While driving approximately 30 mph, a vehicle suddenly cut in front of the driver and caused a chain reaction of sudden stops with several vehicles. The driver rear ended the preceding vehicle. The air bags and Toyota safety sense system failed to activate. The safety sense feature was supposed to stop the vehicle automatically due to objects (vehicle) being in close proximity at a fast rate. The front end of the vehicle had severe damage and was towed. The contact was unsure if a police report was filed. The driver received injuries to the neck and arm area, which required medical attention. The contact was not aware if the vehicle was totaled or able to be repaired. The contact was unaware if the dealer was informed. The manufacturer was not made aware of the failure. The VIN was unknown. The approximate failure mileage was 1,000.
