Air Bag problem #1

On may 18, 2025, 4:00pm, my wife was applying brakes to stop for a stop sign at a t-intersection. Suddenly, the car lurched forward and accelerated uncontrollably. Despite her attempts to stop, her vehicle crossed four traffic lanes, vaulted over curb, nosedived into turf, and then crashed through brick wall into a fire station. My wife was unable to brake and stop the forward progression of the vehicle. My wife steered to avoid hitting trees but could not avoid the brick of the fire station wall. The airbags did not deploy. My wife sustained back injuries and bruising of the front torso. Her right foot is bruised from pressuring the brake pedal. The vehicle sustained heavy damage as did the fire station.

Air Bag problem #2

Passenger air bag malfunction notification randomly going off (with and without items on the front passenger seat). I was told the dealership can’t diagnose it unless the alert is going off at the time the vehicle is dropped off.

Air Bag problem #3

After starting the van, an error pops "srs airbag system malfunction visit your dealer. " this occurs a day after turning the car on and off and driving for several hours. The next day, it may come back on, or it may not. I have taken it to a dealership, and they stated it was a firmware update they completed. After they updated the warning light, it came back on less than a week later.

Air Bag problem #4

Srs airbag warning light is in the dashboard. It is confirmed by the Toyota dealer a faulty radio head unit can not register the srs airbag. In the event of collision, the srs airbag will not be deployed, causing passengers at risk.
