1 Visibility problem

Failure Date: 09/26/2021

The contact owns a 2005 Toyota Sienna. The contact stated while driving 70 mph, the sun roof detached from the vehicle and hit the the vehicle driving behind him. The contact stated he continued to drive and exited the highway to park to inspect the vehicle. The contact stated he was informed by the police department 30 minutes later to informed on the damages the sunroof created. The contact stated the driver of the other vehicle has multiple leisure at unknown locations and did not seek medical attention at that time. A police report was filed. The contact took the vehicle to local dealer, where he was informed that the webasto sunroof was an aftermarket part. Upon investigation, the contact associated the failure with NHTSA campaign number: 11v042000 (visibility), however the VIN was not included. The vehicle was not repaired. The contact called manufacturer webasto and left them a voicemail but has not spoken to them. The failure mileage was approximately 186,000.

2 Visibility problem

Failure Date: 06/10/2011

Navigation system fails displaying fault "no map dvd". In this state the rear camera is inoperable causing blind spots when attempting to operate the car in reverse.

3 Visibility problem

Failure Date: 09/26/2008

The left sliding-door side window in our 2005 Toyota Sienna spontaneously exploded while driving at about 45mph on a day where the temperature was in the 60s. The window was not struck by any object, it just exploded. The window was not in the process of lowering or raising at the time.
