"my 2015 Ford F-150 is experiencing an intermittent, complete loss of the output shaft speed (oss) signal within the 6r80 transmission lead frame. While driving at speed, the speedometer suddenly drops to zero, the 'wrench' light illuminates, and the vehicle performs an unintended, harsh downshift into a lower gear (limp mode/3rd gear). This creates an immediate safety hazard of rear-wheel lockup or a sudden loss of acceleration in traffic. I took my vehicle to a local repair shop where they could not duplicate the fault. I then called my local Ford dealer who informed me unless it would duplicate the fault or show a code they could not repair the vehicle due to it being against warranty policy.

Today I was driving my truck at 55mph on a local highway and all of the sudden I felt/heard/experienced a massive clunk out of nowhere - almost like the truck tried to downshift into 1st or something similarly jarring. It only lasted a second, and then I got a dash light on that said service advancetrac. Based on the service codes I'm seeing (two of them that are both uo100 codes which indicate loss of communication), I did some research and found out about the oss sensor issue and think that's what is going on with my truck too. Unfortunately I'm not sure if I'm going to be able to get a Ford dealership to do anything about it since there isn't an official recall, and I'm afraid to keep driving it as this is likely going to keep happening - but fixing it myself is going to be expensive I think. It is available for inspection upon request. I felt that my safety was and continues to be at risk - downshifting into 1st gear, or whatever that was, while driving high speeds is dangerous and could potentially significantly damage my vehicle and myself. I have not taken it anywhere yet to be reproduced or confirmed - just plugged my scanner in and did a bit of research. No one has inspected it.

Manufacturer recall #24e12 (pcm software update) triggered a po430:00-ed code (catalyst system efficiency below threshold - bank 2) because the new software more strictly monitors the catalytic converter, causing it to detect previously ignored degradation. This reprogram scamming program makes community bothered by paying parts replacements more than 2k, even vehicle was fine before the recall. This needs resolved with free fixed and free catalytic replacement. This is not right to announce a recall with unfinished repair issues left. This needs action. Thanks, so mote it be!.

The vehicle’s 10-speed automatic transmission is experiencing severe, erratic, and life-threatening failures at highway speeds. Erratic shifting & sudden downshifts: while traveling at approximately 60 mph, the transmission unexpectedly downshifts from 10th gear directly to 1st gear without warning. This causes violent deceleration and risks immediate rear-wheel lockup. The vehicle also cycles randomly through gears (e. G. , jumping 10-1-7-10-3) and frequently 'locks out' upper gears, making power delivery completely unpredictable. Total loss of motive power: on approximately 3/10/26, while in the high-speed 'fast' lane of the highway, the vehicle experienced a complete loss of motive power. While the engine remained running, the digital gear indicator on the instrument cluster began flashing and the transmission failed to engage any gear. I was left coasting in the flow of traffic and was forced to navigate across multiple lanes to the shoulder without the ability to accelerate. Safety hazard: these combined issues create an extreme risk of high-speed rear-end collisions and loss of vehicle control. The 'flashing' gear indicator and sudden visible on the dash 10-to-1 downshifts suggest a critical electronic, sensor or software failure that makes the vehicle unsafe for highway operation.

I’ve been experiencing harsh shifts between 1st and 3rd. Slip shifts and delayed engagement between 5th and 8th gears. Recently I just experienced no engagement at all while accelerating from red light loss of complete power and slippage. Truck just stopped accelerating which could cause an accident. I got no warning. This is not a new problem. It’s an ongoing issue that needs to be rectified before serious injuries. 10r80 transmission. Cdf drum bushing failure and outer casing putting metal shavings in unit causing valve body damage.

When parking the vehicle at auto zone, at a very slow speed, the vehicle suddenly accelerated. Without the accelerator being pressed, the vehicle suddenly accelerated on its own. Notes: vehicle has one-pedal always activated. It is a new vehicle. Only has 3k miles. I was taken once to the shop for maintenance. Malfunction components: maybe speed related components and safety features. The airbag did not deploy and non of all the safety features the vehicle has did not activated. The safety risk was extremely serious. The vehicle went over the concrete parking stopper, tore it off, and then crashed into a concrete column at autozone. If anyone had been walking in front of the vehicle, the outcome could have been catastrophic. Despite the impact, the airbags did not deploy and none of the vehicle’s safety systems activated. After the incident, I went to the er due to pain in both hands, shoulder pain, back pain, and pain in my feet. I was evaluated for these injuries as a precaution. A police incident was done. We have a number. Because it was on private. Property there is not a report, but they gave us a police incident number. No warnings, symptoms prior to the incident. It was a dry day, not rain. The incident occurred last Saturday. We tried calling the Ford dealer to have the car towed, but we were unable to reach them. Because of safety concerns, we contacted a towing company and had the vehicle towed to our home instead. We plan to call Ford today, but wanted to report the incident first.

While driving my 2023 Ford F-150 raptor, the vehicle suddenly lost throttle response. The accelerator pedal produced no response and the engine would not rev even though the vehicle remained powered on. A battery warning light briefly illuminated at the time of the incident. I was able to bring the vehicle to a stop, but during the event the steering wheel became locked or extremely stiff and the vehicle did not respond normally to inputs. After stopping, I placed the truck in park and was unable to shift the transmission back out of park. The vehicle appeared to remain powered on but multiple systems were not responding. After pressing the start/stop button and cycling the vehicle, the issue cleared and the truck began operating normally again. Prior to this incident, the vehicle had already been scheduled to go to the dealership because it had been displaying numerous warning lights and diagnostic codes across multiple systems, totaling approximately 30 alerts. The vehicle was taken to a Ford dealership for diagnosis. The dealership reported codes indicating a network communication issue between vehicle modules but stated they were unable to reproduce the failure and no repair was performed. The dealership has requested that I take possession of the vehicle despite the unresolved cause of this failure. Given the loss of throttle response and steering function while the vehicle was in motion, I currently feel the vehicle is unsafe to operate until the underlying issue is identified and corrected. A sudden loss of throttle response and steering control while the vehicle is in motion creates a significant risk of collision, particularly in traffic or at highway speeds. The fact that the failure temporarily cleared after cycling the vehicle suggests a potential intermittent electronic or communication failure affecting critical vehicle control systems.

On two separate occasions, the vehicle downshifted. Once while driving on the interstate at 70 mph, the vehicle downshifted to 3rd gear and locked up the tires. The rpms went up to 6000. The driver was nearly rear ended. In another instance while driving at 45 mph, the vehicle downshifted to 1st gear. The vehicle is available for inspection upon request. The issue cannot necessarily be reproduced on demand as it does not happen every time you drive the vehicle but it has happened more than once. The vehicle has been inspected by by a repair shop and the resolution appears to be a system update. There were no warnings, messages, or other symptoms prior to the events. The first event was in approximately January 2026 and the most recent incident February 22, 2026. The vehicle has not been driven since the last incident as we feel it is unsafe.

2013 Ford f?150 limited – VIN [xxx] on [xxx], my 2013 Ford f?150 limited experienced a sudden and dangerous transmission failure involving the transmission conductor plate (lead frame). My son was driving the vehicle when the dashboard abruptly shut off, multiple warning lights illuminated, and the truck lost engine power. Shortly afterward, while braking, the engine attempted to accelerate on its own, and the vehicle resisted slowing down. This created an immediate and life?threatening situation. These symptoms match the known failure mode of defective 6r80 transmission lead frames, which NHTSA has previously identified as a serious safety hazard due to sudden loss of power, erratic shifting, unintended acceleration, and dashboard failure. The vehicle became undriveable and could not be safely transported to a Ford dealership. It had to be repaired at a nearby independent shop (auto stop elite of purcellville, va). The conductor plate was replaced on February 12, 2026, at a total cost of $1,689. 59. Despite this being a well?documented safety defect affecting many Ford f?150 owners, no recall or service campaign was ever issued for my VIN. No warnings appeared on Ford’s website, carfax, or through any Ford communication. Other owners have received reimbursement from Ford for the same failure, yet Ford provided no notice or remedy in my case. This defect placed my son’s life in danger and represents a clear safety issue that should be addressed. I am requesting that NHTSA review this incident as part of the ongoing pattern of conductor plate/lead frame failures in Ford’s 6r80 transmissions. We are informing you because we have contacted Ford and they have done nothing to resolve this. We had to have it repaired and were never aware that there was a safety recall. We expect to be reimbursed in full for this repair by Ford. To date - we are being ignored and will now pursue with the virginia attorney general's office as well. While driving at about 50 mph, the truck will go from 6th to 1st, and lock up the rear end briefly. I'm the mechanic filing this for my customer, seems to me, its the lead frame failing in the trans, and they have done nothing about it, customer says Ford worked on this transmission, the only updated part I found was 1 extra magnet. Old part number for the lead frame, and a known failing lead frame #, I'm pretty sure you guys have a open case for these trucks.

My 2018 Ford F-150 experiences harsh, erratic, and unpredictable transmission shifting. The truck frequently hesitates during acceleration, followed by sudden and violent gear engagement. It also makes abrupt downshifts while driving, causing unexpected deceleration that can be dangerous in traffic. The truck will shift from 8th, 9th, or 10th gear down to 1st at highway speeds. This causes abrupt deceleration and significant loss of speed on the highway. On multiple occasions, the transmission has delayed engagement when shifting from a stop, creating hesitation when entering intersections or merging, followed by a sudden surge of power. This creates an unsafe driving condition and increases the risk of rear-end collision or loss of control. The shifting behavior is inconsistent and unpredictable, occurring during both in traffic and highway driving. The problem is worsening over time. Given the frequency and severity of these events, I believe this transmission behavior represents a significant safety defect that should be investigated for potential recall or corrective action.

Around curves truck will down shift too many gears. The problem was when it did it recently with the icey roads the tires locked up for a second and I got sideways. I got it straight but if a car was coming I would crashed. My speed was around 50 but I let off the gas to slow down for the curver never applied brakes yet. It does it usually when slowing down its done it since ive owned it and its never bothered me too much until this recent instance. No warning lights, only recall is back up camera I dont have issues with. Ive complained to many Ford dealers and my year dose not qualify, if I want to get it fixed I need to buy new transmission.

The contact stated that the vehicle then stopped accelerating with an unknown " braking" message displayed. The vehicle was then driven approximately 40 miles and then parked. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 6,500.

The contact owns a 2019 Ford F-150. The contact stated that on several occasions, while the vehicle was stopped, the transmission became stuck in fourth gear and the vehicle failed to accelerate while depressing the accelerator pedal. The accelerator pedal was depressed to the floor for the vehicle to respond. The vehicle gradually accelerated. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was 99,500.

The contact owns a 2014 Ford F-150. The contact stated that while shifting into reverse or drive, the vehicle seemed to be in neutral and failed to move. The vehicle failed to shift into gear. The wrench symbol was displayed. After resetting the codes, there was an abnormal squealing sound. The engine light was illuminated. The vehicle went into safe mode. The contact stated that the vehicle was taken to the local dealer and the contact was informed that a diagnostic test was needed. The vehicle was not diagnosed or repaired. The vehicle was previously reprogrammed under NHTSA campaign number: 24v444000 (power train). The manufacturer was contacted and was referred to the local dealer for assistance. The failure mileage was approximately 220,288.

Approximately one month ago, I noticed that the vehicle would not accelerate the way it normally does. A error code came on. I took the vehicle to auto zone for error code check. I was informed that the powertrain control module speed sensor had gone bad. I looked this up and found that Ford had a recall on 2011 Ford F-150s for similar issues. I disconnected the battery, as recommended on an internet site, and the vehicle began to operate normally. About one week later, I noticed that the vehicle would not shift above third gear and was accelerating very sluggishly. On further investigation I fount that the vehicle would not shift out of third gear (up or down). I again disconnected/reconnected the battery, and again it began operating normally. I contacted the local Ford dealership and they said that I should have received a letter in may 2019 about the pcm recall. I didn't recall receiving the letter, but Ford motor CO. Customer service says the pcm was actually reprogrammed on March 29, 2019 to repair the issues related to the recall. My concern is that the pcm speed sensor is malfunctioning again even after the repairs were supposedly performed by Ford. I am concerned that the vehicle may again be potentially unsafe to operate at highway speeds. I wanted to make NHTSA aware of the potential for ongoing safety issues with this vehicle even after Ford's proposed fix.

Truck would randomly go into limp mode while driving. Highways, country roads, middle of town, making it difficult to pull off the road with no ability to accelerate to safely merge off the roadway. The cause was from the lead frame in the transmission which was under an “expired” recall 19n01. Dealership charged me $1,500 to fix this. How is this not a safety issue that should have been taken care of by the dealership at no charge. I just bought the truck mid December 2025 and am not happy to have to shell out $1,500 to fix a known issue like this.

While driving my 2013 f150 eco boost is started shifting erratically. While at 60 mph the truck would down shift on it's own to 1st gear and my truck would skid. Luckily I did not crash but it's dangerous and it's my only transportation. I took it to Ford and they said I needed a $8k transmission replacement. I ended up taking to a transmission shop for a rebuild but shortly after if did it again. The transmission shop replaced the valve but said Ford needed to reflash the tcm. When I asked for them to follow recall 19s07 they said I would need to bring it in and pay them another $400 for a diagnostic and they couldn't just reflash it. Now I'm in a position where I have to drive what feels like a timebomb and I have no way out of it.

While driving at normal speeds, the transmission hesitates and pops when shifting into 4th gear. The vehicle slows down unexpectedly before shifting and will suddenly downshift without warning. Even when I’m in normal gears it will downshift and cause me to slow and lose control even when I am not breaking. It occurs at just 35 mph- highway speeds.

I am an owner of a 2013 f150 with a 5. 0 liter v8 engine my transmission speed sensor malfunction causing my truck to abruptly lose power as I was doing 85mph down the turnpike as to where I had to coast to the shoulder of the road and my dash cluster I had no readings of digital components and I had a wrench symbol on my dash board as I was confused of what just happened I killed the engine and restarted it and I took off to get home ASAP during my ride home I encountered this problem a couple more times , I took it and had a diagnostics on it and it showed a code of p0720 which indicated a transmission speed sensor problem so I looked up any recalls on this issue and found that in fact my truck had been recalled for this issue so I contacted Ford and they have my truck under the customer appreciation program and informed me that I was past the extended warranty program and my truck was over 150,000 miles so basically they were no longer responsible to fix it but my truck is actually under the recall status because it has the 5. 0 liter engine and that recall calls for a programming and molded lead frame replacement and from what I read that recall don't expire because it is a safety issue and the code is stored in my pcm and cant be erased so my question is , is my truck still covered under the 19s19 recall since my engine is the 5. 0 liter v8 coyote engine ?.

This report is being made within 1 1/2 hours after the incident. It has not been inspected by a dealer, service center, nor any other agency. I was stopped at a local fast food restaurant waiting to pay for my purchase. The 2021 Ford F-150 was in drive with my foot on the brake. No warning lights or other indication were present. Without warning, the engine revved to a high rpm and the truck shot forward about 15-20 feet before I could react by putting my full weight on the brake pedal. The engine quickly slowed. I then put the vehicle in reverse and backed up to the window and placed the transmission in park. The truck is equipped with adaptive cruise control but it was not active at the time of the incident. Had there been another vehicle in front of me, I would have rear-ended them. I work from home and use the truck a couple of times per week and it is available for inspection most days.

I was driving home and the truck started to shift very harshly and had died at a stoplight , I put it into park and restarted it and the engine light came on tried to put it into drive and it had jumped forward very rough as soon as I did, started driving again and then the truck shifted into 5th gear and would not shift out of 5th gear at all was very slowly leaving from stop signs and the high speed fans had turned on and ran the entire rest of the time. Causing very irratic shifting where at times it would down shift and skid the tires.

The vehicle experienced sudden, unexpected downshifting and a temporary loss of power while in motion. These incidents occurred frequently and randomly, particularly while accelerating or traversing inclines, creating "unsafe driving scenarios" where the vehicle could have been rear-ended or lost traction due to abrupt gear changes. Furthermore, the vehicle eventually entered "limp mode," where it would "buck" during shifts and fail to accelerate under load, leaving the driver vulnerable in traffic. 3. Yes, by both. Independent center: super tech automotive confirmed on December 11, 2025, that the transmission line pressure dropped from 158 psi to 20 psi under load, causing the vehicle to go into "limp mode". Dealer: south west Ford service department verified the concerns on December 16, 2025, documenting a "torque converter shudder" and "direct drum failure" with "excessive metal debris in fluid". 4. Yes. The vehicle was inspected by a manufacturer-authorized dealer, south west Ford service department, on December 16, 2025 (service advisor ryan forbis; transmission specialist 'dietrich'). 5. Yes. Symptoms: the symptoms first appeared in August 2025 as a "sudden, temporary loss of power or unexpected down-shifting". Warning lamps: initially, no warning lights or error codes were produced, which prevented early detection during multiple diagnostic attempts. Later symptoms: on December 11, 2025, the ses (service engine soon) light finally appeared when the truck entered "limp mode". Prior to the total failure, the transmission also exhibited "harsh/jerky downshifts" and would "hang up in gear".

When driving home from work at night on the highway in a 55mph zone my truck lost all power and the ability to accelerate. While on the highway began to hit the gas a bit more and saw the gears go from gear 10 all the way down to 1st gear at 55mph. Not sure what was happening and the truck began to “gear hunt” (attempting to find a gear that would give power). The truck gave me a check engine light and a alert of reduced power. I started to smell a burning smell in the vehicle and someone behind me was flashing me with their high beams to attempt to alert me something was wrong. I rolled the window down and looked in my mirror and saw white bellowing smoke being produced from behind the vehicle causing other drivers to either change lanes or slow down so they can see. I used what speed I had left from going down a on ramp to pull into a parking spot behind a building on the main road and had to call for a tow truck very late at night after my shift. I had the vehicle towed to that into the service department of a certified Ford dealership. After a few days of waiting for a response on the diagnosis of the vehicle, they stated that the transmission had blown and needed a replacement.

I was attempting to leave a parking spot when while maneuvering the vehicle it suddenly and unexpectedly accelerated to a high speed into a tree. There is a police report and security video as the incident took place in a hospital parking lot. The truck is totaled and settled but insurance company not interested in liability. My wife and I were injured. I have no idea or explanation for the sudden acceleration. Nobody is investigating if there was a vehicle malfunction or not. I just want it documented somewhere in case it is happening a lot. The crash was violent and occurred from a near complete stop on a clear day with no distractions.
