As I drive about 70 mph, the abs and the brake lights come on and the car is momentarily out of control. As I grabbed the tearing and reduce the speed dramatically, the system correct itself and I am able to continue driving at a very low speed.

The contact owns a 2022 Toyota Rav4. The contact stated that on several occasions while driving at undisclosed speeds and accelerating, the transmission momentarily shifted into 2nd gear and then into neutral(n), causing the vehicle to start coasting. The contact released and then depressed the accelerator pedal and instantaneously and the vehicle accelerated normally. The vehicle was taken to an independent mechanic where the transmission fluid was checked but the vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 10,000.

While pulling into what I thought was a parking spot, the left front wheel hit an 8" drop off. Almost immediately after the wheel was back up and level with the other wheels. The vehicle then began surging forward. I hit the brakes but they did not go down. Tried again and the same result. I then hit a hard plastic container and a brick wall. This stopped the vehicle and I was able to put it in park and turn off the engine. Truthfully, once the wheel fell went into the drop off, I had no control of the car. The brick wall has a crack in it and the container is ruined. Toyota product liability was called but they have not contacted me. Thankfully the wall did not collapse and no one else was injured. I have pain in left arm and shoulder but hopefully only bruising. The issue has not been confirmed by anyone and I do not know if there were any warnings because I was focused on trying to get the car stopped. There might have been some alarms, I am not sure.

While changing the lane towards right, suddenly my vehicle over accelerated and was about to crash the vehicle ahead of me and quickly I decided to maneuver towards the right shoulder lane and within few seconds and speedy semi-truck passed from the left and saved our lives of 4 people in the car in the busy highway approaching the o'hare chicago airport. Safety: had I not taken towards right to avoid crash on anther vehicle, the high speed truck from the back would have hit us within a few seconds of my alertness. Yes, during Feb. 2019, due to excess acceleration, this car crashed towards thick ice and snow while driving through a street, and damaged front bumper. (my wife was seated at passenger side) called the dealer and took the vehicle to them(wilde Toyota-west allis), they knew the seriousness, serviced it in their workshop for over a week by changing timing belt, re-programming etc. . Etc. And returned back my car and said now it should work, it is okay as it should be. Yes, my vehicle was inspected twice by insurance company, Toyota officials for airbag inspection, workshop mechanics, due to major road accidents (for not my fault) in oct. 2019 and Jan. 2020 and repaired at another auto body service center in milwaukee(andrew Toyota) and in madison(ballbody-a division of Smart motors). There was no warning signs in the vehicle as the car runs very well on both highway and city roads.

On 10/1/2022 in berwick, PA while pulling into the dollar tree parking lot, smoke appeared in the passenger cabin and the vehicle accelerated on it's own. The brakes were applied and the vehicle continue to accelerate. The vehicle went down through a yard, across a road through another parking lot and crashed into a high curb before stalling out. The airbags deployed. My mother was transported to the hospital and is still in icu with broken ribs, cuts and bruises. She was able to steer the vehicle away from a parked car, a person on a bicycle and missed a rose bush, no one else was injured in the incident. A police report has been completed, the vehicle is still in the custody of the towing company and has not been released to the insurance company.

Intermittent loss/delay of acceleration power from a stop. When coming out of a stop, pressing the gas pedal warrants no response except the slow creep that occurs just as a result of lifting off the brake. This has happened a handful of times when trying to accelerate from being stopped at a red light. The problem occurs intermittently and I never know when to expect it. The most recent time this problem happened, I pressed the gas pedal all the way to the floor, and still the car did not accelerate. When left in the middle of an intersection, a loss of accelerator power can be deadly. I am a sitting duck as oncoming traffic comes towards me and I just hope they see me. Furthermore, when stalled out at a red light or stop sign, other traffic has to drive around me, risking the safety of other drivers as well. I took it into my Toyota dealership for repair, but since they are unable to replicate the issue, they cannot fix it. I was also met with skepticism; however, searching online has shown that other people have experienced the same problem with the same vehicle. The vehicle or component has not been inspected by the manufacturer, police, insurance representatives, or any others as of now. No engine lights appear when this happens; this is also what led the dealership to believe there wasn't actually a problem.
On 8/31/2022 I was turning into the driveway of a friend. All of a sudden my car accelerated. There was another car in the driveway in front of me. I was trying not to hit it, so turned my steering wheel sharply to the right. I left the driveway, turning into a side yard, and then it seemed the brakes wouldn't work. I hit a tree, and went over a large rock, which stopped my car. As it happened, I did hit the other car. Its back right bumper was hit, and my front left was crushed. My car was towed to an auto shop on 9/6/2022. It is not drivable. Incident was reported to my insurance that evening. My local agent told me to report to you on 9/9/2022.

On several occaisions the vehicle has incorrectly detected an impending collision, disconnected the adaptive speed control and applied the brakes forcefully. On one occasion, it happened while driving in heavy slushy snow. Speed control wouldn't reengage. The dealer suggested cleaning off the Toyota logo in front and that resolved the problem. The other times the problem occured, the vehicle was being drive in late afternoon daylight on I 80. The vehicle was in the left lane, going around a tight (for an interstate) curve with another vehicle in the right lane and slightly ahead. It apparently thought that the other vehicle was in the same lane. I. E. , the vehicle had inadequate knowledge of it's position relative to the roadway. Up to that point, it understood that it was in it's own lane. Each time this occurred, the road was (fortunately) relatively empty so no crash occurred.

On August 10 2022 I was pulling slowly into a parking spot with foot lightly on brake to control forward motion car attempted to surge /accelerate . I stepped firmly on brake and since car was nearly stopped I put it in to park. Rpm's went up dramatically and then car reverted to normal rpms. I estimate that the entire process. I estimate that the entire process took place in about 30 seconds or less. I immediately took vehicle to the Toyota dealer and they presumably checked it out and found no problems with vehicle. They "deemed it accidental accelerator pedal engagement by customer" which it was definitely not! a similar situation occurred about 3 months earlier. That time I was pulling slowly into traffic and suddenly car tried to accelerate. I pressed firmly on brake pedal and acceleration stopped. Entire event was very brief seconds and I ignored it as a possible anomaly not requiring attention. I reported that to dealer as well when I took it in for the 10 August incident.

The vehicle without warning will lose all power; "accelerator system malfunction output power reduced visit your dealer" and "pre-collision system malfunction - visit your dealer" are the warning messages that appeared when power was lost. The "check engine" light also illuminated as well as a collision error light appeared. The vehicle has done this twice; the first time, the dealer found a recall which was a software update. However, today the vehicle became unresponsive again with the same error messages as before; we were lucky no vehicle hit us from behind since we were turning towards the frontage road to access the highway. The first time this happened, my wife was entering the ramp towards the expressway when the vehicle lost all power and she had to go to the side. Vehicle is now again at the dealer but cannot be serviced until Monday; however, this needs to be addressed since it is a safety concern. On the Toyota app, it is also listing a hybrid malfunction; this needs immediate attention to avoid a catastrophic event. Losing all power with no warning is completely unacceptable.
My 2020 Rav4 transmission is lagging power during acceleration and will not get up to speed, and also is stumbling and making a pulse like motion during acceleration. My safety and my family safety and others safety are put at risk when entering a highway, feeder, any busy road. We have taken it to the dealer for them to inspect it and the mechanic told us that his personal Rav4 does the same thing but doesn't understand why. It has started a few months ago I don't remember the exact time but however it is becoming more common and there are no warning lights when this problem occurs.

Vehicle happens to accelerate involuntarily when driving, especially on highway. You'll be driving, and then suddenly it starts to accelerate faster than usual. Had another family member drove vehicle to work, just to confirm if they had the same issue, and they did confirm that the vehicle was involuntarily accelerating at a higher speed on its own.
My vehicle has a problem with torque converter shudder that has gotten worse over time. At first it was shuddering and now vehicle bulks and momentarily slows down at the speed of 20mph thus creating an unsafe situation. Toyota has a torque converter shudder warranty enhancement program (zh1) for vehicles of my age and mileage, but my specific VIN number does not fall under this warranty enhancement. I spoke to Toyota brand engagement center senior adviser drew and explained to him that it is a safety issue due to my vehicle unexpectedly slowing down in traffic. But he refused to do anything to mitigate this safety concern.

The contact stated while driving 65 mph and attempted to merge into the next lane, he was depressing on the brake pedal but it began to accelerate independently and lost control of the vehicle. The contact stated he crashed into a concrete shoulder off the road, bounced off the concert shoulder and crashed into another vehicle. The air bags did deploy. The contact stated the brake malfunction warning light was illuminated. The contact sustain injuries on his left shoulder, left neck and right hip area. The contact sought medical attention for his injuries. The contact stated 6 additional people from the other vehicle also sustained injuries but was unaware if they sought any medical attention. A police report was filed. The vehicle was not drivable. The insurance company deemed the vehicle total loss. The vehicle was towed to his residence and then to a collision shop. A dealer was not contacted. The manufacturer had been informed of the failure. The failure mileage was 13,322.
On may 12, 2022 I was driving our 2022 Rav4 hybrid. At about 10 am that morning I arrived at the sherwin williams store in chandler, az. I applied the brakes and put the car in park. But the car did not stop! it continued to forge ahead, smashing into the building barrier and even then it kept trying to move forward. 1. The whole front end was smashed. 2. The first responders had to pry open the door to extract me from the car. 3. It was later determined that I had broken a rib. (I was lucky it was not more. ) 4. The car was later declared “totaled” by the insurance company. By -standers immediately called the police. 1. The police officer confirmed that the car was in park, at the time of the accident. 2. The officer also confirmed with witnesses that the car was in park but was still moving ahead. 3. The officer recorded the witnesses’ names and contacts. 4. I have a copy of the police report, before the accident, both my husband and I had noticed that the car had a tendency to jump ahead even after putting it in park. Since that time I have connected with other Rav4 owners on social media and have discovered that this “ jumping ahead” has been experience by a lot of other owners.
On 12-April, I filed report #11460592 regarding stuttering and uncontrollable acceleration of Toyota Rav4, 2019 model year, <22000 miles, incident occurred 11-April. Vehicle was taken to Toyota dealership service department on 12-April, returned that day as described in previous report. On 13-April, one day after repair, exact same issue occurred. Family (with 2 children) driving on road, 45mph speed limit. Hybrid system malfunction alert occurred again. Speed limited drastically to approximately 15mph, with occasional reaccelerating uncontrollably. Vehicle exited as safely as possible and returned home. Vehicle then taken back to dealership for service. Full text of report #114605 (11-April incident): driving on 35mph limit road. Vehicle began to stutter. Vehicle began to communicate warning lights - "pksb malfunction" and "hybrid system malfunction" and "check engine" and alerted to visit dealer. For rest of trip to home (approximately 2 miles), vehicle would alternate between 10mph limited state and returning to full acceleration which led to very unsafe driving conditions and abrupt changes in speed. Reached home and parked in drive way. After a few hours, returned to vehicle to take to dealer for service. All 3 error messages remained but no further limitation to 10mph or abrupt speed changes. Dropped off for service at dealership. On Monday, March 21, 2022, at about 11:40 am CT, I was at the walgreens pharmacy drive-thru window. I used the brake-hold feature on the vehicle. I did not have my feet on any pedals or the steering wheel. I did not get any notifications, warnings, alerts, or beeps from the vehicle prior to the incident. As I reached my arm out to get my medications and payment from the pharmacy window, the vehicle sped off (accelerated) at a high rate of speed. The vehicle was on break-hold and I did not touch anything. I was able to bring my arm inside and stop the vehicle and put it into the park position. I then put it into reverse and returned to get my medications. When I went to put it back into drive, the brake-hold indicator was still on and so I pressed the button to disable it. I drove the vehicle home and parked it. The vehicle did not roll forward. The vehicle took off at a high rate of speed and my arm was sore afterward. Luckily, there was no one in front of me at the time of the incident. The pharmacy technician stated, "that was close!" the next day, I took the vehicle into service at the Toyota dealership in which the vehicle was purchased. I explained to them about the similar incidents reported I had found online and the outcomes. They kept the vehicle all day, only to get the same response as the previous complainant; they did not find anything wrong. The technician stated if any warning lights turn on to bring it in right away. He stated that it would have given you(me) the warning to keep the brake-hold on, otherwise, the e-brake would have engaged. I asked him, so how do you explain how it took off full speed ahead if it was supposed to stop, not accelerate? I am very afraid of how the vehicle accelerated during this incident. When I got the vehicle back I tried the brake-hold on my driveway; if you tap the gas pedal, it just starts to roll forward; this is not what happened on 21mar22.
I was driving on an interstate highway with th speed control on in extremely light traffic and very heavy, slushy snow when the brakes were applied full and the instrument panel flashed red. There were (fortunately) no other vehicles in front or behind me. Since there was no reason for stopping, I stepped on the accelerator and the vehicle operated normally except that I was no longer able to use the speed control. Calls to Toyota's 800 number were of no help, but a service advisor at sunrise Toyota suggested cleaning slush off the Toyota logo on the grill. After cleaning the logo, the car operated normally. Toyota subsequently examined the car at the dealer and determined that it was operating as designed. I believe that the vehicle's software design is defective and must be redesigned.
While driving I have noticed that the car slow down the speed near other cars, I understand that is the accident prevention feature, but when I was stopped at red light, I press the accelerator lightly, the car sped off, I have my daughter in the car and she told me why are you accelerating mom? I told her I wasn't, my foot was off the accelerator and the car still speeding off. I pressed the breaks and it slow down. I do not feel safe with this car, but I have to use it, as I still have to finish paying it. My battery was replaced today, I had to pay for it, 3 years and gone! they told me it is because I do not drive much daily. My front tires are on yellow tread, not sure how driving 4-8 miles per day can cause all of this damage. I am so tired of car companies sell crappie cars. I had a Subaru before that almost burned (head gasket), of course, when I was finishing paying that happened. By the way I see by the time I finish paying Toyota, many things will need to be replaced, if I do not get into an accident because of weirdness of the car slowing down or speeding. For sure my next car won't be Toyota for safety reasons!.
I had 2 sudden acceleration incidents. The first occurred 10/6/21. My car lurched forward & hit a pole. I was with my mom who saw me press the brake, which did not engage. My car has an auto-braking system which did not function as designed. I took it to Toyota. They determined there were no issues w/the vehicle. I insisted there was something wrong & warned this will happen again & could be much worse. I felt defeated but hoped it was an isolated glitch & hoped for the best. Fast forward 2 months to 12/13/21 I was in a drive-thru using the hold button, which is a feature that holds the brake in place, so my foot was not on any pedal. All of a sudden, my car began to accelerate & the brakes would not engage. An error message on the dash said “brake malfunction”. I could not stop the car. I put in park, but it did not stop the car. The car jumped the curb & drove into oncoming traffic & hit two cars. They were traveling north, I was traveling west. It’s a miracle another car did not hit & kill me. The most frightening experience of my life. I had the car towed to Toyota as the front wheel blew out & could not be driven. Another inspection was done & once again Toyota came back with “they could not find anything wrong with the vehicle. ” and added that the brakes were never engaged, which is an outright lie & utterly insane. I spoke to an attorney & what he told me was quite eye opening. He has been practicing product liability law for 30 years. He told me the letter received from Toyota was a standard letter that all car companies respond with. They never admit a problem unless forced to, usually by the govt, to do a recall after enough people file a complaint. I really believed Toyota would tell the truth. I never imagined they would lie & participate in a cover-up. They would rather fight you in court. As mentioned earlier, my car has an auto braking system, so why didn't it work? clearly it malfunctioned. My hope is that this deceit by car manufacturers ends.
When I driving around 50 miles the car is producing a grinding noise, stalling and reducing the speed, putting in danger the cars driving behind us. That kind of fail is on and off.
1. 5th generation Rav4 le hybrid does not allow adjustment of intermittent wiper intervals that constitutes safety hazard. Every single vehicle on the road has had this feature for decades, its absence constitutes negligence on Toyota's part. 2. Traditional and intuitive industry standard cruise control lever with push-button switch has been replaced with counter-intuitive button controls that distract attention and can hardly be operated without having looked at them.
- I believe the ecm has failed, and I believe this after replacing battery, starter, spark plugs, and spark plug wires. While accelerating, and while maintaining a consistent speed over 50 mph, the tachometer (and engine, accordingly) yanks itself back and forth between 3500 and 5500 rpm. The vehicle physically jumps back and forth between massive acceleration and decceleration, without any change in throttle/pedal placement. This occurs while in cruise control, eco mode, normal mode, and sport mode. There is not yet a discernable pattern or trigger identified in why this occurs. - my safety and the safety of others has been put at risk by the vehicle's jerking, halting, and unplanned acceleration in heavy traffic situations. It scares me and others in traffic, to the point that people have asked if I was okay after being able to visibly notice the car's behavior on the highway, and my reaction. - the problem has been reproduced twice by an independent service center. Toyota dealership read the computer for the vehicle to pull codes, told me nothing was wrong, and sent me away. - the vehicle has been inspected by my local Toyota dealership (lackluster inspection, in my opinion). Nothing resulted from this inspection. - there were (and still are) no warnings lamps, messages, reminders, codes, or notifications of any kind to show that something is abnormal in my engine. This is the most vexing part of this issue. As far as the computer in the Rav4 is concerned, everything is nearly perfect. And it is not.

I was turning into a parking spot at my doctor's office when my car engine revved up and sped through the doctor's office nearly hitting my doctor and another patient. My airbags did not deploy. I had my foot on the brake but it did not stop the car. The automatic emergency braking system and forward collision warning did not work. My car had to be towed and is now waiting to see if it can be fixed but with the safety features that I paid extra for, I'm not sure that I would even buy a new one of this particular model. I came very close to killing my doctor and that patient!.
While using cruse control. It did not slow the vehicle. I could not turn it off. The brakes would not work. I ran thru an intersection against the light. Bounced over the median. I turned off the ignition, still coasted about 1/2 mile. Finally I jamed the gear shift lever into park. Toyota has the vehicle for inspection and reading of the black box.
The Toyota Rav4 2018 car acceleration went out of control and crashed into a retaining wall. The airbags exploded and the steering wheel airbag burst. The car was totaled and is with state farm. I was blessed to still be alive the car speed out of control on its own. I tried to push on the brakes but the car kept going. The accident could have been worse. No one else was injured only another car was hit. I did reach out to Toyota and they said it could take 4-6 weeks for someone to look at the car. I reach out to the dealer and waiting to hear back from the dealer. The police department issued me a ticket, I have court on July 23, 2021. State farm insurance declared the car a total loss. No warning lamps or messages. The first same incident happened on April 3, 2021, and on June 12, 2021, was the day of the accident. The incident and accident happened at the same location. I would like to speak with someone at your organization. The car is at state farm's collision place. I would like for your organization to look at the car. I will call to make sure that the car is still at the collision place.
The first incident happened on June 9, 2021. I was turning right very slowly into a parking spot at a grocery store when the car suddenly accelerated rapidly, even when I depressed the brake very hard. My car hit a row of small trees that formed a border between the parking lot and the street. The impact was so hard that there was over $5,000 in damage to the front of the car. I was in shock as it felt like the car was out of control. The second incident July 27, 2021. Again, I was turning right to reach the exit of a parking garage at a medical clinic. I was going very slow because I was approaching the ticket booth to exit the parking garage. Just as I completed the turn, the car accelerated and lurched forward toward the arm that keeps cars from exiting. I was able to stop the vehicle by pressing the brake hard, and I did not hit the arm at the exit point. Again, it was shocking and very frightening for this to happen.
