluster was the problem. I contacted Toyota customer service, and was told they were aware of the issue. They had issued an extended warranty and sent out a letter to all owners this warranty has expired in 2016. If you purchased your vehicle after 2016 you would not have received any notice regarding this. While driving you can’t see you speed, your gear selection, fuel level, lights and no warning lights. The warning is that your vehicle won’t start or shut off.

The inverter water pump failed while I was driving 50mph, it caused the car to stop working and the brakes malfunctioned as well. When I tried to hit the brakes I had to hit them a lot harder and it still acted like it wasn’t going to stop. I was lucky enough that the light up ahead was still green and the cars in front of me started going forward.

2012 Priusv has intermittent vsc and traction control when cornering left or right at slow speeds, as less than about 20mph, 5-20. This is happening on dry road conditions at unpredictable times. The vehicle may deceleration and have a lungeing behavior with traction and or bracking interrupted momentarily. On accelerating from stop sign turning left or right the vehicle may pause accelerating briefly and pull with the abs brake auto/ intermittently applied. The traction control light may or may not illuminate at the short time interval of these events, but traction control light does not stay illuminated after the event is over. No error code is read from the vehicle with error code reader. These events are very startling and cause sometimes cause the vehicle to speed up or slow down abruptly. These events are happening at low speeds, however with the car surging forward or abruptly slowing on corning and lane changes there is potential issues of keeping the vehicle under proper control at these times. I believe Toyota should correct this problem so that it never occurs. Toyota to find a solution an implement a recall to update vehicle.

I was driving my 2010 Toyota Prius and my car started beeping when I was driving when I tried to slow down the car was not able to stop the breaking system abs started displaying and my car started to beep and it won’t stop beeping. I did take my car to the dealer for a recalled inspection after my car broke down and I was no longer able to drive for my own safety. After I took the car to downey Toyota for hybrid recall they didn’t tell me anything but on the document they gave me it did state that the abs was a problem. Now my car is still not able to be used because of my own safety concerns since the vehicle won’t break when I put the brake pedal and a none stop beeping sound that won’t stop at all.

When the battery cooling fan is running, the car is slow to accelerate or will not accelerate at all. The incident has happened multiple times over the past year when the temperatures are above 90. If at a complete stop, the car will not accelerate, it will only jerk forward. The car has to be turned off and restarted to be able to accelerate. If braking or reducing speed, the car will be sluggish, and I have not been able to get the speed over 40 mph. I have taken it to the dealership, but they said they could not recreate the issue, though I do not believe that they followed the directions I gave them to recreate the problem. No warning lights appear when the issue occurs, so they did not have an answer. The first time the incident occurred, I had just had the scheduled maintenance completed two days earlier and they did not find anything in the inspection that needed to be repaired. The first time the incident occurred, I was at a stop light trying to make a right turn. The incident has occurred twice when trying to pull on to the highway and accelerate up to speed.

I had just changed from the right-hand lane to the center lane of a six-lane expressway when my vehicle lost power and I was able to move over to the right hand shoulder of the road. The accelerator pedal did not work and I just coasted to the side of the road. I noticed that a red warning saying something about radar flashed on the instrument panel when this happened. I turned off the vehicle and tried to restart it, but nothing happened. I then texted the person who was expecting me and explained the problem. Since I had just taken the car to the dealer for its regular 40,000 mile service four days earlier, I intended to call my dealer to ask them what to do. All of this took about 10 minutes with the engine turned off. Before doing this, I again tried to restart it. It started and I inched it forward on the shoulder of the road. Since I did not feel safe parked on the shoulder, I merged into traffic and took it to another Toyota dealer. They said that they could not look at my vehicle to diagnose what might be wrong because all their technicians were going home at 4 pm. I drove to my son's house and he suggested that I report this to you. Something similar happened to him when he was driving this same vehicle about six month ago. He was driving on a city street, did not notice any dashboard warning, and took it to this same dealer. They were not able find anything wrong. I felt I should report this since this is the second time this vehicle suddenly lost power. My incident happened when I was driving at expressway speed and could have caused a serious accident had I not been able to coast to the shoulder of the road.

I need your help to get Toyota to fix 2 items: -abs accumulator: I understand Toyota at one time had a recall on the ‘abs accumulator’, that may have expired. At that time, I did not have any issue, nor was I aware of the recall. Now the ‘abs accumulator’ has gone out and is really hurting the braking. I had to get new brakes & they get locked because of the abs malfunction. It is an expensive item – more than the cost of the car. I intend to keep the car for a long time, if Toyota can help me get the abs accumulator fixed? -mfd: the multi-functional display (mfd) in the middle of the dashboard, has gone blank. It used to just partially blank out before, but now the whole screen is blank all the time, making it very hazardous to drive. Obviously, there is nothing I have done & it is a manufacturing defect, because it should last the life of the car. Please have Toyota fix the 2 problems (abs & mfd). All requests to Toyota have gone unfulfilled. Your help is needed & will be greatly appreciated.

My 2013 Prius' hybrid system failed as I was entering a very busy state highway (where several fatalities have been recorded) exit when a tractor trailer truck unexpectedly bore down on me. My engine reduced to half speed and there was no breakdown lane to pull over to. The dashboard lights lit up and indicated a hybrid system failure, but I could not get to safety for another 2-3 miles. This is an NHTSA recall (20v-369) that was eligible for repair under an expanded warranty, but only after the vehicle has experienced the shutdown. The ipm (intelligent power module/hybrid) system breakdown was confirmed by Toyota dealership's diagnostics, but the warranty period was not told to me and the charges amounted to almost $3000. My research and doggedness discovered that the costs would be covered in toto. But more importantly, I am among numerous Prius owners who experienced this problem and more could occur, risking injury if not death.

My wife hit a deer at highway speed. The airbags deployed. The drivers side lower airbag deployed in the footwell. The driver was unable to get to the break paddle to stop due to the airbag at het feet. She had to use the emergency brake to stop the vehicle. That worked, but she had limited vision of the road ahead and it took some time for her to think of using the emergency brake.

Brake failure / unintended acceleration / unknown cause driving at approx 25mph on urban street (road in good condition no potholes, ramps, etc) I began to slowly brake as I approached a red traffic light. Instead of braking, the car began to accelerate quickly. I pressed the brake pedal again - no response. Pressed down hard for a third time which executed an emergency stop. This all happened within 1 or 2 secs. There were no injuries or involvement of other cars on this occasion. However, had I been any closer to the car in front, I believe I would have rear-ended it. Had this occurred a few moments later, at the busy junction 400m ahead, it could have been fatal. I am absolutely sure it was not driver error. I was not distracted, drive regularly, and have been a confident and safe driver for over 30yrs. I have never experienced this in a car before, and in fact wasn't aware that it could happen until I researched it on this site. No warning lights or indicators came on. The car ran normally for the remaining 2 miles back home. I didn't think anything of it before, but prior to this incident (in our 7 yrs ownership) there were approx. 5 instances when the engine would "roar" as if it wanted to accelerate but didn't when I was breaking or slowing down on a highway. Also, there were perhaps 3 instances when the car felt as though it was "running away" I. E. A slight acceleration - for no obvious reason. As neither of these represented safety issues, I assumed the car was just a bit old and these issues would be addressed as part of any service. Now I wonder if they are related to the sudden acceleration. The car is currently having a "health checks" run, but nothing has been found. I am reluctant to drive this vehicle if neither my dealer or Toyota can discern the problem. Overall we have been happy with this car and its performance, but this is an unacceptable "anomaly" as far as safety is concerned. Vehicle available for inspection.

I went to park in a parallel space when all of a sudden my abs, (!), vsc light come on with the loss of brake power and a loud beeping noise. With this sudden loss of brakes in the middle of driving, I could have harmed myself and others in any cars around me because I also lost steering capability. This is a problem that there was an extended warranty for, but there should be a recall. My car is a 2009 and was only covered before the pandemic and now it is 2021 and I have no recourse, but to try and fork over $3500. 00 for which I don't have. I am a second owner and this is a very common problem amongst Prius owners. I didn't have any other symptoms that I could point out prior to this situation. I have had my hybrid battery, engine, ecu and most recently a stolen catalytic converter replaced. This abs actuator problem needs to be addressed for another 5 years with a free repair. I would hate to have hit some other car and had my car smash into a wall, another car, pedestrian, pole or any other object based on it's ability not to stop.

For the second time (first in 2019) I was seconds and maybe two feet from stopping the car to park when, with my foot on the brake, the car suddenly and without warning lunged forward at what seemed like full throttle. The first "crash" was in my driveway. The force was shocking and took off the front of the car. I insisted Toyota investigate and they ultimately denied any liability. After the car was allegedly repaired, I started noticing another phenomenon: every time I would slow down to brake--approaching an intersection or stop sign, etc. --the car would briefly lunge forward before the brakes engaged to stop the car. I reported this to the dealer 2-3 times and each time I was assured that this was "normal" for a hybrid transitioning from gas to electric at a lower speed. Two weeks ago, with a friend in the car, I was pulling into a parking space, foot on the brake and about to stop, when the same thing occurred: without warning, the car lunged forward, at what seemed like full throttle, and crashed into a low concrete wall. Had it been seconds later, we would have had our seatbelts off and gone into the windshield. There were injuries. Fyi, I have a pretty much perfect driving record and log thousands of miles each year. I had zero problems with the Prius I owned prior to this one. I will never drive this car again.

My steering wheel controls, including odometer, cruise control, horn, and airbag light is on. I took it to the dealership and they said it is the spring (clocking) cable in the steering wheel. Looking up a recall there are quite a few years and models where this exact issue has been recalled on Toyotas, but the dealership says my 2016 Toyota Prius is not cover (same exact issue). I submitted a ticket through Toyota customer service but they have done nothing about this safety issue for the 2016 Toyota Prius. I see lots of blogs and complaints because the airbag default warning light stays lit, so I assume airbag is not functional. I found a site that says to put the complaint through here.

Break actuator is failing in my 2011 Toyota Prius. The car now has a unpredictable abs breaking system. Sometimes the breaks work and sometimes they quickly fail and a error light comes on. The light than goes out as if nothing happened and all is fine. This is a very dangerous problem that will lead to many accidents because the vehicle tells you its good to go, mean while the abs system is failing to stop the vehicle intermittently. I am scared because I dont know if the car will do this on a turn then lose control injuring myself and others.

A check hybrid system alert came on and included lit break, check engine, steering, abs, skid and yellow triangle with an ! mark inside. The car stopped and I could not move it. Good thing I was on a side street with light traffic. This happened again after the inverter was replace; and it happened again after the 12 v battery was replaced.

I was driving on the highway the other day and my 2013 Prius and all of a sudden I felt a pop and I had no gas by the grace of god I was able to pull over and not get killed I called for roadside assistance I did have power I did have steering but there was no gas upon checking at my Toyota dealer they said the piston had gone straight through the engine and I needed a new engine and that it was probably due to abuse of oil and lack of oil changes I googled and saw that this was a known problem and I'm actually a doctor and have seven children and I did change my oil and I had no warning light.

Brake light, abs,slip indicator,brake system warnig light system all came on while driving at high velocity [75m/h] in the rain and lost 75% of the braking power plus the car started to slide. Needless to say this was a catastrophic failure of the entire brake system on a car that has a customer support program notification from Toyota[zjb] that was sent to me with a warning of such occurrence. This is down right irresponsible to a major car manufacturer to allow these vehicles on the road without testing the entire system on each of these affected vehicles and with for a catastrophic failure to occur and cause public harm or even death. I had just taken the vehicle for service a few weeks prior where I explained the service advisor of an issue with brake pulsation. However, the entire brake system was not throughly checked and was just told that its the front rotors that are uneven, thank god I did not fully loose control of the vehicle and cause a catastrophic accident as it would have resulted in bodily harm to my family. I expect this federal government agency to use it's authority to prevent such occurrence to the tax paying citizens of this nation.

Check hybrid system light ignites, slows down vehicle in middle of high speed highway. First incident November of 2020 then several incidents after. Repair shops don't assure problem according to error codes & they recommend possibilities they cannot assure will resolve the problem. Car was taken into local dealership after receiving recall notice for software and airbags prior to November 2020 when the issue began. Software update took a couple of hours. Restart/reboot temporarily allowed driving again. Most recently the system entirely shut down & the car will not start or run. Abandoned car on side of the road and got heat exhaustion from walking the rest of the way home and had vehicle towed. Error lights come on including "check hybrid system" says "shift to p position when starting" and will not allow start.

I've noticed a few times while driving that my Prius started accelerating when my foot was off the pedal. It hasn't caused an accident or anything too scary yet but it is disconcerting. I noticed others had complained about this and wanted to add that I seem to be experiencing the issue too. It's possible it's just my vehicle specifically; I've not had time to take it to a mechanic yet.

Driving speed less than 25mph on familiar surface street with need to stop for a car about 75 feet (estimate) in front; should have been an easy stop; applied brakes but car accelerated (I think only electric motor); further brake pressure did not stop the car, continued to accelerate (estimate 30 mph); fender bender crash of driver side front fender into passenger rear fender and side bumper. Very familiar with car (19 years driving it) and very familiar street near my home; confusion was not a factor; no airbags on either car activated; I was able to stop when I was beyond the struck vehicle. I plan on not driving car further and have asked Toyota dealer to take it back for junk without a definite reply so far.

The car gave a triangle of death and looked it up if any recalls had been made that could possible cause this issue and there was 1 that called for an update to the system which I had done and they had told me if it didn't take they would have to do the invertor which was apart of the recall. A week after the system update the same issue arose and they refused to replace the invertor which drained the hybrid battery and ruined it also . This effects the driving ability and causes the car to shut down the lights to not work . It can stale at any given time and will not have power to continue.

Toyota Prius v 2012 lost power as I was driving out of the parking lot. I had to make emergency stop on the side of the road because inverter/electric motor failed. Mechanic ran computer test and found that problem codes indeed pointed to failure of inverter. I called local dealer mccord Toyota and was told my car does not fall to extended warranty of inverter and did not offer any help. Same day I called Toyota customer care at 1-800-331-4331 and they told me that my car does not fall under inverter warranty based on my VIN number, and gave me case number 2005072419. I was able to call my insurance company to arrange towing from my location to repair shop. Repair shop completed work and vehicle is now in operable condition. I feel lucky that I was not on freeway and did not have my children with me when it happened.

If this defect occurs, various warning lamps on the instrument panel made illuminate, I experience increased stopping distance, reduced hydraulic pressure within your brake system, the deactivation of brake assist, and/or the deactivation of vehicle stability control. Toyota has previously issued at least two safety recalls � d0h and k0l � designed to prevent similar dangerous occurrences. However, hundreds of thousands of generation iii Prius vehicles (2010-2015 model years) were excluded from those safety recalls.

Tl-the contact owns a 2011 Toyota Prius. The contact stated that while driving all the lights on dashboard came on and the vehicle would not stop. The contact had to stand on the brake to the floor in order for the vehicle. To finally stop. The vehicle had a NHTSA recall number: 18v-684000 (hybrid propulsion system) and contact took the vehicle to the dealer marietta Toyota 750 cobb pkwy s, marietta, GA 30060 (470) 938-8430. The contact stated that the vehicle had a salvaged vehicle. The dealer did not complete the repairs before the issue occured. The contact thought that the brake booster system was not included in the recall according to the dealer but the contact thought it should be. The manufacturer stated that they would fix the hybrid soft ware but not fix the brake booster system. The failure milage was 101,000. Tf.
