The contact owned a 2021 Toyota Sienna hybrid. The contact stated that while driving at an undisclosed speed, the check engine light and the malfunctioning hybrid system warning lights illuminated. The contact stated while he was driving the vehicle to a nearby parking lot he noticed smoke coming from under the hood. The contact and his family exited the vehicle. The contact noticed flames coming from the hood area and grabbed a nearby water hose to hose down the fire. The fire department arrived and extinguished the fire. A fire report was filed and there were no injuries. The vehicle was towed to the dealer and a investigation was completed. The results of the investigation were not released to the contact. The vehicle was declared total loss. The manufacturer was made aware of the failure. The failure mileage was approximately 23,500.

For a month prior, there were intermittent incidents of the van shuddering and the rpm needle jumping upon ignition. Sometimes the van would have a hard time accelerating before jerking and driving normally. No issues before this started in late July and no indication when/where would occur. •September drove the van all day running errands. It drove normally. Filled up the gas tank (gulf gas station), then continued errands. About a half hour later, while at a stop sign, a notification appeared that the “traction control system has been turned off” and the check engine light came on. Tried to move through the stop sign, but the van would barely go. Applied the gas, but no results. Able to turn onto a side street and had to have it towed to Toyota of irving. The service advisor stated the issue was due due to bad gas and indicated the van needed to be flushed and the fuel injection system cleaned. Paid $725 for these repairs. •September started van in driveway. Shuddering and rpm needle jumping again. Drove normally. •September same as above. •September drove the van all day with no issues. Filled up with valero plus gas. As we pulled out of the gas station and sitting at the light, the traction control message and check engine light again appeared. The van would barely go to get us through the traffic light and into a nearby parking lot. Once we parked, the screen displayed the following message:"a malfunction in the electronic control of the engine, throttle, or automatic transmission has been detected. " had the vehicle towed to Toyota of irving. The service advisor called 2 days later to say they cannot detect any issues.

I my Sienna had its 12 volt battery depleted over night with no reason. After it was jump started, literally every single piece of equipment was malfunctioning and producing an alert (hybrid system, brake pressure, rcta, bsm, awd,tire pressure and so on). After several restarts of the engine (about 15 min after running engine), all systems went to normal except hybrid drive system, that still asked for maintenance and the tire pressure sensor was still on alert. However I drove the vehicle to a dealership in maine after the parking brake finally decided to get loose again. These guys told me, they had just the same problem with another 2022 Sienna a week before. However, they reseted the errors, and everything worked fine. They had no explanation why two Sienna in one week come to their lot with a depleted battery and as a consequence with a malfunctioning hybrid systen. That's very unsatisfying.

Was driving my Sienna down he highway, as I was passing someone I lightly accelerated and I heard a loud bang sound. Seconds later I see smoke coming out of the hood so I pull over, about 30 seconds before I pulled over the engine bursted into flames. When the vehicle came to a stop it would not unlock and me and my 2 kids were stuck in a burning vehicle, we barley made it out and my arm got burnt from it, this is the 4th time I’ve had a problem with the engine and 6th time the transmission has had problems. I’ve heard this happen on many other Siennas yet it’s not recalled….

I was driving my 2017 Sienna on 5/10/2022 and was approaching a roundabout. As I was going thru the roundabout, my van stalled. I coasted into a gas station. After a few minutes I notice smoke coming from under my hood. I popped the hood and noticed the smoke was coming from the air intake. We called aaa and they towed it to the dealership we bought it new from and had all services done at. The next day we authorized them to do a diognostic on it where they found a burnt airfilter and melted housing. Heiser Toyota in milwaukee WI has not repaired the van yet and we wanted to talk to Toyota customer service and your office to see what we should do. We called insurance and a claim has been started - not sure if they have inspected the van yet. It is available for you to inspect. All warning lights were on when it stalled. There was no power loss or any warning before it stalled.

During heavy rain exhaust system become dislodged and fell to the road, only hanging on by the engine connection. We had the vehicle towed to the dealership only to be told that is not a warranty issue. They did reinstall it in the rubber brackets. Due to my persistence a warranty claim was opened. There claim was that it hit something as there were marks across the muffler however those were made by dragging on the road until vehicle could be driven off the road. Knowing it was most likely water pressure from the water on the road that lifted the exhaust up and out of the rubber brackets, I wanted this documented. Although if the exhaust system had completely broken off from the engine it would have not hurt our vehicle occupants, any vehicles behind could have been hit.

After a short 10-min drive, parked the van in my house's driveway. When I stopped, smoke started coming out of the hood. Which developed into flames and the front of the van was engulfed in flames. Immediately called the fire department to put the fire out. Van is totaly burnt. Managed to move two other vehicles away from the burning van. Aluminum siding next to the garage sustained heat damage.

Engine stalled in heavy traffic after full stop at red light. No warning lights. Managed to pull off the road. Still no warning lights. Turned engine off then started normally. Drove to destination normally. After about one hour parked started engine but it stumbled on and off for the next few minutes but did not die. Has not reoccurred. Outside temps were in the 60's. It was first trip of the day. Incident occurred about 4 miles from origin.

The Toyota 2gr-fe 3. 5l v6 engine have a verry comoon complaint of blown head gasket under 100k miles. I own a 2014 sinenna and currently experiencing same issue at 89k miles. I looked online and found that so many people had the same issue and there was no action from the side of the company, they just waited for the 60k miles/5 year to be over so that they can deny the claim. The funny thing is that my vehicles's previous owner purchased an extended warranty which is still valid till September 2022 and the company doesn't want to honor it. With this issue in mind I cannot take my vehicle for a long with my family, it puts life of my family at risk.

The contact owns a 2015 Toyota Sienna. The contact stated while starting the vehicle he noticed that the vehicle was shuddering and shaking uncontrollably. The check engine warning light was illuminated. The contact attempted to restart the vehicle however, the failure persisted. The contact took the vehicle to an independent mechanic to be diagnosed. The contact was informed that the failure was due to low cylinder pressure on cylinder #6. The mechanic was not able to do the repair on the vehicle, so the contact took the vehicle to the local dealer who diagnosed and confirmed the failure as a misfire and low pressure cylinder #6 and that the spark plug was damaged. The vehicle was repaired however, the failure persisted. The dealer recommended that the engine be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 87,000.

On 10/14/2021, as my wife was pulling into our attached garage, the vehicle suddenly accelerated, hitting a metal storage shelf located at the back wall of the garage. The impact of the crash caused the vehicle and the shelf to extensively damage the dry wall of that area of the garage, as well as to damage the pedestal supporting the house water heater, which resulted in the water heater detaching away from the wall and being destroyed. There was, of course, also major damage to the front bumper area of the vehicle. It is my contention that the cause of the accident lies in some component of the vehicle acceleration system. I believe this to be true because of two prior incidents which occurred within the month before this happened. On 09/16/2021, I was driving this vehicle and wanted to make a u-turn. I was traveling at approximately 10 mph, but as I began the turn, suddenly, the vehicle accelerated to about 25 mph, even though my foot was off the accelerator. My wife and granddaughter were in the vehicle at the time and both expressed alarm, somehow thinking I had purposely made this maneuver, which I assured them that I had not. Then, approximately 7-10 days later (I am unsure of the date), I was driving, alone, in this vehicle and went to make a left-hand turn. The vehicle again accelerated, even though I had removed my foot from the accelerator pedal and was applying the brakes. I estimate that as I began the turn, my speed was 10-15 mph, but the vehicle accelerated to 25-30 mph. I dismissed both of these prior incidents as not too important and attributed them to some unknown anomaly on the road. However, after the latest incident involving the damage to my garage and vehicle, research showed that several Toyota models and specifically the Sienna have had similar issues in the past. I am reporting this incident to Toyota for their evaluation, but wanted to make your agency aware, also.

This is to advise you of a safety issue with our 2013 Toyota Sienna xle minivan having a v6 (3. 5l) engine. Our vehicle currently has 118,169 miles on it and has been well maintained. This vehicle was purchased new and is operated primarily in florida where we live. Over the last 6 - 10 months on three occasions our Sienna minivan suddenly lurched forward as the engine revved up. On all three occasions, I had to take quick action and with significant force press the brake pedal as well as place the minivan in neutral and then into park in order to stop our minivan from surging forward and hitting the car in front of me. On all three occasions, my right foot was on the brake pedal when this occurred and on two of the three occasions, our minivan was slowly accelerating from either a stopped position or a very low speed (5 mph or less). On one of the occasions I was approaching an intersection with a stop sign and our minivan was decelerating to a stop when the engine revved up and the minivan surged forward. This occurred as our minivan was going approximately 5 - 10 mph. There were no warning lamps, messages, or other symptoms prior to any of these incidents. I have reported this condition to our local Toyota dealer but they said they could not duplicate the issue during their test drive. This is a serious safety condition and one that could result in a serious accident with injuries particularly if the driver is not quick to react to this unexpected condition.

There is a Toyota campaign to replace this faulty designed oil line with a full metal line, but for some reason my 2007 Toyota Sienna VIN is not showing up with this campaign. When tow truck operator arrived and tried to start, is caught fire and was severely damaged.

When I entered bedford PA turnpike, the car sounded with different sounds from engine on the ramp when entering the 70/76 freeway. When entered to virginia from maryland border after potomac river crossing on highway 15 , the car suddenly stopped while driving at 30 mph and pulled the car to the side of the road slowly and when I restarted the car, it started normal and drove till chantilly virginia without any stopping. But the engine gave me different noises.

When the car is at idle speed, such as waiting for traffic to clear for a left turn, the car will just die without warning. It will usually restart right away, but this has nearly caused a collision. The dealership in bellevue, wa says that's a normal thing if the car isn't properly maintained. I have brought the car to that dealership for all but one of the maintenance intervals, and the only other time was for maintenance when we were in ohio. Having the throttle body cleaned is not on the routine maintenance schedule. When the car does die there are no codes or other symbols that come up on the display, other than to say that the car died.

Our 2016 Toyota Sienna spontaneously caught fire while parked in our driveway after returning from a short drive. After pulling into our driveway and shutting off the ignition, my wife smelled smoke as she was exiting the parked vehicle. This smoke intensified and quickly accelerated becoming an engine compartment fire. She called for me & then called 911; but in the brief moments it took me to grab a fire extinguisher, the fire further intensified with sizable flames erupting out from around the hood, front grill & underneath the vehicle. I used up 2 fire extinguishers without any significant effect. I did not have any protective safety gear so after hearing several small explosions coming from under the hood & fearing more exposure to noxious fumes, I decided to abandon my attempts to extinguish the fire. The fire dept. Arrived within a few minutes and were able to put the fire out; however, the fire progressed through the initial attack and entered the passenger compartment causing severe damage to the entire vehicle (see attached photos). The Sienna is completely destroyed. Fortunately, there were no injuries. I have been made aware of multiple incidents of engine fires in Toyota Siennas in this & other closely related model years (2014-2017). See: NHTSA id number: 11010601. I can also supply you with some additional media accounts of other Toyota Sienna engine fire reports. Our Sienna was routinely maintained as per manufacturer's recommendations & was last serviced on 9/13/2019 (see attached service receipt). The vehicle had approx. 35k miles on it & exhibited no prior signs of trouble, change in performance or warning indicator lights. We did have an unresolved problem with automatic side doors not operating properly after a recall repair; however, the local Toyota dealer service dept. Could not identify a problem after multiple service complaints.

The traction control system is activating improperly. The vehicle's anti-slip indicator warning light illuminates; an alarm sounds; the abs system activates; and the vehicle sometimes loses power. This happens on dry roads, most commonly either going downhill or else on a curve, even a mild one. This happens above 20 mph. When it happens, the combination of alerts and sensations are very startling to the driver, creating a dangerous situation. It has occurred while trying to accelerate to merge onto a highway. This was exceptionally dangerous as the vehicle began losing power and vehicles behind us almost hit us from behind as we were slowing instead of increasing speed. From my internet searches (including NHTSA complaints), this appears to be a recurring issue with this make/model/year range. This has been happening for the past 3 months. Our vehicle has approximately 135,000 miles on the odometer.

Car was recalled for fuel pump and I repaired in 8/2020 with approximately 11,190 miles. I had complained that car, since time of purchase and had 4662 miles when I insisted Toyota fix this, (hunted for gears, hesitated, lagged when accelerator pushed, and had poorer gas mileage than expected). The car now has 34,875 miles on it and is doing the same thing, hesitating, lunging between gears. Our mechanic confirmed this and stated worse when using a. C. Called Toyota customer service and they said the car was out of warranty for the pump repair, but perhaps not the power train, transmission, etc. Have not gotten back to me yet, but has not been one working day after spending 2. 5 hours on the phone with them yesterday. This is a safety issue, I press on the gas to make a turn and it lags leaving me in traffic. My 2007 Sienna, which is still running for my daughter, never did this and has full power when called upon. Toyota needs to repair the entire fuel system, transmission, if they cannot fix this. There are no warning lights when this happens & it is happening more frequently. Dealership told me they could not put another pump on car and to contact Toyota customer service and gave me the number. When we had the car for about a month, dealership said we were just not used to driving a car with this type of transmission and car was low on oil, we added oil and dealership said we used the wrong type, but we figured some oil was better than no oil. We are boomers and have been driving cars for decades, with clutches and without and we know how to drive a vehicle.

Acceleration paddle has to be pressed very hard, for first few seconds there is no response from engine then latter it suddenly accelerates. I pulled onto a highway and reaching about 25 mph the 2019 Sienna hesitated for at least 10 seconds as if it was not getting gas. I pressed the gas wanting to get out of the way of traffic and it jumped slightly but would not go. Then it kicked in with a few hesitations and took off. Another 100' or so, it did it again. Prior to this I had already brought it to Toyota complaining that there is a hesitation when the van is not warmed up yet between 20-40 mph. It is only slight, but noticeable and feels like it is not getting gas. The van does this every time after it has been sitting long enough to cool down. The long hesitation only happened twice so far (dangerous enough!), there have been a few shorter ones, and then there is the every time slight hesitation. Toyota has told me nothing shows in their diagnostics and they do not know what is wrong. They tried cutting the power to the computer to reset the memory, but this did not change anything.

I drove about 5 miles from my house to the grocery store, when I notice the smoke, I first though was the car in front of me. But was smoke coming from my hood. I was just about to stop at a stop sigh and I look the temperature gate and it was fine, turn of the car and people from the parking lot was already yelling for me to get out of the car. More than 2/3 of the van was totally destroyed in the fire.

The manufacturer extended the warranty; however, the contact was not covered since the vehicle exceeded the warranty. Buckeye Toyota (1903 riverway dr, lancaster, OH 43130, (740) 654-3943) stated that they could not authorize a repair out of warranty unless approved by the manufacturer. The manufacturer provided a case number and stated that they would not cover the cost of the repairs. The contact called the state attorney general's office. The vehicle was not diagnosed or repaired. The failure mileage was 156,000.
