Driver side passenger 2 nd row seat belt stopped working. Won't clip as needed leaving us unable to use seat. This is a safety issue as we are not able to use the seat as needed as one is not able to strap in. This triggers the sensor and this a driving violation. Problem has been reported. Vehicle has been reported and still pending to be replaced. Unkown when issue can be resolved. Passenger seat on the second row was stuck upon purchase. Dealer made aware. They claimed a sock was stuck on railing which was not possible as issue was reported before they could schedule for vehicle to be inspected. Such sock did not exist when incident was reported. Sock was not purchased until day before taking vehicle to schedule appointment. Tension is still there but agency said it is working properly.
Passenger side middle row seat belt has locked up fully retracted. Dealership service center stated on invoice, “nothing found to be causing the binding other than a mechanical issue, quoted replacing the seat belt assembly. ”.

My child [xxx] ) engaged the automatic locking retractor (alr) and as it wound back in it wrapped around his neck. By the time we got to him it was tight enough that it was difficult to move or rotate or slide the seatbelt over his head, and we ultimately cut the seatbelt to free him. In addition to the actual safety incident, I was very surprised to learn that this was even possible. I had never heard of this safety issue, and had absolutely no game plan or awareness about this issue. I saw the sbir program addressing innovation in alr systems which is exciting! however I would also strongly encourage some sort of awareness campaign or warning messaging to raise awareness and ensure parents keep belt cutters in their cars. This recall has been open for 7 months and a remedy has still not been issued - this doesn’t seem reasonable for a safety concern. I have also several times experienced a failure in the “extra” 8th passenger seat (middle row middle seat) sensor - it alarms that the seat belt isn’t buckled even when that seat is not in use and not even installed at the time.

2 issues to report 1) when under medium consistent breaking(slowing down for a light) there are instances where the breaks feel like they let up or slipping. This issue has been reported and with the car since the first year of ownership but Toyota says it’s all within spec 2) second issue is an issue with the rear captions seatbelt and has also been reported since 2022 where upon normal driving the seatbelt locks up and the car has to be stopped and seatbelt unbuckled to release tension. Also brought up multiple times with Toyota and no action. Kids sometime unbuckling while car is driving since they cannot move.
Recall: OCS sensors may short circuit; 3tb15/23ta15 ; I am a owner of 2021 Sienna hybrid (VIN 5tdyrkec0ms004155) and was getting alert for the airbag and seat belt warning even when plugged in. I took it to a dealer in sunnyvale California (stevens creek Toyota; 4202 stevens creek blvd san jose, CA 95129; 408-984-8833) and they are not recognizing the recall. I was charged a diagnostics fee of $ 899. 93 and am being asked to pay $ 2000 to replace the car seat to fix the issues. Your advise is solicited. Best regards. After starting the van, an error pops "srs airbag system malfunction visit your dealer. " this occurs a day after turning the car on and off and driving for several hours. The next day, it may come back on, or it may not. I have taken it to a dealership, and they stated it was a firmware update they completed. After they updated the warning light, it came back on less than a week later.

My young daughter got the seatbelt caught around her waist. The seatbelt was unable to give any slack and was locked up. It could retract a little bit but that would only squeeze around my daughter’s waist tighter. The seatbelt would not pull forward. We eventually had to cut the seatbelt to release her. It caused red marks around her waist and had we tried to retract it any further could have suffocated her. We took the van to the dealer to replace the seatbelt and they had said they heard of this happening before but it wasn’t a malfunction but just a weird design quirk. This incident has not been investigated.
In a couple of instances the latch mechanism in 2 different seats became loose during trip. In one ocassion there was an alarm while in the second ocassion we realized when removing the car seat of an infant.

After a rain, water will leak into the driver and front passenger seatbelt coil housing. The front passenger seatbelt was not used in some time and mold developed with green and black stains all along the seatbelt. After it rained, the driver side seatbelt released about 1 cup of water when I pulled the seatbelt. With my wife as the passenger, she was conflicted about wearing the seatbelt while having mold pressed against her body, clothes, and breathing it in. The smell of mold is now present in the vehicle. This problem has been reproduced via online reports at Siennachat. Com. No warning lamps, messages or symptoms appeared prior, or during, this issue. I am in the process of reporting it to Toyota as soon as the next business day (Monday).

How was your safety or the safety of others put at risk? while driving the vehicle at the rate of 55-60 mph, the vehicle came to a complete/instant stop without warning — potential causing harm or even death to the occupants of the associated vehicle, and it adversely affected my ability to safely control the vehicle. Also, as the incident was occurring, my drivers seatbelt became very, very tight. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? no. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? no, but this is the second occurrence of this same incident within the past 30 days.

Hi, I complained many time about seat belt sign of front passenger side and rear passenger right side. While driving expect me no one in the car but still I get seat belt sign.
The contact owns a 2021toyota Sienna hybrid. The contact stated that while her son was occupying the passenger side, the third row seat. The seat belt locked and failed to release. The occupant was unable to get out of the seat belt. The occupant sustained marks on his stomach but no medical attention was provided. The contact had to cut the seat belt to release the child from the seat. The vehicle was taken to the local dealer but was not diagnosed or repaired. The manufacturer was notified but no additional assistance was provided. The approximate failure mileage was 13,000.
The driver-side second-row seatbelt goes slack for no reason while driving. All tension is released, and the loose belt falls forward. This is a serious safety issue for the child riding in that seat with no working seatbelt. This has happened about a dozen times on various days while driving. The dealer first denied there was a problem, then they claimed they fixed it. But it is still happening.
A few days ago the seat belt sensor would continue to beep and blink even after the seat belt was fastened. Then, on Friday, December 24th, 2021, a piece popped out of the seat belt latching mechanism on the driver's seat. Now it is difficult to get the seat belt to latch, and I am unsure that the latch would hold if the vehicle was in an accident that could cause serious injury to the driver. This is available to be inspected upon request. I haven't taken this to be looked at yet, but would like to know if this is something that I would have to pay for or if the manufacturer would cover the cost of this repair.

Drivers seat belt latch had a spring pop out. It continued to work for a short time although less functional and then it would no longer latch.
One day when we tried to buckle the driver's side seat belt, little pieces flew out of the receiver side and it will no longer buckle. We took it to the Toyota dealer in n syracuse NY and were told it would be a cost of $212 to replace and that it would be a two-week wait for the part to come in. This means that the owner and her two young babies would have to drive in an unsafe car, illegally for two weeks putting her life and the life of her children in danger. The Toyota dealer inspected the seat belt and confirmed it was a defect and had failed. I called Toyota customer service and was told that there were no recalls for this defect and they would take no responsibility for the dangerous situation or consequences because the car was no longer under warranty. In fact, this is a problem that has been affecting many owners as indicated by an online search. Also, the fact that the dealer has to wait two weeks to obtain the part indicates that this seatbelt has failed many owners and they have used up the supply. This should be a recall situation but Toyota will take no responsibility.

From the time I purchased my 2004 Toyota Sienna, the driver-side seat belt has never worked properly. Most of the time it would not retract, but was sometimes fixable by moving the height adjuster up and down. Sometimes it would be over-extended for days, then retract on its own. It is now fully extended and unable to retract. I made an appointment two weeks in advance with a Toyota dealership (80 miles away) to fix it but, because of a miscommunication, they were preoccupied with repairing four recall issues and neglected to address the seat belt problem. Instead, they gave me an estimate of $335 to replace it. The following statement, dated Jan 7, 2016, was sent to owners of 2004-2006 Toyota Siennas. Apparently the former vehicle owner did not follow through. I purchased the vehicle from a maui dealership in October of 2018. "warranty enhancement program � zf2 2004 � 2006 model year Sienna vehicles extension of warranty coverage for front seatbelt assemblies Toyota has received a number of reports regarding front seatbelts in certain 2004 � 2006 model year Sienna vehicles. In these reports, customers have indicated that the front seatbelts (right and/or left) may slowly or non-smoothly retract. Although the front seatbelt assembly is covered by Toyota's new vehicle limited warranty for 5 years or 60,000 miles (whichever comes first), we at Toyota care about the customers� ownership experience. Toyota is now extending the warranty coverage for repairs related to slow or non-smooth retraction of front seatbelts. " since this is obviously a common and hazardous problem with 2004-2006 Siennas, it seems like it should be warrantied under a recall program.
Driver and passenger front seat belts do not retract properly. It gives driver and front passenger a sense that the seatbelts work only to find the seatbelt is loose at the waist. A known problem by Toyota in the 2016 "campaign" whereby they installed driver and front passenger seatbelts. Here it is 2020 and the exact same issue is happening.
The drivers seatbelt on my wife's car stopped functioning and will not latch. Nothing appears to be stuck in it. Looking inside the female part, it looks like it has come apart. The vehicle has never been in an accident. The dealer cannot explain why it broke.
Water intrusion from front sunroof when vehicle is parked at a 5% grade with the nose downward. Multiple vehicles exhibit this issue. My issue started around 2020 that I noticed and every dealership would not help me other than place a large burden on me. This can impact the electrical and the airbag curtains. The issue needs to have a seal sealer added to the sunroof assembly as the fix. As a family vehicle, this is a dangerous issue for multiple occupants in a single 7 or 8 person hauler minivan. The right front passenger seatbelt frequently binds up in the top guide area. The seatbelt is not usable.
The front seat belts gets tangled. It takes a lot of time and effort to get it untangled. Most recently I was unable to get it untangled so I had to take it to my garage to get it untangled. Very serious safety problem if it get tangled while I am out running errands and cannot get it untangled so I can drive home. Hazardous and should be recalled!.
Seat belt will not retract. No accident/collission. It seems the seat belt is defected and dealer refuses to correct, stating seat belt is out of warranty. My child pulled the seat belt to put it on and it just simply did not retract.
On numerous occasions, the front passenger safety belt has gotten bound up and stuck in the guide on the a pillar. This has happened while putting on the belt, as well as while the vehicle is in motion. I'm concerned that if this happens during a collision the front passenger will not be properly restrained and could be injured. I have been able to free the belt with tools before, but am no longer able to do so.

Driver's side seat belt buckle stopped locking in seat belt. Buckle unit then came apart. I called Toyota; was informed that there was a recall on this matter. I was told that I was 2 years past the scope of the recall. I informed them I never received a notice of recall so how would I be aware of problem until it occurred. Spoke to supervisor was told I was no longer covered under recall. Went to 2 mechanics and was unable to obtain needed part. I feel that for this safety item there should be no time limit on a recall and I felt Toyotas help in remeding the situation was poor especially as they tout their vehicles safety.
The first weeks seemed like everything was fine with the vehicle until we started feeling low levels of electrical shocks. It was mostly on the steering wheel so we thought that the sensation would go away, but it actually started going worse to the point that we started having discomfort on the seatbelt and the last three weeks before we dropped the vehicle at the dealership the hazard started to intensify and get stronger. The seatbelt of the 2019 Toyota Sienna had provided a lot of pain to my ovaries (to the point that my period had been altered during the months of October to December 2019), it caused so much pain in my stomach while being a driver and a passenger specially the last three weeks before I dropped the vehicle at the dealership. Also due to the fact that I am five feet tall, the seatbelt rested on my neck which was the most uncomfortable experience. In those past three weeks prior to dropping it at the dealership, I even felt shocks from the seatbelt while it was resting on my neck causing numbness in my legs. On regards of the steering wheel, I felt electrical shocks that caused tingling in my hands going all the way up to my elbow causing pain on my elbows (having greater discomfort on my left arm as I tended to drive most with my left hand) and making my arms feel tingling and eventually numbness.
While buckling up on 6 Dec 2019, the seat attachment to the b-pillar completely, without resistance, detached. The bolt that secured the belt was on the floor, completely pristine in appearance, undamaged, without any evidence of thread-locker application at the factory. The car had been in no accidents and was in excellent shape, without a trace of body rust or deterioration. Dealer replaced the bolt and checked the other belt anchoring bolts and said they were okl. Had to return to the dealer in two days since their repair had left the seat belt twisted. They unbolted, then re-bolted the belt anchor in the un-twisted, correct position. No harm occurred since I put on my belt before starting the car; realizing the defect immediately, I got to work by other means that day. The vehicle was stationary, in my garage at the time of the defect discovery.
1) seatbelt twists and gets jammed in the guide near the retractor. Seat belt pulls out, but then will not retract as it is wedged in the guide. This has happened numerous times, but now it is quite wedged. And unusable at the moment. 2) cable that pulls the side door open and closed has rusted and broken. This was subject to a Toyota voluntary service campaign, but my problem started after the expiration of the service campaign.
